一氧化氮和血管紧张素Ⅱ在糖尿病大鼠早期肾脏高功能状态中的作用

目的 探讨一氧化氮（NO）和血管紧张素Ⅱ（AngⅡ）在早期糖尿病大鼠肾小球高滤过状态中的作用。方法 利用链脲佐菌素诱导大鼠实验性糖尿病模型，将大鼠分为正常对照组、未治疗组、卡托普利组及左旋硝基精氨酸甲基酯（L－NAME）组，观察4周后糖尿病大鼠肾脏血流动力学和血浆、肾组织匀浆中NO、AngⅡ浓度的改变。结果 糖尿病未治疗较正常对照组的肾重／体重、24h尿蛋白、内生肌酐清除率、肾小球滤过率、肾血浆流量、血清及肾组织匀浆中NO3^-和AngⅡ活性显著升高（P＜0．05或P＜0．01）。卡托普利组，除血浆及肾组织匀浆中NO3^-未检测外，上述各项水平均有不同程度下降(P＜0．05或P＜0．01）。L－NAME组，除血浆及肾组织匀浆中AngⅡ未检测及24h尿蛋白无明显下降外，余项均有显著下降（P＜0．05或P＜0．01）。结论 糖尿病早期血浆NO、AngⅡ升高可造成肾小球高功能状态，用卡托普利及L－NAME可减轻糖尿病早期肾脏的高功能状态，延缓糖尿病肾病的发生与发展。     

化浊方对肾小球硬化大鼠肾组织TRPC6 mRNA表达的影响

目的:观察化浊方对大鼠肾小球硬化模型瞬时受体电位阳离子通道蛋白6 mRNA(TRPC6 mRNA)表达的影响,探讨其延缓肾小球硬化的作用机制。方法:72只雄性SD大鼠随机分成正常组、假手术组各6只,模型组、化浊方高、中、低剂量组及厄贝沙坦组各12只,造模后用化浊方治疗8周,检测治疗前及治疗后24 h尿蛋白定量、肾功能,显微镜下观察肾脏病理,荧光定量PCR检测肾组织TRPC6 mRNA表达。结果:治疗8周后,各治疗组24 h尿蛋白定量、肌酐(Scr)、尿素氮(BUN)均低于模型组(P0.05),肾脏病理显示大鼠肾小球纤维增生较模型组改善,均以高剂量组改善明显。模型组大鼠TRPC6 mRNA表达增高,各治疗组肾脏TRPC6 mRNA表达较模型组低(P0.05)。结论:化浊方能降低肾小球硬化模型大鼠24 h尿蛋白、Scr、BUN及改善肾小球纤维增生,其机制可能是通过下调大鼠肾脏TRPC6 mRNA的表达而延缓肾小球硬化。     

阿霉素肾病大鼠肾脏CTGF的表达及意义

目的:观察阿霉素肾病模型大鼠肾脏结缔组织生长因子(connective tissue growing factor,CTGF)的表达情况,探讨其与肾小球硬化发生发展的关系.方法:16只雄性SD大鼠随机分为对照组和模型组(n=8),模型组大鼠采用尾静脉注射阿霉素的方法建立肾病模型.分别检测两组大鼠的24h尿蛋白定量及血生化、肾功能指标,免疫组化染色及荧光定量PCR法分别检测大鼠肾脏CTGF蛋白和mRNA的表达.结果:与对照组比较,模型组大鼠的Alb明显降低,24h尿蛋白定量、TC、BUN、Cr及肾脏CTGF蛋白和mRNA的表达明显升高(P＜0.01).结论:CTGF过度表达是肾小球硬化发生发展的重要因素,可能是通过促进细胞外基质积聚导致肾小球硬化.     

肾宁口服液对兔膜性肾病模型肾保护作用的实验研究

目的观察肾宁口服液治疗膜性肾病的实验研究.方法用阳离子牛血清白蛋白(C-BSA)制备家免膜性肾病模型,设立正常对照组、中药治疗组、激素治疗组和模型组.每周测定一次24h尿蛋白,实验结束时测定血浆白蛋白、血脂和肾功能等生化指标.结果肾宁口服液中药治疗组的24h尿蛋白、血脂和肾功能等生化指标较模型组明显改善(P＜0.01或P＜0.05),而激素治疗组与模型组比较无明显差异(P＞0.05).结论肾宁口服液能降低尿蛋白,提高血浆白蛋白,改善肾功能,对膜性肾病有显著疗效.     

保肾康对阿霉素肾病大鼠血液动力学的影响

目的 进一步探讨保肾康的作用机理。方法：采用阿霉素肾病大鼠的实验方法，观察大鼠24h尿蛋白定量、血浆胆固醇（TG）、甘油三脂（TC）、内皮素（ET）、降钙素基因相关肽（CGRP）、血栓素（TXB2）、6－酮前列腺素F1a(6-Keto-PGF1a)的影响。结果：保肾康组大鼠24h尿蛋白定量可明显低于模型对照组（P＜0.05)，血浆胆固醇、甘油三脂显低于模型对照组，血液动力学指标ET、CGRP、TXB2、6－Keto-PGF1a与模型组相比较均有显意义（P＜0.05)。结论：保肾康可改善肾病模型大鼠脂质代谢，影响其血液动力学指标。从而显减少模型大鼠尿蛋白的丢失。     

曹氏清补通络法治疗慢性肾小球肾炎40例

目的观察曹氏清补通络法治疗慢性肾小球肾炎的临床疗效。方法将80例慢性肾小球肾炎患者随机分为观察组40例和对照组40例。在西医常规治疗的基础上,对照组予缬沙坦80 mg~160 mg/次,1次/d,观察组予缬沙坦联合曹氏清补通络方口服,1月为1疗程,共观察3个疗程,检测治疗前后血清白蛋白、血肌酐、血尿素氮、24 h尿蛋白定量、血尿等指标。结果观察组临床疾病总有效率87.50%,可改善肾功能,升高血清白蛋白(P0.05或P0.01),降低24 h尿蛋白定量及血尿水平(P0.05或P0.01)。结论曹氏清补通络方可有效改善慢性肾小球肾炎患者肾功能、降低蛋白尿及血尿,有较好的治疗作用。     

甘草甜素对大鼠肾小球硬化早期的防护作用

目的 探讨甘草甜素对大鼠肾小球硬化早期的保护作用。方法 采用两次经尾 iv 阿霉素方法建立大鼠肾小球硬化模型。实验随机分为3组,模型组、甘草甜素治疗组和对照组。检测各组大鼠第4、6、8周各项指标的变化,包括24 h 尿蛋白定量、血清尿素氮 (BUN)、肌酐 (SCr)、胆固醇 (TC) 和白蛋白 (Alb)。各组取肾皮质进行光镜等组织病理学检测。应用免疫组织化学方法检测肾组织内转化生长因子β1 (TGF-β1)、结缔组织生长因子 (CTGF) 蛋白的表达。采用标准曲线法进行荧光定量 PCR 检测肾组织内 TGFβ1、CTGF 和金属蛋白酶组织抑制物-1 (TIMP-1) mRNA 的表达。结果 甘草甜素治疗组大鼠 24 h 尿蛋白定量、BUN、SCr、TC 和 Alb 等指标与模型组相比较均有不同程度改善 (P＜0.05)。肾小球硬化程度治疗组明显轻于模型组。治疗组与模型组比较肾组织内 TGF-β1 和 CTGF 蛋白表达均有不同程度降低,表达峰值下降 (P＜0.05)。治疗组与模型组比较 TGF-β1、CTGF 和 TIMP1 mRNA 表达均有不同程度降低,表达峰值下降 (P＜0.05)。结论 从蛋白和 mRNA 水平同时证实甘草甜素对 TGF-β1、CTGF 和 TIMP-1 的表达有明显的抑制作用,证实甘草甜素对大鼠肾小球硬化有早期保护作用。     

益肾通络方对肾病综合征大鼠肾功能的保护作用

目的:观察益肾通络方对实验性肾病综合征大鼠肾功能的保护作用。方法:采用尾静脉注射阳离子化牛血清白蛋白造模。将40只雄性SD大鼠随机分为模型组、益肾通络方组、肾炎四味片组、正常对照组,每组10只。造模后予以药物干预,共给药4w。采用化学比色法检测24h尿蛋白,光镜下切取肾组织进行病理形态学观察。结果:与模型组比较,肾炎四味片组和益肾通络方组大鼠24h尿蛋白量明显降低(P<0.01);而2组之间比较,益肾通络方组大鼠24h尿蛋白量降低更显著(P<0.01)。病理形态学观察:模型组肾小球呈局灶性节段性硬化,肾小管上皮细胞水肿,部分小管腔内可见蛋白管型,肾间质纤维化;肾炎四味片组以上改变较同期模型组轻,而益肾通络方组显著减轻。结论:益肾通络方能降低大鼠尿蛋白,保护肾功能。     

赤芍对早期糖尿病肾病大鼠肾脏AngⅡ、ET-1、PKC及TGF-β1的影响

目的 观察赤芍对早期糖尿病肾病大鼠的作用及赤芍对肾脏血管紧张素Ⅱ(AngⅡ)、内皮素-1(ET-1)、蛋白激酶C(PKC)及转化因子-β1 (TGF-β1)的影响.方法 腹腔注射STZ制备糖尿病肾病大鼠模型,随机分为正常对照组、糖尿病肾病模型组以及高、中、低赤芍组(腹腔注射5、15、30g/kg),8周后测定大鼠血糖、血脂及肾功能指标(尿素氮、血肌酐、24h尿蛋白),放射免疫法测定肾脏AngⅡ、ET-1含量,免疫组织化学染色法检测肾脏TGF-β1及PKC表达.结果 与正常对照组相比,糖尿病肾病组大鼠血糖、血脂、尿素氮、血肌酐及24h尿蛋白明显升高,AngⅡ、ET-1含量和TGF-β1、PKC表达增强(均P<0.01); 与糖尿病肾病组相比,赤芍能呈浓度依赖性的降低糖尿病肾病大鼠血脂、尿素氮、血肌酐及24h尿蛋白,明显下调肾脏ET-1、TGF-β1 及PKC(均P<0.01),AngⅡ虽有下调趋势,但与糖尿病肾病组相比无明显差异性(P>0.01).结论 赤芍能改善糖尿病早期肾功能,其机制可能主要与抑制肾脏ET-1表达继而下调PKC及TGF-β1水平有关.     

甘草甜素对大鼠阿霉素肾病的保护作用

目的观察甘草甜素对大鼠阿霉素肾病肾组织层黏连蛋白(laminin,LN)的表达及尿蛋白排泄的影响,探讨甘草甜素对大鼠阿霉素肾病的发生、发展是否具有保护作用。方法18只SD雄性大鼠随机分为3组正常对照组、模型组、甘草甜素治疗组。模型组和甘草甜素治疗组大鼠采用尾静脉注射阿霉素的方法建立阿霉素肾病模型。甘草甜素治疗组予以甘草甜素灌胃8周,正常对照组和模型组大鼠予以等量生理盐水灌胃。检测各组大鼠24h尿蛋白定量及血清肾功能指标。观察各组大鼠肾组织病理学改变。采用免疫组织化学方法检测肾组织LN的表达水平。结果甘草甜素治疗组于用药第4、8周24h尿蛋白定量与同期模型组相比,均有明显改善,差异有统计学意义。甘草甜素治疗组大鼠肾组织病理学改变亦较模型组明显减轻,且肾组织LN的表达亦明显下降,差异有统计学意义。结论甘草甜素对大鼠阿霉素肾病具有一定的保护作用,可以减少尿蛋白的排泄、改善肾功能及减轻肾小球硬化的病理学改变。甘草甜素延缓肾小球硬化的进程,可能与降低肾组织LN的表达从而抑制细胞外基质的积聚相关。     

Acupuncture Treatment of Vomiting

CASE HISTORY A female patient, 46 years old, head of the foreign affairs department of a certain university in Beijing, paid her first visit on October 9, 2006, with the chief complaint of vomiting for one month. She got vomiting after meals in early September. Before that, she had discomfortable sensation in the stomach due to angry with others, but she didn't pay much attention. Later, it developed into vomiting after eating. After the vomiting, the discomfort would be relieved, but with slight hypodynamia. She was once diagnosed as having 'neurogenic vomiting'. Having taken some western and Chinese drugs, the above symptoms were a little bit improved, but she would have nausea upon eating and with regurgitation. Because of the fear for vomiting, she did not dare to have food intake, with body weight reduction of 6 kilos in one month.     

Qiu Baoguo''''s Experience in Treating Consumptive Syndromes after Radio-Chemotherapies

Radiotherapy and chemotherapy are the important modern medical therapies for malignant tumors,yet they can also bring about serious local and systemic toxic side reactions so to decrease the patient;'s life quality,manifested by a series of consumptive symptoms.Having engaged in the combined work of Chinese and western medicine for nearly 50 years,the research fellow Qiu Baoguo in Henan Provincial Academy of TCM has developed his unique views on the TCM study of consumptive syndromes.The author of this essay had once the fortune tO follow Dr.Qiu in clinic,and specially would like to introduce in the following Dr.Qiu's experience in treating consumptive syndromes after radio-chemotherapies for patients with malignant tumor.     

Insomnia Due to Deficiency of Both the Heart and Spleen Treated by Acupuncture-Moxibustion and Chinese Tuina

OBJECTIVE: To observe therapeutic effects of the comprehensive therapy of acupuncture-moxibustion and Chinese Tuina for treatment of insomnia due to deficiency of both the heart and spleen. METHODS: 92 cases were divided randomly into the treatment group (treated by acupuncture-moxibustion and Chinese Tuina) and the control group (treated by acupuncture-moxibustion). RESULTS: The therapeutic effect of the treatment group was obviously superior to that of the control group (the CHI2 test showed P < 0.01). CONCLUSIONS: The comprehensive therapy of acupuncture-moxibustion and Chinese Tuina can give marked therapeutic effects for treatment of insomnia due to deficiency of both the heart and spleen.     

Analysis of CK-19,PDX-1,Nestin,Ngn3 in different donor islet purity in rats

Objective: To investigate if there are the CK-19, PDX-1, Nestin, Ngn3 positive cells in the donor islets of different purity in rats. Methods: Thirty male adult SD rats were randomly divided into 3 groups. Islets were isolated using digestion by ductal injection of collagenase. Group Ⅰ (n=10): Separating cell preparations were not purified, Group Ⅱ(n=10): Islet sediment was purified with 25% Ficoll400 ,Group Ⅲ (n=10): Islet sediment was purified with 25% and 11% Ficoll-400. The levels of protein of CK-19, PDX-1, Nestin and Ngn3 were detected by immunohistochemistry and the mRNA of CK-19, PDX-1, Nestin, Ngn3 was amplified by RT-PCR. Results: After two different purification methods applied, three islet preparations of different purities were obtained. The difference of islet purity was significant among various groups (P＜0.05). Compared with group Ⅱ and group Ⅲ,the protein and mRNA of CK-19, PDX-1, Nestin,Ngn3 were both higher in group Ⅰ; group Ⅲ was poorly expressed. Conclusions: The three different islet purity donor islet have different CK-19, PDX-1, Nestin, Ngn3 positive cells within them, indicating that there are some islet stem cells in the purified donor islet.     

A preliminary screening study on the associated proteins in human psoriasis vulgaris by serum proteomics technologies

Objective：To investigate the optimum screening conditions of associated proteins in human psoriasis vulgaris by serum proteomics technique, and to screen the different expression proteins related with psoriasis vulgaris. Methods：Serum samples of peripheral blood were collected from newly diagnosed psoriasis vulgaris patients in the clinic, and 20 matched healthy persons. Serum albumin IgG was removed by filtering with ProteoExtract Albumin/IgG. After comparative proteomics analysis the different protein spots were identified using 2-DE and MS. Results ：Electrophoresis figures with high resolution and reproducibility were obtained. Three different expression proteins were found only in the serum from psoriasis vulgaris patients, while nine other different proteins expressing from healthy volunteers. Conclusion：The protein expression was different in the serum between the psoriasis vulgaris patients and healthy volunteers. It was hoped that we could find the biomarkers related to psoriasis vulgaris by using proteomics.

Acupuncture Treatment of Chest Bi Syndrome

CASE HISTORY A male,a retired officer of 61 years old,paid his first visit on May 12,2006.The patient stated that 2 weeks before he suddenly got paroxysmal precordial choking pain,with irregular attacks.Each attack would last several seconds to several minutes.The attack was accompanied with heavy oppressed sensation in the precordial region,palpitation,lassitude,and in severe cases,sweating,which was once diagnosed by a hospital as'coronary heart disease'.Recalling the case history,the patient said that several days before he got angry with others because of some affairs in work,and he was not so used to the life pattern since he had just retired from his busy working post.At usual times,he was quick tempered and peevish.In recent days,he had poor appetite,and loose stool.
