烧伤后手指瘢痕挛缩畸形综合治疗的临床研究 总被引:7,自引:1,他引:6
目的通过比较手术结合康复综合治疗方案与常规治疗方案对烧伤后手指瘢痕挛缩畸形的治疗效果,探讨综合治疗的优越性及经验。方法1999年6月~2004年6月,共56例烧伤后手指瘢痕挛缩畸形患者纳入试验。综合治疗组(A组)28例,男20例,女8例;年龄14~47岁,平均31岁;患手共47只129指,以手术结合系统康复治疗。常规治疗组(B组)28例,男17例,女11例;年龄18~51岁,平均30岁;患手共42只107指,采用常规手术,术后仅作简单的康复性锻炼指导,由患者在院外自己完成功能锻炼。采用Swanson法对患指运动功能进行评定,九孔柱试验法对患手整体运动功能进行评定,比较两组患者治疗前后手指和手整体运动功能的差异。结果A组患者术后均获随访12~19个月,B组患者均获随访13~20个月。治疗前A组患指的强直失能指数(indexofankylosis,IA)值为82%±20%,B组为78%±17%;治疗后A组为45%±13%,B组为52%±14%;治疗前后A组IA差值为37%±15%,B组为26%±15%,两组比较差异有统计学意义(P<0.05)。治疗前A组九孔柱测量值28.34±5.62s,B组为27.47±5.78s;治疗后A组为20.73±4.25s,B组为21.86±4.12s;治疗前后A组九孔柱测量差值为7.61±2.27s,B组为5.61±2.94s,两组比较差异有统计学意义(P<0.05)。结论烧伤后手指瘢痕挛缩畸形采用手术结合康复综合治疗的效果明显优于常规治疗,值得推广。 相似文献

目的动态观察肝脏热缺血再灌注损伤(WIRI)对细胞周期检查点基因的表达影响,研究其分子机制帮助了解那些与热缺血再灌注相关的疾病。方法采用成组对照的实验设计,利用微阵列芯片和逆转录-聚合酶链反应(PCR)联合分析技术,分析SD大鼠热缺血再灌注损伤处理后的基因表达变化(其中芯片分析8只、RT-PCR定量分析16只);运用系统生物学分析方法对功能基因进行聚类。结果分别得到了热缺血再灌注损伤处理后肝脏的细胞周期各检查点基因及早期即刻基因表达数据,将其中与细胞周期密切相关的的功能基因(上调的223条、下调的62条变化幅度均大于3倍)做进一步分析。结论肝脏热缺血再灌注损伤通过细胞周期/检查点控制基因影响细胞周期G1/S、G2/M的运行,抑制DNA的合成和染色体的分裂。推测可能影响受损的DNA双链自我修复和后期的组织细胞再生、凋亡。 相似文献

Abstract
Background: Does there exist a difference in the outcome of severely injured children and severely injured healthy adults?
Methods: The data of 1,566 severely injured patients, treated between May 1998 and December 2002 in our emergency department of the
University Essen/Germany, were analyzed. Patients with an injury severity score (ISS) > 24 were included in the present study.
Patients younger as 18 (17) years were located to the children group c. Patients aged 18 and up to the age of 54 were included
in the adult group a.
Results: Fifty-four children and 252 adults met the selection criteria. ISS and the Glasgow coma scale (GCS) before intubation were
not statistically different in both groups. Seriously injured children stayed significantly shorter on the intensive care
unit, required significantly less ventilator days. Furthermore, the incidence of single organ failure (SOF) and multiple organ
failure (MOF) was significantly lower in the children group. Mortality in the children group (29.6%) was lower than that in
the adult group (33.7%). There was no death due to MOF in the children group as compared to 2.4% (n = 6) in the adults.
Conclusion: The incidence of SOF and MOF was significantly lower in the children group although there was no difference in ISS, GCS and
injury patterns. The prognosis of severely injured children was found to be better than those of adults. Moreover, there was

目的探讨谷氨酸(Glu)在大鼠脑损伤局部的异常释放以及其对乳酸(Lac)含量变化的影响。方法采用大鼠局部脑损伤动物模型,分为对照组、损伤组、干预组。伤前15min干预组注射Riluzole(一种Glu突触前释放抑制剂),损伤组注射等容量的生理盐水,对照组仅开骨窗不损伤脑。应用微透析技术检测各组伤后不同时间透析液中Glu含量([Glu]d)及Lac含量([Lac]d)变化。结果[Glu]d及[Lac]d在伤后15min、30min和45min干预组明显低于损伤组(P<0.05),而明显高于对照组(P<0.05);在伤后60min,损伤组仍明显高于对照组(P<0.05)。伤后不同时间[Glu]d和[Lac]d变化呈显著正相关(P<0.01)。结论脑损伤后受损脑组织细胞液中Glu水平的升高是Glu神经元末梢大量释放Glu所致,并继而引起了Lac的含量升高。 相似文献

目的初步探讨CT腹部模式常规和非常规扫描次数、高曝光剂量扫描对家兔急性脾组织病理变化的影响。方法将46只大白兔随机分成3组:(1)高曝光剂量CT扫描组36只。采用约为常规剂量2倍的腹部CT扫描模式,对大白兔进行全身扫描,再将其分两大亚组,每组18只。①腹部模式一次性扫描组(一次性扫描组):CT扫描层厚为2.5mm分别连续扫描3、6、9、12、15和18次,48h后剖杀;②腹部模式重复扫描组(重复扫描组):CT扫描层厚以5mm分别连续扫描2、4、6、8、10和12次,24h后重复上述扫描,24h后剖杀。(2)常规CT扫描组:5只,采用常规次数和曝光剂量对大白兔进行全身扫描,48h后剖杀。(3)健康对照组:5只。所有实验动物取脾组织作病理分析。结果光镜病理结果:①一次性扫描组:连续扫描6次时出现红髓髓窦略扩张、淤血;连续9-18次扫描后,脾脏出现红髓髓窦扩张、淤血,可见网状组织细胞增生和吞噬含铁血黄素。②重复扫描组:共8次扫描时出现红髓髓窦轻度扩张、淤血,散在中性粒细胞浸润;16-24次扫描后,脾脏出现红髓髓窦扩张、淤血,散在中性粒细胞浸润,间或有组织细胞增生。结论常规脾脏CT扫描是非常安全的;在非常规扫描次数和高曝光剂量下,CT辐射可能会引起家兔急性脾组织的异常病理变化。 相似文献

PURPOSE: To discuss injury research as a phenomenon of concern in nursing science. ORGANIZING CONSTRUCT: Injury is defined as the physical damage that results when the human body is briefly subjected to intolerable levels of energy. It is the leading cause of death in the first 4 decades of life in high-income nations and is second only to infectious diseases as a leading cause of death in low- and middle-income nations. METHODS: Review and discussion of relevant scientific and theoretical literature in both injury and nursing science. FINDINGS: Nurse scientists can apply unique perspectives to increase understanding of injury and its consequences. Fertile areas for nursing inquiry include identifying people at risk, developing models to explain the association between risk-taking and injury, testing interventions to prevent and limit injury, and creating and refining interventions that are culturally relevant to subpopulations most at risk for injury. CONCLUSIONS: The mandate to improve global heath should lead to nursing inquiry about this phenomenon, including developing and testing interventions to prevent and reduce injury. 相似文献

Objective To investigate the role of poly-lactic acid and agarose gelatin in promoting the functional recovery of the injured spinal cord. Methods Poly-lactic acid ( PLA) or agarose was embedded in the space between two stumps of the hemisectioned spinal cord. Immunohistochemistry was used to show astroglia proliferation and the infiltration of RhoA-positive cells. Locomotor activity recovery was evaluated by testing the function of hindlimbs. Results Astrogli-as and RhoA labeled non-neuronal cells accumulated in the area adjacent to the implant, while the number of RhoA-positive cells was decreased dramatically in the absence of implant. Animals implanted with agarose gelatin recovered more quickly than those with PLA, concomitant with a higher survival rate of the neurons. Conclusion Both PLA and agarose gelatin benefited the recovery of spinal cord after injury by providing a scaffold for astroglia processes. Modulation of the rigidity, pore size and inner structure of PLA and agarose gelatin might make these biodegradable materials more effective in the regeneration of the central nervous system (CNS). 相似文献

目的探讨腺苷与异丙酚预处理对犬心肌缺血再灌注损伤的保护作用。方法 21只杂种犬,雌雄不拘,随机分为3组(n=7):缺血再灌注组(A组)、异丙酚预处理组(B组)、异丙酚与腺苷联合预处理组(C组)。A组冠状动脉的左前降支结扎60min后松开结扎,再灌注:120min;B组缺血前30min经静脉以5.6mg·kg-1·h-1速率持续泵注异丙酚30min;C组缺血前10min经主动脉根部一次性注入腺苷(10mmol·L-1,10ml),其余处理同B组。记录心脏血液动力学指标,并行节段性室壁运动评分(RWMS)。结果缺血即刻出现了缺血性心电图的变化。与基础值比较,A组缺血期及再灌注期 LVEDP升高,CO、SV、LVEF、CPP、RPP降低,再灌注期MAP降低,HR减慢,B、C组上述缺血再灌注诱导的血液动力学变化减弱,A组缺血期及再灌注期RWMS增加,但B、C组缺血期RWMS低于A组(P< 0.05或0.01)。结论异丙酚预处理对犬心肌缺血再灌注损伤有一定的保护作用,腺苷预处理并未增强其保护作用。 相似文献
