This case report presents the effect of Cognitive Functional Therapy (CFT) in a patient with chronic non-specific neck pain. The patient believed that pain signified tissue damage, and demonstrated pain catastrophizing, hypervigilance, stress sensitivity, and movement impairment of the neck, during extension and rotation. The CFT intervention integrated a cognitive approach with manual therapy and active exercises to encourage the patient to trust her neck again. One month after the first appointment, the patient had recovered confidence, and the pain and disability had disappeared almost entirely.  相似文献

Neck pain (NP) is a common occupational health problem associated with a number of professions. Many studies indicate that NP is common among teachers, yet no published study was found to address the prevalence and risk factors of NP in Iranian school teachers. The purpose of the current study was to assess the prevalence and risk factors for NP among school teachers in Iran. A cross-sectional study was conducted on 586 randomly selected primary and high schools teachers from 22 schools in Tehran, Iran. Point, last month, last 6 months, annual, and lifetime prevalence rates of NP were 24%, 29%, 33%, 37%, and 43%, respectively. There was a significant association and increased prevalence of NP with a number of risk factors such as; being female, age, general health, length of employment, regular exercise and job satisfaction (P < 0.05 in all instances). Therefore, some individual and occupational factors may make conditions relevant for the development of NP among teachers.  相似文献

Objective

The purpose of the study was to determine the association between trapezius muscle tenderness and tension-type headache among female office workers.

Methods

Through a questionnaire survey, 256 female office workers with tension-type headaches reported the level of palpable tenderness (“no,” “some,” or “severe tenderness”) in the trapezius muscle. The number of days with headache (“0-7,” “8-14,” or “>14”), intensity (“low,” “moderate,” or “high”), duration of headache (“<8 hours per day,” “>8 hours per day,” and “all day”), and use of analgesic medications were reported. Odds ratio (OR) for tenderness in the trapezius muscle (“no/some” vs “severe tenderness”) as a function of days with headache, intensity of headache, duration of headache, and use of analgesic medications were calculated using a binary logistic regression controlling for age and body mass index.

Results

After adjustments for confounders, a strong association was found between the level of trapezius muscle tenderness and intensity of headache (moderate intensity, OR 2.45; 95% confidence interval [CI] 1.08-5.54; high intensity, OR 7.51 [95% CI 2.65-21.29]) and days with headache (>14 days, OR 4.75 [95% CI 1.41-15.89]). No association was observed for duration of headache or use of analgesic medications.

Conclusions

For the participants studied, there was a strong association between trapezius muscle tenderness and the level of intensity and the number of days with a headache among female office workers. No association was seen for duration of headaches or use of analgesic medications.  相似文献

Objective

The purpose of this study was to investigate the influence of thoracic high-velocity low-amplitude thrust (HVLAT) manipulation on quantitative and qualitative 3-dimensional cervical spine kinematic patterns in a subgroup of patients with acute neck pain.

Methods

Thirty patients with acute neck pain, aged 20 to 59, received a thoracic HVLAT manipulation. Three-dimensional kinematics of the cervical spine were registered pretreatment and posttreatment using an electromagnetic tracking system. Quantitative and qualitative parameters were calculated for axial rotation, lateral bending, and flexion-extension movement. Subjective pain ratings were measured with the visual analogue scale and the Neck Disability Index and were collected pretreatment and posttreatment.

Results

After treatment, the range of motion of the main motion improved significantly for axial rotation (P = .034), lateral bending (P < .001), and flexion-extension (P = .031). Although for axial rotation as the main motion, the smoothness of the flexion-extension movement improved significantly after treatment (P = .036), the reverse was true for flexion-extension as the main motion. Visual analogue scale scores exhibited a statistically (P < .001) and clinically significant reduction of pain sensation. The mean change in Neck Disability Index scores only exhibited a statistically significant improvement 1 week after treatment.

Conclusion

Thoracic HVLAT manipulation led to positive changes in quantitative and qualitative aspects of 3-dimensional cervical spine kinematics. Because of the 1-intervention group design, external factors influencing the healing process could not be eliminated.  相似文献

Objective

The objective of this study was to assess changes in upper trapezius myoelectric activity and pain in patients with nonspecific neck pain after a single session of acupuncture (ACP).

Methods

A blinded randomized clinical trial was conducted. Fifteen patients with nonspecific neck pain and 15 healthy participants were enrolled in a randomized, single-blinded, crossover study. Each participant was subjected to a single session of ACP and sham acupuncture (SACP). The electromyography (EMG) signal of the upper trapezius muscle was recorded during different step contractions of shoulder elevation force (15%-30% maximal voluntary contraction) before and after ACP treatment.

Results

Significant effects were confirmed after the treatment (ACP and SACP) for Numeric Rating Scale scores (F1,28 = 51.61; P < .0001) and pain area (F1,2 = 32.03; P < .0001). Significant decreases in the EMG amplitude were identified for the nonspecific neck pain group (NPG) (F1,112 = 26.82; P < .0001) and the healthy participant group (HPG) (F1,112 = 21.69; P < .0001) after ACP treatment. No differences were identified between the ACP and SACP treatment protocols for Numeric Rating Scale score (NPG: F1,28 = 0.95; P = .33), pain area (NPG: F1,28 = 1.97; P = .17), or EMG amplitude (NPG: F1,112 = 0.47; P = .49; HPG: F1,112 = 0.75; P = .38).

Conclusion

The effect of ACP at acupoints triple energizer 5 and large intestine 11 triple energizer 5, or in close proximity, contributes to pain relief among patients with nonspecific neck pain. The electromyographic analysis indicated a greater resistance to muscle fatigue and decrease of activity of the upper trapezius muscle among healthy participants and patients with nonspecific neck pain.  相似文献

目的:探讨股骨近端空心钉锁定板内固定治疗股骨颈骨折的临床疗效。方法:股骨颈骨折行股骨近端空心钉锁定板内固定术13例,观察骨折愈合时间以及髋关节功能恢复情况。结果:术后13例骨折一期愈合,随访6-20个月,无退钉及断钉,1例股骨头坏死。Harris评分优8例,良4例,优良率92.3%。结论:股骨近端空心钉锁定板内固定治疗股骨颈骨折是有效的内固定方法,固定牢靠,便于患肢早期活动,改善髋关节功能,降低并发症,是较为理想的内固定方法。  相似文献

目的探讨中老年人颈围与血液生化指标的相关性,为血液生化指标异常的筛查探索更简便的方法。方法选择成都市参加大型免费体检的742名城乡中老年居民作为研究对象,进行体格测量、问卷调查以及血液生化指标测定。按照本课题组前期研究的颈围对肥胖的最佳预测切点(男性为37.05 cm,女性为32.73 cm)将研究对象分为颈围增大组(400名)和颈围正常组(342名),探讨颈围与血液生化指标的关系。结果不同性别人群颈围增大组的腰围、体质指数(BMI)、体重、腰臀比和小腿围均大于颈围正常组,差异均有统计学意义(P0.01)。颈围增大组空腹血糖、甘油三酯和极低密度脂蛋白胆固醇水平高于颈围正常组,高密度脂蛋白胆固醇水平低于颈围正常组,差异均有统计学意义(P0.05,P0.01)。另外,男性老年人群颈围增大组血红蛋白水平高于颈围正常组,差异有统计学意义(P0.05)。多因素logistic回归分析结果显示,男性中老年人群空腹血糖水平(OR=1.24,95%CI:1.07~1.45)、血红蛋白水平(OR=1.04,95%CI:1.01~1.06)与颈围呈正相关,高密度脂蛋白胆固醇水平(OR=0.07,95%CI:0.02~0.18)与颈围呈负相关(P0.05,P0.01),女性中老年人群甘油三酯水平(OR=1.51,95%CI:1.01~2.18)与颈围呈正相关,高密度脂蛋白胆固醇水平(OR=0.33,95%CI:0.16~0.65)与颈围呈负相关。结论颈围的测量有利于筛查发现血生化异常人群。  相似文献

Summary Results of chemotherapy in head and neck cancers are reported with a regimen of cisplatinum, bleomycin and methotrexate. In 63 previously untreated patients, the overall response rate was 73%, including 13/63 CR and 33/63 PR. The response rate in 20 previously treated tumors was 20%. The chemotherapy protocol was well tolerated without severe complications. Initial chemotherapy as a third modality in addition to radiotherapy and/or surgery is discussed.  相似文献

Background:Technological advancements in modern military and acrobatic jet planes have resulted in extraordinary psychophysiological loads being exerted upon flying personnel,including inducing neck and back pain.The purpose of this study was to examine the effects of 12 weeks of functional strength training on 1) the volume and strength of the neck and shoulder muscles and 2) muscular activity upon exposure to helmets of different masses and elevated Gz forces in a long-arm centrifuge in high-performance aircraft personnel.Methods:Eighteen participants underwent 12 weeks of functional strength training (n=12) or the control protocol(n=6) without additional strength training.Pre-and post-intervention tests included evaluations of isometric strength of the head extensor muscles,flexion,and lateral flexion and rotation,as well as magnetic resonance imaging (MRI)to measure the volume of the m.stemocleidomastoideus,m.trapezius,and deep neck muscles.Furthermore,during a long-arm centrifuge (+1.4 and +3Gz) protocol,the muscular activity levels of the m.sternocleidomastoideus,m.trapezius and m.erector spinae muscles were assessed without a flight helmet,with a helmet,and with a helmet and night vision goggles.Each participant's perception of muscular strain was noted immediately after the long-arm centrifuge protocol.Results:The maximal isometric strength in all exercises and muscle volumes increased in the training group but not the control group (P<0.05).Relative muscle activity (%MVC) with a helmet decreased after the intervention in the training but not the control group (P=0.01).Relative muscle activity while wearing a helmet and night vision goggles was higher after intervention in the control group than in the training group (P<0.01).The perceived muscular strain of the neck muscles induced by the long-arm centrifuge did not differ between the groups.Conclusions:Twelve weeks of functional strength training improves the maximal isometric strength and volume of neck and shoulder muscles and leads to lower relative muscle activation upon exposure to elevated Gz forces in a long-arm centrifuge.  相似文献
