PYRIDOXINE DEFICIENCY AND PHOSPHOLIPID METABOLISM

Pyridoxine deficiency decreased phospholipid methylation in rat liver microsomes to a greater degree than did food restriction alone.     

SECONDARY CYSTATHIONINURIA

Cystathioninuria was detected in 6 of 230 hospitalized children. Three had convulsions when admitted to the hospital, two severe liver malfunction of unknown cause (suspect neonatal hepatitis), and one child had severe congenital malformations and bronchopneumonia and was in a terminal stage. Cystathioninuria disappeared in 4 children (two with convulsions and two with liver malfunction) treated with pyridoxine orally. Pyridoxine treatment (2×40 mg daily) also resulted in disappearence of convulsions. The possible background to these findings is discussed.     

Pyridoxine responsive hereditary sideroblastic erythropoiesis and iron overload: Two microcytic subpopulations in the affected male,one normocytic and one microcytic subpopulation in the obligate female carrier

Mild hepatic iron overload has been demonstrated by magnetic susceptibility measurements in a 22-year-old man with hereditary sideroblastic erythropoiesis despite hemoglobin levels in the normal range and a normal erythropoietin level. His grandfather's sideroblastic anemia has been found to be responsive to pyridoxine; his mother's hemoglobin has persisted in the normal range but red cell volume distribution analysis demonstrated two subpopulations; 30% with estimated geometric mean of 68 fl and 70% an estimated mean of 93 fl. Red cell distribution analysis of the grandson demonstrated two microcytic subpopulations; 46% with an estimated geometric mean of 45 fl and 54% an estimated mean of 70 fl. A therapeutic regimen is outlined to reduce to normal his iron stores and to prevent the future development of excessive iron overload. © 1993 Wiley-Liss, Inc.     

Megavitamins And The Hyperactive Child

Megadoses of niacin, ascorbic acid, pyridoxine and pantothenate had no beneficial effect on hyperactivity and were associated with gastrointestinal complaints and hepatotoxicity.     

A study of a female with congenital sideroblastic anemia

A female infant with congenital refractory sideroblastic anemia is described. A market reduction of δ-aminolevulinic acid (ALA) synthetase activity of erythroblasts was noticed with and without treatment of pyridoxal phosphate. Mitochondrial neutral protease activity of erythroblasts, which inactivates specifically the apo form of ALA synthetase, was normal and the sensitivity of apo form of ALA synthetase to the neutral protease was also normal. It was speculated that the reduction of ALA synthetase activity is not due to the high speed of destruction, but rather due to the impairment in production of ALA synthetase, which could explain the unresponsiveness to pyridoxine therapy in this case.

Effect of magnesium chloride and magnesium L-aspartate on seizure threshold in rats under conditions of dietary magnesium deficiency

We studied the effect of Mg-L-aspartate, MgCl₂, and their combinations with vitamin B₆, magneB₆, and MgSO₄ on seizure threshold in rats with dietary Mg²⁺ deficiency. Mg²⁺ deficiency was followed by a decrease in the threshold dose of corazol (from 79.20 to 49.48 mg/kg), shortening of the latency of the first jerk (by 33.6%, p=0.012), and reduction of the time to the first episode of clonic seizures (by 32.6%, p=0.011). Seizure threshold and latencies of the first jerk and first episode of clonic seizures increased over 3 weeks after peroral administration of Mg²⁺ salts. VITAMIN B6 DEFICIENCY AND IMMUNE RESPONSES

The effect of vitamin B₆ deficiency on the immune response is briefly reviewed. The mechanisms involved in the immunosuppressive effect of vitamin B₆ deficiency are unclear.     

Recurrence of gestational diabetes.

The recurrence rate of gestational diabetes in 58 patients who had had the foregoing pregnancy complicated by diabetes was estimated to be 30% if our former criteria for abnormal glucose tolerance were strictly applied and 25% if our new, more stringent criteria were used. The recurrence rate is not influenced by prophylactic administration of pyridoxine. The perinatal morbidity complicating the 'second' pregnancy of former gestational diabetics was not increased in those patients who were not treated again, as compared with those who were. Recurrent gestational diabetes is associated with a degree of overdiagnosis in an attempt to detect all gestational diabetics. It is suggested that recurrent gestational diabetes occurs mainly in prediabetic patients.
