The Joint Commission on Accreditation of Healthcare Organizations hasn't told hospitals enough about the new demands that its "Agenda for Change" will place on hospitals' information systems. Hospitals will have to be able to abstract medical records electronically and analyze their data to fully benefit from the program, executives at one test site say.

The publication on the web will include indicators from the National Quality Forum, Joint Commission on the Accreditation of Healthcare Organizations, Agency for Healthcare Research and Quality, and Centers for Medicare & Medicaid Services. Only national sets will be used to maintain transparency for consumers and competitors. Kentucky hospital contends this new initiative will cause staff to pay greater attention to quality.

The Joint Commission on Accreditation of Healthcare Organizations is getting mostly positive reviews on its recently approved standards focused on patient safety and medical error reduction in hospitals. But experts warn that integrating these new standards into existing safety protocols will be no easy task. The new standards augment the nearly 50% of current Joint Commission standards related to patient safety. Requirements for establishing ongoing patient safety programs in organizations accredited under the Comprehensive Accreditation Manual for Hospitals will be added in the areas of leadership, management of information, and other functions.

The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) has recently released its 1992 Accreditation Manual for Hospitals. A shorter, more concise version of its 1991 counterpart, the 1992 version marks the beginning of a multi-year transition to standards that emphasize the application of continuous quality improvement principles and concepts. In an effort to clarify some of the changes, Michigan Hospitals asked JCAHO officials to provide answers to some of the most commonly asked questions about the new standards. Following are their responses.

When the Joint Commission on Accreditation of Healthcare Organizations began conditionally accrediting hospitals two years ago, hospitals projected dire consequences. But interviews with hospitals that have been put on probation for quality problems by the JCAHO show that all regained full accreditation and few suffered any significant financial hardships.

The race is on to determine which organization will become the accreditation agency of choice for the managed care industry. At stake is enormous clout at the state and national levels--and a lot of money. Squaring off against the traditional accreditation leader, the Joint Commission on Accreditation of Healthcare Organizations, are: the National Committee for Quality Assurance, the American Association of Preferred Provider Organizations, and the Utilization Review Accreditation Commission.

Hospital accreditation and state certification are the means that the Centers for Medicare & Medicaid Services (CMS) employs to meet quality of care requirements for medical care reimbursement. Hospitals can choose to use either a national accrediting agency or a state certification inspection in order to receive Medicare payments. Approximately, 80% of hospitals choose the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). The purpose of this paper is to analyze and discuss improvements on the structure of the accreditation process in a Principal-Agent-Supervisor framework with a special emphasis on the oversight by the principal (CMS) of the supervisor (JCAHO).

New pain assessment and management standards instituted by the Joint Commission on Accreditation of Healthcare Organizations will become a part of surveys conducted this year, and could be part of the Joint Commission scores for hospitals and other agencies beginning in 2001. This month, we provide a run-down of what those standards include.
