Family medicine has matured as an academic and scientific discipline with its own core concepts, knowledge, skills, and research domains. It has acquired much expertise in studying common illnesses; the integration of medical, psychological, social, and behavioral sciences; patient-centered care; and health services delivery. Many health care challenges in the 21st century will place a great demand on primary care, which can serve its purpose only if it is of high quality and evidence based. Family medicine research can contribute to many areas of primary care, ranging from the early diagnosis to equitable health care. Stakeholders, such as the World Health Organization, governments, and funding agencies, are becoming more supportive to family medicine research because they recognize its key importance in improving the quality of primary care and bridging the gap between biomedical research and clinical practice. Family medicine can play a leading role in shifting the paradigm of medical research from the laboratory to the person. The 21st century should be a golden age of family medicine research because the time is right for the discipline, the health care environment is most suitable, and stakeholders are supportive. Family medicine must prepare for it by building up its research track record and capacity.  相似文献

The German statutory health insurance scheme is confronted with a steadily rapid progress of medical sciences and increasing difficulties to mobilize the financial resources necessary for applying the new scientific knowledge in health care. Therefore it is absolutely imperative to intensify the efforts to improve the effectiveness and efficiency of health care. Health policies based on health targets, the development of patterns requiring integrated care, redefining the bunch of health insurance benefits, and a more regular use of methods of economic evaluation have been proposed as promising approaches towards optimizing resource allocation in health care. However, an analysis of these approaches demonstrates that a valid appraisal of their potential to improve the effectiveness and efficiency of health care requires further research. In addition, European integration may produce tendencies towards a convergence of the national health care systems; this makes it rather difficult to assess how the room for autonomous national health policies will develop in the future.  相似文献

The medical doctorate and the subsequent advanced research qualification in medicine have an exceptional position within the natural sciences. While, in the German system, graduation to the degree of a medical doctor is often an initiation into scientific practice, the in-depth scientific education of medical doctors may be achieved in various configurations. In recent years, structured programs for doctorates in medicine and natural sciences have found increasing acceptance, following recommendations of national scientific councils (“Deutsche Forschungsgemeinschaft” and“Hochschulrat”). Hannover Medical School has been offering such programs for a number of years. The StrucMed program increases the quality of medical doctorate studies, typically performed in the third and fourth years of university studies. The Hannover Biomedical Research School (HBRS) combines several programs for a doctorate in natural sciences, creating a platform for an internationally oriented education of post-graduates in various disciplines of life sciences. Evaluating the achievements and career paths of the trainees will contribute to the successful integration of research work in an efficiency-oriented clinical environment.  相似文献

Since the foundation of the People's Republic of China in 1949, there has been remarkable developments in public health in the country. These achievements are primarily attributed to the public health services and patriotic public health campaigns, although the contribution of high-technology medical applications is also recognized. However, along with the recent socio-economic developments and scientific and technological progress, medical disciplines have become more and more specialized, and clinical and preventive medicine have become further separated from each other. Conventional Chinese wisdom says 'when long divided they must unite, when long united they must divide'. At the onset of the new round of reforms of health care in China, it seems important to revisit the discussions on the urgency for integration of health sciences in medicine in China. Several issues and viewpoints on integrating medicine are discussed in this paper. The biopsychosocial model for health calls for broad integration. Primary care development in China requires integration in education and practice, and in treatment and prevention. Control of chronic diseases requires integrated and united action. Integration of traditional Chinese medicine with Western medicine requires creativity. The integration perspective should be instilled in the minds of medical students. Integration also entails integrated practice. After all, integration entails integrated education and practice in public health education. Changing the current public health education system still has a long way to go. True integration requires integration of concepts, policies, resources and measures, as well as changes in the organization of health care including public health, prevention and treatment. This needs to be a systematic process. Finally, success of integration relies on social mobilization, advocacy, promotion and attention of the entire society.  相似文献

Scarcity of resources and the economic aspects of health and health care set the starting point for health economics. The discipline has emerged in the post-war period. Its roots reach back to the times when social policy was first formulated, when human life was quantitatively valued, and when institutionalized health care was established. Health economics is defined as the scientific analysis of the economic aspects of health and health care. In spite of a number of developments of its own, the methodological basis of the discipline is founded in general economics and in its sub fields. This is amended by contributions from neighbouring disciplines. Health economics is a heterogeneous and applied discipline, which plays an original role in health sciences. Regularly, this discipline is expected to provide consultancy for health policy and other decisions. Next to this consultancy, sufficient attention should be devoted to the further development of theories and methods for the analysis of health production and the analysis of the organisation of health care systems.  相似文献

Orthodox medicine works in a scientific framework which often discounts knowledge arising outside biomedical models and the statistical means by which these are tested. Alternative medicine cannot meet these standards because it is holistic and individual in its orientations toward the understanding and treatment of human illness. But in fact the dominant model also has problems with surgery and other areas such as family practice as sub-disciplines where individualised caring solutions are important. These prominently include areas in which wider social and economic concerns directly impinge on health care so that we need a more liberal attitude to medical knowledge and discovery. I suggest that this wider conception is more in keeping with the Hippocratic ethos as a whole and with the idea of a healing praxis. Because our aim as doctors and health care professionals is to help people, and our knowledge is directed towards furthering this end, medicine is a practical science not able to stand apart and build theories in detached contemplation from within the ivory tower of the academy. However the practicality of medicine and the assurance needed to poison people or inflict grievous bodily harm in an effort to help them often puts a premium on certainties in our thinking about clinical medicine before the scientific basis for such certainty has been established. Therefore, hand in hand with the professional calling that is health care, goes the need for a certain style of belief in what one is doing and its ultimate rightness. This leads to an almost unique profile for medicine among the sciences.  相似文献
