Polymorphisms in gene encoding TRPV1-receptor involved in pain perception are unrelated to chronic pancreatitis

Background  

The major clinical feature in chronic pancreatitis is pain, but the genetic basis of pancreatic pain in chronic pancreatitis is poorly understood. The transient receptor potential vanilloid receptor 1 (TRPV1) gene has been associated with pain perception, and genetic variations in TRPV1 may modify the presence and phenotype of chronic pancreatitis. The aim of our study was to investigate the genetic variation of TRPV1 in Dutch patients with chronic pancreatitis and healthy controls.

Lidocaine plasters for postherpetic neuralgia?

Each year in the UK, about 1 in 2,500 people experiences neuropathic pain that is still present 3-6 months after acute herpes zoster (shingles). This condition, known as postherpetic neuralgia, is the most common complication of herpes zoster and can be chronic, intractable and distressing. Treatments used in an attempt to reduce postherpetic neuralgia include tricyclic antidepressants (e.g. amitriptyline--an unlicensed indication), antiepileptics (e.g. gabapentin) and opioid analgesics, as well as topical treatments such as capsaicin. However, such treatments may only provide partial pain relief, and tolerability can be a problem, particularly in older patients. Versatis (Grunenthal Ltd), a topical preparation of lidocaine formulated in a plaster, has recently been licensed for treating patients with postherpetic neuralgia. Does it offer useful benefit?     

What role for tigecycline in infections?

Tigecycline (pronounced tie-ge-sigh-cleen; Tygacil--Wyeth) is a broad-spectrum antibacterial and the first glycylcycline to be marketed in the UK. It is active against certain resistant bacteria, including meticillin-resistant Staphylococcus aureus (MRSA) and bacteria that produce extended-spectrum beta-lactamase (ESBL). Tigecycline is licensed for intravenous treatment of adults with complicated skin and soft tissue infections, and complicated intra-abdominal infections. We review tigecycline and assess its place for these infections.

一项多中心随机对照研究发现，对几组危重患儿进行严格的血糖控制虽然会增加发生低血糖的危险，但是可以减少他们的住院时间。危重患者出现应激性高血糖是很常见的并发症，并且与预后不良有关。成年危重患者进行严格血糖控制是否有益的争论一直存在，而在儿童方面的研究数据很少。目前被接受的处理方法是对于危重患者不要试着去控制血糖浓度。     

德国新指南推动器官捐献

德国卫生部向医院颁布新的指南。以增加捐献器官的数量。2008年这个数字下降了9％。指南指出：所有具有重症监护病房的医院需配有移植协调员，其将尝试确认可能的器官捐献者并且与国家器官获取机构合作。     

医生呼吁对儿童医疗服务进行“根本性”的一揽子改革

英国皇家儿科医学院（RCPCH）在一份报告中警示：为了保护和促进英国儿童及青少年的健康，儿童医院的服务亟待从根本上重新整改。     

四分之一患者在某些欧洲医院内被感染
