<正>209349 Perioperative complications of interventional therapy for lower extremity arteriosclerosis obliterans/Duan Pengfei(段鹏飞,Dept Vascul Surg,2nd Affil Hosp Soochow Univ,Suzhou 215004)…∥China J Gen Surg.-2009,24(6).-466～469Objective To evaluate the prevention and management for the complications in interventional therapy for the lower extremity arteriosclerosis obliterans (ASO).Methods In this study 207 ASO cases received interventional therapy.The relationship between complication rates and different TASC type and the influence of diabetes mellitus(DM) with coronary heart disease (CHD) on critical organs complication rates was analyzed.Results The intervention succeeded in 190 cases.There was 17 failures,including 13 procedure failures and 4 deaths due to postoperative critical organ complications.Among the complications there were puncture site hemorrhage in 12,pseudoaneurysm in 4,alimentary tract hemorrhage in 2,arteriorrhexis in 6,cerebral infarction in 8,acute heart failure in 9,respiratory failure in 13,renal failure in 6,thrombosis in 5 and blue-toe syndrome in 1.The intraoperative complication rate of femoral-political type(39.84%,51/128)was higher than that of aorta-iliac type(18.99%,15/79)(P<0.05).Those critical organ complication rates of patients with DM and patients with DM&CHD were 27.66%(13/47)and 24.49%(12/49)respectively,they were higher than that of patients without(5%,2/40)(P<0.05).Conclusion The high complication rates during interventional therapy of ASO may result from misoperation and complicated pathological type and compliation with DM and CHD.These rates could be decreased when we treated DM and CHD before operation and chosen a reasonable method and shorten the duration of operation.8 refs,3 tabs.

<正>209189 Experience of endovenous radiofrequency combined with TriVex in treatment of chronic venous insufficiency in lower extremity/Lin Shaomang(Dept Vascul Surg,2nd Affil Hosp,Guangzhou Med Col,Guangzhou 510260)…∥Chin J Surg.-2009,47(4).-271~274Objective To evaluate therapeutic results of endovenous radiofrequency in combination with TriVex in treatment of venous insufficiency in lower extremities.Methods One hundred and fifty patients with chronic venous insufficiency(150 limbs) were randomly assigned to Group A(75 limbs) and Group B(75 limbs).Patients in Group A were treated with long saphenous veins radiofrequency ablation procedures in combination with TriVex.Patients in Group B were treated with long saphenous veins traditional stripping operation in combination with TriVex.The postoperative pain,average hospital stay and short-term results in hospital were compared between the two groups.Self-assessment of the opertion 4 weeks later,changes of CEAP classification,venous clinical severity score(VCSS) and chronic venous insufficiency questionnaire(CIVIQ) score were compared between the two groups.Results The operation time in Group A was (67±11) min,compared with (59±9) min in Group B(P>0.05).Postoperative pain and average hospital stay in Group A was higher than that in Group B 4 weeks after operation(P<0.05).The change of CEAP classification,VCSS and quality of life were signifcant after operation in both groups.The VCSS of Group A decreased by 4.6±2.5 compared with 4.3±2.7 in Group B (P>0.05).Conclusion Endovenous radiofrequency combined with TriVex for treatment of venous insufficiency in lower extremity is available,effective and with less trauma and faster recovery.CEAP classification,VCSS and CIVIQ are useful tools for assessing outcomes after radiofrequency in these patients.10 refs,4 tabs.
