We report the first case of echocardiographically detected Lambl's excrescences on the pulmonary valve in a 72-year-old man who was referred for transesophageal echocardiography as a part of an evaluation for ischemic stroke. A total of four excrescences were noted on the arterial aspect of the pulmonary valve; two of them were on the anterior cusp, one was on the left cusp, and one was on the right cusp. The excrescence on the left cusp was the largest, measuring 5 mm in length. These valvular strands (Lambl's excrescences) represented an incidental finding and were not associated with any disease process. 相似文献

Self-stimulation (SS) rewarding experience induced alterations in the density of excrescences in the apical dendrites of CA3 neurons were studied in adult male Wistar rats. SS experience was provided daily for an hour over a period of 10 days, through bipolar stainless steel electrodes implanted bilaterally in lateral hypothalamus and substantia nigra-ventral tegmental area. The results revealed a significant (P<0.001) increase in the number of excrescences in both main shaft and sub branches of the apical dendrites in SS experienced group compared to control groups of rats. The increased number of excrescences in CA3 neurons might be due to an enhancement in the synaptic transmission in the mossy fiber pathway following SS experience. 相似文献

A 54-year-old female with rheumatic heart disease was found to have a mass in the left ventricle by echocardiography and angiography. Subsequently, giant Lambl's excrescences of papillary muscle and aortic valve were confirmed by operation and pathologic examination. Possible complication from cardiac catheterization could be avoided by the utilization of echocardiography prior to catheterization. 相似文献

In this retrospective study, we identified 7 cases where Lambl's excrescences were identified by two‐dimensional transesophageal echocardiography (2DTEE) and also had live/real time three‐dimensional transesophageal echocardiography (3DTEE) studies available for comparison. We subsequently assessed them for the presence of Lambl's excrescences (LE) and nodules of Arantius (NA) on the aortic valve. After their identification, we qualitatively and quantitatively organized our findings by number, cusp location, measurements, and orientation if applicable. A greater number of LE was found by 3DTEE than 2DTEE (19 vs. 11, respectively). In all 3DTEE studies, their cusp attachment site, their x‐, y‐, and z‐axis measurements, and orientation were clearly visualized and described. Only 3DTEE studies provided confident visualization of the cusp attachment sites. Similarly, a greater number of NA was found by 3DTEE than 2DTEE (21 vs. 5, respectively). The triad of NA was visualized in all 3DTEE studies and each was described using its x‐, y‐, and z‐ axis measurements. Only three 2DTEE studies provided reliable identification of the NA. In conclusion, we present further evidence of the incremental value of 3DTEE over 2DTEE in the qualitative and quantitative assessment of cardiac structures including LE and NA on the aortic valve. 相似文献

Acute aortic dissection is a life-threatening condition mainly caused by hypertension, atherosclerotic disease and other degenerative diseases of the connective tissue of the aortic wall. Mesothelial/monocytic incidental cardiac excrescences (cardiac MICE) is a rare benign reactive tumor-like lesion composed of admixture of histiocytes, mesothelial cells, and inflammatory cells set within a fibrinous meshwork without a vascular network or supporting stroma. Cardiac MICE occurring in association with aortic dissection is exceptionally rare (only one such case reported to date). We herein report on the surgical repair of two Stanford type A aortic dissections caused by idiopathic giant cell aortitis in a 66-year-old-woman and by atherosclerotic disease in a 58-year-old-man, respectively. In both cases, the dissections could be visualized via computed tomography. Histopathology showed cardiac incidental MICE within the external aortic wall near the pericardial surface which was confirmed by immunohistochemistry. 相似文献

Lambl's excrescences are thin, mobile, filiform structures, often referred to as valvular strands in the echocardiography literature. These valvular strands may occur as a single strand, in rows, or clusters. The differential diagnosis for these excrescences includes fibroelastoma, myxoma, thrombi, vegetations, and cardiac neoplasms and metastases. Most difficult is the distinction between Lambl's excrescences and fibroelastomas. Studies regarding the clinical importance of these valvular strands are contradictory. Surgical intervention versus conservative observation should be determined on a case-by-case basis. The transesophageal views of Lambl's excrescences in this case report are meant to serve as a classic example of this commonly misinterpreted lesion. 相似文献

We present three cases of so-called mesothelial/monocytic incidental cardiac excrescences (MICE) of the heart and a brief
review of related literature. Case 1 was a 51-year-old woman who underwent mitral- and aortic-valve replacement. A tissue
sample was submitted as a thrombus attached to the left atrial endocardium. Case 2 was a 69-year-old woman who underwent mitral-valve
replacement. The sample was incidentally obtained as whitish clot-like fragments, but its exact origin was not known. Case
3 was a 68-year-old woman who underwent mitral-valve replacement for suspected infective endocarditis. The sample adherent
to the pericardium was removed after valvular surgery. Histologically, these lesions were composed of a mixture of plump histiocytoid
cells, a papillary arrangement of cuboidal cells, various sized vacuoles, and fibrin. The nests of cuboidal cells resembled
cancer cells but showed features of mesothelial cells and no proliferative activity, immunohistochemically or ultrastructurally.
In all cases, a suction tube placed in the left atrium was occasionally used to remove overflowing intrapericardial fluid
during the surgery. The tip of the suction tube was covered with spiral wire, which is likely to transfer the stripped pericardial
mesothelial cells to the left atrium. The significance of MICE is their possibility of being misdiagnosed as metastatic carcinoma
by pathologists and a risk of arterial embolization by mesothelial debris clinically.
