An epidemiological study on patients undergoing dacryocystorhinostomy

Dacryocystorhinostomy is performed in patients with naso-lacrimal duct obstruction to bypass the site of obstruction so as to relieve the patient of the irritating and socially embarrassing symptoms of epiphora. We discuss the various epidemiological aspects especially the vast difference by which the females out-number the males in patients undergoing DCR and the likely explanations for this difference in our study on 74 patients which underwent DCR in our institute.     

Evaluation of Audio-Rhinological Changes During Pregnancy

A number of physiological changes occur during pregnancy and amongst them, audiological and nasal changes are quite significant. These are mainly due to the changing levels of sex hormones and return to normal once the pregnancy is over. This study was conducted to document these changes. Forty (pregnant 40 and non-pregnant 40) consenting subjects in age group of 20–35 years were assigned to test and control groups. They underwent complete ENT and Obstetric examination. In test group Pure Tone Audiometry was performed in all trimesters of pregnancy and within 3 months of delivery. The subjects in the control group underwent pure tone audiometry only once. The nasal patency was measured by Gertner’s plate method. Results from each trimester and postpartum period were compared. A highly significant difference in pure tone thresholds was observed at frequencies ranging from 125 to 1000 Hz (P < 0.001). However frequencies higher than 1000 Hz demonstrated no significant correlation. Nasal patency as measured by mean area of vapour condensation in all trimesters and control groups was highly significant (P < 0.001). The results of this study confirm that these changes occur in the first trimester and gradually improve during the subsequent trimesters returning to normal in post partum period. However number of pregnancies bear no relationship with these changes

Multispiral computed tomographic angiography of renal arteries of live potential renal donors: a review of 118 cases

OBJECTIVES: Review of the angiographic data of 118 live kidney donors was performed to assess the renal vessel anatomy; compare the findings with the perioperative findings using multislice spiral (MS) computed tomographic angiography (CTA) with the use of 50 mL of intravenous contrast; determine the sensitivity of this technique in the workup of live potential renal donors; and finally to discuss and compare the results of the present study with the reported results using single-slice spiral (SS) CTA, magnetic resonance angiography (MRA), and conventional angiography (CA). METHODS: Retrospective analysis of the angiographic data of 118 of prospective live kidney donors was performed. All donors underwent renal angiography on MSCTA scanning using 50 mL of intravenous contrast with 1.25-mm slice thickness followed by maximum intensity projection and virtual rendering techniques postprocessing algorithms. Analysis was made on imaging and intraoperatively for the number of renal arteries as well as their bifurcation pattern, location, vessel caliber, length, and venous anatomy, and these were then compared with each other. RESULTS: MSCTA showed clear delineation of the main renal arteries in all the donors with detailed vessel morphology. The study also revealed a 100% sensitivity in the detection of accessory renal vessels, which had an overall incidence of 26.67%, with the most common distribution in the perihilar region. CONCLUSIONS: The present study showed a 100% sensitivity and specificity in the visualization and detection of main and accessory renal vessels with the use of only 50 mL of intravenous contrast with similar results seen with CA which has so far been considered the "gold standard." The results on MSCTA were also better than those with the use of SSCTA and MRA in the workup of liver renal donors, with the above technique also proving to be more cost effective. The overall incidence of supernumerary vessels was the same as reported in the literature; however, a higher incidence of single aberrant vessels was seen on the right side, contrary to what has been suggested so far and was attributable to improved detection of accessory vessels less then 2 mm in diameter.     

S M R of inferior turbinate in chronic hypertrophic rhinitis

A study was undertaken to evaluate the role of unilatcral/bilateral submucous resection of the inferior turbinates in fifty cases of chronic hypertrophic rhinitis. Patients associated with deflected nasal septum or sinus infectious were excluded from the study. Decongesiton of turbinates was done to exclude the cases with predominantly mucosal hypertrophy. Gertner (1984) plate method was used to asses the nasal patency preoperatively and then post-operatively at 1, 3 and 6 months follow up. The analysis of observations made revealed SMR of inferior turbinate to be a very effective modality tor relieving nasal obstruction due to bony turbinal hypertrophy. The procedure has least interference with nasal mucosal integrity and functions and complications associated with the procedure have been found to be minimal. Histopathological examination of mucosa and of ostenid tissue revealed infiltration by chronic inflammatory cells suggesting chronic nonspecific inflammation either due to non-specific infections and / or allergy).     

Transciliary filtration using the fugo blade<Superscript>TM</Superscript>

Transciliary filtration is an approach to manage glaucoma that departs from classic filtration surgery. This approach uses the tissue ablation and noncauterizing, hemostatic capabilities of the Fugo Blade^TM (Plasma Blade). A nonbleeding micropore is created, which drains aqueous from behind the iris and into subconjunctival lymphatics. No peripheral iridectomy is needed. Mitomycin and excess cautery are contraindicated. Surgical time for this procedure is approximately 5 minutes. The authors have stated that they do not have a significant financial interest or other relationship with any product manufacturer or provider of services discussed in this article.     

Primary Chondrosarcoma of Male Breast: A Rare Case

Sarcomas of the breast are relatively rare and account for 1% of all primary malignant tumors of the breast. Pure and primary chondrosarcoma of the male breast is an extremely rare tumor. It may arise either from the breast stroma itself or from underlying bone or cartilage. Differential diagnoses include cystosarcoma phyllodes and breast metaplastic carcinoma with chondroid differentiation.     

Tuberous sclerosis--a multi system disease

Tuberous sclerosis is a genetic disease with autosomal dominant inheritance, associated with hamartomata in several organs and various skin findings. A case of a ten year old boy is presented here to highlight the multisystem involvement in tuberous sclerosis. The child had seizures, facial papular naevi and periungual fibromas. MRI revealed cortical tubers, white matter lesions and subependymal nodules. Orbital ultrasound showed retinal hamartoma on the left side. Ultrasound of the abdomen revealed a soft tissue mass at the upper pole of left kidney with a small cyst in right kidney.
