AF内固定治疗胸腰椎骨折88例

目的 探讨AF椎弓根内固定系统治疗胸腰椎爆裂骨折的疗效.方法 应用AF经椎弓根内固定系统治疗胸腰椎爆裂骨折88例.结果 平均随访12个月.88例椎体前缘高度由术前平均48%恢复至平均92%.椎管内移位骨块复位率达90%.神经损伤的恢复按ASIA分级:A级中9例无变化,余79例均提高1～2级.结论 AF椎弓根内固定系统能达到满意复位固定及椎管有效减压,是一种治疗胸腰椎爆裂性骨折理想的方法.     

后路间接减压治疗椎管占位的胸腰椎爆裂骨折

目的探讨无脊髓神经损伤伴有大骨块突入椎管的胸腰椎爆裂骨折后路间接减压与椎弓根固定技术的应用价值。方法对42例无脊髓神经损伤伴有大骨块突入椎管的胸腰椎爆裂骨折患者行椎弓根螺钉内固定及后路间接减压术,未进行椎板切除,伤椎上下椎体与伤椎同时植钉进行撑开间接减压,术后随访观察伤椎椎体高度的恢复,节段后凸角的纠正,椎管内骨块复位情况以及有无神经损伤症状。结果 42例患者均获随访,术后平均随访30个月。间接减压患者伤椎前后缘高度分别是:术前53.8%、82.3%,术后91.7%、95.3%;节段后凸角分别是:术前27.1°,术后5.3°;椎管截面积术前平均51.2%,术后83.7%;术后各项指标与术前相比有差异(P<0.01)。患者术后随访均无迟发性神经损伤发生。结论对于椎管内骨块占位但无脊髓及神经损伤的胸腰椎爆裂骨折行后路间接减压椎弓根钉内固定可以达到良好的治疗效果。     

短节段加伤椎后路椎弓根螺钉治疗胸腰椎爆裂骨折

目的总结短节段加伤椎后路椎弓根螺钉在胸腰椎爆裂骨折中的治疗体会。方法对65例胸腰椎爆裂骨折患者行术前、术后X线、CT片变化及功能恢复情况进行观察和分析。结果经6-15个月的随访,65例术后伤椎高度及钉棒稳定性调查,本组伤椎高度与术后相比较无塌陷,无脊柱不稳、断钉、断棒及螺钉松动现象发生,无脊髓神经损伤加重病例。结论短节段加伤椎行椎弓根螺钉固定治疗胸腰段椎体爆裂性骨折,能最大限度地恢复椎体高度、恢复生理弧度,减少后凸畸形。     

经后路椎弓根内固定系统治疗胸腰椎爆裂性骨折

目的:分析经后路椎弓根内固定系统治疗胸腰椎爆裂性骨折的临床疗效.方法:回顾性分析76例行经后路椎弓根内固定系统治疗胸腰椎爆裂性骨折的患者的临床资料.结果:平均手术时间110分钟,术中平均出血量600ml.术后CT或MRI复查显示脊髓受压解除,无明显椎管狭窄,无神经症状加重及晚期迟发性神经功能障碍.Cobb's角由术前的22°恢复到5°.IVW术前0.63±0.05,术后0.91±0.03.螺钉植入椎弓根位置的优良率85.53%,椎体压缩高度恢复优良率89.47%;术后随访12～24个月,4枚椎弓根螺钉折断,3枚弯曲,1例连杆折断,2例后期发现Cobb角有平均5°再丢失.结论:后路短节段椎弓根钉固定脊柱胸腰段爆裂性骨折,是治疗胸腰椎骨折的有效方法之一.     

AF内固定系统治疗胸腰椎爆裂型骨折疗效观察

目的:评价AF椎弓根螺钉内固定系统治疗胸腰椎爆裂型骨折的效果。方法:分析AF系统内固定治疗28例胸腰椎爆裂型骨折。观察患者手术前后椎体高度变化和椎管恢复情况。结果:术后随访1~4年,28例患者椎体高度全部恢复17例,基本恢复7例,过度撑开3例,1例椎体高度基本恢复,椎体侧方移位改善。所有病例后凸畸形均纠正。椎体前缘高度由术前41%恢复到术后的96%,椎体矢状面水平位移由术前平均31%纠正到术后6%。结论:AF内固定治疗胸腰椎爆裂骨折复位确切,固定可靠,是一种理想的治疗胸腰椎爆裂骨折的方法。     

植骨漏斗经椎弓根椎体内植骨结合椎弓根钉固定治疗86例胸腰椎爆裂骨折

目的：探讨自制植骨漏斗经椎弓根椎体内植骨结合椎弓根钉固定系统治疗胸腰椎爆裂骨折的技术特点和疗效。方法2005年5月至2013年3月,应用此方法治疗胸腰椎爆裂骨折86例,随访6～36个月,平均18个月。结果伤椎高度由57．8％恢复到正常的85．9％,矢状位cobb角由术前平均21．6°矫正至7．2°,脊髓神经损伤患者,按美国脊髓损伤协会（ASIA）标准恢复1级及以上。结论自制植骨漏斗经椎弓根椎体内植骨治疗胸腰椎爆裂骨折能很好恢复椎体高度、加强了椎体内部强度、克服了单纯椎弓根钉固定和后路植骨的缺点,疗效满意。     

经后路AF椎弓根钉系统内固定加钛网植骨治疗合并三柱不稳的胸腰椎爆裂型骨折

目的 探讨经后路AF椎弓根钉系统加钛网植骨治疗合并三柱不稳的胸腰椎爆裂型骨折的疗效.方法 应用AF椎弓根钉系统加钛网植骨治疗合并三柱不稳的胸腰椎爆裂型骨折30例.结果 30例患者经过平均24个月的随访,植骨全部融合,后凸角及高度的丢失不明显,无内固定松动、断裂和钛网移位,脊髓功能均有不同程度的恢复.结论 后路AF椎弓根钉系统加钛网植骨具有减压彻底、复位满意、固定可靠、创伤小、并发症少、力学性能好等优点.是治疗合并三柱不稳的胸腰椎爆裂型骨折的一种较理想的方法.     

后路经伤椎椎弓根螺钉内固定治疗胸腰椎爆裂型骨折疗效分析

目的 探讨后路经伤椎椎弓根螺钉内固定治疗胸腰椎爆裂型骨折的临床疗效.方法 选取2007-02~2010-11收治的51例胸腰椎爆裂型骨折患者,其中采用经伤椎椎弓根螺钉内固定治疗33例（6钉组）,跨伤椎双平面椎弓根螺钉内固定治疗18例（4钉组）.所有患者术前、术后、随访均行X线检查,测量伤椎椎体前缘高度值和Cobb＇s角.术后随访3~24个月,平均13.5个月.结果 三椎均顺利完成椎弓根螺钉植入,放置横连杆.螺钉位置及稳定性能良好,无切口感染,无脊髓神经症状加重.伤椎前缘高度和Cobb＇s角恢复满意.术后随访1年,6钉组效果优于4钉组（P=0.002）.结论 后路经伤椎椎弓根螺钉内固定治疗胸腰椎爆裂型骨折可增加骨折固定的稳定性,有效恢复和维持伤椎高度,矫正后凸畸形.     

AF内固定系统治疗胸腰椎爆裂型骨折128例

目的评价AF椎弓根螺钉内固定系统治疗胸腰椎爆裂型骨折的效果。方法分析AF系统内固定治疗128例胸腰椎爆裂型骨折。观察患者手术前后椎体高度变化和椎管恢复情况。结果术后随访1-5年，128例患者椎体高度全部恢复108例，基本恢复17例，过度撑开3例。病例后凸畸形均纠正，椎体前缘高度由术前41％恢复到术后96％，椎体矢状面水平位移由术前平均31％纠正到术后6％。结论AF内固定治疗胸腰椎爆裂型骨折复位确切，固定可靠，是一种理想的治疗胸腰椎爆裂骨折的方法。     

AF椎弓根钉内固定治疗胸腰椎爆裂型骨折的15例体会

目的 总结AF椎弓根钉后路治疗胸腰椎爆裂型骨折的一些心得.方法 后路入路用AF椎弓根钉内固定治疗胸腰椎爆裂型骨折15例.结果 全部病例随访12-24个月.术后椎体高度恢复满意.神经功能除Franket A级外均有提高.结论 AF椎弓根钉内固定手术治疗胸腰椎爆裂型骨折的疗效肯定,且操作相对简单、安全、固定可靠.     

Acupuncture Treatment of Vomiting

CASE HISTORY A female patient, 46 years old, head of the foreign affairs department of a certain university in Beijing, paid her first visit on October 9, 2006, with the chief complaint of vomiting for one month. She got vomiting after meals in early September. Before that, she had discomfortable sensation in the stomach due to angry with others, but she didn't pay much attention. Later, it developed into vomiting after eating. After the vomiting, the discomfort would be relieved, but with slight hypodynamia. She was once diagnosed as having 'neurogenic vomiting'. Having taken some western and Chinese drugs, the above symptoms were a little bit improved, but she would have nausea upon eating and with regurgitation. Because of the fear for vomiting, she did not dare to have food intake, with body weight reduction of 6 kilos in one month.     

Qiu Baoguo''''s Experience in Treating Consumptive Syndromes after Radio-Chemotherapies

Radiotherapy and chemotherapy are the important modern medical therapies for malignant tumors,yet they can also bring about serious local and systemic toxic side reactions so to decrease the patient;'s life quality,manifested by a series of consumptive symptoms.Having engaged in the combined work of Chinese and western medicine for nearly 50 years,the research fellow Qiu Baoguo in Henan Provincial Academy of TCM has developed his unique views on the TCM study of consumptive syndromes.The author of this essay had once the fortune tO follow Dr.Qiu in clinic,and specially would like to introduce in the following Dr.Qiu's experience in treating consumptive syndromes after radio-chemotherapies for patients with malignant tumor.     

Insomnia Due to Deficiency of Both the Heart and Spleen Treated by Acupuncture-Moxibustion and Chinese Tuina

OBJECTIVE: To observe therapeutic effects of the comprehensive therapy of acupuncture-moxibustion and Chinese Tuina for treatment of insomnia due to deficiency of both the heart and spleen. METHODS: 92 cases were divided randomly into the treatment group (treated by acupuncture-moxibustion and Chinese Tuina) and the control group (treated by acupuncture-moxibustion). RESULTS: The therapeutic effect of the treatment group was obviously superior to that of the control group (the CHI2 test showed P < 0.01). CONCLUSIONS: The comprehensive therapy of acupuncture-moxibustion and Chinese Tuina can give marked therapeutic effects for treatment of insomnia due to deficiency of both the heart and spleen.     

Analysis of CK-19,PDX-1,Nestin,Ngn3 in different donor islet purity in rats

Objective: To investigate if there are the CK-19, PDX-1, Nestin, Ngn3 positive cells in the donor islets of different purity in rats. Methods: Thirty male adult SD rats were randomly divided into 3 groups. Islets were isolated using digestion by ductal injection of collagenase. Group Ⅰ (n=10): Separating cell preparations were not purified, Group Ⅱ(n=10): Islet sediment was purified with 25% Ficoll400 ,Group Ⅲ (n=10): Islet sediment was purified with 25% and 11% Ficoll-400. The levels of protein of CK-19, PDX-1, Nestin and Ngn3 were detected by immunohistochemistry and the mRNA of CK-19, PDX-1, Nestin, Ngn3 was amplified by RT-PCR. Results: After two different purification methods applied, three islet preparations of different purities were obtained. The difference of islet purity was significant among various groups (P＜0.05). Compared with group Ⅱ and group Ⅲ,the protein and mRNA of CK-19, PDX-1, Nestin,Ngn3 were both higher in group Ⅰ; group Ⅲ was poorly expressed. Conclusions: The three different islet purity donor islet have different CK-19, PDX-1, Nestin, Ngn3 positive cells within them, indicating that there are some islet stem cells in the purified donor islet.     

A preliminary screening study on the associated proteins in human psoriasis vulgaris by serum proteomics technologies

Objective：To investigate the optimum screening conditions of associated proteins in human psoriasis vulgaris by serum proteomics technique, and to screen the different expression proteins related with psoriasis vulgaris. Methods：Serum samples of peripheral blood were collected from newly diagnosed psoriasis vulgaris patients in the clinic, and 20 matched healthy persons. Serum albumin IgG was removed by filtering with ProteoExtract Albumin/IgG. After comparative proteomics analysis the different protein spots were identified using 2-DE and MS. Results ：Electrophoresis figures with high resolution and reproducibility were obtained. Three different expression proteins were found only in the serum from psoriasis vulgaris patients, while nine other different proteins expressing from healthy volunteers. Conclusion：The protein expression was different in the serum between the psoriasis vulgaris patients and healthy volunteers. It was hoped that we could find the biomarkers related to psoriasis vulgaris by using proteomics.

Acupuncture Treatment of Chest Bi Syndrome

CASE HISTORY A male,a retired officer of 61 years old,paid his first visit on May 12,2006.The patient stated that 2 weeks before he suddenly got paroxysmal precordial choking pain,with irregular attacks.Each attack would last several seconds to several minutes.The attack was accompanied with heavy oppressed sensation in the precordial region,palpitation,lassitude,and in severe cases,sweating,which was once diagnosed by a hospital as'coronary heart disease'.Recalling the case history,the patient said that several days before he got angry with others because of some affairs in work,and he was not so used to the life pattern since he had just retired from his busy working post.At usual times,he was quick tempered and peevish.In recent days,he had poor appetite,and loose stool.
