军队医院信息系统的现状及对我军的展望

医院信息系统是在医院内建立以计算机为中心的系统网络，其目的是实现医院信息管理的现代化，近年来计算技术，通过技术等信息技术的发展，为医院信息系统的发展提供了有力的技术保障，本文介绍了国内国队医院的信息化建设的发展过程及现状，展望了我军医院信息系统的发展方向。     

Softcopy interpretation of head and abdominal computed tomograms: comparison of 1024 × 768 to 2048 × 2048 monitors

The purpose of this study was to determine the accuracy of interpretation of computed tomograms (CT) on 1K × 1K PC-based review workstations used by clinicians, and compare them to 2K × 2K diagnostic workstations used by radiologists. We reviewed 733 CT examinations, of which 390 were head and 198 abdominal. All studies were first reviewed on the PC-based workstation, and the findings were compared to those from the diagnostic workstation that served as the control. In all, 167 head and 49 abdominal CTs were negative. Forty head CTs had incidental findings. Of the 364 positive studies, 215 were head CTs and 149 were abdominal CTs. Findings were not apparent on the PC-based monitor in 3 out of the positive head CT cases (1.4 %) and 3 out of the positive abdominal studies (2 %). The combined missed rate was 1.65 %, which we consider to be an acceptable error rate for review workstations.     

Anforderungsdefinition und -spezifikation für PAC-Systeme (Picture Archiving and Communications System) Ein Leistungsverzeichnis in Anlehnung an den Standard„IEEE Recommended Practice for Software Requirements Specifications”

Zusammenfassung Hintergrund: Die Formulierung von Anforderung bildet die Grundlage für die Kontrolle der Zielvorgaben in einem PACS-Projekt. Sie soll ferner Art und Umfang der PACS-Funktionalit?t kl?ren, die zur Unterstützung radiologischer Arbeitsabl?ufe notwendig ist. Methodik: Anforderungsdefinitionen und -spezifikationen werden in Anlehnung an den IEEE-Standard „Recommended Practice for Software Requirements Specifications” systemunabh?ngig formuliert. Definitionen werden im Request for Information (Anforderung eines L?sungsvorschlags), Spezifikationen im Request for Proposal (Angebotsanforderung) festgelegt, wobei zwischen funktionalen und nicht-funktionalen Anforderungen unterschieden wird. Bewertet werden Umfang, Angemessenheit und Qualit?t der angebotenen Systeml?sungen. Ergebnisse: Es wurde eine PACS-Checkliste erstellt, die die genannten Grunds?tze berücksichtigt und auf den Webseiten der Arbeitsgemeinschaft Informationstechnologie (AGIT) der Deutschen R?ntgengesellschaft ver?ffentlicht wird (http://www.uni-marburg. de/mzr/agit). Schlu?folgerungen: Mit der Checkliste wird ein Diskussionsforum angeboten, das zur Festlegung einer akzeptierten PACS-Basisfunktionalit?t beitragen soll.

分布式混合结构PACS系统设计及实施

近年来,医学影像存储和通讯系统(Picture archiving and communications system,PACS)开始出现在国内大型医院,是管理医疗影像设备如CT、MRI、DR等产生的医学图像的信息系统.PACS的先进性、实用性是不容置疑的,然而其推广的进程却十分缓慢,尤其是中小医院.究其原因:其一,整套全院PACS(C/S结构服务器/客户端)费用昂贵,报价数以百万计,而高档的影像服务器在购置后不能被完全利用,造成资源浪费,而随时间延长,计算机硬件的飞速发展而贬值[1].其二,由于医院信息系统(HIS)在国内的发展早于PACS,很多需要实施PACS的医院已有基础网络及硬件环境,就要求PACS的实施要充分利用HIS、RIS系统的硬件及网络.要加快中小医院数字化进程,迫切需要解决如何用最小成本实现具有较好扩展性的全院PACS问题.我们经过实践探索,在我们医院设计实施了分布式PACS系统.     

固定内容寻址存储技术在我院信息网络中的应用

本文介绍了CAS(固定内容寻址存储)技术的存储原理及特点。我院的PACS与HIS系统数据存储归档采用了CAS技术,既保证了网络安全又为医院节约了大量人力和物力资源。
