目的:分析腹腔镜胆囊切除术的临床疗效,分析其对血清应激指标的影响。方法:将160例来我院行胆囊切除术的患者随机分为两组,对照组和观察组各80例。对照组采用开腹胆囊切除术,观察组则采用腹腔镜胆囊切除术。比较两组间手术时间、术中出血量、肠功能恢复时间以及住院天数差异,比较治疗前后两组患者间血清应激相关指标皮质醇(COR)、去甲肾上腺素(NE)、促肾上腺皮质激素(ACTH)、多巴胺(DA)以及胰岛素(INS)、三碘甲状腺原氨酸(T3)、甲状腺素(T4)、促甲状腺激素释放激素(TSH)水平差异。结果:观察组患者手术时间、术中出血量、肠功能恢复时间以及住院天数均显著少于对照组(P<0.05)。血清COR、NE、ACTH、DA和INS、T3水平在围手术期均发生了显著变化,各项指标均在胆囊分离时就明显升高,术后24 h开始下降(P<0.05);术后观察组上述指标均显著低于对照组(P<0.05)。结论:腹腔镜胆囊切除术对机体应激反应影响小于开腹手术,更利于患者术后康复。  相似文献

Nerve growth factor (NGF) and NGF receptors were measured in cortex and hippocampus of rats treated with drugs affecting cholinergic neurotransmission. High (Kd= 0.045nM) and low (Kd= 21nM) affinity125I-NGF binding sites were present in both cortical and hippocampal membranes with hippocampus containing higher numbers of both sites than cortex. Chronic treatment of rats with the muscarinic receptor antagonist scopolamine (5 mg/kg, twice daily) decreased the density of high- and low-affinity sites by 50–90% in cortical and hippocampal membranes. These changes were seen after 7 days, but not 3 days, of scopolamine treatment. Chronic infusion of physostigmine (1 mg/kg/day) using minipumps increased the number of high- and low-affinity sites in cortex 3- and 6-fold, respectively. The changes in receptor-binding parameters induced by physostigmine were transient as they were evident after 3 days of treatment, but returned to control levels after 7 days. NGF content in cortex and hippocampus was reduced by about 50% following 7, but not 3, days of chronic physostigmine infusion. In contrast, scopolamine treatment failed to change NGF levels in the cholinergic neuronal target regions but it decreased NGF content in the septal area. The content of NGF mRNA in the cortex measured by Northern blot analysis failed to change following either scopolamine or physostigmine treatment. The results suggest that levels of NGF and NGF receptors in the target regions of cholinergic neurons are regulated by the extent of cholinergic neurotransmitter activity.  相似文献

目的对29例肾上腺肿瘤患者行射频消融(RFA)治疗,研究其近期局部治疗效果、不良反应和副作用。方法肾上腺肿瘤患者共29例,病灶总数31个,其中直径≤2.0cm的病灶共5个,2.1-4.0cm者18个,4.1-6.0cm者5个,≥6.1cm者3个,经RFA治疗1个月后行螺旋CT双期增强扫描评价肿瘤治疗效果。结果上述病灶经消融治疗后达到完全坏死者分别为5个、18个、3个、2个。患者无严重并发症出现。结论RFA治疗安全可靠,副作用小,是治疗肾上腺恶性肿瘤的有效方法之一。  相似文献

目的总结后腹腔镜技术治疗肾上腺肿瘤的临床经验.方法采用后腹腔镜切除肾上腺肿瘤35例,男12例,女23例.年龄25岁~72岁,平均49.8岁.左侧15例,右侧20例.其中嗜铬细胞瘤5例,原发性醛固酮增多症10,柯兴氏病2例,神经节细胞瘤2例,无功能性腺瘤16例.腰部取3个套管针穿刺入路,用自制的水囊扩张器扩张后腹腔,用超声刀将肿瘤切除.结果 35例手术均成功,无中转开放手术病例.术中出血量10~500 ml,平均74 ml.病人均未输血.手术时间45~200 min,平均90 min.结论后腹腔镜手术切除肾上腺肿瘤手术效果确切,手术时间短,术中出血少,损伤小,术后恢复快,可以替代绝大多数肾上腺肿瘤开放手术.  相似文献

Summary: : The hypothalamic-pituitary-gonadal axis is a complex system within which both positive and negative feedback occur among its elements and higher brain systems. The occurrence of seizures and changes in the secretion of pituitary hormones can affect the feedback loop. Both seizures and antiepileptic drugs can affect the hypothalamic-pituitary-gonadal axis of males and females and cause changes in hormones and sexuality. Reproductive dysfunction has a social impact because of reduced fertility. Once conception occurs, live birth rates are not diminished. Prospective studies of men and women with epilepsy are needed.  相似文献

Extracellular single-unit recordings were made in somatosensory cortical barrels of fentanyl-sedated rats. Whiskers were deflected singly or in paired combinations. lontophoretically-applied (−)-baclofen disproportionately reduced weak responses, and phaclofen disproportionately increased them, resulting in more tightly focused or more broadly focused receptive fields, respectively. Both drugs had only minor effects on surround inhibition. In light of previous findings, we conclude that GABAA and GABAB mechanisms both act to enhance spatial contrast, but that the former plays a much greater role in enhancing temporal resolution.  相似文献

目的探讨腹腔镜卵巢穿刺内凝对多囊卵巢不孕症的治疗价值。方法151例腹腔镜卵巢穿刺内凝术(实验组)和151例开腹卵巢楔切术(对照组)治疗多囊卵巢不孕症配对对比研究。结果实验组的手术时间33.1±7.2min,术中失血15.6±4.7ml,总排卵率92.05%,总妊娠率49.67%,均优于对照组(P<0.01);术后雄激素、LH/FSH下降幅度小于对照组(P<0.01)。随时间推移,两组术后排卵巢、妊娠率均有下降倾向,而术后雄激素、LH/FSH则有逐渐回升的倾向。结论腹腔镜卵巢穿刺内凝术对多囊卵巢不孕症有较好的治疗效果  相似文献

为了探讨雄激素受体(AR)、雌激素受体(ER)、孕激素受体(PR)和表皮生长因子受体(EGFR)在前列腺带性结构中的分布,应用单饱和剂量测定法,分别测定了前列腺移行带、外周带组织中AR、ER、PR和EGFR的含量。结果表明,在11例前列腺增生症(BPH)患者22份标本中,移行带和周围带AR全部阳性;尽管AR浓度个体差异很大,但是两带之间仍有密切相关性(P<0001);周围带AR浓度高于移行带,两带AR浓度均高于PR、ER;PR和ER在前列腺带性结构上没有明显差异。EGFR在移行带明显高于周围带。研究证实了AR、ER、PR,EGFR在前列腺移行带和周围带上的分布特征。  相似文献

The effects of MPP+ (2.5–20 mg/kg) on the adrenal glands and heart were investigated in rats. At various periods after s.c. drug administration the rats were decapitated and tissue catecholamine levels were determined by means of HPLC with electrochemical detection. Adrenal dopamine (DA) levels were reduced at 2–8 h after MPP+ administration, but this decrease was followed by an elevation after 16 h and return to the control values after one week. Three successive injections of MPP+ caused a statistically significant elevation in adrenal DA, one day, with a tendency to elevation four and seven days after the last injection, whereas a severe (up to 96%) decrease in heart noradrenaline (NA) was found one day after the last injection. Seven days after the last injection a 50% depletion of NA in the heart was still observed. Pretreatment with GBR 12909 (30 mg/kg, 4 h) blocked the MPP+ (10 mg/kg, 2 h) induced reduction of adrenal DA levels, but at the same time GBR 12909 failed to block the effects of MPP+ in the heart. One day after three successive daily injections of MPP+ (10 mg/kg each), the DA-uptake inhibitor GBR 12909 (30 mg/kg, 6 h) could still induce an increase in adrenal DA.MPP+ appears to lack persistent cytotoxic action in the adrenal medulla but rather to cause a transient inhibition of DA synthesis followed by a compensatory stimulation. The inhibition can be blocked by specific inhibitor of the DA-uptake mechanism, suggesting a direct effect of MPP+ taken up by adrenomedullary cells. The data obtained so far do not suggest any involvement of peripheral dopaminergic nerves in the action of MPP+ on the adrenal medulla. The responses of human neocortical neurons to iontophoretic application of excitatory amino acids and their modulation by dopamine (DA) were studied in vitro. Brain slices were obtained from children undergoing surgery for intractable epilepsy. Application of N-methyl-D-aspartate (NMDA) to the slices induced slow depolarizations accompanied by decreased input conductances and sustained action potentials in cortical neurons. Glutamate produced rapid depolarizations and firing with few changes in input conductances. Quisqualate also induced depolarization and firing, but input conductances increased during the rising phase of the membrane depolarization. Iontophoretic application of DA alone produced no change in membrane potential or input conductance. However, when DA was applied in conjunction with the excitatory amino acids, it produced contrasting effects. With either bath application of DA or when iontophoresis of DA preceded application of NMDA, the amplitude of the membrane depolarizations and the number of action potentials were increased, whereas the latency of these responses decreased. In contrast, DA decreased the amplitude of the depolarizations and the number of action potentials evoked by glutamate or quisqualate. The fact that DA affects responses to NMDA and glutamate or quisqualate in opposite directions is of considerable importance to the understanding of cellular mechanisms of neuromodulation and the role of DA in cognitive processing and in epilepsy.  相似文献
