Treatment of aortic pseudoaneurysm with interventional procedure

The prognosis of aortic pseudoaneurysm is bad because it usually threats to life by hemorrhea. Conventional surgical repair is usually associated with high morbidity and mortality, especially in patients with poor condition. Pseudoaneurysm formation after infected modified Blalock-Taussig shunt: echocardiographic findings

The modified Blalock-Taussig (B-T) shunt is well recognized as a palliative procedure for cyanotic congenital heart disease in infants. Pseudoaneurysm formation after a modified B-T shunt is a rare complication. We present the case of a 9-month-old girl with tetralogy of Fallot who had undergone a modified left B-T shunt at 5 months of age and developed a pseudoaneurysm after an episode of infective endocarditis as detected by echocardiography. Compression of the left bronchus, displacement of the mediastinum to right chest, and paralysis of the left hemidiaphragm were found. Magnetic resonance images, computed tomography scans, aortograms, and selective angiograms demonstrated the presence of a large pseudoaneurysm. The compression syndrome gradually disappeared after aneurysmectomy.     

彩色多普勒超声在股动脉假性动脉瘤诊治中的应用价值

目的探讨彩色多普勒超声对医源性股动脉假性动脉瘤的诊断价值及彩超引导下压迫修复的疗效。方法对22例经股动脉穿刺术后，穿刺部位出现博动性肿块和（或）听诊有血管杂音的患者行彩色多普勒超声检查，并在彩超引导下压迫假性动脉瘤与股动脉相通处，直至血流及频谱信号消失。治疗后经彩超复查评价其疗效。结果22例股动脉假性动脉瘤患者全部检出，与临床符合率100％（22／22）：彩超引导下2l例股动脉假性动脉瘤经压迫治愈、1例压迫后因未修复而行彩超引导下假腔内注射凝血酶后治愈。结论彩色多普勒超声对股动脉假性动脉瘤有极高的诊断价值：彩超引导下压迫修复医源性假性动脉瘤是一种简单、有效、安全的方法，可作为临床首选的治疗方式。     

Lower extremity arterial thrombosis following sonographically guided thrombin injection of a femoral pseudoaneurysm

Sonographically guided percutaneous thrombin injection is a minimally invasive and highly successful treatment of arterial pseudoaneurysms. Despite a very low complication rate, several severe arterial thrombotic events have been reported following thrombin injection of pseudoaneurysms. Native arterial thrombosis, though recognized as a severe complication of thrombin injection, has not been well described in the literature. We report a case of successful surgical management of arterial thrombosis after percutaneous thrombin injection of a femoral artery pseudoaneurysm in a 69-year-old woman. The pathophysiology, risk factors, diagnosis, and treatment options for this complication are discussed.     

Angio-Embolization of Renal Artery Pseudoaneurysm after Renal Biopsy: A Case Report

Renal artery pseudoaneurysm is a rare clinical entity that has been reported after renal biopsy, percutaneous renal surgery, penetrating trauma, and rarely blunt renal trauma. We present the case of a 37-year-old man with ruptured renal artery pseudoaneurysm accompanied by massive gross hematuria, urinary clot retention, and bladder tamponade, which were the presenting signs seven hours after renal biopsy. Abdominal CT scan showed a large perinephric, intracapsular hematoma of left kidney. His angiogram revealed a left renal segmental artery pseudoaneurysm that measured 1 cm × 1 cm. He was successfully treated by selective embolization of the arterial branch supplying the pseudoaneurysm.     

Digital Embolus Arising from a Pseudoaneurysm after Radial Artery Catheterization: a Case Report

Heterotopic ossification is defined as the formation of mature, lamellar bone within soft tissues other than the periosteum. We present a case of a 9-year-old girl with formation of heterotopic bone tissue in the elbow after surgical treatment of a fracture of the lateral humeral condyle. Eight months after the first operation the girl was re-operated to resect the heterotopic bone tissue. We believe that radical surgical excision of heterotopic bone, following physiotherapy around the elbow in children can improve the function of the extremity.

胰腺假性囊肿并脾动脉假性动脉瘤及结肠瘘致消化道出血一例

1 临床资料 患者男,64岁.因便秘及腹泻2d,无明显诱因突发性排新鲜血便1次,总量约1000 ml,伴头晕、乏力,于2011年6月29日急诊入院.患者3年前诊断为急性胰腺炎行保守治疗后痊愈.体格检查:心率90次/rin,血压100/70 mm Hg(1 mm Hg =0.133 kPa),神志清楚,贫血貌.腹平软,无压痛,肝脾肋下未触及,腹部未触及包块,移动性浊音阴性.肛门指诊未见异常.血常规:RBC 2.71×1012/L、Hb 84 g/L.给予药物止血、输血、补液等对症处理.入院12h后预行结肠镜检查而给予清洁灌肠,期间患者间断排新鲜血便6次,并有血凝块,总量约1700 ml,遂给予输注同型红细胞悬液6U.     

Hemosuccus Pancreaticus Due to the Rupture of a Pseudoaneurysm That Developed in an Intraductal Papillary Mucinous Neoplasm

A 76-year-old woman with branch duct intraductal papillary mucinous neoplasm (IPMN) was admitted with epigastric pain and vomiting. She had received warfarin due to a history of deep vein thrombosis. A blood test showed decreased serum hemoglobin and elevated serum amylase. Contrast-enhanced computed tomography revealed acute pancreatitis and formation of a pseudoaneurysm in the IPMN. We suspected rupture of a pseudoaneurysm and performed trans-catheter angiography. Angiography showed extravasation from the posterior superior pancreaticoduodenal artery, and coil embolization was performed. It is important to be alert for the formation of pseudoaneurysm in patients with cystic neoplasms.
