幽门螺杆菌阳性十二指肠球部溃疡患者红细胞免疫功能的变化

目的:探讨幽门螺杆菌(Hp)阳性十二指肠球部溃疡患者红细胞免疫功能的变化。方法:47例Hp阳性十二指肠球部溃疡和32例健康体检者入选,前者以标准方案治疗7d,后者不予处理作为对照。治疗前后分别以流式细胞仪测定其血液红细胞上CR1的数量,以免疫粘附酵母菌花环法测定其活性。结果:与正常人相比,Hp阳性十二指球部溃疡红细胞C3b受体花环率、红细胞CR1数量降低(P<0.01),红细胞免疫复合物花环率增高(P<0.01)。结论:Hp的感染可能是导致十二指肠球部溃疡患者红细胞免疫功能低下的原因。     

Sedimentation and resuspendability evaluation of pharmaceutical suspensions by low-field one dimensional pulsed field gradient NMR profilometry

In order to study the sedimentation of pharmaceutical suspensions using low-field one dimensional pulsed field gradient nuclear magnetic resonance (1D pfg NMR) profilometry, the accuracy of signal acquisition as well as the spatial resolution of a commercial spectrometer operating at 23.4?MHz was investigated. The use of a solid Teflon spacer revealed that the accuracy of signal acquisition was independent of spatial position (height). The standard deviation of distance determinations was less than 150 µm, whereas the accuracy of water content determination was within 2% in the central part of the detection zone and deteriorated to 4% in the outer parts. The study of aqueous paramagnetic MnCl₂ solutions indicated an exponential relationship between the relative signal intensity and the transverse relaxation decay constant. From this relationship, the relative water content of suspensions could be derived from their signal intensity relative to that of water. Using concentrated paliperidone palmitate dispersions as model suspensions, low-field 1D pfg NMR profilometry has been proven to be suitable for the evaluation of both the sedimentation and resuspendability behavior of viscous, opaque suspensions, for which visual detection of homogeneity may be difficult.     

红细胞膜免疫磁珠检测成分血中抗A/抗B的研究

目的比较应用红细胞膜免疫磁珠法及传统方法进行成分血中抗A/抗B效价检测之间的相关性。建立应用红细胞膜免疫磁珠检测血浆标本中高效价抗体的方法。方法采用新鲜细胞试管法、微柱凝胶法及红细胞膜免疫磁珠法进行血浆样本抗体效价检测。将试管法检测效价≥128的样本10倍、15倍、20倍稀释后,用红细胞膜免疫磁珠检测,测定其凝集强度。结果微柱凝胶法最敏感,其次为试管法,红细胞膜免疫磁珠法低于试管法1个滴度。将试管法检测效价≥128的待检样本15倍稀释后,用红细胞膜免疫磁珠检测,其凝集强度≥2+。结论红细胞膜免疫磁珠可做为标准红细胞抗原应用于抗体效价检测,将样本进行15倍稀释,若凝集强度≥2+,则可视其带有高效价抗体。     

The Natural Course of Serum D-Dimer,C-Reactive Protein,and Erythrocyte Sedimentation Rate Levels After Uneventful Primary Total Joint Arthroplasty

BackgroundThis study aimed to assess the baseline levels of D-dimer, C-reactive protein (CRP), and erythrocyte sedimentation rate (ESR) and monitor the natural course of these serum markers after uneventful primary total joint arthroplasty.MethodsThis prospective study enrolled 81 patients undergoing primary total knee arthroplasty or total hip arthroplasty. The level of serum D-dimer, CRP, and ESR was measured preoperatively and on postoperative days 1, 3, 5, 15, and 45. Mean peak values, peak times, and distribution were compared between D-Dimer, CRP, and ESR.ResultsThe mean preoperative serum D-dimer, CRP, and ESR level was 412 ± 260 (range 200-980) ng/mL, 2.93 ± 2.1 (range 1-18) mg/L, and 22.88 ± 17.5 (range 3-102) mm/h, respectively. The highest mean peak for D-dimer, CRP, and ESR was at postoperative day 1, 3, and 5, respectively.ConclusionD-dimer levels reached peak levels on postoperative day 1 and then declined rapidly to a plateau level by postoperative day 3. A second, albeit small, peak in the level of D-dimer occurred on postoperative day 15. The level of CRP and ESR remained elevated for much longer with CRP returning to baseline on postoperative day 45 and the level of ESR had not returned back to normal on postoperative day 45.     

One-year recombinant growth hormone therapy does not improve hemoglobin state and morphology of erythrocytes in growth hormone deficient children

An increase in growth rates of children suffering from growth hormone deficiency (GHD) subjected to recombinant growth hormone treatment (rGHT) was shown to be accompanied by acceleration of metabolic processes that may stimulate oxygen consumption in various organs and tissues. Therefore, oxygen-transporting properties of RBC should undergo considerable changes during the rGHT. The aim of this study was to examine the effects of rGHT on erythrocyte shape and hemoglobin state in GHD children. The level of oxyhemoglobin (Oxy-Hb) in RBC was analyzed by Raman spectroscopy. The RBC count, mean corpuscular hemoglobin (MCH), mean corpuscular volume (MCV) and other parameters were calculated. The blood of eleven treatment-naive prepubertal children with GHD (aged 3–9, median 5.7 years) was examined and compared with control group (aged 5–7; median 6.0 years) at three time points: 0, 3 and 12 months of rGHT. Before rGHT, the MI in GHD children was higher (median 0.48 vs 0.14 p = 0.0018) and the RBC count was lower (median 4.20 vs 4.96 10¹² cells/L p = 0.0022) than in control group. After the treatment, cell count in GHD patients did not differ significantly from the control group, but Oxy-Hb level became higher (median 0.64 vs 0.41 p = 0.0075). During rGHT, MCV decreased (median 80.3 vs 83.2 μm³ p = 0.0231). Morphological and functional characteristics of erythrocytes in GHD children were shown to differ significantly from the healthy control group. A twelve-month rGHT partially improved some of the studied parameters but Oxy-Hb level and echinocyte count remained high.     

XC-A30A型全自动血沉分析仪的应用评价

目的探讨XC-A30A型全自动血沉分析仪测定红细胞沉降率(ESR)的重复性、准确性及影响因素。方法抽取高、中、低三组血沉值共27份进行重复性试验,对做血沉检查的198例患者,采用两法测定,以统计学方法分析仪器法的准确性,抽取10例贫血患者标本(压积<0.30)和10例压积正常且魏氏法ESR值在21～100 mm/h的标本对照,分析贫血标本用仪器测定的可行性。结果重复性:除1例(CV为16.1%)外,其余26例CV均<5%。准确性:ESR值在2～100 mm/h各组差异无统计学意义(P>0.05),但ESR值>100 mm/h者两组差异有统计学意义(P<0.05)。贫血标本采用两法测定的差值与对照组比较差异有统计学意义(P<0.05)。结论 XC-A30A型全自动血沉分析仪具有良好的重复性,准确性较高,贫血标本用该仪器测定准确性也较高,适合医院快速准确的报告血沉结果。但对于ESR>100 mm/h时建议用手工法复查。     

网织红细胞计数监测对乳腺癌化疗致骨髓抑制的评估价值

[目的]探讨网织红细胞(RET)计数动态监测对乳腺癌化疗致骨髓抑制的评估价值.[方法] 110例乳腺癌患者均接受化疗治疗,1疗程21 d,干预1～4疗程,通过全自动血细胞分析仪对各疗程化疗前、化疗后1周、2周、3周网织红细胞百分比(RET％)、高荧光强度网织红细胞百分率(HFR)、中荧光强度网织红细胞百分率(MFR)进行测定.[结果]与化疗前比较,第1～4疗程化疗后1周、2周RET％、HFR均显著降低,且差异有显著性(P<0.05);第1～3疗程化疗后3周RET％、HFR、MFR与化疗前比较差异无显著性(P>0.05);第3～4疗程化疗后2周MFR与化疗前比较差异无显著性(P>0.05);第4疗程化疗后3周HFR明显低于化疗前(P<0.05).[结论] 乳腺癌化疗不同疗程期间行RET相关指标动态监测对骨髓抑制、开始恢复情况评估具有重要作用.     

温阳补肾法对高原类风湿关节炎患者血清25羟维生素D3水平的影响

目的:观察温阳补肾法对高原类风湿关节炎患者血清25羟维生素D3水平的影响。方法:选取2015年7月至2017年8月青海省中医院收治的高原类风湿关节炎患者132例作为研究对象,随机分为对照组和观察组,每组66例。对照组应用常规西医治疗,观察组在对照组治疗基础上应用温阳补肾法治疗,2组均连续治疗3个月。比较2组治疗后临床疗效,统计2组治疗前后主要临床症状与体征、健康状况评定量表(HAQ)评分和DAS28评分,检测2组治疗前后实验室检查指标、血清25羟维生素D3以及血液流变学指标水平。结果:治疗后,观察组临床治疗总有效率显著高于对照组(P 0. 05); 2组关节肿胀指数、关节疼痛指数、关节压痛指数、晨僵时间、关节功能分级和20 m步行时间均较治疗前显著降低(P 0. 05),且观察组均显著低于对照组(P 0. 05); 2组HAQ评分、DAS28评分、类风湿因子(RF)、C反应蛋白(CRP)和红细胞沉降率(ESR)水平均较治疗前显著降低(P 0. 05),且观察组均显著低于对照组(P 0. 05); 2组血清25羟维生素D3水平均较治疗前显著升高(P 0. 05),且观察组显著高于对照组(P 0. 05); 2组红细胞电泳指数、红细胞压积、血浆黏度、全血低切黏度、全血高切黏度水平均较治疗前显著降低(P 0. 05),且观察组均显著低于对照组(P 0. 05)。结论:温阳补肾法治疗高原类风湿关节炎,疗效满意,同时能明显增加血清25羟维生素D3水平。     

白细胞介素-6对初次膝关节置换术后早期假体周围感染筛查作用研究

目的探讨白细胞介素-6(IL-6)对膝关节置换术后早期假体周围感染的诊断价值。方法选取2014年12月至2016年1月在沈阳军区总医院行初次膝关节置换的患者44例,对其术前及术后3、6、9 d血液中IL-6、血沉(ESR)、C-反应蛋白(CRP)的检测结果进行分析。结果所有患者术后IL-6值快速回落,但直至术后第9天,IL-6值也未回落到正常范围内。本研究患者术后3、6、9 d的血清IL-6、CRP回落曲线具有相关性,IL-6值的回落曲线更迅速,而ESR值回落曲线变化幅度较小。结论 IL-6可作为筛查膝关节置换术后早期假体周围感染的敏感指标,动态监测患者术后IL-6值回落曲线,能够提高膝关节置换术后早期假体周围感染的诊断率。
