Background

Patellofemoral joint biomechanics contribute to anterior knee pain, instability, and dysfunction following total knee arthroplasty (TKA). Information about specific factors leading to anterior knee pain and dysfunction is currently limited. Changes in patellofemoral joint offset (PFO) refers to a mismatch between the preoperative and postoperative anteroposterior geometry of the patellofemoral joint. It remains unclear whether these changes lead to adverse outcomes in TKA.

Methods

A retrospective radiographic review of 970 knees pre-TKA and post-TKA was completed to correlate the radiographic and clinical outcomes of changing the PFO using a posterior-stabilized single knee design with patellar resurfacing.

Results

A total of 970 patients were reviewed. Postoperatively, the anterior femoral offset, anteroposterior femoral size, and anterior patellar offset were changed in 40%, 60%, and 71% of knees, respectively, compared to preoperative values. The Western Ontario and McMasters Osteoarthritis Index total score as well as subscale scores for pain and function were not significantly affected by an increase or decrease in PFO. Similarly, Knee Society Scores and range of motion were not significantly affected. Increased anterior patellar offset was, however, associated with increased postoperative patellar tilt. Postoperative patellar tilt was not correlated with adverse patient satisfaction scores or loss of range of motion.

Conclusion

Changes in PFO (decreased, maintained, or increased) are common post-TKA and are not associated with a difference in clinical outcomes. Increases in anterior patellar offset led to increased patellar tilt, which was not associated with adverse patient satisfaction scores.  相似文献

Background

Patellofemoral complications are one of the main problems after total knee arthroplasty (TKA). The design of the TKA component may affect the patellar biomechanics, which may be associated with this postoperative complication. The purpose of this study was to assess the influence of TKA and prosthesis designs on the patellar kinematics and patellofemoral pressure.

Methods

Using fresh-frozen cadavers, we measured the patellofemoral pressure, patella offset, and patella tilt in the following 4 conditions: normal knee (patella replacement only), cruciate-retaining TKA, condylar-stabilizing TKA, and posterior-stabilized TKA.

Results

The patellofemoral pressure increased significantly after the cruciate-retaining TKA and condylar-stabilizing TKA compared with the normal knee. The patella offset in the normal knee decreased with increasing knee flexion angles, while the patella offset in the TKA knees did not change significantly through the full range of motion. The amount of lateral patella tilt in the normal knee was significantly larger than the TKA knees in the full range of motion.

Conclusion

Although the femoral components are designed to reproduce an anatomical patellar tracking, the physiological patellar kinematics were not observed. Relatively high patellofemoral pressure and kinematic change after TKA may be associated with postoperative complications such as the anterior knee pain.  相似文献

Background

Lack of consensus exists on the use of cementless total knee arthroplasty (TKA) in patients with knee osteonecrosis. Therefore, this study was conducted to evaluate (1) implant survivorship; (2) clinical outcomes and complications; and (3) radiographic outcomes of primary cementless TKA in knee osteonecrosis.

Methods

This study included 46 patients (49 knees) who had knee osteonecrosis and underwent primary cementless TKA and had a mean follow-up of 44 months (range 36-96). Kaplan-Meier analysis was used to evaluate implant survivorship. Follow-up was performed post-operatively at 6 weeks, 3 months, and annually thereafter. Clinical outcomes including the Knee Society Scores (KSS) for pain and function, changes in range-of-motion, complications, and radiographic outcomes were analyzed.

Results

Aseptic implant survivorship was 97.9% (95% confidence interval 1.01-0.93) and all-cause implant survivorship was 95.9% (95% confidence interval 1.01-0.9), with 1 septic and 1 aseptic failures. The mean KSS for pain was 93 points (range 85-100) and the mean KSS for function was 84 points (range 70-90). Additionally, 1 patient had superficial wound necrosis and was treated with local wound care with no further sequela. Otherwise, no evidence of loosening, subsidence, or progressive radiolucencies were noted on radiological evaluation.

Conclusion

Excellent implant survivorship, clinical, and radiographic outcomes of primary cementless TKA in the setting of knee osteonecrosis was demonstrated. Although further long-term study is needed to validate survivorship, new generation cementless TKA implants provide promising results in this subset of patients.  相似文献

Background

This study estimates the cost-effectiveness of patellar resurfacing in total knee arthroplasty (TKA).

Methods

We conducted a cost-effectiveness analysis using a decision analytic model representing a hypothetical TKA cohort, with or without patellar resurfacing, using data from the 2014 Australian Registry. The model represents 3 possible postoperative health states: (1) well, (2) patellofemoral pain, or (3) serious adverse event (any event resulting in a revision). Our effectiveness outcome was the quality-adjusted life year, from published utility scores. We estimated cost-effectiveness from a Canadian public healthcare payer perspective. Costs and quality of life were valued in 2015 United States dollars and discounted annually at 5%.

Results

Our results suggest that TKA with resurfacing is cost-effective compared to nonresurfacing. Unresurfacing the patellae resulted in higher costs ($13,296.63 vs $12,917.01) and lower quality-adjusted life year (5.37 vs 6.01) at 14 years. Sensitivity analysis suggests that if rates of secondary resurfacing are <0.5%, there was no cost difference.

Conclusion

Over 14 years postoperative, patellar resurfacing appears to be cost-effective, due to higher revision rates for unresurfaced TKA. Although our results suggest resurfacing improves quality of life, our model is limited by the availability and validity of long-term utility outcomes reported for TKA. Our cost-effectiveness analysis showed superiority of the resurfacing compared to retention of the patella.  相似文献

Background

The purpose of this study is to compare the clinical and radiographic outcomes of total knee arthroplasty (TKA) with patellar retention in accordance with the severity of patellofemoral arthritis.

Methods

We retrospectively reviewed patients who underwent TKA with patellar retention using the NexGen LPS or LPS-flex system between September 2010 and May 2015. The radiographic severity of patellofemoral arthritis was categorized according to the Iwano classification system, and subjects were divided into mild (stage 0-I) and moderate to severe (stage II-IV) groups. Clinical outcomes were evaluated using the Hospital for Special Surgery score, Knee Society Score, function score, Western Ontario and McMaster Universities Osteoarthritis Index, and Feller score. Radiographic outcomes were assessed using the congruence angle, patellar tilt angle, and lateral patellar displacement. The minimum follow-up for clinical and radiographic evaluation was 2 years. Clinical and radiographic outcomes were compared between the 2 groups preoperatively and at the time of the last follow-up.

Results

Four hundred seventy-four knees were enrolled and assigned to mild (n = 208) or moderate to severe (n = 266) groups. The preoperative Feller score was significantly lower in the moderate to severe group (P = .030), whereas the postoperative clinical and radiographic results did not differ significantly between the 2 groups.

Conclusion

Clinical and radiographic outcomes did not differ in accordance with the severity of patellofemoral arthritis after a minimum 2 years of follow-up of patients treated with TKA with patellar retention. Good outcomes were obtained with patellar retention in TKA, even in patients with advanced patellofemoral osteoarthritis.  相似文献

Background

Metaphyseal bone loss is commonly encountered in revision total knee arthroplasty (TKA). Anderson Orthopaedic Research Institute types 2 and 3 defects generally require some form of metaphyseal fixation or augmentation. This study evaluates the midterm results of stepped, porous-coated metaphyseal sleeves in revision TKA in the setting of severe bone loss.

Methods

Patients who underwent revision TKA using metaphyseal sleeves from March 2006 to May 2014 at our institution were identified from a prospective research database. Preoperative patient characteristics and operative data were reviewed. Postoperative outcomes were compared with preoperative values. Primary study outcomes included complications, reoperations, radiographic assessment of sleeve osteointegration, and survivorship.

Results

One hundred sixteen knees (108 patients) underwent revision TKA with 152 metaphyseal sleeves (111 tibial and 41 femoral). Anderson Orthopaedic Research Institute defect classification included 5 type 2A, 89 type 2B, and 17 type 3 tibial defects; and 3 type 2A, 34 type 2B, and 4 type 3 femoral defects. There were 3 intraoperative fractures (1.9%) associated with sleeve preparation and/or insertion. Six knees (5 patients) were lost to follow-up and 5 patients (6 knees) died before 2 years. Of the remaining 104 knees (98 patients, 134 sleeves), mean follow-up was 5.3 years (range 2-9.6 years). Nineteen knees (16.4%) required reoperation, most commonly for recurrent infection. Only one sleeve demonstrated radiographic evidence of failed osteointegration, but did not require revision. Two sleeves (1.5%) required removal and/or resection for recurrent infection.

Conclusion

This large retrospective series illustrates the utility of porous metaphyseal sleeves in revision TKA with a low rate of intraoperative complications, excellent osteointegration, and long-term fixation.  相似文献

Background

Ceramic bearings are not commonly used in total knee arthroplasty (TKA). So far, little information is available about whether long-term survivorship and good clinical outcomes can be ensured with ceramic knee implants. The purposes of the present study were to evaluate the clinical and radiological outcomes, and to assess the long-term durability of a ceramic tri-condylar implant.

Methods

A total of 507 consecutive TKAs were carried out using a ceramic tri-condylar femoral implant. The posterior cruciate ligament was sacrificed, and all components were fixed with bone cement. Clinical outcomes were assessed retrospectively with the Knee Society scoring system. Kaplan-Meier survivorship was calculated to determine the cumulative survival rate.

Results

One hundred sixty-seven knees (114 patients) were available for clinical outcomes. The average range of flexion improved from 118.1° preoperatively to 123.7° at a minimum 15-year follow-up (P < .001). The average Knee Society knee score improved from 39.1 to 92.8 (P < .001). The functional score also improved from 36.0 to 47.0 (P < .001). With revision for any surgery or radiographic failure as the end point, Kaplan-Meier survivorship at 15 years was 94.0%. With revision of any component as the end point, the corresponding survivorship was 96.2%.

Conclusion

Clinically, the postoperative knee flexion range and Knee Society scores were good after long-term follow-up. The survivorship of the ceramic knee implant was excellent over the 15-year follow-up, and long-term durability was achieved, making ceramic a promising alternative material for the femoral component in TKA.  相似文献

Background

In total knee arthroplasty (TKA), handling of the patella surface is still quite controversial. We carried out a prospective randomized study to compare circumpatellar electrocautery plus patella resurfacing vs circumpatellar electrocautery only in the single-staged bilateral TKA in Chinese population.

Methods

One hundred five patients diagnosed with late-staged osteoarthritis who received single-staged bilateral TKA were screened and 53 patients were included. All patients received the same posterior cruciate-stabilizing total knee prostheses. Patients were randomized to receive circumpatellar electrocautery plus patellar resurfacing or circumpatellar electrocautery only for the first TKA, and the second knee received the opposite treatment. All patients were followed for a minimum of 2 years.

Results

No differences were found with regard to Knee Society Score, Feller score, anterior knee pain, and revision rates. Fifty-two percent of patients had no preference with regard to pain and function, 27% of patients preferred the resurfacing plus circumpatellar electrocautery knee while 21% of the patients preferred the circumpatellar electrocautery only knee. The Insall-Salvati index and the patella tilt were a little smaller in the resurfacing group. One patient (2.1%) in the circumpatellar electrocautery group underwent a patella resurfacing revision for severe anterior knee pain and patella subluxation.

Conclusion

Equivalent clinical results for circumpatellar electrocautery plus resurfacing and circumpatellar electrocautery alone of the patella in TKA were demonstrated in selective Chinese population with thick enough patella.  相似文献

Background

The purpose of this study was to evaluate the joint gap kinematics in posterior-stabilized total knee arthroplasty (PS TKA).

Methods

Between January 2010 and April 2011, 44 consecutive patients (55 knees) who underwent primary PS TKA using the navigation system were assessed. There were 37 women and 7 men with a mean age at operation of 63 years (range; 58–73 years). After fixation of all components with cement and insertion of polyethylene trial insert, medial and lateral joint gap measurements were carried out without distraction force and separately using a navigation system on each flexion angles. The joint gaps were measured before and after deflation of tourniquet, first with patella everted and then reduced.

Results

Deflation of tourniquet did not effect on gap. In patellofemoral joint reduction situation, the medial gap relatively maintained throughout the range of knee motion, but the lateral gap is gradually decreasing with knee flexion from 30° to 120° flexion. However, in patellar eversion situation, the medial gap showed a significant increase and also statistically significantly increased compared with patellar reduced position over 90° knee flexion status, but the lateral gaps are decreased compared with patellofemoral reduction situation throughout the range of knee motion except full flexion status. In more physiologic status that is patella reduced and tourniquet deflated status, the average medial joint gap change was 2.30 ± 1.64 mm and the average lateral joint gap change was 2.90 ± 1.53 mm throughout the range of motion.

Conclusions

Medial and lateral joint gaps showed different patterns with patellar eversion and patellofemoral joint reduction. However, such changes occurred within 3 mm in average throughout whole range of knee motion even with multi-radius femoral component.  相似文献

Background

The management of patella in total knee arthroplasty (TKA) is a controversial topic. The elevated risk of patellofemoral complications seems to support those who are not eager to perform such procedure. The aim of this study is to carry out a retrospective assessment of patients who underwent patellar resurfacing during TKA from 2004 to 2009.

Methods

From January 1, 2004, to December 31, 2009, 1280 patients underwent TKA with patellar prosthesis implantation. Of them, 861 patients (639 women and 222 men with an average age of 67.7 ± 12.3 years) were available for a telephonic interview consisting of the questions included in the Knee Pain Score (KPS). Patients presenting KPS >36 and those subjected to reoperation were then evaluated clinically and radiologically.

Results

Of the 861 patients included in the study, 801 showed a KPS <36. Among the remaining 60 patients, 33 had a KPS >36, while 27 had been reoperated at the target knee. In the former case, a decrease in normal Insall-Salvati index and patellar tilt values (from 1.08 to 1.02 and from 9.3 to 9.1, respectively) and average anatomic axis (from ?2.6° to 1.1°) was found comparing preoperative with last follow-up evaluation. Only in 0.6% of the cases of reintervention, patellofemoral complications were found to be the cause.

Conclusion

This study conducted on a large and homogenous group of patients confirms a very low rate of complications related to patella resurfacing during TKA.  相似文献

Background

There is interest in uncemented total knee arthroplasty due to the hope for long-term biologic fixation, but limited data are available regarding uncemented tantalum patellar components. The purpose of this study was to evaluate the radiographic outcomes of uncemented tantalum patellar implants at midterm follow-up.

Methods

We retrospectively reviewed a consecutive series of 30 knees in 29 patients who underwent cementless total knee arthroplasty with an uncemented metal-backed tantalum patella between September 2006 and April 2009. Patients were required to have a minimum radiographic follow-up of 2 years. Anteroposterior and lateral radiographs of the knee were evaluated for signs of implant fracture or gross loosening. Clinical follow-up was obtained by reviewing each patient's most recent orthopedic record.

Results

Thirty knees in 29 patients met inclusion criteria. The mean age of the cohort was 59.1 years with a mean body mass index of 31.9 kg/m2. Mean postoperative radiographic follow-up time was 5.5 years. Six fractures of the patellar component were noted. This represented a fracture rate of 20% among the entire cohort and 35% among the 17 knees with visible patellae on anteroposterior radiograph. All fractures had a transverse pattern. No gross patellar component loosening was noted. Among patients with component fractures, 2 required revisions for instability and 1 revision was for infection.

Conclusion

Our results suggest a minimum 20% rate of component fracture at midterm follow-up. Although many of these patellar component fractures were asymptomatic, they have the potential to impact revision rates in the longer term.  相似文献

Background

Patellar crepitus is a complication most commonly seen in patients implanted with a posterior-stabilized total knee arthroplasty (TKA). Recently, design changes in the patellofemoral geometry and the intercondylar box ratio have been optimized in newer TKA designs. A comparative study was performed to analyze the incidence of patellar crepitus between a historical vs modern TKA design.

Methods

A retrospective review of all patients at our institution that underwent a primary TKA with either a PFC Sigma or Attune posterior-stabilized TKA (DePuy, Inc, Warsaw, IN), with a minimum of 1-year follow-up duration was performed. A total of 1165 participants implanted with the PFC Sigma and 728 with the Attune design were analyzed. Patellar crepitus incidence, functional scores, and range of motion were recorded at each follow-up appointment. Statistical analyses were performed between the 2 groups to determine if there were differences in clinical outcomes.

Results

The incidence of crepitus in participants implanted with the Attune was 0.55% vs 6.26% in the PFC Sigma cohort (P < .001) at 1 year vs. 0.83% vs 9.4%, respectively at 2 years post operatively (P < .001). There were small differences in extension, flexion, and Knee Society Scores between the 2 groups that were not clinically meaningful.

Conclusion

The Attune posterior-stabilized TKA demonstrated substantially less patellofemoral crepitus incidence than the historical control. We hypothesize that these findings are related to femoral component changes including a thinner and narrower anterior flange and a reduced femoral intercondylar box ratio.  相似文献

Background

Prior studies comparing unicompartmental knee arthroplasty (UKA) with total knee arthroplasty (TKA) in the elderly are limited by heterogeneity in arthritic disease patterns and patient selection. We report the results of UKA and TKA in patients 75 years and older with isolated medial compartmental arthritis, with special emphasis on immediate postoperative recovery, complications, reoperation rates, and implant survivorship at midterm follow-up.

Methods

A retrospective review was performed of all patients 75 years and older who underwent UKA or TKA at our institution between 2002 and 2012. All TKA preoperative X-rays were reviewed by a blind observer to identify knees with isolated medial compartmental arthritis considered acceptable candidates for UKA. Patients with less than 2 years of follow-up, flexion contracture greater than 10°, and rheumatoid arthritis were excluded. The final sample included 120 UKA (106 patients) and 188 TKA (170 patients) procedures. Patient records were reviewed to determine early postoperative recovery, complications, reoperations for any reason, and implant survivorship.

Results

UKA patients experienced significantly shorter operative time, shorter hospital stay, lower intraoperative estimated blood loss, lower postoperative transfusions, greater postoperative range of motion, and higher level of activity at time of discharge. Two UKA and 2 TKA patients required revision surgery. There was no statistically significant difference in postoperative Knee Society Scores. There were no differences in 5-year survivorship estimates.

Conclusion

Due to its less invasive nature, patients older than 75 undergoing UKA demonstrated faster initial recovery when compared to TKA, while maintaining comparable complications and midterm survivorship. UKA should be offered as an option in the elderly patient who fits the selection criteria for UKA.  相似文献

Background

There is none, to our knowledge, about comparison of high-flexion fixed-bearing and high-flexion mobile-bearing total knee arthroplasties (TKAs) in the same patients. The purpose of this study was to determine whether clinical results; radiographic and computed tomographic scan results; and the survival rate of a high-flexion mobile-bearing TKA is better than that of a high-flexion fixed-bearing TKA.

Methods

The present study consisted of 92 patients (184 knees) who underwent same-day bilateral TKA. Of those, 17 were men and 75 were women. The mean age at the time of index arthroplasty was 61.5 ± 8.3 years (range 52-65 years). The mean body mass index was 26.2 ± 3.3 kg/m2 (range 23-34 kg/m2). The mean follow-up was 11.2 years (range 10-12 years).

Results

The Knee Society knee scores (93 vs 92 points; P = .531) and function scores (80 vs 80 points; P = 1.000), WOMAC scores (14 vs 15 points; P = .972), and UCLA activity scores (6 vs 6 points; P = 1.000) were not different between the 2 groups at 12 years follow-up. There were no differences in any radiographic and CT scan parameters between the 2 groups. Kaplan-Meier survivorship of the TKA component was 98% (95% confidence interval, 93-100) in the high-flexion fixed-bearing TKA group and 99% (95% confidence interval, 94-100) in the high-flexion mobile-bearing TKA group 12 years after the operation.

Conclusion

We found no benefit to mobile-bearing TKA in terms of pain, function, radiographic and CT scan results, and survivorship. Longer-term follow-up is necessary to prove the benefit of the high-flexion mobile-bearing TKA over the high-flexion fixed-bearing TKA.  相似文献

Background

A systematic review and meta-analysis was performed to investigate the risks associated with anterior knee pain (AKP) following primary total knee arthroplasty (TKA).

Methods

A computerized search was performed of the following databases: MEDLINE, Embase, and Cochrane Central (published prior to July 2017). A total of 37 studies, which included 1641 cases of AKP and 168,090 TKAs, were included in the meta-analysis.

Results

A subgroup analysis revealed that compared with those without the following medical conditions, patients who had an infrapatellar fat pad excision and more than 12 months of follow-up (odds ratio [OR] 12.58, 95% confidence interval [CI] 3.245-48.781) were more likely to have AKP after TKA. Circumpatellar electrocautery (>12 months: OR 0.50, 95% CI 0.326-0.760; ≤12 months: OR 0.59, 95% CI 0.408-0.867) and patellar resurfacing (OR 0.25, 95% CI 0.131-0.485) may decrease the risk of AKP. Other factors, including the prosthesis bearing type (mobile bearing or fixed bearing) and the approach (midvastus compared with the medial parapatellar approach), were not significant risk factors for AKP.

Conclusion

The use of strategies such as patellar denervation and patellar resurfacing in primary TKA is recommended because they are safe and result in good clinical outcomes in preventing AKP. Caution should be taken when using an infrapatellar fat pad excision, because there is an increased risk of AKP at long-term follow-up (>12 months). Future studies should investigate these different strategies to confirm the underlying mechanisms and help prevent the occurrence of AKP after TKA. The timing of AKP onset remains unclear and requires further research.  相似文献

Background

The purpose of this study was to compare mid-term clinical and radiographic results of conversion total knee arthroplasties (TKA) following a closed-wedge high tibial osteotomy (HTO) and primary TKAs.

Methods

A total of 32 conversion TKAs (29 patients) were compared with a matched control group of patients who had undergone primary TKA. The clinical results were evaluated using the Knee Society Knee and Function score, the Western Ontario and McMaster Universities score, patella score, and range of motion (ROM). The radiographic results were evaluated using femorotibial angle, mechanical axis (MA), the method employed by the American Knee Society, joint line height (JLH), the amount of tibial bone resection, and the Insall–Salvati ratio. The clinical and radiographic results of conversion TKAs were compared with primary TKAs. The thickness of the polyethylene insert was also compared.

Results

No significant differences were observed in the clinical scores or ROM between two groups. No significant differences were detected in femorotibial angle, MA, and position of the components between two groups. The amount of tibial bone resection and pre-operative JLH both were significantly lower in the conversion group than those in the control group, although postoperative JLH and pre and postoperative Insall–Salvati ratio did not differ between two groups. No significant difference in the thickness of the polyethylene insert was identified between two groups.

Conclusions

The previous closed-wedge HTO itself had no detrimental effect on the mid-term outcome of the subsequent TKA, including patellar symptom and position. No significant differences in the postoperative joint line and patella height were detected between the conversion and primary TKA groups.  相似文献

Background

Patella resurfacing is performed in >80% of primary total knee arthroplasties (TKAs) in the United States, yet far fewer patellae are resurfaced internationally. Recent registry data have begun to question the long-held belief that patellar resurfacing yields lower revision rates. Multiple current meta-analyses have not shown a difference in patient satisfaction, anterior knee pain, or knee society scores based on patellar resurfacing.

Methods

We sought to determine how the rates of patellar resurfacing have changed over the past 10 years worldwide (2004-2014). Data were abstracted from the annual reports of 7 national joint registries, literature review, or via direct correspondence with registry administrators.

Results

Average rates of patellar resurfacing from 2004 to 2014 ranged from 4% (Norway) to 82% (United States). The largest decrease in resurfacing rates was in Sweden (15%-2%), whereas the biggest increase was in Australia (44%-59%). In 2010, only 48,367 of 137,813 (35%) primary TKAs from all registries outside the United States were resurfaced. Meta-analyses have demonstrated no difference in anterior knee pain or satisfaction scores but do consistently report increased revision rates for unresurfaced patellae. Recent Swedish registry data, however, showed a reverse trend toward higher revision rates after resurfacing.

Conclusion

Despite recent registry data and meta-analyses demonstrating equivalent outcomes among resurfaced and unresurfaced patellae in primary TKA, worldwide trends in patellar resurfacing have changed little over the past decade. Most countries outside the United States continue to resurface a much smaller proportion of patellae.  相似文献

Background

Minimally invasive surgery (MIS) has perceived advantages in the early postoperative stage for total knee arthroplasty (TKA). It is not clear whether the improved radiographic alignment achieved by computer-assisted navigation surgery (CAS) improves midterm clinical outcomes. The aim of this study was to compare patient outcomes of MIS TKA performed with and without CAS after a minimum follow-up of 7 years.

Methods

Between 2007 and 2009, 50 patients underwent CAS and MIS TKA, and 50 patients underwent jig-based MIS TKA in this prospective study. Ninety-six patients were evaluated after a mean follow-up of 7.7 years, and clinical and radiological evaluations were performed.

Results

Midterm results demonstrated that the Knee Society knee score, function score, and range of motion were comparable in the 2 groups. The percentage of patients with the mechanical axis within ±3° of neutral was significantly higher in the CAS group than in the jig-based group (94% vs 79%, respectively; P = .038). No knees had loosening after TKA. However, 1 patient in the CAS group demonstrated late infection 4 years postoperatively.

Conclusion

CAS did not improve midterm outcomes after MIS TKA compared with jig-based surgery, although CAS reduced outliers in coronal alignment.  相似文献

Background

To resurface or not to resurface the patella remains a controversy in total knee arthroplasty (TKA). The purpose of this study was to assess the long-term outcomes associated with selectively not resurfacing the patella.

Methods

This was a historical cohort study of 15,497 patients with 21,371 primary TKA procedures performed at a single institution between 1985 and 2010. The cohort included 402 (2%) knees with unresurfaced patellae and 20,969 knees with all-polyethylene patellae designs. Reasons for not resurfacing the patella were documented. Multivariable Cox regression analyses were used to estimate the risk of complications and revisions among procedures with unresurfaced patellae.

Results

According to the surgeon, reasons for not resurfacing were normal cartilage (226, 56%), young patient (30, 8%), thin patella (53, 13%), and surgeons' choice (93, 23%). In age, sex, and calendar year-adjusted analyses, the risk of complications (hazard ratio [HR]: 1.25, 95% confidence interval [CI]: 1.06, 1.46) and all-cause revisions (HR: 1.39, 95% CI: 1.02, 1.89) were significantly higher after TKA with unresurfaced patellae. However, after adjusting for femoral component types and operative diagnoses, these associations were no longer significant. The only group with significantly worse outcomes were those with a thin patellae with increased risk of complications (HR: 2.66, 95% CI: 1.70, 4.17) and revisions (HR: 5.94, 95% CI: 2.35, 15.02). Yet, the excess risk in the thin patellae group was mainly due to infections, and not related to unresurfaced patellae.

Conclusion

Selectively not resurfacing the patella seemed to provide similar results compared with routine resurfacing.  相似文献

Background

Anterior knee pain (AKP) and/or crepitation are important causes of dissatisfaction after total knee arthroplasty (TKA). Aim of this prospective, matched-pair study was to compare 2 different designs of patellofemoral (ie, trochlear groove) TKA. The Attune knee has an anatomic trochlear groove with a medialized dome patellar component vs the PFC Sigma with a single radius trochlear groove with a domed shaped patella.

Methods

Between January 2010 and December 2014, 100 consecutive Attune TKAs were matched to 100 PFC Sigma TKAs based on age, gender, side, and body mass index. All surgeries were performed via medial parapatellar approach and used cemented, posterior-stabilized implants with patellar resurfacing. Clinical evaluations were assessed using Knee Society Scores (KSS) and a patient-administered questionnaire at 2-year follow-up (range, 1.5-3 years).

Results

Based on the KSS clinical rating system, excellent clinical results were achieved in 89.4% and 90.7% of Attune and PFC Sigma TKAs, respectively. There were no significant differences in the KS pain or function scores. The overall incidence of AKP was significantly lower with the Attune knee compared to that of the PFC Sigma (12.5% vs 25.8%; P = .02). The incidence of hearing or feeling noise was also less with the Attune knee (17.7% vs 30.9%; P = .02). The incidence of painful crepitation was low in both groups (1.0% vs 4.1%) with no significant difference between groups (P = .37). Both groups had similar high satisfaction scores of 8.6 and 8.4 for the Attune and PFC groups, respectively (P = .09).

Conclusion

At 2-year follow-up, there were no differences between the PFC Sigma and Attune knees in KSS or satisfaction. However, the Attune group had a lesser incidence of AKP and crepitation. Further studies should focus on AKP and noise in cruciate-retaining knees.  相似文献
