PURPOSE: To evaluate the safety and efficacy of laser in situ keratomileusis (LASIK) for correction of high astigmatism after penetrating keratoplasty, and to assess the refractive results and predictability of the procedure. METHODS: LASIK was performed on 19 patients (19 eyes) with high astigmatism after penetrating keratoplasty, using the Chiron Automated Corneal Shaper and the Chiron-Technolas Keracor 116 excimer laser. The amount of preoperative refractive astigmatism ranged from 6.50 to 14.50 D (mean, 9.21 +/- 1.95 D) and the spherical component of manifest refraction ranged from -7.00 to +1.25 D (mean, -2.14 +/- 2.11 D). All patients completed a minimum follow-up of 12 months. RESULTS: Refraction was stable after 3 months. At 1 year after LASIK, the amount of refractive astigmatism was reduced to a mean of 1.09 +/- 0.33 D (range, 0.50 to 1.75 D), with 57.9% of the eyes within +/- 1.00 D of refractive astigmatism. The mean percent reduction of astigmatism was 87.9 +/- 3.7%. The postoperative spherical component of manifest refraction ranged from -1.00 to +1.75 D with a mean of +0.43 +/- 0.82 D. Vector analysis showed that the mean amount of axis deviation was 1.1 +/- 1.3 degrees and the mean percent correction of preoperative astigmatism was 92.6 +/- 8.4%. There were no intraoperative complications. Spectacle-corrected visual acuity was not reduced in any eye, and improved by 2 or more lines in 42.1% of eyes after LASIK. CONCLUSION: LASIK with the Chiron-Technolas Keracor 116 excimer laser was effective for correction of both astigmatism and myopia after penetrating keratoplasty. The procedure proved to be safe and gave fairly predictable and stable refractive results.  相似文献

PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) retreatment on eyes with residual myopia with or without astigmatism. METHODS: LASIK retreatment was performed on 35 eyes of 23 patients for correction of residual myopia, with or without astigmatism, with a mean manifest spherical equivalent refraction of -2.17+/-0.82 D (range, -1.00 to -3.87 D) and mean refractive astigmatism of -0.55+/-0.61 D (range, 0 to -1.75 D). Retreatment was performed 3 to 18 months after primary LASIK (mean, 5.1+/-2.6 mo). The corneal flap of the previous LASIK was lifted and laser ablation was performed using the Chiron-Technolas Keracor 116 excimer laser. Follow-up was 12 months for all eyes. RESULTS: At 1 year after retreatment, manifest spherical equivalent refraction was reduced to a mean -0.23+/-0.28 D (range, 0 to -0.87 D), and refractive astigmatism was reduced to a mean -0.16+/-0.25 D (range, 0 to -0.75 D). Thirty-two eyes (91.5%) had a manifest spherical equivalent refraction within +/-0.50 D of emmetropia, and 33 eyes (94.3%) had 0 to 0.50 D of refractive astigmatism. Uncorrected visual acuity was 20/20 or better in 11 eyes (31.4%). Spectacle-corrected visual acuity was not reduced in any eye after retreatment. There were no significant complications. CONCLUSION: LASIK retreatment was effective for correction of residual myopia or astigmatism after primary LASIK. Refractive results were predictable with good stability after 3 months. Lifting the flap during LASIK retreatment was relatively easy to perform and did not result in visual morbidity in eyes treated from 3 up to 18 months after primary LASIK.  相似文献

PURPOSE: To evaluate excimer laser in situ keratomileusis (LASIK) for hyperopia and its predictability. METHODS: We performed a retrospective study of 100 eyes that had LASIK for hyperopia to assess predictability and long-term stability of refractive results. The Chiron Automated Corneal Shaper was used to create the flap and the Keracor 117CT Chiron-Technolas excimer laser with the plano-scan program was used to ablate all corneas. RESULTS: Mean baseline spherical equivalent manifest refraction was +4.50 +/- 1.73 D (range, +1.25 to +8.50 D). Six months after LASIK, mean manifest spherical equivalent refraction was +0.72 +/- 1.87 D (range, -1.75 to +2.50 D), at 1 year, +0.88 +/- 1.73 D (range, -1.25 to +2.50 D), and at 2 years, +0.85 +/- 1.74 D (range, -0.50 to +2.75 D). Two years after LASIK, 45 eyes (74%) were within +/-1.00 D of intended correction and within +/-1.00 D of emmetropia. Uncorrected visual acuity was 20/40 or better in 50 eyes (82%) at 2 years; 29 eyes (37%) saw 20/20 or better. Undercorrection occurred more frequently in eyes with preoperative keratometric power of more than 45.00 D, when ablation zones less than 6 mm were used and when higher amounts of hyperopic correction were required. CONCLUSION: LASIK with the Keracor 117CT excimer laser appears to be an effective and safe procedure to correct hyperopia. Preoperative keratometric power, amount of hyperopia, and ablation zone diameter affect the efficacy and long-term stability of the procedure.  相似文献

PURPOSE: To assess the efficacy, predictability, stability, and safety of laser in situ keratomileusis (LASIK) in patients with hyperopia and to evaluate the visual and refractive results of the procedure. METHODS: LASIK was performed on 85 eyes of 53 patients for correction of hyperopia, with a preoperative mean manifest spherical equivalent refraction of +3.31 +/- 0.69 D (range, +1.25 to +5.00 D) and mean refractive astigmatism of +0.91 +/- 1.06 D (range, 0 to +3.00 D). The Carriazo-Barraquer (Moria) manual microkeratome was used to create the corneal flap, and laser ablation was performed using the Technolas Keracor 117C excimer laser with an ablation zone diameter of 6.0 mm and a transition zone diameter to 9.0 mm. Follow-up was 12 months for all patients. RESULTS: Refraction was stable by 3 months after surgery. At 1 year after LASIK, the mean manifest spherical equivalent refraction was +0.43 +/- 0.57 D (range, -1.25 to +2.00 D) and refractive astigmatism was reduced to a mean of 0.36 +/- 0.30 D (range, 0 to 1.00 D). Fifty-two eyes (61.2%) had a manifest spherical equivalent refraction within +/- 0.50 D of emmetropia, and 76 eyes (89.4%) were within +/- 1.00 D. Uncorrected visual acuity was 20/20 in 21 eyes (24.7%) and 20/40 or better in 79 eyes (92.9%). Spectacle-corrected visual acuity was reduced by two lines in one eye (1.2%) and improved by two lines in five eyes (5.9%). There were no significant complications. CONCLUSION: LASIK was an effective, safe, and predictable procedure for the correction of hyperopia up to +5.00 D and hyperopic astigmatism up to +3.00 D with the Technolas Keracor 117C excimer laser. The large size of the corneal flap obtained by the Carriazo-Barraquer (Moria) manual microkeratome facilitated laser ablation entirely in the exposed corneal stromal bed.  相似文献

BACKGROUND: To assess the results of toric laser in situ keratomileusis (LASIK) correction of myopic astigmatism. METHODS: A prospective study was performed over a 20 week period for consecutive patients treated for myopic astigmatism. Spectacle-corrected visual acuity, uncorrected visual acuity, spectacle refraction, videokeratography, and complications were recorded. Vector analysis was performed by the ASSORT program. RESULTS: Sixty-five eyes of 42 patients underwent toric LASIK with a Summit Technologies Apex Plus excimer laser. Mean preoperative spherical equivalent refraction at the spectacle plane was -6.24 +/- 2.42 D (range, -1.63 to -14.63 D) and mean pre-operative refractive cylinder magnitude was 1.99 +/- 1.35 D (range, 0.75 to 7.00 D); mean attempted refractive cylinder correction was 1.90 +/- 1.00 D. Six months after LASIK (43 eyes followed), mean spherical equivalent refraction at the spectacle plane was -0.40 +/- 0.55 D and 31 eyes (72.1%) were within +/- 0.50 D of emmetropia. At 6 months, mean refractive cylinder magnitude was 0.74 +/- 0.70 D, mean surgically induced astigmatism was 1.46 +/- 0.86 D, mean absolute angle of error was 10.33 degrees, mean astigmatic correction index was 0.93 +/- 0.36 D, and mean index of success was 0.46 +/- 0.39. Uncorrected visual acuity was 6/12 or better in 34 eyes (79.1%) and 6/6 in 15 eyes (35%); spectacle- corrected visual acuity was 6/9 or better in 41 eyes (95.35%). Six eyes (14.0%) lost 1 line of spectacle-corrected visual acuity at 6 months and one eye (2.3%) lost 2 lines. Ten eyes (23.3%) gained 1 line at 6 months. CONCLUSION: Toric LASIK with an ablatable mask using the Summit Apex Plus excimer laser is a safe and relatively accurate procedure for the correction of myopic astigmatism.  相似文献

PURPOSE: To evaluate the efficacy, predictability, stability, and safety of laser in situ keratomileusis (LASIK) to correct residual astigmatism after cataract surgery. METHODS: LASIK was performed on 20 eyes of 20 patients with refractive myopic or mixed astigmatism (3.00 to 6.00 D) at least 1 year after extracapsular cataract extraction with posterior chamber intraocular lens implantation without complication. Each eye received bitoric LASIK with the Nidek EC-5000 excimer laser and the Automated Corneal Shaper microkeratome. RESULTS: At 6 months after LASIK, mean refractive cylinder decreased from 4.64+/-0.63 D to 0.44+/-0.24 D (P<.001). Mean percent reduction of astigmatism was 90.4+/-5.0% (range 80% to 100%). Mean spherical equivalent refraction decreased from -2.19+/-0.88 D (range -1.00 to -3.88 D) to -0.32+/-0.34 D (range -1.25 to +0.38 D) (P<.001). Vector analysis showed that the mean amount of axis deviation was 0.7+/-1.2 degrees (range 0 degrees to 4.3 degrees) and the mean percent correction of preoperative astigmatism was 92.1+/-5.9% (range 85.6% to 108%). Eighty-five percent of all eyes had a mean spherical equivalent refraction and mean cylinder within +/-0.50 D of emmetropia. Change in spherical equivalent refraction and cylinder from 2 weeks to 6 months was < or = 0.50 D in 90% (18 eyes) and 95% (19 eyes), respectively. Spectacle-corrected visual acuity was not reduced in any eye. Diffuse lamellar keratitis occurred in three eyes (15%) after LASIK, and were treated successfully with eyedrops. CONCLUSION: LASIK was an effective, predictable, stable, and safe procedure for correction of residual myopic or mixed astigmatism ranging from 3.00 to 6.00 D with a low spherical component after cataract surgery.  相似文献

PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) in pediatric patients with myopic anisometropia and amblyopia, and to assess the predictability, safety, and efficacy of the procedure in children. METHODS: Fourteen patients aged 7 to 12 years with myopic anisometropia and amblyopia had LASIK in the more myopic eye (14 eyes) using the Chiron Automatic Corneal Shaper and the Chiron-Technolas Keracor 116 excimer laser. Preoperative spherical equivalent manifest refraction ranged from -4.62 to -12.50 D (mean, -7.87 D) and spectacle-corrected visual acuity ranged from 20/40 to 20/100 (median, 20/50). All patients completed a minimum follow-up of 12 months. RESULTS: One year after LASIK, spherical equivalent manifest refraction ranged from 0 to -1.50 D (mean, -0.55 D). Spectacle-corrected visual acuity improved in all eyes (range, 20/20 to 20/40; median, 20/25). Six eyes (42.9%) had a postoperative spectacle-corrected visual acuity of 20/20. Uncorrected visual acuity was 20/40 or better in 10 eyes (71.4%) (range, 20/20 to 20/70; median, 20/30). Uncorrected visual acuity exceeded preoperative spectacle-corrected visual acuity in all eyes by an average of 2 lines. There were no significant complications. CONCLUSION: LASIK was effective for correction of myopic anisometropia in this small group of children and reversed refractive amblyopia. LASIK in these children was safe, predictable, and provided good visual results.  相似文献

PURPOSE: To evaluate the visual and refractive results of conventional (non-wavefront) laser in situ keratomileusis (LASIK) for treatment of myopia and myopic astigmatism using the Alcon LADARVision 4000 excimer laser system and nomogram adjustment techniques. METHODS: A retrospective analysis of 499 eyes that had LASIK for myopia and myopic astigmatism was performed. Preoperative manifest spherical equivalent refraction ranged from -0.43 to -6.00 D and preoperative astigmatism ranged from 0 to -4.75 D. Patients were evaluated during 3 months following surgery. RESULTS: One month after surgery, 72% of eyes examined (298/415 eyes) had uncorrected visual acuity (UCVA) of 20/20 or better. Three months after surgery, 83% of eyes examined (216/261 eyes) had UCVA of 20/20 or better. One and three months after surgery, 82% and 83% of eyes, respectively, were within +/-0.50 D of attempted correction; 97% of eyes were within +/-1.00 D at both 1 and 3 months. No eye lost more than 1 line of best spectacle-corrected visual acuity (BSCVA) at 3 months after surgery. At the 3-month examination, 83% of eyes had UCVA better than or equal to preoperative BSCVA. CONCLUSIONS: Conventional LASIK to correct myopia and myopic astigmatism was safe and effective using the Alcon LADARVision 4000 excimer laser system. Outcomes were substantially improved throughout development of an accurate nomogram, derived from continually updated regression analysis of previous refractive results.  相似文献

PURPOSE: To evaluate the efficacy, safety, predictability, and surgically induced astigmatism (SIA) of laser in situ keratomileusis (LASIK) for the correction of myopia and myopic astigmatism. SETTING: Department of Ophthalmology, National Taiwan University Hospital, Taipei, Taiwan. METHODS: This retrospective study comprised 69 eyes that had LASIK to correct myopia and 74 eyes that had LASIK to correct myopic astigmatism. The excimer laser keratectomy was performed using a Summit Apex Plus machine. Refraction, visual acuity, and computerized corneal videokeratography data from the preoperative and postoperative examinations were collected. The astigmatic change was calculated by the Alpins vector analysis method. RESULTS: The preoperative spherical equivalent at the glasses plane in the myopia and myopic astigmatism groups was -8.08 diopters (D) and -9.73 D, respectively. At 6 months, the spherical equivalent and residual corneal astigmatism were -0.25 D and 0.85 D, respectively, in the myopia group and -0.71 D and 0.82 D, respectively, in the myopic astigmatism group. In the myopia group, 88% of eyes were within +/-1.0 D of the intended myopia correction and in the myopic astigmatism group, 85% were within +/-1.0 D of the targeted spherical equivalent and 90% were within +/-1.0 D of the intended astigmatism correction. The uncorrected visual acuity was 20/40 or better in 94.1% of eyes in the myopia group and 92.5% of eyes in the myopic astigmatism group. The SIA magnitude was 0.66 D with the axis randomly distributed in the myopia group. The mean astigmatism correction index was 0.97, the mean magnitude of error was 0.13 D +/- 0.62 (SD), and the mean angle of error was -3.70 +/- 13.73 degrees in the myopic astigmatism group. CONCLUSION: Laser in situ keratomileusis had similar predictability, safety, and efficacy in the treatment of myopia and myopic astigmatism. The astigmatism correction was effective, but the results suggest that subjective astigmatism of less than 1.0 D need not be treated with the Summit Apex Plus laser.  相似文献

PURPOSE: To evaluate the efficacy of excimer laser in situ keratomileusis (LASIK) in the treatment of refractive errors after penetrating keratoplasty. METHODS: Eight eyes underwent LASIK after a mean 71 months (SD 60) following the initial penetrating keratoplasty. A full ophthalmic assessment was performed before LASIK and at 1 week, 1, 3, 6, and 12 months after surgery. Mean follow-up was 8.6 months (SD 3.2). RESULTS: No eyes lost any Snellen lines of best spectacle-corrected visual acuity at the latest follow-up. Mean reduction in spherical equivalent refraction was 91% from -6.79 D (SD 4.17) to -0.64 D (SD 1.92) and mean reduction of cylinder was 72% from -6.79 D (SD 3.28) to -1.93 D (SD 1.17) at 6 months. Mean surgically induced astigmatism was 5.50 D (SD 2.42) and the index of surgically induced astigmatism divided by the initial cylinder expressed as a percentage was 81%. Mean outcome indices were: correction index 0.87, index of success 0.31, and angle of error 0.95 degrees. Three eyes (43%) achieved a spherical equivalent refraction of +/-0.50 D and 4 eyes (57%) achieved a spherical equivalent refraction of +/-1.00 D of emmetropia. CONCLUSIONS: The efficacy of LASIK after penetrating keratoplasty was good compared to incisional or surface based excimer laser methods and has the advantage of reducing the myopic spherical equivalent refraction in addition to astigmatism, thus improving the uncorrected visual acuities.  相似文献

目的:观察准分子激光原位角膜磨镶术(laser in situ kera-tomileusis,LASIK)治疗近视散光的临床疗效。方法:应用鹰视准分子激光治疗系统对120例234眼近视散光进行LASIK手术治疗。随访12mo,比较手术前后的视力、屈光度、散光及其散光轴位的变化。结果:术后随访12mo,裸眼视力≥术前最佳矫正视力(BCVA)者108例213眼(91%),屈光度在±1.00D以内者112例222眼(94.8%),散光残留平均为-0.52D。结论:LASIK治疗近视散光效果好,稳定性高,安全可靠。  相似文献

PURPOSE: To evaluate the safety, efficacy, and predictability of excimer laser photorefractive keratectomy (PRK) for compound myopic astigmatism using the VISX StarS2 excimer laser system with international version 3.1 software. METHODS: We report a prospective consecutive study of myopic excimer laser PRK, performed in a multi-surgeon environment with 200 eyes of 117 patients, to correct naturally occurring compound myopic astigmatism of between -0.50 to -5.90 D manifest refractive sphere and up to -3.50 D manifest refractive astigmatism. Patients were assessed prior to surgery and at 1, 3, 6, and 12 months after treatment. RESULTS: One hundred and ninety-eight of 200 treatments (99%) were reviewed 1 year after surgery; 193 of 198 eyes (97%) achieved 20/40 or better uncorrected visual acuity and 163 of 198 eyes (82%) achieved 20/20 or better. One eye lost two lines of Snellen visual acuity assessed at 12 months but recovered acuity when assessed at 18 months. Mean spherical equivalent corneal plane power was reduced from -3.50 to +0.90 D 1 month after treatment and 0 D at 12 months (SD 0.67 D). Three eyes of three patients underwent further treatment, two with LASIK and one with PRK for residual refractive error. Refractive astigmatism of >1.00 D was reduced from a mean -1.70 to -0.70 D at 1 year after treatment. Vector magnitude was 79% of that intended and mean vector axis error (absolute) was 8.5 degrees. No eye had a severe haze response. Pelli-Robson contrast acuity was significantly reduced after treatment from a mean 1.72 D preoperatively to 1.63 D at 12 months (P<.01). CONCLUSIONS: PRK for myopia using the VISX StarS2 excimer laser system was effective in the treatment of low myopic astigmatism, although there was a significant reduction in Pelli-Robson contrast sensitivity.  相似文献

PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) for mixed and simple myopic astigmatism using bitoric ablation. METHODS: A retrospective study was performed in 65 eyes of 38 consecutive patients to evaluate uncorrected (UCVA) and best spectacle-corrected visual acuity (BSCVA) and cyclopegic and manifest refraction, before and 3 and 6 months after LASIK (Moria LSK-ONE microkeratome, Nidek EC-5000 excimer laser). RESULTS: At 3 and 6 months after LASIK, 40 eyes of 24 patients (64.5%) were available for follow-up examination. Mean age was 25.9 +/- 6.6 years (range 18 to 43 yr). Mean preoperative manifest spherical equivalent refraction was -1.40 +/- 0.80 D (range -3.80 to +0.50 D) mean preoperative cylinder was -3.30 +/- 1.30 D (range -1.00 to -6.00 D). At 6 months follow-up, mean manifest spherical equivalent refraction was +0.30 +/- 0.46 D (range -0.38 to +1.88 D), mean cylinder was -0.73 +/- 0.61 D (range -2.25 to 0 D). There was a 77.8% decrease in astigmatism magnitude. According to vector analysis, mean achieved vector magnitude was 80% of intended. Fifty percent (20 eyes) had a cylinder within +/- 0.50 D of emmetropia. Twenty-three eyes (57.5%) had a spherical component within +/- 0.50 D. Eighty-five percent (34 eyes) had postoperative UCVA of 20/40 or better. Ten percent (four eyes) lost two lines of Snellen BSCVA, whereas 35% (14 eyes) gained one or more lines. CONCLUSIONS: Bitoric LASIK with the Moria LSK-ONE microkeratome and Nidek EC-5000 excimer laser was effective and safe for the reduction of cylinder in mixed and simple myopic astigmatism. Moderate undercorrection of the cylinder was evident.  相似文献

PURPOSE: To identify risk factors in a series of patients who developed steep central islands after laser in situ keratomileusis (LASIK). METHODS: We analyzed and compared the refractive and topographic outcome of a study group composed of 83 eyes of 44 patients who underwent LASIK using the VISX Star3 excimer laser with a refraction-matched control group of 83 eyes treated later. The vacuum aspirator of the excimer laser was abnormally positioned during the surgeries performed in the study group. RESULTS: Mean preoperative spherical equivalent refraction in the study group was -6.75 +/- 2.50 D. Four eyes with a mean preoperative spherical equivalent refraction of -9.27 +/- 2.29 D developed steep central islands. Thirty-three (38%) of 83 eyes treated needed retreatment for residual myopia or myopic astigmatism. In the control group, mean preoperative spherical equivalent refraction was -6.76 +/- 2.50 D. Ninety-three percent of eyes were within +/- 1.00 D of target refraction. Five (6.02%) of 83 eyes required retreatment and no eyes developed central islands. CONCLUSION: The abnormally positioned vacuum aspirator coupled with the higher preoperative refractive correction were the likely causative factors for central island formation and the increased incidence of undercorrection in these patients.  相似文献

PURPOSE: To compare the axis and magnitude of surgically induced refractive astigmatism (SIA) after hyperopic and myopic photorefractive keratectomy (PRK). SETTING: Department of Ophthalmology, University of Texas Southwestern Medical Center, Dallas, Texas, USA. METHODS: In this single-center retrospective study, the VISX Star S2 excimer laser was used to create a peripheral annular ablation profile to correct spherical hyperopia in 23 eyes of 16 consecutive patients. Attempted corrections ranged from +0.50 diopter (D) to +4.25 D with 0 to 1.00 D of astigmatism. The same laser was used to create a central ablation profile to correct spherical myopia in 25 eyes of 17 consecutive patients. Attempted corrections ranged from -2.25 to -6.50 D with 0 to 1.00 D of astigmatism. The absolute change in refractive astigmatism was calculated by taking the difference in magnitudes of astigmatism before and after laser treatment without regard to axis. Axis and magnitude of SIA were analyzed by vector differences. Magnitudes were compared using the Student t test, and axial shifts were compared using the chi-square test. All patients were followed for a minimum of 6 months. RESULTS: The mean changes in absolute astigmatism were 0.29 +/- 0.28 D at 3 months and 0.34 +/- 0.29 D at 6 months after hyperopic PRK and 0.40 +/- 0.35 D at 3 months and 0.39 +/- 0.36 D at 6 months after myopic PRK. The mean vectoral magnitudes were 0.49 +/- 0.29 at 3 months and 0.52 +/- 0.25 at 6 months after hyperopic PRK and 0.48 +/- 0.39 at 3 months and 0.44 +/- 0.38 at 6 months after myopic PRK. The mean values for SIA (the centroid) were 0.10 +/- 0.57 D x 113 degrees at 3 months and 0.15 +/- 0.57 D x 131 degrees at 6 months after hyperopic PRK and 0.04 +/- 0.63 D x 160 degrees at 3 months and 0.08 +/- 0.58 D x 171 degrees at 6 months after myopic PRK. There was no statistically significant difference between the 2 groups in vectoral axis or magnitude of SIA. CONCLUSION: Surgically induced astigmatism after hyperopic PRK was comparable to astigmatism induced by myopic PRK. A peripheral annular ablation for hyperopic correction, similar to a central ablation in myopic PRK, did not appear to result in uneven corneal healing causing astigmatism.  相似文献

PURPOSE: To determine the safety, efficacy, predictability, and stability of laser in situ keratomileusis (LASIK) in treating patients with myopic astigmatism using the Meditec MEL 70 G-Scan excimer laser. METHODS: Seventy-four eyes of 42 patients with myopia ranging from -4.50 to -9.88 D and astigmatism ranging from 0.50 to 4.00 D who underwent LASIK with the Meditec MEL 70 G-Scan excimer laser and the Summit Krumeich-Barraquer microkeratome were studied. Patients were followed for 1 year. RESULTS: Mean baseline spherical equivalent refraction was -7.12 +/- 1.70 D and cylinder was 1.62 +/- 1.72 D. At 12 months, mean postoperative spherical equivalent refraction was -0.49 +/- 0.57 D and mean refractive cylinder was 0.59 +/- 0.32 D. Mean change in spherical equivalent refraction between 1 and 12 months after surgery was -0.09 +/- 0.31 D, toward myopia. At baseline, no eyes had best spectacle-corrected visual acuity (BSCVA) of 20/20 or better, and 65% had 20/40 or better. At 12 months after surgery, 10% of eyes had BSCVA of 20/20 or better and 88% of eyes had 20/40 or better. The unusually low postoperative BSCVA results were thought to be due to amblyopia or existence of a higher percentage of some optical aberrations in this group of eyes. Mean uncorrected visual acuity was -1.38 +/- 0.42 LogMAR units (20/400) at baseline and -0.29 +/- 0.25 LogMAR units (20/40) at the 12-month postoperative examination. No eyes lost two or more lines of spectacle-corrected visual acuity. No vision threatening complications were observed. CONCLUSIONS: LASIK with the Meditec MEL 70 G-Scan excimer laser appeared to be safe, effective, reasonably predictable, and stable for correction of myopic astigmatism with a spherical component between -4.25 and -8.25 D and a cylindrical component between 0.50 and 4.00 D. However, astigmatism was slightly undercorrected with the algorithm used.  相似文献

PURPOSE: To examine the refractive outcome in 950 consecutive eyes having laser in situ keratomileusis (LASIK) by 1 surgeon with experience in keratomileusis. SETTING: Outpatient excimer laser surgical facility. METHODS: This study comprised 950 consecutive eyes of 475 patients having LASIK as a primary procedure with a LaserSight 200 excimer laser (8.51 software). A nasal hinged flap and a Chiron microkeratome were used. Preoperative cycloplegic refraction was done only in patients younger than 25 years and in all hyperopic cases. Subjective preoperative and postoperative manifest refractions were done after autorefractometry in all cases. In cases of hyperopia, the software was modified by adding 30% to the refractive error. Enhancement results are not included. RESULTS: Of the 950 eyes, 893 (94.00%) were myopic and 57 (6.00%), hyperopic. In the low myopia group (1.00 to 3.99 D) of 223 eyes (24.97%), mean spherical equivalents (SEs) were -2.90 D +/- 0.56 (SD) preoperatively, -0.46 +/- 0.6 D 3 months postoperatively, and -0.41 +/- 0.5 D 6 months postoperatively. In the moderate myopia group (4.00 to 5.99 D) of 205 eyes (22.96%), respective mean SEs were -4.90 +/- 0.7 D, -0.90 D +/- 0.9 D, and -0.67 +/- 0.7 D. In the high myopia group (6.00 to 9.99) of 266 eyes (25.30%), the respective means were -7.70 +/- 1.3 D, -0.76 +/- 0.99 D, and -0.60 +/- 0.8 D. In the extreme myopia group (10.21 to 30.00) of 199 eyes (22.28%), the respective means were -13.30 +/- 2.9 D, -1.30 +/- 1.4 D, and -1.13 +/- 1.3 D. For the entire myopic group, the mean astigmatism was +1.55 +/- 1.38 D, +1.09 +/- 0.92 D, and +0.87 +/- 0.77 D, respectively. The low hyperopia group (1.00 to 2.99 D) of 39 eyes (68.42%) had a mean preoperative SE of +1.80 +/- 0.59 D and mean postoperative SEs of +1.00 +/- 0.76 D at 3 months and +1.16 +/- 0.52 D at 6 months. The respective means in the moderate hyperopia group (3.00 to 6.00) of 18 eyes (31.57%) were +4.62 +/- 1.19 D, +3.71 +/- 1.12 D, and +4.00 +/- 1.07 D. CONCLUSIONS: Laser in situ keratomileus for myopia using the LaserSight 200 excimer laser was stable with time and safe for the correction of different degrees of myopia. In the hyperopic group, marked regression occurred in a large percentage of patients. Thus, we will not perform LASIK for hyperopia until the software improves.  相似文献

PURPOSE: To compare the postoperative visual outcomes after implantation of a Collamer toric implantable contact lens (ICL) and after wavefront-guided laser in situ keratomileusis in high myopic astigmatism. SETTING: Department of Ophthalmology, Kitasato University, Kanagawa, Japan. METHODS: This study comprised 30 eyes (18 patients) having toric ICL implantation and 24 eyes (17 patients) having wavefront-guided LASIK (Technolas 217z) to correct high myopic astigmatism (manifest spherical equivalent [SE] or=1.0 D). The safety, efficacy, predictability, stability, and adverse events were assessed preoperatively and 1 week and 1, 3, and 6 months postoperatively. RESULTS: At 6 months, the mean safety index was 1.28+/-0.25 (SD) in the ICL group and 1.01+/-0.16 in the LASIK group and the mean efficacy index, 0.87+/-0.15 and 0.83+/-0.23, respectively. All eyes in the ICL group and 71% of eyes in the LASIK group were within +/-1.00 D of the targeted SE correction at 6 months. The mean change in manifest refraction from 1 week to 6 months was -0.04+/-0.24 D in the ICL group and -0.60+/-0.49 D in the LASIK group. There were no significant complications in the ICL group; 2 eyes (8.3%) in the LASIK group required enhancement ablations. CONCLUSION: Toric ICL implantation was better than wavefront-guided LASIK in eyes with high myopic astigmatism in almost all measures of safety, efficacy, predictability, and stability, suggesting that toric ICL implantation may become a viable surgical option to treat high myopic astigmatism.  相似文献

PURPOSE: To determine the safety, efficacy, predictability, and stability of laser in situ keratomileusis (LASIK) to correct high myopia or high myopic astigmatism in patients with high anisometropia in whom conventional treatments have failed. METHODS: Nine eyes of nine patients, three boys and six girls with high anisometropia (>3.50 D), were included in this study. Patient age was 8 to 15 years. Minimum follow-up was 12 months. All patients were treated with the Chiron Technolas 217 excimer laser. RESULTS: Three months after LASIK, uncorrected visual acuity improved by at least five Snellen lines in all eyes. Three eyes (33.3%) were 20/40 or better without correction. Due to dense amblyopia, no eye had uncorrected visual acuity of 20/20 or better. The mean spherical equivalent refraction decreased from -7.66 to -0.22 D, and mean preoperative astigmatism decreased from -3.11 to -0.69 D. Eight eyes (88.8%) were within +/-1.00 D of emmetropia. Two eyes (22.2%) were within +/-0.50 D of the desired outcome. All patients were able to function binocularly. CONCLUSION: In this study of selected older children, LASIK was a safe and effective option for the correction of high anisometropia and for improved binocularity, when conventional therapies had failed. Further study is necessary to determine the long-term refractive stability in these young patients.  相似文献

PURPOSE: To report the results of the laser in situ keratomileusis (LASIK) using a second-cut hinged corneal flap to treat regression 1 year after primary LASIK for high myopia. SETTING: Istanbul University, Eye Research Center, Istanbul, Turkey. METHODS: Six eyes of 4 patients who had a mean myopic regression of -6.20 diopters (D) +/- 1.10 (SD) 1 year after primary LASIK were retreated with secondary LASIK performed using the Automated Corneal Shaper (Chiron Vision) and a 193 nm excimer laser (Chiron Technolas, Keracor 116). Fluence was 130 mJ/cm2 and the repetition rate, 10 Hz. RESULTS: Mean follow-up was 12.16 +/- 0.75 months. At the last follow-up, the mean spherical equivalent was -0.18 +/- 0.77 D (range +0.12 to -1.25 D). Two eyes (33.3%) remained within +/- 0.50 D of emmetropia; 5 eyes (83.3%) were within +/- 1.00 D of emmetropia. No patient lost best spectacle-corrected visual acuity. No complications occurred such as detachment of the former corneal flap and rolling, irregularity, or tearing of the new corneal flap. CONCLUSION: Secondary LASIK was an effective, safe method to treat regression after primary LASIK for high myopia.  相似文献
