Vascular injuries that occur during traffic accidents are a commonly neglected aspect that can add more detail to the framework of a case. In this study, we analysed a case series of 150 traffic accidents, 39 of which were marked by microscopically identifiable vascular lesions. The purpose was to identify the presence of carotid injuries in individuals who died due to traffic accidents and had nonpenetrating trauma of the neck. We focused on the discrepancies regarding the macroscopical aspect and the histology and demonstrated how histological analysis of the carotids in cases of trauma can reveal injuries that are attributable to the trauma itself. We conducted a histological analysis of the lesions to describe their distribution and type and investigate potential correlations. The study offers insight on how to examine road accidents that involve traumatic injury of the carotid arteries. Indeed the main task of the forensic pathologist in the case of death is to establish the existence of a causal relationship between the micro- or macroscopic alterations observed in the autopsy and the traumatic event that led to the death of the subject. Thus, further morphological elements were provided to the forensic practitioners that may reveal injuries attributable to the trauma itself and should be evaluated in cases of trauma in traffic accidents. 相似文献

Background and purposeMultiply occurring intracranial dural arteriovenous fistulas (dAVFs) have been documented but rarely occur, and neither pathogenesis nor prognosis is clearly understood. This study was conducted to analyze angiographic characteristics of multiple dAVFs and to chronicle our treatment experience.MethodsBetween April, 2002 and January, 2018, data prospectively collected from 310 patients with intracranial dAVFs were systematically reviewed, assessing clinical and anatomic outcomes of endovascular treatment in 32 patients with multiple dAVFs (≥ 2 fistulas each). Lesions were categorized as multifocal or diffuse type, depending on presentation, and further characterized as progressive or non-progressive disease.ResultsOverall, 18 patients (56.3%) experienced aggressive presentations, including intracerebral hemorrhage or venous infarction. Cortical venous reflux (CVR) was observed in 26 patients (81.3%), and sinus thrombosis or occlusion was seen in 24 (75.0%). Clinical outcomes in patients with multifocal fistulas (n = 11) were excellent (100%), marked by a moderately high rate of complete occlusion (54.5%). Those with progressive disease (n = 10) regularly displayed certain angiographic findings, namely diffuse configuration (100%), sinus thrombosis (100%), and CVR (100%). Complete anatomic obliteration was achieved in 12 patients (37.5%), and in 26 patients (81.3%), clinical outcomes were favorable.ConclusionMultiple dAVFs are typically aggressive at presentation, given strong associations with CVR and sinus thrombosis. In diffuse-type fistulas, the potential to recur or progress is high. Although definitive treatment poses a challenge, outcomes of endovascular therapeutics may be still optimized in this setting through strategic procedural modifications and careful follow-up monitoring. 相似文献

IntroductionUltrasound is a useful first-line imaging modality for diagnosing and monitoring vascular pathologies. Compared with other modalities, it is relatively low cost, requires no ionizing radiation, is often available at bedside, and is noninvasive. However, the modality can have limitations when differentiating normal from pathologic tissues. In this review, we discuss the role of contrast-enhanced ultrasound (CEUS) in carotid, aortic, and peripheral vascular conditions.DiscussionCEUS is a developing modality that supersedes standard vascular ultrasound imaging and complements other modalities such as computed topography and magnetic resonance angiograms. Administered intravenously, the contrast are microbubbles filled with gas, surrounded by a stabilizing shell. They have the ability to enhance the quality of images and quantify vascular pathologies by acting as intravascular tracers of ultrasound energy. Based on these properties, CEUS has the potential to play a pivotal role in the management of vascular pathologies through its utility in detection, diagnosis, risk-stratification, follow-up, and monitoring.ConclusionStudies have suggested that CEUS is superior compared with standard ultrasound and on-par with computed topography angiograms in the detection of vascular pathologies, concluding that CEUS should be part of standardized routine practice. 相似文献

ATP is the major excitatory transmitter from chemoreceptor type I cells to sensory nerve endings in the carotid body, and has been suggested to be released by exocytosis from these cells. We investigated the mRNA expression and immunohistochemical localization of vesicular nucleotide transporter (VNUT) in the rat carotid body. RT-PCR detected mRNA expression of VNUT in extracts of the tissue. Immunoreactivity for VNUT was localized in a part of type I cells immunoreactive for synaptophysin (SYN), but not in glial-like type II cells immunoreactive for S100 and S100B. Among SYN-immunoreactive type I cells, VNUT immunoreactivity was selectively localized in the sub-population of tyrosine hydroxylase (TH)-immunorective type I cells associated with nerve endings immunoreactive for the P2X3 purinoceptor; however, it was not detected in the sub-population of type I cells immunoreactive for dopamine beta-hydroxylase. Multi-immunolabeling for VNUT, P2X3, and Bassoon revealed that Bassoon-immunoreactive products were localized in type I cells with VNUT immunoreactivity, and accumulated on the contact side of P2X3-immunoreactive nerve endings. These results revealed the selective localization of VNUT in the subpopulation of TH-immunoreactive type I cells attached to sensory nerve endings and suggested that these cells release ATP by exocytosis for chemosensory transmission in the carotid body. 相似文献

目的 研究同型半胱氨酸(homocysteine,Hcy)是否为绝经后女性颈动脉粥样硬化的独立风险因素。 方法 对2017年12月—2018年12月北京协和医院健康体检的1 204名55岁以上绝经后女性,进行Hcy检测和颈动脉超声检查。使用多变量logistic回归方法分析Hcy及其他传统风险因素对颈动脉粥样硬化的影响。 结果 Hcy平均值(12.4±3.3)μmol/L,高Hcy血症176人(14.62%)。Hcy与年龄和BMI正相关(分别r=0.19,P<0.001;r=0.11, P<0.001)。颈动脉粥样硬化患者的Hcy显著升高(Z=-2.56,P=0.011)。logistic回归显示,对于颈动脉粥样硬化只有年龄(OR=1.11,95%CI:1.09~1.14)和收缩压(OR=1.02,95%CI:1.01~1.02)是独立风险因素;对颈动脉斑块形成年龄(OR=1.10,95%CI:1.08~1.12)、收缩压(OR=1.02,95%CI:1.01~1.03)、糖化血红蛋白(OR=1.27,95%CI:1.07~1.50)和高血压病史(OR=1.40,95%CI:1.06~1.87)是独立危险因素。 结论 在绝经后女性,Hcy不是动脉粥样硬化发生的独立风险因素,筛查高Hcy血症在预防该人群颈动脉粥样硬化的意义仍需进一步证明。 相似文献
