Pilot-induced factors in fatal light aircraft accidents in the United Kingdom: 1969-1981

The author reviews 52 personally investigated fatal light aircraft accidents involving 8 professional pilots and 44 private pilots. In 22 of the 52 accidents the presence of any pre-crash defect or malfunction of airframe, engine, or flying controls was definitely excluded. In the opinion of the author there was a "pilot-induced" factor in 35 (69.3%) of the 52 fatal accidents. The author discusses the pilot-induced factors associated with some of these accidents and refers to some of the mechanical defects encountered.     

Elbow arthrography: brief technical considerations.

The author describes his technique for elbow arthrography. Linear tomography is advocated as a useful supplemental technique in both single and double contrast examinations.

Neurinoma of the ulnar nerve detected by ultrasonography: case report and discussion

The author describes the observation of nervus ulnaris neurinoma in a 10-year boy, who manifested the tumour clinically as a palpation -sensitive resistance. It was represented in the ultrasonographic picture as a sharply demarcated ovoid lesion of strongly hypo-echogenic character, homogenous echo-structure, while distal enhancement was mildly expressed behind the tumour. Based on the analysis of literature data the author discusses in detail ultrasonographic picture of tumours of peripheral nerves.

Painful Shoulder in Swimmers: A Diagnostic Challenge

In brief: Painful shoulder can cause significant disability in swimmers, and in elite swimmers it may even force premature retirement. Treatment of this problem has been difficult, and surgery has been perceived as generally unsuccessful. The author asserts that “swimmer's shoulder” is not an anatomically based diagnosis and that the frequent lack of a specific diagnosis probably has hindered treatment. New investigative tools, such as arthrography and NMR imaging, have improved the chances of reaching an accurate diagnosis. The author lists the many regional problems that can cause shoulder pain, describes physical, clinical, and laboratory tests for use in diagnosis, and outlines approaches to management of the problem.

Jung v. Association Of American Medical Colleges: the lawsuit challenging our system of graduate medical education

This article examines the antitrust case in which three physicians have challenged the Match Program and various other aspects of the system of graduate medical education in the United States. After describing the parties to the litigation, the author explains the plaintiffs’ theories of liability. He suggests how the plaintiffs are likely to claim damages in the multibillion dollar range and speculates about the injunctive relief that the plaintiffs are likely to seek. The author then sets forth some of the principal defenses to the case, as well as the basis for the defendants’ position that the plaintiffs have not been injured at all. The author goes on to explore developments that have occurred since the lawsuit was filed on May 7, 2002. These include a motion by the National Residency Matching Program to refer the case to arbitration; motions by certain teaching hospital defendants asserting that they are not subject to suit in the District of Columbia; and motions by the American Medical Association, the Accreditation Council for Graduate Medical Education, and other organization defendants to the effect that the plaintiffs have not alleged conduct by them that violates the antitrust laws. The author then discusses the motion by the plaintiffs to have the case certified as a class action. Finally, he considers legislative initiatives that the litigation has engendered.
