异基因造血干细胞移植中真菌感染的预防和护理体会

真菌感染是异基因造血干细胞移植的主要并发症和致死原因之一,积极地预防和科学的护理,早期发现并预防真菌感染已成为移植成功的关键问题之一。我院自1995年以来,共进行异基因造血干细胞移植37例,仅6例发生真菌感染,该6例真菌感染的患者,经积极的治疗和细致的护理均已康复。现将真菌感染的预防和护理体会介绍如下。     

腹腔镜手术全麻前后安置低截石位的效果比较

重型再生障碍性贫血（severe aplastm anemia，SAA）病情凶险，死亡率高，临床常规治疗效果差，而如对此类患者应用抗淋巴细胞球蛋白（antilymphocyte globulin，ALG）可显著改善SAA患者的预后。目前，对无法采用异基因骨髓移植进行治疗的患者，ALG为首选的治疗方法。但ALG是一种免疫血清，不良反应较大，易引起过敏反应，甚至过敏性休克，因此加强用药前后的观察和护理，     

溴化异丙托品与消炎痛雾化吸入对COPD病人的疗效观察

以随机公开对照试验比较对COPD病人的支气管扩张作用及副反应,雾化吸收溴化异丙托品(50ug)为A组16例;雾化吸入消炎痛(0.5mg)为B组15例;雾化吸入溴化异丙托品(50ug)合并消炎痛(0.5mg)为C组15例。结果:A组15分钟起效;B组60分钟起效,其FEV1(第一秒用力肺活量)及FVC(用力肺活量)的最大改善率(22.0%、22.7%)与A组(21.5%、23.2%)相比无差异(P>0.05);C组5、15分钟FEV1的改善率(8.7%、13.0%)高于B组(0%、2%);180、360分钟FEV1的改善率(23.9%、17.3%)高于A组(13.7%、5.8%)(P<0.05)、FVC与FEV1的改变相近,三组均无严重的副反应,提示:COPD患者雾化吸入消炎痛具有与溴化异丙托品相近的气道扩张作用,但起效慢。两者合用时,起效快,持续时间长,作用优于单药应用。     

麝香保心丸联合消心痛治疗心绞痛的临床观察

笔者应用中成药麝香保心丸联合西药消心痛片治疗冠心病心绞痛70例，取得了满意的疗效，现报道如下。     

Effect of 5-Aminoisoquinolinone on the Adhesion of Colon Carcinoma

Objective： 5-Aminoisoquinolinone, a water-soluble, potent inhibitor of the activity of poly （adenosine 5＇-diphosphate ribose） polymerase, plays an important role in the tissue injury associated with ischaemia-reperfusion injury and inflammation by inhibiting the activity of poly （adenosine 5＇-diphosphate ribose） polymerase and the expression of cell adhesion molecules such as ICAM-1, P-selectin et al. But how about it in the tumor is not clear. The aim of the present study was to study the effects of 5-Aminoisoquinolinon on the adhesion of colon carcinoma line HT-29 cells to human umbilical vein endothelial cells; and the effects of 5-Aminoisoquinolinon on the expression of ICAM-1, P-selectin and the activity of poly （adenosine 5＇-diphosphate ribose） polymerase in colon carcinoma HT-29 cells. Methods： The adhesion of HT-29 cells to human umbilical vein endothelial cells was detected by adhesive experiment. Immunocytochemically Streptavidin-Peroxidase method was used to investigate the expression of ICAM-1, P-selectin and Poly （adenosine 5＇-diphosphate ribose）（ the product of poly （adenosine 5＇-diphosphate ribose） polymerase activation）. Results： the results of the adhesion assay of HT-29 cells to HUVEC showed that the OD570 value in each 5-AIQ-treated group was significant lower than that in the control group （5-AIQ-untreated） in a dose-dependent manner. The expression of ICAM-1, P-selectin and Poly （adenosine 5＇-diphosphate ribose） was significant lower in 5-Aminoisoquinolinone-treated HT-29 cell group than that in 5-Aminoisoquinolinoneuntreated groups. Conclusion： The data suggest that 5-Aminoisoquinolinone can inhibit the adhesion of HT-29 cells to human umbilical vein endothelial cells. 5-Aminoisoquinolinone also can inhibit poly （adenosine 5＇-diphosphate ribose） polymerase activation and the expressions of ICAM-1 and P-selectin in HT-29 cells. 5-Aminoisoquinolinone probably contributes to the prevention of tumor cell metastasis. Further study is needed.
