The Région Languedoc-Roussillon is the umbrella organisation for an interconnected and integrated project on AHA covering the 3 pillars of the European Innovation Partnership on Active and Healthy Ageing. All sub-activities (A1: electronic pharmaceutical file, A2: falls prevention initiative, A3: frailty, B3: chronic respiratory diseases, chronic diseases with comorbidities, oral health and hepatitis virus C chronic infection, C2 and D4 active and independent living and handicap) are included in MACVIA-LR that has a strong political commitment and includes all stakeholders (public, private, patients, policy makers). It is one of the Reference Sites of the European Innovation Partnership on Active and Healthy Ageing built around chronic diseases, ageing and handicap. The framework of MACVIA-LR has the vision that the prevention and management of CDs is essential for AHA promotion and for the reduction of handicap. The main objective of MACVIA-LR is to develop innovative solutions for a network of Living Labs in order to improve the care of patients affected by CDs in the Languedoc-Roussillon area and to disseminate the innovation.  相似文献

Parkinson's disease is the second most common neurodegenerative disease worldwide, leading to a wide range of disability and medical complications. Managing patients with Parkinson's disease in the perioperative hospital setting can be particularly challenging. Suboptimal management can lead to medical complications, prolonged hospital stays, and delayed recovery. This review aims to address the most important issues related to caring for patients with Parkinson's disease perioperatively who are undergoing emergent or planned general surgery. It also intends to help hospitalists, internists, and other health care providers mitigate potential in-hospital morbidity and prevent prolonged recovery. Challenges in managing patients with Parkinson's disease in the perioperative hospital setting include disruption of medication schedules, “nothing by mouth” status, reduced mobility, and medication interactions and their side effects. Patients with Parkinson's disease are more prone to immobility and developing dysphagia, respiratory dysfunction, urinary retention, and psychiatric symptoms. These issues lead to higher rates of pneumonia, urinary tract infections, deconditioning, and falls compared with patients without Parkinson's disease, as well as prolonged hospital stays and a greater need for post-hospitalization rehabilitation. Steps can be taken to decrease these complications, including minimizing nothing by mouth status duration, using alternative routes of drugs administration when unable to give medications orally, avoiding drug interactions and medications that can worsen parkinsonism, assessing swallowing ability frequently, encouraging incentive spirometry, performing bladder scans, avoiding Foley catheters, and providing aggressive physical therapy. Knowing and anticipating these potential complications allow hospital physicians to mitigate nosocomial morbidity and shorten recovery times and hospital stays.  相似文献

Posterior alien hand syndrome is a new addition to a poorly understood group of movement disorders. Historically, anatomical lesions causing uncontrolled limb movement and a feeling of foreignness were found to be located in the corpus callosum or frontal lobe. Recent case reports, however, demonstrate the typical symptoms of alien hand syndrome with lesions located in the parietal/occipital lobes. Disturbance of normal function in these regions tends to produce less complex motor activity, such as hand levitation, along with a sensory component characterized by feeling of estrangement. We discuss a patient who presented with unusual symptoms following an outpatient procedure and was found to have posterior alien hand syndrome.  相似文献

This paper is a reflection on a Developmental Movement Play (DMP) programme based on Sherborne Developmental Movement (SDM) for parents and their young children experiencing emotional, behavioural and/or mental health difficulties. A focus on embodiment through movement experiences, dance and relationship play (Sherborne, 2001 Sherborne, V. (2001) Developmental movement for children: Mainstream, special needs and pre-school (2nd ed.). London: Worth Publishing  [Google Scholar]) is described within the programme. This is then discussed in relation to the bonding between parent–child, which takes place through shared movement experiences. Participants in the programme were referred through multi-disciplinary agencies with concerns for the parent–child relationship. The final group comprised 10 children (aged 18 months to eight years) and their mothers. The study uses a combination of quantitative and qualitative research methods to investigate the effectiveness of using DMP/SDM to facilitate changes in attitudes, feelings, moods and parenting styles. Results of the research are discussed and an examination of theory underpinning SDM explores Laban's principles of human movement (1998) and attachment theories (Ainsworth, Bell, Blehar, & Main, 1971; Bowlby, 1951 Bowlby, J. Abstract

In the past, vertical intrusive movement of teeth has been considered difficult and most routine clinical vertical movement of teeth has been confined to extrusion. It has been suggested that attempts at intrusion may result in an increased incidence of root resorption and also in occasional devitalization.

The displacement and resulting stress fields associated with such treatment can be successfully studied using the finite element method. In the case being considered initial movements are known to be small; therefore, the assumption in the study that the material behaves linear-elastically is considered to be reasonable.

This study of vertical tooth movement demonstrated that the maximum cervical margin stress in the periodontal ligament was 0·0046 N/mm2, whilst the highest apical stress was 0·00205 N/mm2 when intrusive and extrusive forces of 1 Newton were applied to the buccal surface of the crown of a tooth model. These stresses were evaluated in the light of previous studies and found to be within the suggested clinical optimum level. However, the periodontal stress distribution following orthodontic loading within this three-dimensional finite element model was found to be highly complex.  相似文献

Fathers have been excused from involvement with their families because of traditional gender roles, which invite fathers to be more involved with work and somewhat less involved with the emotional well-being of his family. Family therapy is often no exception to fathers' lack of participation in the family. Family therapists, however, are calling for inclusion of fathers in family therapy. This is occurring because most therapists prefer to have as many family members involved in therapy as possible, and because the feminist movement has called for family therapists to reject the "status quo," in this case, lack of fathers involvement in family therapy. Lastly, there is some empirical evidence that fathers inclusion in therapy improves the outcome of family therapy. This paper is written in response to these changes, so that therapists may gain insight into ways they can involve fathers in family therapy.  相似文献
