IntroductionIn recent years the proportion of predatory invitations by email of the total number of emails received has grown dramatically. The aim of the current study was to objectify the increase in predatory journal and congress requests per email over a 5.5-year period and their relatedness to the receivers’ speciality in Foot and Ankle surgery.Material and methodA comprehensive Microsoft Outlook® analysis of 5.5 years’ worth of junk email from one surgical Foot & Ankle specialist working at a Level-1 trauma centre was evaluated. The topic of each email from the last year was scored according to the ‘degree-of-relatedness’.ResultsBetween July 1st 2013 and December 31th 2018 a total of 41,687 emails were collected in the junk email folder of Microsoft Outlook®. A total of 17.0 percent was attributed to emails from predatory journals and congresses. Over the 5.5-year period the percentage of total emails that consisted of invitations from predatory journals and congresses rose from 4.5% to 40.8%. Only an insignificant amount (2.3%) was directly related to the authors medical sub-speciality of Foot and Ankle surgery.ConclusionThis is the first study showing the individual burden of unsolicited emails from predatory publishers and journals. The authors acknowledge the risk that this publication, like all others, will likely result in yet even more invitations from predatory journals and congress invitations. All should be warned of a huge increase in emails from predatory journals and congresses, which has likely not peaked yet. Appropriate action is needed, as we urge foot–ankle interested scientists to publish in indexed journals with proper peer review.  相似文献

The aim of this study was to compare orphan drug access in a sample of Balkan countries: five EU Member States (Bulgaria, Croatia, Greece, Romania, Slovenia) and two EU Candidates (Serbia, Montenegro). The comparative analysis was based on a cross-sectional study and included medicinal products with an active orphan designation and market authorisation on January 1, 2017.Access to orphan drugs is an ongoing challenge in these countries. Three clusters of countries were identified in terms of orphan drug access: Greece and Slovenia, making the top tier, Romania, Bulgaria, and Croatia, being in the middle, and EU Candidates, Serbia and Montenegro, forming the bottom tier, where a substantial number of EU market approved orphan drugs was not even registered. Available public health resources and market size are probably among the contributing factors for such inequalities. Sizeable part of EMA market authorised orphan medicinal products is not even priced in the Balkan countries. This is a serious issue, which is putting rare disease patients from this region in a particularly vulnerable situation.There is a need for further improvement in accessibility of orphan drugs in the Balkan countries. Cross-border collaboration in the field of pricing, health technology assessment, and reimbursement negotiation of orphan drugs may help to address these challenges.  相似文献

The aim of this case report was to show the concept of guided endodontics in a maxillary first molar with limited interocclusal space. Guided endodontics involves merging cone-beam computed tomographic imaging and a surface scan of the tooth in order to create a guide to perform a drill path into the seemingly obliterated root canal. In the molar region, the interocclusal space is often too small to accommodate the guide, bur, and handpiece at once, and, therefore, a modified approach is presented. A 52-year-old man was referred because the dentist had failed to localize the distobuccal root canal of a maxillary molar (#3) associated with apical pathosis. After reopening and rubber dam placement, a glide path was established for both the palatal first mesiobuccal root canal and the second mesiobuccal using a size 10 hand file and coronal flaring. Further instrumentation to the working length was achieved by reciprocating file size 25. All 3 canals were temporarily filled with calcium hydroxide. For the distobuccal root canal, guided endodontics was chosen in order to avoid further impairment of the tooth because negotiation of the canal failed even with the use of the operating microscope. Following the merged data obtained from the cone-beam computed tomographic and surface scans, a translucent SICAT Optiguide (SICAT, Bonn, Germany) was constructed containing a sleeve representing the proper direction of a drill path in order to reach the distobuccal root canal. The access cavity was temporarily filled with a composite material made for light-curing. Before light curing, the Optiguide was replaced on the teeth, and a steel pin was pressed through the sleeve and the composite whereby the proper drill path direction was transferred into the composite. After polymerization and removal of the Optiguide and pin from the composite base sleeve, the guided drilling could be performed. This case report is the first on guided access preparation in a molar with pulp canal obliteration and limited interocclusal space. The demand for more interocclusal space was solved by transforming the virtual drill path into a composite-based intracoronal guide. The use of digital technology was essential.  相似文献

The past decade has witnessed a significant increase in the incidence of GI diseases across Europe. There are clear differences in outcomes for patients in Europe based on geographical and economic differences, and there is a worrying inequality in the provision of healthcare across the continent. Recent demographic studies have highlighted the heavy burden of GI disease across Europe. There is increasing demand for endoscopic procedures which are becoming increasingly more complex and demand further expertise and training. A co-ordinated and cohesive approach to research, specialist training and healthcare funding is required to overcome these inequalities.  相似文献

BackgroundCommon salt is a safe, effective and cheap home-made remedy for umbilical granuloma. The aim of this scoping review is to identify and summarize the available evidence and examine the research conducted on salt treatment for umbilical granuloma.MethodsA literature search was performed in the second week of September, 2022 using Google scholar, PubMed, MEDLINE and EMBASE databases using the keywords ‘umbilical granuloma’ and ‘salt treatment’ to identify all English articles pertaining to salt treatment for umbilical granuloma. Tables were made to summarize the methodological characteristics, results and the dosage regimens of salt used by different authors. The Cochrane Collaboration's tool was used for assessing risk of bias in RCTs. The indexing statuses of the journals publishing these studies were also noted. The overall efficacy with the use of common salt was calculated by adding the success rates mentioned in each study.ResultsTwenty-four articles (2 systematic reviews, 6 Randomized Controlled Trials, 11 prospective cohort studies, 1 case control study, 3 retrospective case series and 1 case report) were included. An overall 93.91% success rate (1033/1100) was seen with common salt application, without any reports of complications/recurrences.ConclusionTopical application of common salt for umbilical granulomas is simple, effective and inexpensive. This scoping review provides a broader outlook at the existing level of evidence and may help in planning interventional comparative studies, so that recommendations can be formulated. ObjectiveTo study the mechanical behavior of endodontically-treated teeth with minimally invasive endodontic access cavities and resin composite restorations under different bonding conditions using finite element analysis (FEA).MethodsFour Class-II endodontic access cavities including the mesio-occlusal minimally-invasive (MO-MIE), mesio-occlusal conventional (MO-CONV), disto-occlusal minimally-invasive (DO-MIE), and disto-occlusal conventional (DO-CONV) cavities were prepared in 3D-printed maxillary first molars. Each tooth was subjected to root canal preparation and scanned using micro-CT to provide a 3D structural model which was virtually restored with resin composite. An intact 3D-printed molar was used as control. FEA was conducted under a 250-N vertical load. Three different interfacial bonding conditions between dentin/enamel and resin composite were considered, i.e. fully bonded, partially debonded, and fully debonded. The maximum principal stress of dentin and the normal tensile stress at the interfaces were recorded. The risk factor of failure for each component was then calculated.ResultsIn the fully-bonded tooth, the dentin-composite interface showed significantly higher stress and a higher risk factor than dentin, indicating that debonding at the dentin-composite interface would occur prior to dentin fracture. With the dentin-composite interface debonded, the enamel-composite interface exhibited higher stress and a higher risk factor than dentin, indicating that debonding at the enamel-composite interface would occur next, also prior to dentin fracture. With the resin composite fully debonded from the tooth, stress in dentin increased significantly. Irrespective of the bonding status, the CONV groups exhibited higher median stresses in dentin than the MIE groups.SignificanceWithin the limitation of this study, it was shown that debonding of the resin composite restoration increased the stress in dentin and hence the risk of dentin fracture in endodontically-restored teeth. Minimally-invasive access cavities could better safeguard the fracture resistance of interproximally-restored teeth compared to conventional ones.  相似文献
