The purpose of the present study was to investigate whether altered longitudinal bending stiffness (LBS) levels of the midsole of a running shoe lead to a systematic change in lower extremity joint lever arms of the ground reaction force (GRF). Joint moments and GRF lever arms in the sagittal plane were determined from 19 male subjects running at 3.5 m/s using inverse dynamics procedures. LBS was manipulated using carbon fiber insoles of 1.9 mm and 3.2 mm thickness. Increasing LBS led to a significant shift of joint lever arms to a more anterior position. Effects were more pronounced at distal joints. Ankle joint moments were not significantly increased in the presence of higher GRF lever arms when averaged over all subjects. Still, two individual strategies (1: increase ankle joint moments while keeping push-off times almost constant, 2: decrease ankle joint moments and increase push-off times) could be identified in response to increased ankle joint lever arms that might reflect individual differences between subjects with respect to strength capacities or anthropometric characteristics. The results of the present study indicate that LBS systematically influences GRF lever arms of lower extremity joints during the push-off phase in running. Further, individual responses to altered LBS levels could be identified that could aid in finding optimum LBS values for a given individual.  相似文献

ObjectiveTo assess speech alterations in mouth-breathing children, and to correlate them with the respiratory type, etiology, gender, and age.MethodA total of 439 mouth-breathers were evaluated, aged between 4 and 12 years. The presence of speech alterations in children older than 5 years was considered delayed speech development. The observed alterations were tongue interposition (TI), frontal lisp (FL), articulatory disorders (AD), sound omissions (SO), and lateral lisp (LL). The etiology of mouth breathing, gender, age, respiratory type, and speech disorders were correlated.ResultsSpeech alterations were diagnosed in 31.2% of patients, unrelated to the respiratory type: oral or mixed. Increased frequency of articulatory disorders and more than one speech disorder were observed in males. TI was observed in 53.3% patients, followed by AD in 26.3%, and by FL in 21.9%. The co-occurrence of two or more speech alterations was observed in 24.8% of the children.ConclusionMouth breathing can affect speech development, socialization, and school performance. Early detection of mouth breathing is essential to prevent and minimize its negative effects on the overall development of individuals.  相似文献

BackgroundDifferent therapeutical options exist for the treatment of hallux rigidus, depending on the clinical symptoms. These include within the surgical treatment joint-preserving and -resection procedures, whereby the arthrodesis of the first metaphalangeal (MTP-I) joint has the reputation for a particular safe treatment option.Material and MethodsThe aim of this review is to present the surgical treatment of hallux rigidus with an arthrodesis of the first MTP-I- joint.ConclusionsBesides other surgical procedures such as a cheilectomy or artificial arthroplasties, e.g. Cartiva, MTP-I-arthrodesis continues to be the gold standard therapy for hallux rigidus in advanced stages. It represents a safe and simple surgical therapy option and, according to current literature, has the highest degree of evidence (grade B). In addition, MTP-I arthrodesis is a proven salvage procedure after failed arthroplasties and hallux valgus corrections.  相似文献
