In the past 2 decades, advancements have been made in procedures for preserving and restoring the function of the proximal humerus when treating glenohumeral (GH) osteoarthritis. This review addresses two surgical options for GH osteoarthritis in the young patient, stemless total shoulder arthroplasty and resurfacing arthroplasty. These procedures preserve bone stock, have low complication rates, ease future revision if indicated, and report equal if not superior results compared with stemmed implant options. Comparative studies especially of long-term clinical outcomes of humeral resurfacing and stemless implants are still needed to make an informed treatment decision, especially in the young patient.  相似文献

ObjectiveThis study, firstly, investigates the effect of using an anatomical versus a functional axis of rotation (FAR) on knee adduction moment (KAM) in healthy subjects and patients with knee osteoarthritis (KOA). Secondly, this study reports KAM for models with FAR calculated using weight-bearing and non-weight-bearing motion.DesignThree musculoskeletal models were created using OpenSim with different knee axis of rotation (AR): transepicondylar axis (TEA); FAR calculated based on SARA algorithm using a weight-bearing motion (wFAR) and a non-weight-bearing motion (nwFAR). KAM were calculated during gait in fifty-nine subjects (n = 20 healthy, n = 16 early OA, n = 23 established OA) for all models and groups.ResultsSignificant differences between the three groups in the first peak KAM were found when TEA was used (p = 0.038). However, these differences were no longer present when using FAR. In subjects with established OA, KAMs were significantly reduced when using nwFAR compared to TEA models but also compared to wFAR models.ConclusionThe presence of excessive KAM in subjects with established KOA showed to be dependent on the definition of the AR: anatomical versus functional. Therefore, caution should be accounted when comparing KAM in different studies on KOA patients. In patients with end-stage knee OA where increased passive knee laxity is likely to exist, the use of weight-bearing motions should be considered to avoid increased variability in the location and orientation of a FAR obtained from activities with only limited joint loading.  相似文献

Background: Osteoarthritis (OA) is a major cause of pain and physical disability, and general practitioners (GPs) are usually the first point of contact for patients. Physiotherapy has been shown as effective in the management of lower-limb OA. The aim was to explore the beliefs of GPs on the physiotherapy management of lower-limb OA in primary care. Methods: This is a qualitative study evaluating GP views about physiotherapy in Sheffield, South Yorkshire, UK. Participating GPs were recruited by systematic sampling, and invitation was given to GPs in 10 practices in the four localities in Sheffield. Semistructured interviews were completed and framework analysis was used to analyze the data. Results: Eight GPs were interviewed and six themes emerged from analysis of the data: perspective on OA, management strategy, views on patients, views on physiotherapy, working collaboratively, and suggestions for service improvements. GPs had a positive impression and knowledge of physiotherapy, but lacked understanding of the processes involved in treatment and limited awareness of clinical guidelines regarding the management of OA. Improvements in communication and collaborative working were critical issues suggested by the participants. Conclusion: This study found that GPs who were interviewed had a limited understanding on the role of physiotherapists and of clinical guidelines. Interprofessional communication was not as good as it should have been. A reconfiguration of the Sheffield musculoskeletal pathway may help achieve more effective collaborative working and a better outcome for patients.  相似文献

ObjectiveOpticin is a class III member of the small leucine-rich repeat proteoglycan (SLRP) family, produced in articular joint tissues. In normal and osteoarthritic (OA) cartilage, opticin is degraded. This study aimed to assess whether human cartilage opticin is degraded by the main proteases involved in OA pathophysiology, and to determine the protease cleavage sites of this SLRP.MethodsWe analyzed the proteolytic activity of matrix metalloproteinases (MMPs)-1, -2, -3, -7, -8 and -9, and ADAMTS-4 and -5 on proteoglycan extracts from normal and moderately fibrillated OA human cartilage, and on recombinant human opticin. Opticin degradation was analyzed by Western blotting and cleavage sites were determined by sequence analysis.ResultsAll eight proteases digested opticin from proteoglycan extracts from both normal and OA samples, as well as recombinant human opticin, MMP-2 and MMP-7 are the proteases that degrade recombinant human opticin most efficiently. The opticin cleavage site determined for these MMPs was between the glycosylation and leucine-rich repeat domains. MMP-7 had two additional digestion sites near the N-terminal end of opticin.ConclusionOpticin is a substrate for several MMPs and aggrecanases involved during OA cartilage degradation, and seems to be a preferential substrate for MMP-7. The role of opticin in cartilage degeneration could be related to decreased levels of intact opticin, followed by its proteolytic degradation, which in turn may stimulate some of the modifications observed in the OA cartilage, such as neovascularisation and changes in the extracellular matrix.  相似文献

Assessment of changes in standing balance following an intervention requires accurate measurement of balance parameters. The reliability of centre of pressure measures of balance during single-leg standing has not been reported in individuals with knee osteoarthritis. The purpose of this study was to assess the test re-test reliability of force platform centre of pressure measures during single-leg standing in older adults with knee osteoarthritis. Twenty-five adults with radiographic evidence of knee osteoarthritis performed single-leg standing balance trials on a laboratory-grade force platform on two occasions, no more than 14 days apart. Participants were asked to stand on their more symptomatic limb for three, ten second trials. Centre of pressure measures collected included: standard deviation in the mediolateral and anteroposterior directions, mean path length, velocity, and area. The mean of the three trials was calculated. Intraclass correlation coefficients, standard error of measurement, Bland and Altman plots and the minimum detectable change were calculated. Intraclass correlation coefficients ranged from 0.54 to 0.87, suggesting mixed reliability of measures. Reliability was lowest for the centre of pressure area (intraclass correlation coefficient = 0.54), and highest for centre of pressure velocity and path length (intraclass correlation coefficient = 0.87 for both). Standard error of measurement values were low for standard deviation in the mediolateral direction and high for centre of pressure area. These results suggest that centre of pressure values, in particular path length and velocity, are appropriate for assessment of standing balance in people with medial knee osteoarthritis.  相似文献

ObjectiveTo determine the comorbidities associated with disability in patients with OA in Mexico (2013–2015).Material and methodsA cross-sectional, retrospective and multicentre IMPACTAR study (n = 7703) in Mexican patients (2013–2015). Comorbidities associated with disability were identified in 4971 patients diagnosed with OA from the IMPACTAR registry (n = 7073). An adjusted logistic regression analysis was carried out by demographic, economic, clinical and medical variables.ResultsMean age was 63 years; and 75% of the patients were women. Subjects with OA and presence of comorbidities are 42% more likely to develop disabilities than patients without associated comorbidity, considering age, sex, family income, OA diagnosis duration, and education level. The highest rate of people with disability (28.9%) was concentrated in Region 7, which corresponds to Mexico City. There are also significant differences between median family incomes, when the income of persons with disability is under $13 000 (IQR: 9000–16 000) Mexican pesos, compared to patients without disability. Almost half of the subjects (49.6%) reported having at least one comorbidity. Arterial hypertension was the risk factor with a statistically significant difference (32.8%) among those with disability (34.7%).ConclusionsPrograms and interventions for OA patients should take into consideration comorbidity factors, being female, family income, and the region of residence as variables that may increase the possibility of developing an OA-associated disability.  相似文献
