目的 研究职业X射线暴露对放射工作人员非癌疾病的影响。方法 选取1980年普查四川省重庆市辖区内医用X射线工作者为调查对象。2020年,选取之前的91家医疗机构,分布在重庆市41个区县,其中三级医疗机构53家,运用前瞻性队列研究的方法,第5次随访1980年重庆市医用X射线放射工作人员(放射组)和同医院同时期非放射科工作人员(对照组)的个人信息、职业射线接触情况、疾病史等信息。分析该队列各系统疾病发病率、各系统疾病相对危险度(RR)及其95%置信区间。结果 该队列共计937人,共统计非癌疾病359例。放射组与对照组相比,其心血管系统、眼科和其他未分类疾病的相对危险度(RR)分别为1.50、2.03和2.64(χ2=5.97、3.97、4.25,P <0.05)。调整混杂因素的结果显示,女性放射组中心血管系统、眼科和消化系统等疾病发病率的RR普遍高于对照组(RR=2.33、2.59、7.55;χ2=7.28、4.17、8.64,P<0.05);在25~29岁参加工作人员中,放射组成员心血管系统、眼科等疾病发病率的RR普遍高于对照组(RR=2.26、5.07;χ2=8.22、4.91,P<0.05);累积剂量分组中,放射组与对照组比较,心血管系统、眼科和消化系统等疾病发病率的RR普遍较高(RR=1.86、2.91、3.59;χ2=9.83、8.21、5.58,P<0.05)。结论 受到长期职业照射的X射线工作者非癌发病危险增高,其中心血管系统、消化系统、眼科疾病等发病危险可能与这种职业照射有关。  相似文献

Determinants of tuberculosis (TB) syringomyelia, its management options and outcomes are still under investigation. The aim of this study is to present a case of TB syringomyelia with markedly improved symptoms status-post surgery and to understand the clinical characteristics and outcomes of 33 TB syringomyelia cases reported in the literature. Specifically, we examined the differences between patients who were managed medically and those who underwent surgical intervention. Inclusion criteria for the cases were (1) syringomyelia caused by TB infection rather than co-occurrence of these conditions, (2) management protocol described, and (3) post-treatment outcome described. The median age was 30 years (interquartile range (IQR): 23–40) with 55% males. The median time between TB onset to syringomyelia diagnosis was 2 years. Nineteen patients were surgically treated, 11 were medically treated, and 3 received no treatment. Twenty-one patients showed improvement in at least one prior symptom, but no patient experienced a full recovery. Those that underwent surgical intervention were more likely to have TB meningitis (95% vs. 64%, p < 0.05) upon initial TB presentation and have a greater interval between TB onset and syringomyelia presentation (median of 2.6 vs. 0.33 years, ns). A greater proportion of the surgically managed patients experienced improvement in any symptom (74% vs. 45%, ns). Future case-controlled studies with larger sample sizes are required to validate and further understand the outcomes of surgically-managed TB syringomyelia.  相似文献

Objectives

Biological monitoring of organophosphorus insecticide (OP) metabolites, specifically dialkylphosphates (DAP) in urine, plays a key role in low-level exposure assessment of OP in individuals. The aims of this study are to develop a simple and sensitive method for determining four urinary DAPs using high-performance liquid chromatography with tandem mass spectrometry (LC–MS/MS), and to assess the concentration range of urinary DAP in Japanese children.

Methods

Deuterium-labeled DAPs were used as internal standards. Urinary dimethylphosphate (DMP) and diethylphosphate (DEP), which passed through the solid-phase extraction (SPE) column, and dimethylthiophosphate (DMTP) and diethylthiophosphate (DETP), which were extracted from a SPE column using 2.5 % NH3 water including 50 % acetonitrile, were prepared for separation analysis. The samples were then injected into LC–MS/MS. The optimized method was applied to spot urine samples from 3-year-old children (109 males and 116 females) living in Aichi Prefecture in Japan.

Results

Results from the validation study demonstrated good within- and between-run precisions (<10.7 %) with low detection limits (0.4 for DMP and DMTP, 0.2 for DEP and 0.1 μg/L for DETP). The geometric mean values and detection rates of the urinary DAPs in Japanese children were 14.4 μg/L and 100 % for DMP, 5.3 μg/L and 98 % for DMTP, 5.5 μg/L and 99 % for DEP, and 0.6 μg/L and 80 % for DETP, respectively.

Conclusions

The present high-throughput method is simple and reliable, and can thereby further contribute to development of an exposure assessment of OP. The present study is the first to reveal the DAP concentrations in young Japanese children.  相似文献

A number of studies have shown that mental challenge under controlled experimental conditions is associated with elevations in inflammatory markers such as interleukin-6 (IL-6) and C-reactive protein (CRP). However, relatively little work has been done on the effects of ‘naturalistic’ stressors on acute changes in inflammatory markers. The present study examined whether perceived arousal, valence and dominance in musicians are associated with pro-inflammatory and oxidative responses to a concert situation. Blood and salivary samples obtained from 48 members of a symphony orchestra on the day of rehearsal (i.e. control situation) and on the following day of premiere concert (i.e. test situation) were used to determine changes in salivary cortisol, pro-inflammatory markers (plasma myeloperoxidase, serum CRP, plasma IL-6), oxidative stress markers (paraoxonase1 activity and malondialdehyde), and homocysteine, a risk factor for vascular disease. Results of regression analyses showed a significant trend to increased myeloperoxidase (MPO) response in individuals with low valence score. Both affective states, valence and arousal, were identified as significant predictors of cortisol response during concert. In addition, control levels of plasma malondialdehyde were positively correlated with differences in IL-6 levels between premiere and rehearsal (r = .38, p = .012), pointing to higher oxidative stress in individuals with pronounced IL-6 response. Our results indicate that stress of public performance leads to increased concentrations of plasma MPO (20%), IL-6 (27%) and salivary cortisol (44%) in musicians. The decreasing effect of pleasantness on the MPO response was highly pronounced in non-smokers (r = −.60, p < .001), suggesting a significant role of emotional valence in stress-induced secretion of MPO. Additional studies are needed to assess the generalizability of these findings to other ’naturalistic’ stress situations.  相似文献

While high intensity exercise is associated with improvement of both motor and non-motor symptoms in Parkinson’s disease (PD), there is limited evidence on its impact on sleep disturbances in PD. This scoping review aims to provide a preliminary statement of the potential size and scope of available evidence for the interaction between exercise and sleep in people with PD. Research to date on non-motor symptoms of PD is broad, with scarce information regarding specific effects of exercise on sleep. A systematic literature search was conducted through three phases. Fifteen articles met the inclusion criteria for this study. Resistive exercise and multimodal exercise programs were frequently studied and found to improve sleep and decrease sleep-related disorders. These findings suggest that exercise programs may improve sleep and other non-motor symptoms of PD. The research was inconclusive when comparing the effects of high and low intensity exercises. Further research on the interaction between exercise and sleep in PD may have implications for rehabilitative therapy interventions.  相似文献

ObjectiveThe aim of this research, which represents an additional and longer follow-up to a previous trial, was to evaluate a 5-year follow-up study of a combined treatment (pharmacological + psychoeducational and cognitive-behavioral therapy) as compared with a standard pharmacological treatment in patients with refractory bipolar disorder.MethodForty patients were randomly assigned to either an Experimental group–under combined treatment — or a Control group — under pharmacological treatment. Data were analyzed by analysis of variance (ANOVA), with repeated measures at different evaluation time points.ResultsBetween-group differences were significant at all evaluation time points after treatment. Experimental group had less hospitalization events than Control group in the 12-month evaluation (P = 0.015). The Experimental group showed lower depression and anxiety in the 6-month (P = 0.006; P = 0.019), 12-month (P = 0.001; P < 0.001) and 5-year (P < 0.001, P < 0.001) evaluation time points. Significant differences emerged in mania and misadjustment already in the post-treatment evaluation (P = 0.009; P < 0.001) and were sustained throughout the study (6-month: P = 0.006, P < 0.001; 12-month: P < 0.001, P < 0.001; 5-year: P = 0.004, P < 0.001). After 5-year follow-up, 88.9% of patients in the Control group and 20% of patients in the Experimental group showed persistent affective symptoms and/or difficulties in social-occupational functioning.ConclusionsA combined therapy is long-term effective for patients with refractory bipolar disorder.
