足三里穴位注射654-2治疗婴幼儿腹泻146例临床观察

目的：观察足三里穴位注射654．2治疗婴幼儿秋冬季腹泻的疗效。方法：选择2月至2岁幼儿腹泻病例共284例，随机分为治疗组和对照组。治疗组146例采用足三里注射654-2 0．5-1．0mg/kg，同时口服补液盐；对照组138例使用补液、肌注病毒唑，口服庆大霉素及助消化剂。结果：治疗组治愈率93．2％，好转率4．1％，平均治愈时间2．49d；对照组治愈率57．9％，好转率31．6％，平均治愈时间4．55d。结论：足三里穴位注射654．2疗效显著，且简单易行、经济方便，可在临床推广使用。     

Expression of p27 and p53: comparative analysis of uterine carcinosarcoma and endometrial carcinoma

The aim of the study was to assess both p27 and p53 expression in the stromal and epithelial component of carcinosarcoma and to assess if their expression in the latter is different than in endometrial carcinoma. Immunohistochemical staining for p27 and p53 was performed on paraffin-embedded tissue blocks of 18 uterine specimens with carcinosarcoma and their expression assessed. Their expression in the epithelial element was also compared to that in 35 paraffin-embedded tissue blocks of endometrial endometrioid carcinoma. Reduced p27 expression was observed in a similarly high proportion of the stromal (77.8%) as well as of the epithelial component (66.7%) of carcinosarcoma. Although statistically not significant, the proportion of reduced p27 expression in endometrial carcinoma (85.7%) was higher than in the epithelial element of carcinosarcoma. The percentage of p53 overexpression in both elements of carcinosarcomas and in endometrial carcinomas was low and also similar (27.8 and 20.0%, respectively). Our results indicate that reduced p27 expression is common and p53 overexpression is infrequent in carcinosarcoma. Their similar rates of expression in the stromal and epithelial elements of the tumor support the contention of a monoclonal origin of carcinosarcoma. Unexpectedly, reduced p27 expression is more common in endometrial carcinoma than in the epithelial element of carcinosarcoma, in spite of the less favorable prognosticators and outcome in the latter. Further studies of p27 expression in carcinosarcoma are indicated to establish its clinical value in this aggressive malignancy.     

Mechanism of uptake and retention of F-18 BMS-747 158-02 in cardiomyocytes: a novel PET myocardial imaging agent

Background BMS-747158-02 is a novel fluorine 18-labeled pyridazinone derivative designed for cardiac imaging. The uptake and retention mechanisms of F-18 BMS-747158-02 in cardiac myocytes were studied in vitro, and the biodistribution of F-18 BMS-747158-02 was studied in vivo in mice. Methods and Results Fluorine 19 BMS-747158-01 inhibited mitochondrial complex I (MC-I) in bovine heart submitochondrial particles with an IC₅₀ of 16.6±3 nmol/L that was comparable to the reference inhibitors of MC-1, rotenone, pyridaben, and deguelin (IC₅₀ of 18.2±6.7 nmol/L, 19.8±2.6 nmol/L, and 23.1±1.5 nmol/L, respectively). F-18 BMS-747158-02 had high uptake in monolayers of neonatal rat cardiomyocytes (10.3%±0.7% of incubated drug at 60 minutes) that was inhibited by 200 nmol/L of rotenone (91%±2%) and deguelin (89%±3%). In contrast, an inactive pyridaben analog, P-0 (IC₅₀ value>4 μmol/L in MC-1 assay), did not inhibit the binding of F-18 BMS-747158-02 in cardiomyocytes. Uptake and washout kinetics for F-18 BMS-747158-02 in rat cardiomyocytes indicated that the time to half-maximal (t_1/2) uptake was very rapid (approximately 35 seconds), and washout t_1/2 for efflux of F-18 BMS-747158-02 was greater than 120 minutes. In vivo biodistribution studies in mice showed that F-18 BMS-747158-02 had substatial myocardial uptake (9.5%±0.5% of injected dose per gram) at 60 minutes and heart-to-lung and heart-to-liver ratios of 14.1±2.5 and 8.3±0.5, respectively. Positron emission tomography imaging in the mouse allowed clear cardiac visualization and demonstrated sustained myocardial uptake through 55 minutes. Conclusions F-18 BMS-747158-02 is a novel positron emission tomography cardiac tracer targeting MC-I in cardiomyocytes with rapid uptake and slow washout. These characteristics allow fast and sustained accumulation in the heart.     

微小切口胆囊切除术2260例效果观察

目的 总结微小切口胆囊切除术治疗胆道系统疾病的经验。方法 对2002-2004年连续收治的2260例胆囊、胆道系统疾病实施微小切口手术的临床观察资料进行回顾性分析。结果 本组2260例微小切口胆囊切除术的患者无一例因手术原因死亡；术后无残留小胆囊；无胆汁漏及腹腔脓肿，亦无伤口全层裂开及切口疝形成。结论 该术式损伤小、局部显露清楚、不易发生副损伤，可同时行胆总管探查术，特殊情况下可随时改变手术方式。     

5-Br-PADAP催化光度法测定锰的研究

实验中发现Mn（Ⅱ）能够催化5-Br-PADAP与过氧化氢之间的氧化还原反应，并找到了Mn^2＋浓度与催化褪色反应速度间的线性关系，线性浓度范围为0．5-10ng／mL，最小检出浓度为0．2ng／mL。方法灵敏度高，选择性好。     

Insulin resistance Type A and short 5th metacarpals.

BACKGROUND/AIMS: Insulin resistance is associated with a number genetic syndromes and a variety of defects of insulin action. METHODS: We describe three members of an extended family spanning two generations with insulin resistance Type A and short 5th metacarpals. The proband had secondary amenorrhoea, male pattern hair distribution, acne, hirsutism, deep voice, acanthosis nigricans, polycystic ovaries, diabetes, features of acromegaly, raised creatine kinase and triglyceride levels and short 5th metacarpals. Her growth hormone, adrenal steroid and testosterone levels were normal. The proband's daughter had severe acne, hirsutism, acanthosis nigricans, polycystic ovaries, raised triglyceride, glucose and testosterone level short metacarpals and normal insulin receptor gene. The proband's son had a muscular build, raised creatine kinase, hypertriglyceridaemia and short 5th metacarpals. His fasting insulin levels were normal but pro-insulin was raised. RESULT/CONCLUSION: There are many familial and genetic syndromes associated with insulin resistance. This family was diagnosed as having insulin resistance Type A. This family does not conform entirely to any of the previously described syndromes and a number of family members have the phenotype of short 5th metacarpals, which appears to be associated with the features of insulin resistance Type A.

p53、nm23蛋白在宫颈鳞癌中的表达及其临床意义

目的研究p53、nm23基因在宫颈鳞癌中的表达及其临床意义.方法采用免疫组织化学S-P法检测45例宫颈鳞癌组织、10例宫颈上皮内瘤变和10例正常宫颈鳞状上皮中p53、nm23蛋白的表达.结果p53、nm23蛋白在宫颈鳞癌中的阳性表达率分别为64.44%、46.67%;在CIN中阳性表达率分别为0、30.00%;在NE组均无表达.p53蛋白在宫颈鳞癌中阳性表达与在CIN、NE中的不表达差异均有显著性(P＜0.01).nm23蛋白在宫颈鳞癌组织中的阳性表达与在NE中的不表达差异有显著性(P＜0.05),但与在CIN中的表达差异无显著性(P＞0.05);与病理学分级、淋巴结转移均呈负相关(P＜0.05,P＜0.01).结论p53、nm23基因参与了宫颈鳞癌的发生发展过程.p53蛋白的过表达是宫颈癌组织恶性变的生物学标志;检测nm23蛋白对判断宫颈癌淋巴结转移和预后有重要意义.     

64层螺旋CT诊断先天性冠状动脉起源异常的价值

目的：探讨64层螺旋CT冠状动脉造影诊断冠状动脉起源异常的临床价值。材料和方法：回顾性总结2005年12月至2006年12月间1800例64层螺旋CT冠脉造影的结果，统计冠状动脉起源异常的发生率。结果：1800例检查者中发现冠脉起源异常51例，发病率2．8％。包括右冠状动脉变异11例（21．6％）、左冠状动脉变异8例（15．7％）、左右冠均发生变异1例（2％）、单支冠状动脉1例（2％）、分支变异28例（54．9％）。结论：64层螺旋CT是诊断冠状动脉起源异常的安全有效方法，对临床治疗有重要的意义。     

螺旋CT多功能重建在骨关节外伤中的应用

目的 探讨螺旋cT功能重建在骨与关节外伤中的应用，提高重建技术的使用能力和诊断水平。方法 对使用双层螺旋CT，SSD，MPR，MIP等多种方式重建检查的49例复杂的骨与关节外伤病例进行回顾性分析。结果 49例骨关节外伤重建成像，其中，颅底3例，颌面部7例，颈、胸、腰、骶椎共17例，肩胛骨3例，髋关节8例，膝关节11例。SSD在显示碎骨片明显移位的空间关系方面较MIP，MPR为佳，它可以直观地、立体地、清晰地、多角度显示骨关节损伤，而MIP在显示骨折细节方面较优，尤其在外伤中颅窝观察有关神经孔方面。结论 螺旋CT的SSD，MPR和MIP技术的综合应用，对复杂的骨折、关节损伤的显示效果良好，具有较高的临床应用价值。
