颊脂肪垫瓣在修复口腔颌面部缺损中的应用   总被引:2,自引:0,他引:2  
目的探讨颊脂肪垫瓣在修复口腔颌面部缺损的应用和效果.方法将颊脂肪垫形成一蒂在后方的颊脂肪垫组织瓣转移至缺损区,在无张力下与创缘缝合,表面轻轻用碘仿纱布缝线包压法固定.用以修复口腔颌面部的良恶性肿瘤手术或损伤留下的缺损以及口腔上颌窦瘘共18例.其中修复腭部缺损2例,上颌骨切除后缺损7例,颊黏膜癌切除后缺损3例,颊部良性肿瘤切除后缺损4例,磨牙后区黏表癌切除后缺损1例,封闭拔牙后口腔上颌窦瘘1例.结果术后10 d去除碘仿纱布,见颊脂肪垫轻度水肿,2~4周后水肿明显消退,颊脂肪垫表面逐渐上皮化,6~8周内表面完全上皮化,3个月后再生黏膜与正常口腔黏膜相似.全部病例效果满意,无感染、坏死等并发症发生.结论带蒂颊脂肪垫瓣修复口腔颌面部缺损,方法简单、易行,效果满意,值得推广.  相似文献

目的 了解无糖尿病家族史、糖代谢正常的脾虚痰湿型、气滞痰阻型肥胖女性体脂改变及β细胞功能变化,分析两者之间的关系。方法 60例入选对象按体重指数(BMI)分为非肥胖者和肥胖者,肥胖者按中医辨证分为脾虚痰湿型、气滞痰阻型。采用双能X线骨密度仪(DEXA)检测体脂,通过静脉葡萄糖耐量试验(IVGTT)评估急性相胰岛素分泌(AIR)、胰岛素曲线下面积(AUCins)、胰岛素曲线下面积/血糖曲线下面积(AUCins/AUCglu)等β细胞功能指标。结果 与非肥胖者比较,脾虚痰湿型、气滞痰阻型肥胖者BMI、腰围(W)、脂肪含量增加,但两种证型肥胖者间BMI、W差异无统计学意义。与脾虚痰湿型肥胖者比较,气滞痰阻型肥胖者躯干、总体脂肪含量、躯干脂肪质量/总质量比增加,AIR、AUCins、AUCins/AUCglu亦明显增加。AIR、AUCins、AUCins/AUCglu与BMI、W、躯干和总体脂肪含量呈良好相关。多元线性回归分析显示躯干脂肪含量是影响AIR的重要因素。结论 气滞痰阻型肥胖者具有更高的体脂(尤其是躯干脂肪)含量和伴胰岛素抵抗的AIR增加,该人群是临床干预的重点对象。  相似文献

BACKGROUND: It is known that intravenous anesthetic etomidate fat emulsion has cerebral protection. Now many scholars focus on the research of its cerebral protection from molecular biology, but the mechanism of cerebral protection is still fully unclear. OBJECTIVE: To observe the influence of etomidate fat emulsion on the [Ca2+]i in hippocampal neurons during the transient cerebral ischemia injury in rats. DESIGN: Randomized controlled observation. SETTING: Weifang Medical College. MATERIALS: This study was carried out in the functional laboratory of Weifang Medical College between October 2005 and March 2006. Twenty-four male healthy Wistar rats, aged 3 to 4 months, were involved. Etomidate fat emulsion was provided by the limited company of En-hua Medical Bloc in Jiangsu Province (code of H20020511) and the other agents and materials were provided by Laboratory Center of Weifang Medical College. METHODS: The 24 Wistar rats were randomized into 3 groups: sham-operation group, model group and etomidate preconditioning group, with 8 rats in each. Rat models of transient cerebral ischemia injury were made by the ligation of bilateral carotid arteries combined with descending blood pressure in the latter two groups. Before ischemia (ligation of bilateral common carotid artery), rats in the etomidate preconditioning group were intraperitoneally injected with 12 mg/kg etomidate fat emulsion and then persistently intraperitoneally injected with etomidate fat emulsion at 1.0 mg/kg per minute. Rats in the model group were not administrated. Rats in the sham-operation group were only performed bilateral common carotid artery isolation. When rats were modeled, their brain tissues were quickly taken out and detected. MAIN OUTCOME MEASURES: Change of the fluorescence pixel value of the [Ca2+]i in each group by the laser scanning confocal microscope. RESULTS: Twenty-four rats were involved in the final analysis. Fluorescence pixel value in the sham-operation group was in the low level. Fluorescence pixel value in the model group was significantly higher than that in the sham-operation group (P < 0.01). Fluorescence pixel value in the etomidate preconditioning group was significantly lower than that in the model group (P < 0.01). CONCLUSION: The protection of etomidate fat emulsion to the transient cerebral ischemic injury in rats is associated with the inhibition to the increase of [Ca2+]i to some extent.  相似文献

Aerobic exercise and beta-blocking drugs are regularly prescribed as treatment for hypertension and as a prophylactic for patients at risk from coronary heart disease and for those recovering from an infarct. Some beta blockers, particularly non-beta1-selective drugs, may make exercise more difficult, possibly by interfering with substrate metabolism during exercise. This study examined the effects of low and high doses of a beta1-selective blocker, metoprolol, and a nonselective beta blocker, propranolol, on exercise metabolism. The study involved 20 healthy subjects (10 men, 10 women) who walked on a treadmill at 50% of their maximal oxygen uptake for 1 h on five occasions, separated by 7 days. On each of the five occasions they received one of the following treatments, given in random order: placebo, metoprolol 50 mg, metoprolol 100 mg, propranolol 40 mg, or propranolol 80 mg, all taken twice daily. Fat oxidation, expressed as a percentage of total energy expenditure, was significantly lower than with placebo for all of the active treatments except metoprolol 50 mg (placebo: 42.7 ± 11.6%; metoprolol 50 mg: 38.7 ± 14.1%, p = NS; metoprolol 100 mg: 36.3 ± 13.7%, p = 0.05; propranolol 40 mg: 31.2 ± 9.3%, p = 0.01; propranolol 80 mg: 29.5 ± 10.9%, p = 0.01); and significantly lower with propranolol than with metoprolol (propranolol 40 mg: p = 0.0036; propranolol 80 mg: p = 0.01). Plasma ammonia concentration was significantly higher than with placebo with propranolol 40 mg, propranolol 80 mg, and metoprolol 100 mg (p = 0.01 for all); with metoprolol 50 mg, there was no difference from placebo (p = NS). Both beta blockers in this study reduced fat metabolism and increased perceived exertion to some degree. Additional inhibition of fat oxidation occurred with the nonselective drug, probably in intramuscular rather than adipose lipolysis, and was probably beta2 mediated. The results of this study suggest that a selective beta blocker has less of an adverse effect on substrate metabolism than does a nonselective beta blocker. Beta1-selective drugs may offer advantages in patients who undertake regular aerobic exercise.  相似文献

Summary. In order to investigate fat metabolism and the regulation of lipolysis and blood fuel metabolites by insulin, nine patients with chronic obstructive pulmonary disease (COPD) with chronic hypoxaemia and seven healthy control subjects of similar age were investigated by determination of the turnover rate of free fatty acids (TOR), using 1-14C-oleic acid as a tracer, and arterial concentrations of FFA, glycerol and 3-hydroxybutyrate. The measurements were performed in the basal state and during insulin and glucose infusion, aiming at euglycaemia at insulin levels of 50 and 100 mU l-1. The subjects' ages were 64±2.7 and 66±1.1 (mean±SEM) years in the COPD and control groups, respectively. TOR was 0.73±0.06 and 0.52±0.02 mmol min-1 (P<0.05) in the basal state, 0.33±0.04 and 0.30±0.02 at an insulin level of 50 mU I-1 and 0.32±0.08 and 0.24±0.02 at an insulin level of 100 mU I-1, in the COPD and control groups, respectively. Arterial FFA concentration was 0.98±0.08 and 0.75±0.06 mmol 1-1 (P<0.05) in the basal state in the COPD and control groups, respectively. During the clamp, the decrease in FFA mirrored that in TOR. The results show that the state of lipolysis is increased in severe COPD patients with chronic hypoxaemia. Furthermore, the results suggest a reduced effect of insulin in lipolysis.  相似文献

研究了2种预处理工艺结合真空油炸对毛豆水分质量分数、脂肪质量分数和色差的影响.结果表明,与冷冻预处理和直接油炸(对照)相比,预干燥可以有效地降低毛豆的脂肪质量分数,且没有显著性地影响毛豆的色泽(P>0.05).随着前期热风预干燥时间的延长,真空油炸后毛豆的水分与脂肪质量分数逐渐减小,毛豆油炸干燥速度逐渐降低;另一方面,随着真空油炸时间的延长,毛豆自由水分逐渐减少,其后期干燥速率也逐渐降低.  相似文献

目的 探索一种治疗体表脂肪瘤及局部脂肪堆积的新方法。方法 自 1 998年 1 0月以来笔者对 1 2例患者在局麻下采用耳鼻咽喉科的鼻内窥镜系统及旋转切削刀具治疗体表脂肪瘤及局限性脂肪堆积。结果 随访 6个月至 2年所有病例均取得了满意的效果。结论 该手术方法操作简便、安全、疗效可靠 ,也符合美容外科的原则。是治疗体表脂肪瘤及局限性脂肪堆积值得推广的手术方法之一  相似文献

脂肪乳剂、葡萄糖、氨基酸、电解质、维生素及微量元素按一定程序混合配成10种全营养素混合液。在4℃14天贮存期内,10种混合液的pH和渗透压变化不大,不同混合液在不同时期的脂粒平均直径变化为0.3135±0.1012μm~0.4813±0.1893μm,94.93%脂粒的直径<0.6μm,无直径>6μm的脂粒,细菌培养阴性。74例共输全营养素混合液1,796次,无不良临床反应和异常实验室检查结果;用1周以上者大都显示氮平衡改善,用2周以上者血清转铁蛋白和白蛋白浓度可有不同程度升高。  相似文献

目的矫正重睑成形术后的上睑粘连、重睑较宽、眼睑上抬受限等畸形。方法另设计重睑线,彻底松解眼睑内的组织粘连,释放眶隔内脂肪及转移眼轮匝肌下脂肪垫充填。结果两年来为32例重睑成形术后畸形者用此法治疗得到满意效果。结论此方法简单易行,效果好。  相似文献
