Objectives: Functional Capacity Evaluation methods (FCE) claim to measure the functional physical ability of a person to perform work-related tasks. The purpose of the present study was to systematically review the literature on the reliability and validity of four FCEs: the Blankenship system (BS), the ERGOS work simulator (EWS), the Ergo-Kit (EK) and the Isernhagen work system (IWS). Methods: A systematic literature search was conducted in five databases (CINAHL, Medline, Embase, OSH-ROM and Picarta) using the following keywords and their synonyms: functional capacity evaluation, reliability and validity. The search strategy was performed for relevance in titles and abstracts, and the databases were limited to literature published between 1980 and April 2004. Two independent reviewers applied the inclusion criteria to select all relevant articles and evaluated the methodological quality of all included articles. Results: The search resulted in 77 potential relevant references but only 12 papers were identified for inclusion and assessed for their methodological quality. The interrater reliability and predictive validity of the IWS were evaluated as good while the procedure used in the intrarater reliability (test–retest) studies was not rigorous enough to allow any conclusion. The concurrent validity of the EWS and EK was not demonstrated while no study was found on their reliability. No study was found on the reliability and validity of the BS. Conclusions: More rigorous studies are needed to demonstrate the reliability and the validity of FCE methods, especially the BS, EWS and EK.  相似文献

The physical environment has emerged as a potential correlate to physical activity, and altering the environment could be one way to increase physical activity on a population level. The aim of the present study was to assess the test–retest reliability of the Swedish version of an environmental instrument developed for population-based studies. The study population was recruited from a larger study performed at the same time. A random sample of Swedish adults, age 18–74, was invited to participate in the reliability testing of 17 questions regarding the local physical environment. The questions addressed variables such as presence of sidewalks, bike paths and recreational facilities as well as questions regarding aesthetic, social and safety attributes. A total of 98 subjects completed the test and retest questionnaires. Overall percent agreement ranged from 55.1–92.9%. Intraclass correlation (ICC) for the total sample ranged from 0.36–0.98. The questions regarding motorized vehicles produced the highest total reliability score (ICC=0.98) and the question regarding safety from crime during the day produced the lowest (ICC=0.36). Small differences were seen between men and women for a few variables, but no apparent pattern was identified. The test–retest of the Swedish version of the environmental module resulted in substantial agreement for most variables. The instrument showed an acceptable reliability and can therefore be used in future studies regarding local environmental attributes and physical activity in Sweden.  相似文献

The objectives of this study were to establish test-retest reliability of lifting and carrying of a functional capacity evaluation (FCE) on two consecutive days and to verify the need for a 2-day protocol. A cohort of 50 patients (39 men, 11 women) with nonspecific low back pain were evaluated using a 2-day FCE protocol. Intraclass correlation coefficients (ICC) were calculated for weight lifted and carried. Predictive relationschips between test and retest were explored by means of a regression analysis. The results of ICC were lifting low 0.87, lifting overhead 0.87, and carrying 0.77. Performances on day 2 were on an average 6–9% higher. Other than the amount of weight handled on day 1, no variable was found to predict performance on day 2. It was concluded that test-retest reliability of lifting and carrying was good, and the need for a 2-day protocol could not be confirmed.  相似文献

Objectives To synthesize the evidence on the psychometrics functional capacity evaluation (FCE) methods. Methods A systematic literature search in nine databases. The resulting articles were screened based on predefined in- and exclusion criteria. Two reviewers independently performed this screening. Included studies were appraised based on their methodological quality. Results The search resulted in 20 eligible studies about nine different FCE methods. The Baltimore Therapeutic Equipment work simulator showed a moderate predictive validity. The Ergo-Kit (EK) showed moderate variability and high inter- and intra-rater reliability. Low discriminative abilities and high convergent validity were found for the EK. Concurrent validity of the EK and the ERGOS Work Simulator was low to moderate. Moderate to high test–retest, inter- and intra-reliability was found in the Isernhagen Work-Systems (IWS) FCE. The predictive validity of the IWS was low. The physical work performance evaluation (PWPE) showed moderate test–retest reliability and moderate to high inter-rater reliability. Low internal and external responsiveness were found for the PWPE, predictive validity was high. The predictive validity of the short-form FCE was also high but need to be further examined on several psychometric properties. Low discriminative and convergent validity were found for the work disability functional assessment battery. The WorkHab showed moderate to high test–retest, inter- and intra-rater reliability. Conclusion Well-known FCE methods have been rigorously studied, but some of the research indicates weaknesses in their reliability and validity. Future research should address how these weaknesses can be overcome.  相似文献

Reliable and valid measurements of meat intake are needed to advance understanding of its health effects and to evaluate interventions to reduce meat consumption. Here, we describe the development and reliability of the Oxford Meat Frequency Questionnaire (MFQ). It asks individuals to report the number of servings of meat and seafood products they consumed in the last 24 h and is administered daily over 7 days. The MFQ combines food portion size data from the UK Food Standards Agency with estimates of meat content in composite dishes from the UK’s National Diet and Nutrition Survey. Adults who self-reported to eat meat (n = 129) completed a 3-week web-based test–retest reliability study assessing the MFQ twice, with a wash-out week in the middle. Two-way random intraclass correlation coefficients (ICC) revealed moderate to good agreement on all meat outcomes (total meat ICC = 0.716; minimum–maximum individual components = 0.531–0.680), except for fish and seafood (ICC = 0.257). Participants reported finding the questionnaire easy to use and, on average, completed it in less than 2 min. This new MFQ offers a quick, acceptable, and reliable method to measure changes in an individual’s meat intake in a UK population.  相似文献

The objective of the study was to assess the reliability and validity of the Dartmouth Primary Care Cooperative Information Project/World Organization of National Colleges, Academies, and Academic Associations of General Practice/Family Physicians (COOP/WONCA) questionnaire in outpatients with chronic obstructive pulmonary disease (COPD). The test–retest reliability of individual items of the COOP/WONCA questionnaire was assessed using a weighted -statistic, and construct validity was assessed by correlating items of the COOP/WONCA with the EQ-5D health status measure. Discriminant validity was assessed by comparing scores for known groups, at the same time comparing the results with those of a lung-specific health status questionnaire. The individual items of the COOP/WONCA had test–retest reliabilities of 0.67–0.78 (weighted ). Spearman's rank correlations between COOP/WONCA single-item scores and corresponding EQ-5D ranged 0.45–0.72, which were generally higher than associations between non-corresponding items. Four of the five COOP/WONCA items did not discriminate between patient groups divided according to forced expiratory flow in 1 sec (FEV1) in percent of predicted and 6-min walking distance, while four of five items of the lung-specific questionnaire discriminated well between these groups. The COOP/WONCA chart system was reliable and showed properties supporting the construct validity of the measure. The items, however, did not discriminate well between known groups, indicating that this questionnaire is not very sensitive in patients with COPD. The reliability of the COOP/WONCA items was acceptable for use at group level, but lower than current recommendations for use in individual patients.  相似文献

OBJECTIVE: Measurement of exercise capacity is essential in patients with non-specific chronic low back pain (CLBP). However, the conventional Astrand bicycle test is not feasible in patients with a very poor aerobic capacity. Therefore the Astrand bicycles test for non-specific CLBP patients based on lean body mass (LBM) was developed as an alternative. The aim of this study was to evaluate reliability and validity of the LBM-based Astrand test. SUBJECTS: Twenty patients with non-specific CLBP and 20 healthy subjects were included for the reliability evaluation, and 19 healthy subjects for the validity evaluation. METHOD: Patients and healthy subjects were assessed twice. Intra class correlation (ICC), repeatability coefficient (RC) and the limits of agreement (LOA) were calculated as a measure of test re-tests reliability. An ICC >or= 0.75 was considered acceptable. Validity was tested by calculating ICC between the LBM-based Astrand test and a maximal bicycle test. RESULTS: The LBM-based Astrand test shows good reliability, reflected by an ICC >or= 0.91 and 95% of the 20 patients could perform the test. However, differences with the estimated true value reflected by the RC and natural variation reflected by the LOA were substantial in patients. Validity was good, reflected by ICC >or= 0.88. CONCLUSION: The present study shows that the LBM-based Astrand test is a reliable, valid, and feasible method for patients with non-specific CLBP. However, a substantial amount of variation should be taken into account in patients when interpreting the test results clinically.  相似文献

Validity and reliability of a Singaporean English EQ-5D self-report questionnaire (EQ-5D) were evaluated among consecutive outpatients with rheumatic diseases attending a tertiary referral hospital in Singapore (a multi-ethnic, urban Asian country). Subjects were interviewed twice within a 2-week period using a standardized questionnaire containing the EQ-5D, Short Form 36 Health Survey (SF-36) and assessing demographic and psychosocial characteristics. To assess validity of the EQ-5D, 13 hypotheses relating responses to EQ-5D dimension/Visual Analogue Scale (EQ-VAS) to SF-36 scores or other variables were examined using the Mann–Whitney test, Kruskal–Wallis test, or Spearman's correlation coefficient. Test–retest reliability was assessed using Cohen's . Sixty-six subjects were studied (osteoarthritis: 9, rheumatoid arthritis: 26, systemic lupus erythematosus: 23, spondyloarthropathy: 8; female: 72.7%; mean age: 44.3 years). Ten of 13 a-priori hypotheses relating EQ-5D responses to external variables were fulfilled, supporting the validity of the EQ-5D. Cohen's for test–retest reliability (n = 52) ranged from 0.29 to 0.61. The Singaporean English EQ-5D appears to be valid in measuring quality of life in Singaporeans with rheumatic diseases; however, its reliability requires further investigation. These data provide a basis for further studies assessing the validity of the EQ-5D in Singapore.  相似文献

The short form of impact of weight on quality of life (IWQOL)-Lite is a 31-item, self-report, obesity-specific measure of health-related quality of life (HRQOL) that consists of a total score and scores on each of five scales – physical function, self-esteem, sexual life, public distress, and work – and that exhibits strong psychometric properties. This study was undertaken in order to assess test–retest reliability and discriminant validity in a heterogeneous sample of individuals not in treatment. Individuals were recruited from the community to complete questionnaires that included the IWQOL-Lite, SF-36, Rosenberg self-esteem (RSE) scale, Marlowe–Crowne social desirability scale, global ratings of quality of life, and sexual functioning and public distress ratings. Persons currently enrolled in weight loss programs or with a body mass index (BMI) of less than 18.5 were dropped from the analyses, leaving 341 females and 153 males for analysis, with an average BMI of 27.4. For test–retest reliability, 112 participants completed the IWQOL-Lite again. ANOVA revealed significant main effects for BMI for all IWQOL-Lite scales and total score. Females showed greater impairment than males on all scales except public distress. Internal consistency ranged from 0.816 to 0.944 for IWQOL-Lite scales and was 0.958 for total score. Test–retest reliability ranged from 0.814 to 0.877 for scales and was 0.937 for total score. Internal consistency and test–retest results for overweight/obese subjects were similar to those obtained for the total sample. There was strong evidence for convergent and discriminant validity of the IWQOL-Lite in overweight/obese subjects. As in previous studies conducted on treatment-seeking obese persons, the IWQOL-Lite appears to be a reliable and valid measure of obesity-specific quality of life in overweight/obese persons not seeking treatment.  相似文献
