From April 1989 to December 2004, we performed liver biopsy on 475 patients and obtained biopsy proven 35 cases of non-alcoholic fatty liver. Among them, 18 cases were diagnosed as non-alcoholic steatohepatitis (NASH). During the last three years, we have tried to detect NASH using ultrasonography and elevated value of serum ferritin (> 300 ng/ml). All of the eligible 7 cases biopsied during the course were diagnosed as NASH. In these 7 cases, ALT levels improved after the body weight loss accompanied by the parallel decrease of serum ferritin levels. Measurement of serum ferritin is useful in the detection of NASH but the normal value of ferritin cannot rule out the possibility of NASH. 相似文献

In order to estimate the diagnostic validity of chemical fecal occult blood tests, i.e. orthotolidine (Shionogi A) and guajac (Shionogi B) slides for detecting cancers of the esophagus, stomach and colorectum, the authors followed up all the examinees (n=3,449) of comprehensive medical check-ups at the Center for Adult Diseases, Osaka, by means of record linkage to the Osaka Cancer Registry's files. Then, diagnostic validity was calculated based on the results of two years' follow-up. Sensitivity for the respective cancers was 20.0%, 11.8% and 62.5% for Shionogi A, and 20.0%, 5.9% and 43.8% for Shionogi B slides. Likelihood ratio for the respective cancers was 1.4, 0.8 and 4.5 for Shionogi A, and 3.3, 1.0 and 7.5 for Shionogi B. Specificity was analogous among the three cancer sites, being 86% for Shionogi A and 94% for Shionogi B. These results suggest that the diagnostic validity of chemical occult blood tests for detecting cancers of the esophagus and the stomach is very poor, and therefore imply that close examinations of these sites for screening positives is unnecessary in mass screenings for colorectal cancer. 相似文献

The results of repairing cleft lip by aesthetic plastic surgery are now excellent. However, the cleft lipnose deformity is still very difficult to repair with the present techniques. A technique that can repair the cleft lip-nose deformity with good results is presented. The technique is divided into three parts: Part I consists of nasal repair of the primary cleft lip. Part II is nasal reconstruction as a secondary operation with or without lip repair. For example, nasal reconstruction may be secondary to repair of deformities of the sill, rim, limen nasi, septum, or nasal bones. Decreased response of beta-adrenergic receptor has been considered to he one of the causes of increased responsiveness of the bronchi in asthma. Since beta-adrenergic receptor has two subtypes, beta1 and beta2 , and the bronchodilating effect of beta stimulants is mediated by beta2 -receptor, responsiveness of the bronchi is expected to correlate to the cyclic AMP response of lymphocytes to a beta2 -stimulant. Responsiveness of the bronchi was expressed as respiratory threshold to acetylcholine (RT-Ach), which was the minimal concentration of acetylcholine solution to cause an initial decrease of FEV1 of more than 20% of the baseline value. Beta1 and heta2 -responses were expressed as the increments of cyclic AMP content of 106 lymphocytes incubated with norepinephrine (beta1 -stimulant) and salbutamol (beta2 -stimulant).
RT-Ach showed a significant correlation with the beta2 -cyclic AMP response of lymphocytes, but not with the beta1 -response among patients with asthma. Sixteen symptomatic patients on continuous beta-stimulants showed lower RT-Ach value and diminished beta2 -receptor activity of lymphocytes compared with 14 patients in remission. These results suggest that selective beta2 -adrenergic blockade may he one of the causes of bronchial hypersensitivity in asthma, though it should be noted that in this study beta-adrenergic responses were examined in lymphocytes and were compared with the responsiveneness of the bronchi. The concept of antiphospholipid syndrome(APS) has been widely accepted. Antiphospholipid antibodies originally included anticardiolipin antibodies and lupus anticoagulants as serological marker of APS. However, recent advances have shown that most pathogenic antiphospholipid antibodies are directed to phospholipid binding proteins such as beta 2-glycoprotein I and prothrombin as well as phospholipids. The preliminary classification criteria for definite APS have been advocated as the "Sapporo criteria". Further prospective investigations are required to re-evaluate the clinical significance of so-called antiphospholipid antibodies. 相似文献

A wireless recording system was developed to study the electroencephalogram (EEG) in unrestrained, male Landrace piglets. Under general anesthesia, ball-tipped silver/silver chloride electrodes for EEG recording were implanted onto the dura matter of the parietal and frontal cortex of the piglets. A pair of miniature preamplifiers and transmitters was then mounted on the surface of the skull. To examine whether other bioelectrical activities interfere with the EEG measurements, an electrocardiogram (ECG) or electromyogram (EMG) of the neck was simultaneously recorded with the EEG. Next, wire electrodes for recording movement of the eyelid were implanted with EEG electrodes, and EEG and eyelid movements were simultaneously measured. Power spectral analysis using a Fast Fourier Transformation (FFT) algorithm indicates that EEG was successfully recorded in unrestrained piglets, at rest, during the daytime in the absence of interference from ECG, EMG or eyelid movements. These data indicate the feasibility of using our radiotelemetry system for measurement of EEG under these conditions. 相似文献

To examine whether the central chemoreceptors of respiration are located in the perfused area of the anterior inferior cerebellar artery (AICA), we occluded arteries or injected hypercapnic blood into arteries in the ventral surface of the medulla in anesthetized, paralyzed, and peripherally chemodenervated cats. Phrenic nerve activity, as an index of respiratory output, was augmented by an injection of hypercapnic blood into the vertebral artery. This vertebral-injection response decreased during bilateral occlusion of AICA. However, responses of phrenic nerve activity to the occlusion of AICA were complicated; activity increased in 19 cats, did not change in 10, and decreased in 9 during occlusion. In experiments with blood of various PCO2 levels being bilaterally injected into AICA, phrenic discharges increased with increases of PCO2. During the injection of constant PCO2 blood into AICA, phrenic response to alveolar PCO2 decreased by 80% compared with the original response. From these results, the blood flow and blood PCO2 level of AICA seemed to be related to the central chemosensitivity for respiration. To examine the perfused area, the ventral surface pH of the medulla was measured with a micro-combination pH electrode (2 mm diameter). During the injection into AICA, pH in the rostral medulla depended on the PCO2 of injected blood, and pH in other areas depended on the PCO2 of systemic blood. Also, histological study of India ink injection into AICA showed that ink-filled vessels were exclusively observed in the rostral medulla. Thus, we conclude that at least part of the central chemoreceptors of respiration are located in the perfused area of AICA, that is, in the rostral medulla. 相似文献
