For hundreds of years various types of sutures and threads have been placed in the human body by surgeons. The technique of thread augmentation involves the use of colorless absorbable (Vicryl) and non-absorbable (Neurolon) suture material on a long, straight Keith needle tunneled sub-dermally beneath prominent facial wrinkles in an effort to make the deep grooves less severe and to augment atrophic areas about the lips and oral commissures. This may be done as an isolated procedure or as an ancillary procedure at the time of a blepharoplasty or cervicofacial rhytidectomy.     

A refinement of the concept “Reticence”

1. 1. Made a distinction between speech disorders involving defects and those that are primarily reticence, but in any case identified speech disorders as negotiated states as opposed to fixed disease states.

2. 2. Argued that there is a clear analytical separation between speech disorders and speech behaiors which are indicative of neurotic disorders.

3. 3. Attempted to remove anxiety as a component of the communication process by according it its rightful place, which is central in human personality. Furthermore, we have attempted to dispel the notion that anxiety is evil and the perennial (and sole) cause of speech disorders.

4. 4. Identified a specialist known as the rhetoritherapist (a specially trained speech teacher) as the particular kind of professional qualified to deal with speech disorders (in conjunction with speech pathologists and/or psychotherapists where necessary). We have accorded to the rhetotherapist the province of instruction and training in all aspects of invention, delivery, and reception of rhetorical speech without reference to its moral intent.

5. 5. Identified “reticence” as the most useful of the various imprecise terms used to refer to people with speech problems, because it is devoid of connotations that go beyond the speech process.

Further refinement of speific categories of speech disorders is necessary to order to expand the repertoire of available treatment strategies. Such refinement will probably include reference to the various subprocesses of human speech identified earlier in this paper as they are related to the rhetorical situation. The rhetoritherapist thus emerges as the trouble-shooter, but not the “psychotherapist,” of speech pedagogy.     

Pleomorphic adenoma of the nose

Pleomorphic adenoma is the most frequent tumor of the major salivary glands. It is rarely located in the lacrymal glands and is unusual in the nasal cavity. A rare case of a pleomorphic adenoma of the nose is presented along with a review of the literature. The use of nasal endoscopy in removing tumors of the nose and paranasal sinuses is discussed.

Mixed parotid tumour of the nose is uncommon, Salivary gland tumours develop near or within the parotid gland nearly twenty times as often as they do in the submandibular gland. About 40% of all minor salivary tumours arise in the hard palate, 20% each in oral cavity and maxillary antrum, 10% in oropharynx and rest 10% are in upper respiratory tract. The average age of the patients with salivary tumours is 45 years, but the age range is wide, extending from childhood to adulthood and even over 80 years. Most series show a preponderance of females, especially if only tumours of the parotid glands are considered.     

Pleomorphic adenoma of nasal septum

The pleomorphic adenoma is the most common benign tumour of the major and minor salivary glands. We report a 40-year-old female patient, who presented with progressive swelling and deformity of the left side of her nose for last 2 years. On a lateral rhinotomy a mass (4 cm X3 cm in size) was found to be arising from the cartilaginous part of the nasal septum. The mass was emicleated from its capsule and the walls excised. The histopathological examination revealed it to be pleomorphic adenoma.     

Pleomorphic adenoma of the larynx

Tumors arising from minor salivary glands are extremely rare neoplasms in the larynx. Of the few reports of pleomorphic adenomas in this site , most have subglottic locations while only one case has involved the true vocal cord and seven cases have had supraglottic locations. We present a case of benign mixed tumor located in the posterior commissure which, to our knowledge, is the first reported in the world literature.
