半腱肌、半膜肌肌腱移位治疗膝关节内侧副韧带断裂12例分析

<正>随着交通和体育事业的发展,膝关节韧带的损伤日益增多,尤以内侧副韧带损伤最为常见。2008年～2010年,我院骨科采用半膜肌、半腱肌肌腱移位治疗新鲜膝关节内侧副韧带断裂12例,术后取得较好效果。现报告如下。     

半腱肌移位静力性重建修复膝内侧副韧带体部损伤

目的探讨半腱肌移位静力性重建修复膝内侧副韧带的方法和疗效。方法对33例膝内侧副韧带断裂患者进行手术治疗,采用游离半腱肌,保留远端止点,游离并切断近端,通过骨隧道挤压螺钉固定的方法修复内侧副韧带。结果33例患者采用lysholm评分法评估患者手术前后的功能,术前平均43.88分,术后平均81.9分。结论半腱肌移位静力性重建修复膝内侧副韧带,发挥了膝内侧副韧带固有的生物力学效能,关节功能恢复满意。     

膝关节镜半腱肌及股薄肌肌腱重建前交叉韧带的临床观察倡

目的:探讨膝关节镜半腱肌及股薄肌肌腱重建前交叉韧带应用效果. 方法:对42例前交叉韧带损伤患者临床资料进行回顾性分析,指定具有专业知识及丰富经验的临床医生完成所有患者膝关节镜半腱肌及股薄肌肌腱重建前交叉韧带手术治疗,记录其治疗前后膝关节Lysholm及IKDC评分变化情况、不良反应发生情况,给予统计学分析后得出结论. 结果:42例前交叉韧带损伤患者经膝关节镜半腱肌及股薄肌肌腱重建前交叉韧带手术治疗后,Lysholm及IKDC评分均较治疗前显著改善,对比结果具有统计学意义( P<0.05);其中5例患者发生相关不良反应(发生率仅为11.90%). 结论:对前交叉韧带损伤患者给予膝关节镜半腱肌及股薄肌肌腱重建前交叉韧带手术治疗,可有效提高患者疗效及预后,有利于保障其生活质量.     

自体半腱肌肌腱在治疗膝关节后内侧结构损伤中的体会

目的介绍自体半腱肌肌腱在膝关节后内侧结构损伤中解剖加强膝关节后内侧结构的治疗体会。方法取自体半腱肌肌腱用Aesculap2号聚乙烯缝线编织缝合肌腱两端,在胫骨及股骨PMC附着点处钻骨隧道,用缝线纽扣钢板悬吊固定。结果11例患者均获得随访,随访时间11～61个月,国际膝关节编制委员会主观评分92.3分,客观评分91.7分,膝关节的稳定性良好。结论应用自体半腱肌肌腱治疗膝关节后内侧结构损伤中解剖加强膝关节后内侧结构的效果可靠,值得推广。

关节镜下自体半腱肌、股薄肌腱重建前交叉韧带

目的:探讨关节镜下利用自体半腱肌、股薄肌腱重建膝关节前交叉韧带的临床疗效。方法:对本院42例前交叉韧带损伤患者,关节镜下利用自体半腱肌、股薄肌腱,用可吸收界面挤压螺钉、股骨横向钉、胫骨端用inra-fix固定系统固定,重建前交叉韧带,术后随访8～20个月,平均12个月,Lysholm评分。结果:术后Lysholm评分较术前有显著性提高(t=23,P<0.01)。结论:关节镜下自体半腱肌、股薄肌腱重建膝关节前交叉韧带,该手术具有创伤小、材料选择理想,固定妥当,能早期锻炼,功能恢复快的优点。     

关节镜辅助下半膜肌腱移位动力加强治疗膝关节后内侧角急性损伤

目的观察关节镜辅助下半膜肌动力加强治疗膝关节后内侧结构损伤(PMC)手术对膝关节后内侧角急性损伤的治疗效果。方法 2009年7月~2011年12月共治疗21例后内侧结构损伤中12例合并前交叉韧带(ACL)断裂,5例合并后交叉韧带(PCL)断裂,4例为单纯后内侧角损伤。所有合并后内侧角损伤患者均行半膜肌腱移位动力加强替代后内侧结构的方法,术后随访12个月,采用Lysholm(2000国际膝关节文献委员会主观症状评分表)评分(膝关节韧带损伤的条件特异性评分)和Tegner膝关节运动水平对疗效进行综合评价及IKDC评级,并比较术前后外翻应力实验,Slocum试验。结果本组21例均获随访,平均随访12个月(6~18个月),关节功能评定.Lysholm评分术前(12.4±12.5)分低于术后(92.03±0.91)分(P<0.05),IKDC术后12个月评级:A级17例(80.95%),B级2例(9.52%),C级0例;术后12个月Tegner膝关节运动水平评分较术前提高5分,关节活动度提高50°。运动试验检查:外翻应力试验Ⅲ°及Slocum试验均为阴性。关节屈膝活动术前(70±18)°,术后12个月(121±19)°;疗效评定:优良18例,中2例.结论半膜肌移位动力加强治疗膝关节后内侧角急性损伤能够显著恢复膝关节后内侧解剖结构稳定性,术式简单,节省时间。可以收到较好的手术效果。     

关节镜下联合应用半腱肌腱和股薄肌腱双股双束重建前十字韧带20例的报告

目的：探讨关节镜下联合应用半腱肌腱和股薄肌腱双股双束重建前交叉韧带股骨端悬吊的方法及疗效。方法：20例前交叉韧带损伤患者行半腱肌腱股薄肌腱双股双束重建前交叉韧带术。将半腱肌腱及股薄肌腱对折套扎编织,于ACL前内侧束和后外侧束附着部据腱粗细分别钻骨隧道,用半腱肌腱重建前内侧束,股薄肌腱重建后外侧束,将肌腱自股骨隧道外口引入,胫骨隧道外口引出,股骨端采用悬吊固定,胫骨端分别行可吸收界面螺钉挤压固定。所有患者术前及术后12个月进行Lachman试验评估膝关节稳定性,用Lysholm评分方法评定膝关节功能。结果：术前Lachman试验均为阳性,术后18例为阴性,2例为阳性。术前Lysholm评分为36—61分,术后Lysholm评分为90～95分。结论：半腱肌腱和股薄肌腱双股双柬重建前交叉韧带,股骨端采用悬吊固定,动态稳定性好,疗效满意。     

膝内侧副韧带松弛合并陈旧性前交叉韧带损伤的手术治疗

目的 探讨应用自体骨-髌腱-骨重建受损的前交叉韧带(ACL),同时半腱肌腱转位联合内侧副韧带股骨止点上移治疗内侧副韧带松弛的临床疗效.方法 采用自体骨-髌腱-骨重建受损的ACL,同时半腱肌腱转位联合内侧副韧带股骨止点上移的方法治疗内侧副韧带松弛20例,其中男14例,女6例,年龄16～64岁,平均年龄32.6岁.术前X线评估Ⅱ度1例,Ⅲ度19例,MRI评估均为3度损伤,术前Lysholm膝关节评估法评分35～65分(平均48.9分).结果 术后20例全部获得随访;随访时间6～59个月,平均19.2个月.术后Lysholm评分50～96分,平均83.9分,优良率90%.术后1例关节屈曲90°,1例关节疼痛加重,其余膝关节功能均正常.结论 自体骨-髌腱-骨重建及半腱肌腱转位联合MCL股骨止点上移是一种可靠的治疗膝内侧副韧带松弛合ACL损伤的手术方法.     

半腱肌腱股薄肌腱在重建前交叉韧带中的运用

目的:探讨用无内固定物的半腱肌腱股薄肌腱重建前交叉韧带的临床运用和效果。方法:南京医科大学第一附属医院自2002年12月至2004年6月对33例前交叉韧带损伤的患者进行了无内固定物的半腱肌腱股薄肌腱重建前交叉韧带术,其中13例移植肌腱采用二股半腱肌腱和二股股薄肌腱,另20例仅为二股半腱肌腱,术前和术后进行Lachman试验评估膝关节的稳定性,用Lysholm评分方法评定膝关节功能。结果:术前Lachman试验均为阳性,术后29例为阴性,4例为阳性。术前Lysholm评分为36￣57分,术后Lysholm评分为71￣96分。结论:无内固定物的半腱肌腱股薄肌腱重建前交叉韧带为生物固定,固定可靠,费用少,短期效果好,中长期效果有待观察。     

Acupuncture Treatment of Vomiting

CASE HISTORY A female patient, 46 years old, head of the foreign affairs department of a certain university in Beijing, paid her first visit on October 9, 2006, with the chief complaint of vomiting for one month. She got vomiting after meals in early September. Before that, she had discomfortable sensation in the stomach due to angry with others, but she didn't pay much attention. Later, it developed into vomiting after eating. After the vomiting, the discomfort would be relieved, but with slight hypodynamia. She was once diagnosed as having 'neurogenic vomiting'. Having taken some western and Chinese drugs, the above symptoms were a little bit improved, but she would have nausea upon eating and with regurgitation. Because of the fear for vomiting, she did not dare to have food intake, with body weight reduction of 6 kilos in one month.     

Qiu Baoguo''''s Experience in Treating Consumptive Syndromes after Radio-Chemotherapies

Radiotherapy and chemotherapy are the important modern medical therapies for malignant tumors,yet they can also bring about serious local and systemic toxic side reactions so to decrease the patient;'s life quality,manifested by a series of consumptive symptoms.Having engaged in the combined work of Chinese and western medicine for nearly 50 years,the research fellow Qiu Baoguo in Henan Provincial Academy of TCM has developed his unique views on the TCM study of consumptive syndromes.The author of this essay had once the fortune tO follow Dr.Qiu in clinic,and specially would like to introduce in the following Dr.Qiu's experience in treating consumptive syndromes after radio-chemotherapies for patients with malignant tumor.     

Insomnia Due to Deficiency of Both the Heart and Spleen Treated by Acupuncture-Moxibustion and Chinese Tuina

OBJECTIVE: To observe therapeutic effects of the comprehensive therapy of acupuncture-moxibustion and Chinese Tuina for treatment of insomnia due to deficiency of both the heart and spleen. METHODS: 92 cases were divided randomly into the treatment group (treated by acupuncture-moxibustion and Chinese Tuina) and the control group (treated by acupuncture-moxibustion). RESULTS: The therapeutic effect of the treatment group was obviously superior to that of the control group (the CHI2 test showed P < 0.01). CONCLUSIONS: The comprehensive therapy of acupuncture-moxibustion and Chinese Tuina can give marked therapeutic effects for treatment of insomnia due to deficiency of both the heart and spleen.     

Analysis of CK-19,PDX-1,Nestin,Ngn3 in different donor islet purity in rats

Objective: To investigate if there are the CK-19, PDX-1, Nestin, Ngn3 positive cells in the donor islets of different purity in rats. Methods: Thirty male adult SD rats were randomly divided into 3 groups. Islets were isolated using digestion by ductal injection of collagenase. Group Ⅰ (n=10): Separating cell preparations were not purified, Group Ⅱ(n=10): Islet sediment was purified with 25% Ficoll400 ,Group Ⅲ (n=10): Islet sediment was purified with 25% and 11% Ficoll-400. The levels of protein of CK-19, PDX-1, Nestin and Ngn3 were detected by immunohistochemistry and the mRNA of CK-19, PDX-1, Nestin, Ngn3 was amplified by RT-PCR. Results: After two different purification methods applied, three islet preparations of different purities were obtained. The difference of islet purity was significant among various groups (P＜0.05). Compared with group Ⅱ and group Ⅲ,the protein and mRNA of CK-19, PDX-1, Nestin,Ngn3 were both higher in group Ⅰ; group Ⅲ was poorly expressed. Conclusions: The three different islet purity donor islet have different CK-19, PDX-1, Nestin, Ngn3 positive cells within them, indicating that there are some islet stem cells in the purified donor islet.     

A preliminary screening study on the associated proteins in human psoriasis vulgaris by serum proteomics technologies

Objective：To investigate the optimum screening conditions of associated proteins in human psoriasis vulgaris by serum proteomics technique, and to screen the different expression proteins related with psoriasis vulgaris. Methods：Serum samples of peripheral blood were collected from newly diagnosed psoriasis vulgaris patients in the clinic, and 20 matched healthy persons. Serum albumin IgG was removed by filtering with ProteoExtract Albumin/IgG. After comparative proteomics analysis the different protein spots were identified using 2-DE and MS. Results ：Electrophoresis figures with high resolution and reproducibility were obtained. Three different expression proteins were found only in the serum from psoriasis vulgaris patients, while nine other different proteins expressing from healthy volunteers. Conclusion：The protein expression was different in the serum between the psoriasis vulgaris patients and healthy volunteers. It was hoped that we could find the biomarkers related to psoriasis vulgaris by using proteomics.

Acupuncture Treatment of Chest Bi Syndrome

CASE HISTORY A male,a retired officer of 61 years old,paid his first visit on May 12,2006.The patient stated that 2 weeks before he suddenly got paroxysmal precordial choking pain,with irregular attacks.Each attack would last several seconds to several minutes.The attack was accompanied with heavy oppressed sensation in the precordial region,palpitation,lassitude,and in severe cases,sweating,which was once diagnosed by a hospital as'coronary heart disease'.Recalling the case history,the patient said that several days before he got angry with others because of some affairs in work,and he was not so used to the life pattern since he had just retired from his busy working post.At usual times,he was quick tempered and peevish.In recent days,he had poor appetite,and loose stool.
