The modified outside-in suture: vertical repair of the anterior horn of the meniscus after decompression of a large meniscal cyst

This article describes a modified suture technique designed for the vertical repair of the anterior horn of the meniscus after arthroscopic decompression of a large meniscal cyst. This procedure comprises of three steps: first, the meniscus was pierced vertically using a suture hook and a No. 0 PDS suture. Second, both ends of the No. 0 PDS on the femoral and tibial surfaces of the meniscus were pulled to the outside of the joint capsule using a spinal needle preloaded with suture material. Finally, a skin incision was made adjacent to the suture materials, and both ends were tied. We recommend this technique not only for the vertical repair of the anterior horn of the meniscus after decompression of large meniscal cyst, but also to repair a longitudinal tear of the meniscus.     

耳垂复合组织游离移植修复鼻小柱过短畸形

目的 探讨因鼻小柱过短所致鼻尖低平畸形的矫治方法.方法 对8例鼻尖低平伴鼻小柱过短者,采用鼻小柱切口,首先应用肋软骨或Medpor行支撑杆移植矫正撑起鼻尖,鼻小柱切口的继发创面采用耳垂复合组织游离移植修复.结果 8例游离移植的耳垂复合组织均成活良好,随访1～2年形态满意,色泽与周围组织无明显差别,供区无明显继发畸形.结论 应用耳垂复合组织游离移植修复鼻小柱过短畸形,不仅适用于外伤性缺损修复,而且也适用于鼻部的美容性修复,是一种患者容易接受、操作简单安全、疗效好、供区无后遗明显继发畸形的修复方法.     

改良直线缝合法修复双侧唇裂的体会

目的介绍改良直线缝合法修复双侧唇裂的方法和体会。方法在VeauⅢ术式的基础上,于双侧唇裂的两侧唇部设计A、B皮肤黏膜瓣,衬垫于前唇部的内侧,以增加创面的接触面积并加厚前唇部;在前唇部上方内侧设计C黏膜瓣加深唇龈沟,并将两侧唇肌肉分离固定于鼻前嵴、鼻翼基底及前唇,以达到唇肌的功能性修复。结果本组12例患者,术后上唇及唇红较丰满,动态畸形不明显,效果满意。结论采用改良直线缝合法修复双侧唇裂,能增加切口抗张力和改善术后上唇形态,减轻术后动态畸形及上唇塌陷。同时,此法还能保留较多的上唇组织,有利于唇裂Ⅱ期畸形的修复。     

面斜裂整形修复中泪道重建术的探讨

为了解决面斜裂整形修复中泪道重建的难题，１９８９年３月～１９９５年１月，对４例患者治疗中设计了三种手术方法：泪小管鼻腔吻合术；静脉血管或口腔粘膜移植重建泪道术；改良的泪囊鼻腔吻合术。术后重建的泪道置入导管３～６个月。经术后６个月～１年的观察，面部畸形整复，溢泪消失，红汞试验及泪道检查均通畅，效果满意。认为，应根据泪道畸形程度的不同，选择合适的手术方法，以取得最佳疗效。     

Arthroscopic Latissimus Dorsi Transfer

The patient is placed in lateral decubitus. A 6-cm incision made in the axilla allows access to the latissimus dorsi tendon and its neurovascular pedicle. Holding the arm in internal rotation, the surgeon detaches sharply the tendon off the humeral shaft and then reinforces it with wrapping sutures. Pulling the free limbs of the sutures exposes the under surface of the muscle and helps to identify the neurovascular pedicle. Special lighting retractors suited for a large diameter scope are helpful. Mobilization is completed when 2 cm of the tendon crosses the posterior edge of the acromion. The standard lateral portal is used for visualization. A silicon drain tube stiffened by a Wissinger rod is advanced from the posterior portal under direct visualization in the space between teres minor and deltoid, exiting in the auxiliary incision. A suture loop passed down the tube retrieves the tendon sutures out the posterior portal. These are then moved out the anterior portal, thus pulling the tendon over the tuberosity. The first anchor is inserted at the anterior aspect of the greater tuberosity, close to the articular cartilage and long head of the biceps tendon. Two to 3 anchors are inserted fixing the tendon to the tuberosity until it is stable.     

自体腹白线片修补十二指肠溃疡穿孔(附13例报告)

目的探讨用游离自体腹白线片修补急性十二指肠溃疡穿孔的应用价值。方法从2006年1月至2006年7月对13例用自体腹白线片修补急性十二指肠溃疡穿孔的病人的临床资料和随访情况进行回顾性分析,其中2例穿孔大于2cm2,平均手术时间60分钟,平均失血量20ml,平均住院天数9±1天。结果游离自体腹白线片修补急性十二指肠溃疡穿孔13例均痊愈出院。随访15天至6个月,无手术并发症。结论本方法操作较简单、安全、效果好,其适应症广,是一种可行的新方法。     

Ubiquitous Presence in Mammalian Cells of Enzymatic Activity Specifically Cleaving 8-Hydroxyguanine-containing DNA

Here we report the finding of enzymatic activity that specifically cleaves DNA containing 8-hydroxyguanine (oh⁸Gua) residues in various mammalian cells. To detect this activity, we used a synthetic double-stranded DNA containing a single oh⁸Gua at a defined position as the substrate, and analyzed the products of enzymatic digestion by polyacrylamide gel electrophoresis. Two cleavage sites near the oh⁸Gua residue were detected with partially purified fractions from cow brain and rat liver, and also with preparations from all mammalian tissues examined. These results suggest that enzymatic activity for the removal of oh⁸Gua from DNA is widely distributed in mammalian cells.     

腕部广泛损伤的修复与康复治疗

报道30例腕部损伤患者,伤情比较相同。其中12例采用传统外科手术治疗,不作或仅作不正规的理疗;18例采用显微外科技术治疗,仔细修复各种组织结构,术后系统地进行康复治疗。经随访3个月～2年,传统方法治疗组优良率仅为8.3％;采用显微外科技术及康复治疗组优良率达67％。
