Atypical tongue-tie due to congenital tongue-palate fusion

We are presenting a case report of a male neonate brought in an emergency room with feeding inability and signs of respiratory difficulty due to oral passage blockade - a developmental anomaly caused by tongue-palate fusion. Surgical correction instantly relieved respiratory distress and established the normal oral feeding mechanism.     

Mesenteric involvement in neurofibromatosis type 1: CT and MRI findings in two cases

We report computerized tomographic and magnetic resonance imaging findings of neurofibromatosis type 1 with mesenteric involvement in two patients. The first patient was a 13-year-old female with a panmesenteric plexiform neurofibroma with segmental involvement of the bowel wall; she had a 3-year history of abdominal pain. The second patient was a 60-year-old female who presented with malignant transformation of multiple mesenteric neurofibromas 4 months after primary operation.     

Effect of monthly intravenous cyclophosphamide in rapidly deteriorating multiple sclerosis patients resistant to conventional therapy

Fourteen consecutive clinically definite relapsing-remitting multiple sclerosis (MS) patients were treated with monthly intravenous cyclophosphomide (CTX) for 6 months. All had experienced severe dinical deterioration during the 12 months prior to treatment with CTX despite treatment with conventional immunomodulating agents and intravenous methylprednisolone. Treatment with CTX led to improvement and neurologic stability within 6 months which was sustained for at least 18 months after the onset of treatment with CTX. Therapy with CTX was well tolerated. CTX may be of benefit in MS patients who experience rapid clinical worsening and are resistant to conventional therapy.     

Treatment of a giant orbitofacial vascular malformation and ophthalmic artery aneurysms with intralesion glue injections: case report

We describe a giant left orbital vascular malformation that was treated with both percutaneous and transarterial embolization. Feeder artery aneurysms thrombosed as a result of retrograde reflux of embolic material into the distal ophthalmic artery. In this presentation, we emphasize the efficacy of percutaneous embolization and retrograde thromboses of two intradural saccular ophthalmic artery aneurysms.     

Clonality of acquired primary pure red cell aplasia: effectiveness of antithymocyte globulin

Primary pure red cell aplasia (PRCA) was diagnosed in two male patients, 65 and 69 years old respectively. In both, surface markers of peripheral blood nuclear cells revealed the presence of TCR alphabeta+ phenotype. Clonality of T cells was confirmed by the polymerase chain reaction in both patients, in whom, prednisone at a dose of 1 mg/kg/day improved the anemia and lower doses caused its renewal, resulting in the reappearance of the patient's transfusion requirement. On the other hand, the anemia seems to have been treated permanently (second case) with horse antithymocyte globulin (ATG) (20 mg/kg/day 1 to 8 +) since his hemoglobin was about 15 g/dl at the time of writing. In the first patient, the hemoglobin level was 10.5 g/dl one month after the administration of ATG (15 mg/kg/d 1 to 5 +), but unfortunately, the patient died because of a massive gastrointestinal bleeding on the fortieth day following this treatment. We, therefore, suggest that, patients with acquired primary PRCA should be screened to detect the presence of a T-cell clone and recommend that, treatment should start earlier with ATG, if the PRCA is due to a T-cell clonal disorder.     

MALIGNANT ECCRINE SPIRADENOMA ON THE LATERAL MARGIN OF NOSE AS AN INFREQUENT LOCALIZATION

Malignant eccrine spiradenomas are exceedingly rare tumors. They can arise from a preexisting eccrine spiradenoma or occur as a primary malignant tumor. Clinical features of these tumors may include a history of enlargement in a previously stable lesion. Tumor can be of low or high grade. Low-grade malignant eccrine spiradenoma has a better prognosis. We present a malignant eccrine spiradenoma arising from a preexisting eccrine spiradenoma, which has an infrequent localization between lateral edge of nose and medial canthus.     

Effects of exposure to cotton dust on energy expenditure in the textile industry

Summary An assessment of energy expenditure has been made in 50 male textile workers and 30 male office cleaners. There was a statistically significant difference (P < 0.001) in energy expenditure between textile workers and office cleaners amounting to 12 to 16%. While 33.3% of the textile group complained of chest disorders, none of the cleaners had such a complaint. FEV₁ and FVC values of the textile workers were significantly lower (P < 0.01) compared to those of the controls. There were no differences in body weight, lean body mass and socio-economic conditions of the two groups, however, the textile workers were younger and taller than the unexposed group. Exposure to cotton dust and respiratory and ventilatory impairments are suspected of causing a reduction in the energy expenditure of the textile workers
