目的:探讨自体前脂肪细胞凝胶植入法预防腰椎术后硬膜外腔瘢痕粘连的效果。方法:将60只SD大鼠随机分为A组(前脂肪细胞凝胶植入组)、B组(自体脂肪颗粒植入组)、C组(空白对照组),将A组动物先进行前脂肪细胞培养,待其脂肪细胞培养传代至适量后,制备各组大鼠的椎板缺损模型,A组取培养的单层脂肪细胞与生物蛋白胶制成混合物,B组取自体脂肪颗粒分别植入椎板缺损区,C组不植入任何物质,于术后8周行大体、光镜、电镜及MRI检查。结果:A组预防硬膜外腔瘢痕粘连效果良好,优于B组。结论:自体前脂肪细胞与生物蛋白胶制成的混合物植入预防硬膜外腔瘢痕的形成与粘连有良好的效果,是一种预防椎板切除术后硬膜外腔瘢痕形成与粘连的新方法。  相似文献

目的通过动物实验观察几丁糖对预防椎板切除术后硬膜瘢痕粘连的作用。方法24只成年山羊行腰2腰4短节段椎弓根钉系统固定及取自体髂骨行后外侧植骨融合术,腰3水平全椎板切除,切除口约3cm×1.5cm,腰3硬膜外分别放置几丁糖或空白对照各12只。于术后24周进行大体形态、组织学观察,并进行硬膜外瘢痕粘连程度分级。结果几丁糖组硬膜与瘢痕无明显粘连,空白对照组硬膜与瘢痕紧密粘连。结论几丁糖能有效地预防椎板切除术后硬膜外瘢痕粘连。  相似文献

目的探讨纤维蛋白封闭剂预防术后硬膜外粘连的中长期效果。方法成年健康犬10只。均行L2及L5全椎板切除术,制作椎板缺损模型。随机取5只犬,于L2缺损区喷涂纤维蛋白封闭剂,L5作为空白对照;于另5只犬的L5缺损区喷涂纤维蛋白封闭剂,L2作为空白对照。均在术后6个月处死,取出腰椎进行大体观察、组织学及MRI研究。结果对照组可见大量瘢痕组织增生,硬膜与后方瘢痕组织粘连紧密。纤维蛋白封闭剂组硬膜与瘢痕之间有间隙。无明显粘连。结论纤维蛋白封闭剂中长期防粘连效果是可靠的。  相似文献

目的:观察几丁糖膜对预防椎板切除术后硬膜周围粘连的作用。方法:24只成年新西兰白兔L3、L5水平切除椎板10×05cm,L3硬膜外放置几丁糖膜(A点),L5作为空白对照(B点)。分别于术后3周、6周、12周进行大体形态、组织学观察。结果:A点硬膜与瘢痕无明显粘连,硬膜外有空隙,未见明显异物、炎症反应;B点硬膜与瘢痕紧密粘连,瘢痕侵入椎管内,对硬膜有压迫。结论:几丁糖膜具有明显的预防硬膜周围粘连的作用。  相似文献

目的 观察壳多糖膜预防椎板切除术后硬膜周围粘连的作用。方法 24 只成年新西兰白兔 L3 、 L5 水平切除椎板1 .0 cm ×0 .5 cm , L3 硬膜外放置壳多糖膜( A 点) , L5 作为空白对照( B 点) 。分别于术后3 、6 、12 周进行大体形态、光镜观察。结果  A 点硬膜外粘连程度明显低于 B 点;光镜观察 A 点粘连范围明显低于 B 点, A 点硬膜外有空隙,未见明显异物、炎症反应, B 点硬膜外与瘢痕紧密粘连,瘢痕侵入椎管内,对硬膜有压迫。结论 壳多糖膜具有明显的预防硬膜周围粘连的作用。  相似文献

目的:探讨自体脂肪颗粒(FP)与医用生物蛋白胶(FG)联合应用预防椎板切除术后硬膜外粘连的效果。方法:90只SD大鼠随机分为5组。1组为对照组(C组),其它4组分别于L1~L2椎板切除术后在硬膜外腔放置FP(FP组)、自体游离脂肪片(FF组)、FG(FG组)及FP FG(FPFG组)。术后分别于4、8、12周对各组动物进行大体、光镜观察和计算机图像分析。结果:FPFG组硬膜外瘢痕较其它各组显著减少(P<0.05),脂肪坏死面积比率较FP及FF组显著降低(P<0.01),而移植脂肪中再生毛细血管数量较FP及FF组显著增加(P<0.01)。结论:自体脂肪颗粒与医用生物蛋白胶联合应用可防止脂肪颗粒流失,提高脂肪存活率,起到三维的遮挡保护作用,能有效地防止椎板切除术后硬膜及神经根周围的纤维化与粘连。  相似文献

目的观察聚乙交酯(PGA)膜对预防椎板切除术后硬膜周围粘连的作用.方法在新西兰兔L3、L5部位切除椎板1.0cm×0.5cm,一缺损处在硬膜外放置PGA膜,另一缺损部放置明胶海绵或空白对照,于术后3、6、9、12周作形态和组织学观察.结果大体形态观察PGA膜3周时开始降解,6周时分解成较大碎片,9周时吸收60%~80%,12周时仅残留部分小碎片,硬膜外无瘢痕粘连.明胶海绵和空白对照组瘢痕粘连发生率为88%和100%.组织学检查PGA膜组各时点均未见硬膜外纤维组织产生,硬膜与背侧组织无粘连,明胶海绵组(除3周外)和空白对照组均产生明显的粘连.结论PGA膜能有效防止椎管术后瘢痕粘连.  相似文献

同种异体骨板覆盖预防硬膜外瘢痕粘连的实验研究   总被引:8,自引:0,他引:8  
目的 探讨经冻干辐照处理后的同种异体骨板预防椎板切除术后硬膜外瘢痕粘连的作用。方法 12只成年雄性绵羊,随机分成A、B两组,每组6只。两组均作L3,4和L4,5椎板切除,A组的L4,5和B组的L3,4椎板缺损用“H”形同种冻干辐照骨板覆盖,为实验节段;A组的L3,4和B组的L4,5不覆盖骨板作为对照节段。于术后4、8、12、16、20和24周取材,行大体、X线平片和组织切片观察;并对4、24周取材行CT扫描。结果 术后4周,实验节段可见同种骨板形态、位置保持完好,椎管内无狭窄,硬膜囊无压迫,镜下椎板咬除缘有多量新生骨小梁生成,周边有大量软骨细胞及其分泌黏多糖和坏死后所留下的空腔,骨板内侧有增生纤维组织及少量炎性细胞浸润;对照节段可见大范围致密的纤维组织夹杂变性坏死肌肉嵌入椎板缺损处,硬膜广泛粘连,硬膜囊受压。术后24周,实验节段椎管重建基本完成,覆盖骨板吸收完全,并与再生椎板融合,椎管形态完好,硬膜外未见瘢痕组织;对照节段椎板再生未完成,瘢痕组织自椎管外嵌入椎管内,压迫硬膜囊及脊髓,硬膜外间隙几乎消失。统计学分析表明手术后期(16~24周),实验节段与对照节段相比硬膜粘连程度明显减轻,椎管矢状径均较对照节段大,硬膜囊形状保持良好,无明显压迫。结论 同种异体冻干辐照骨板在实验羊椎板切除后的硬膜外能有效减少瘢痕形成和粘连。  相似文献

目的通过动物实验探索预防椎板切除术后硬膜外粘连的方法及其效果。方法64只新西兰兔随机分为A、B、C、D组,每组16只。手术切除L5椎板造成12 mm×6 mm硬脊膜裸露区,探查神经根。A组注入生理盐水1m l,B组充填高分子量透明质酸钠凝胶1 m l,C组用自体L6棘突重建L5椎板,D组用高分子量透明质酸钠凝胶1 m l充填后再进行L5椎板重建。术后2、4、6、8周行大体观察及组织学观察,于6周时行硬膜外瘢痕面积和致密度定量分析。结果①大体观察:A组术区明显粘连,B、C组轻度粘连,D组无明显粘连;②瘢痕面积百分比:C、D组分别为15.89%±1.88%、13.94%±1.89%,显著小于A、B组(22.66%±2.89%、20.70%±2.82%,P<0.05);③瘢痕密度:B、D组分别为42.03%±7.36%、36.50%±9.08%,显著小于A、C组(63.73%±6.06%、52.11%±4.10%,P<0.05)。结论高分子量透明质酸钠凝胶充填和椎板重建术对预防椎板切除术后硬膜外粘连具有协同作用,效果显著。  相似文献

目的比较局部应用他克莫司(FK506)和丝裂霉素C(MMC)预防腰椎板切除手术后硬膜外瘢痕粘连的效果。方法 36只SD大鼠用于制作椎板切除术模型(L1椎体),并将其随机分成3组:0.1 g/L FK506组、0.1 g/L MMC组和生理盐水对照组,检测用药前后后肢运动功能评分(BBB评分)的变化。术后4周分别行大体标本观察、羟脯氨酸(HPC)含量检测和组织病理学观察。结果用药前后各组BBB评分差异无统计学意义(P>0.05),FK506组存在轻度粘连,MMC组无明显粘连,对照组形成紧密的硬膜外瘢痕粘连。FK506组和MMC组硬膜外瘢痕中HPC含量、成纤维细胞计数较对照组明显降低,且MMC组优于FK506组(P<0.05)。结论局部应用FK506可减少椎板切除术后瘢痕形成,有效预防硬膜外粘连,但MMC防粘连效果优于FK506。  相似文献

目的 观察椎板切除术后用川芎嗪的同时在椎板缺损处植入自体骨膜预防硬膜外粘连的效果.方法 48只SD大鼠随机分成4组(n=12),均制作L2椎板损伤模型,A组硬膜外涂布生理盐水,B组硬膜外涂布川芎嗪,C组硬膜外覆盖自体骨膜,D组硬膜外涂布川芎嗪的同时加自体骨膜覆盖.术后12周对4组椎板切除部位进行大体观察,对粘连度、胶原含量、组织学评分及骨转化生长因子定量分析进行比较.结果 B、C、D组的改良RydellBalazs粘连韧度评分、胶原含量、改良Nussvuaum组织学评分均优于A组,差异有统计学意义(P<0.05),D组优于B、C组,差异均有统计学意义(P<0.05);TGF-β灰度值C组与D组高于A组与B组,差异均有统计学意义(P<0.05).结论 联合运用川芎嗪和自体骨膜能有效预防硬膜外瘢痕粘连和形成,比单独应用川芎嗪和(或)自体骨膜预防效果好.  相似文献

Fibrin glue inhibits intra-abdominal adhesion formation   总被引:4,自引:0,他引:4  
To determine the effect of fibrin glue on intra-abdominal adhesion formation, 45 rats were randomized to three groups. Each animal received two adhesion models. Group 1 received no further treatment. Groups 2 and 3 had the adhesion models covered with fibrinogen from fresh frozen plasma (1.77 g/L) and cryoprecipitate (23.6 g/L), respectively. In group 1, 13 of 15 rats had high-grade adhesions in both models. In group 2, high-grade adhesions were noted in nine of 15 rats in model 1 and in 12 of 15 rats in model 2. In group 3, however, high-grade adhesions were seen in only three of 15 rats in model 1, with 11 rats having no adhesions, and in only two of 15 rats in model 2. Histologic analysis suggested accelerated healing in group 3. We conclude that (1) fibrin glue inhibits intra-abdominal adhesion formation in rats, (2) the inhibitory action is dependent on the fibrinogen concentration of the fibrin glue, and (3) adhesions are reduced by fibrin glue regardless of whether the peritoneal defect is closed.  相似文献

Background It has been well established that the immediate postoperative intraperitoneal administration of chemotherapeutic agents such as 5-fluorouracil (5-FU) after curative colon resection for colon cancer destroys disseminated cancer cells and inhibits micrometastases but also inhibits anastomotic healing. On the other hand, the application of fibrin glue constitutes a physical barrier around the anastomosis and may prevent anastomotic leakage. The purpose of this experimental study was to determine the effect of 5-FU plus interferon (IFN)-α-2a on the integrity of colonic anastomoses covered with fibrin glue when injected intraperitoneally immediately after colon resection. Materials and Methods Sixty rats were randomized to one of four groups. After resection of a 1-cm segment of the transverse colon, an end-to-end sutured anastomosis was performed. Rats of the control and the fibrin glue groups were injected with 6 ml of 0.9% sodium chloride (NaCl) solution intraperitoneally. Rats in the 5-FU + IFN and the 5-FU + IFN + fibrin glue groups received 5-FU plus IFN intraperitoneally. The colonic anastomoses of the rats in the fibrin glue and in the 5-FU + IFN + fibrin glue groups were covered with fibrin glue. All rats were sacrificed on the 8th postoperative day, and the anastomoses were examined macroscopically. The bursting pressure measurements were recorded, and the anastomoses were graded histologically. Results Only the 5-FU + IFN group had anastomoses rupture, and the rupture rate (33%) in this group was significantly greater than in the other groups, where there were no ruptures (P = 0.015). The adhesion formations score was, on average, significantly higher in rats of the 5-FU + IFN group compared with the control group (P = 0.006) and the 5-FU + IFN + fibrin glue group (P = 0.010). Bursting pressures were significantly lower in the control group when compared to the fibrin glue and 5-FU + IFN + fibrin glue group (P < 0.001). Rats in the 5-FU + IFN + fibrin glue group developed significantly more marked neoangiogenesis than rats in the other groups. Inflammatory cell infiltration, collagen deposition, and fibroblast activity did not differ significantly among the four groups (P = 0.856, P = 0.192 and P = 0.243, respectively). Conclusion The immediate postoperative intraperitoneal administration of 5-FU plus IFN impairs colonic healing. However, when the colonic anastomoses were covered with fibrin glue, the injection of 5-FU plus IFN had no adverse effects on the integrity of the anastomoses.  相似文献

Objective: To determine the efficacy of polylactic acid glue in preventing epidural scar adhesion after laminectomy in rabbits. Methods: Twenty-four Japanese white rabbits underwent laminectomy (including the attached ligaments) at Lz and Ls. After laminectomy at Ls, polylactic acid glue was sprayed on the dura and nerve roots and this segment was taken as the experimental group. After laminectomy at Lz, nothing was used and this segment was enrolled as the self control group. Four rabbits were killed every two weeks postoperatively till the end of the experiment at 12 weeks. Then the operated spine was observed grossly, histologically and ultrastructurally to check the degree of scar formation, the status of epidural scar adhesion, the absorption of the glue, and the intraceHular structure of fibroblasts. Results: The glue coagulated immediately after spraying and showed excellent hemostatic effect. The glue membrane was easy to be taken away from the dura mater of the samples for 2 weeks and there were no cells in the epidural space in the experimental group. But the dura mater was covered by hematoma in the control group,which formed mild adhesion, with fibroblasts proliferating actively. In the 4th week, some glue shivers remained in the epidural space with fibroblasts increasing a little, and the dura mater was smooth in the experimental group. However, in the control group, the formed scar was fragile and conglutinated with the dura mater diffusely and fibroblasts were much more than those in the experimental group. In the 6th-12th weeks, there was a potential interspace between the scar and the dura mater, and the polylactic acid glue was absorbed completely in the experimental group. Much tough scar was found in the control group, which was very difficult to dissect from the dura mater and the surrounding tissues. From the ultrastructural observation of the fibroblasts, the nucleus became much bigger and the rough endoplasmic reticulum was much more plentiful in the control group than that in the experimental group. Conclusions: Polylactic acid glue can effectively reduce epidural cicatrization and adhesion.  相似文献

目的 比较纤维蛋白胶粘合法与神经外膜缝合法修复周围神经损伤的效果。方法 将雄性Wistar大鼠20只随机分为两组,即粘合组和缝合组,每组10只。以大鼠左侧坐骨神经为修复神经模型进行实验。术后8周,各组取8只大鼠进行电生理、组织学检查,每组其余2只取标本做透射电镜观察。结果 术后8周粘合组神经纤维较缝合组平直,粘合组与缝合组比较,运动神经潜伏期延迟比、吻合口神经纤维通过比、有髓纤维截面积恢复比、肌湿重恢复比差别具有显著性意义(t值分别为2.32、2.43、2.39、2.36,P值分别为0.032、0.029、0.030、0.031)。结论 应用纤维蛋白胶粘合修复周围神经损伤是一种较为有效的神经接合方法。  相似文献

The effect of fibrin glue on inhibition of pericardial adhesions was tested using 26 beagle dogs. Dacron patches were sutured to the heart and tincture of iodine was applied to promote adhesions. Fibrin glue (3 ml) was sprayed over the patches in 15 dogs (test group), and was not separated in the remaining 11 dogs (control group). All animals in the test group had minimal adhesions between the pericardium and the epicardium or patched region, and an accumulation of gelatinous material was found in the subpericardial space. Marked fibrosis and a poor demarcation of the subpericardial space were found in the control group. The adhesion score and the visibility of coronary anatomy in the test group were significantly better than in the control group. The tension strength in the test group was significantly less than in the control group. We concluded, therefore, that fibrin glue may also be useful as an adhesion inhibitor.  相似文献

目的研究联合应用生长激素(GH)和生物蛋白胶(FS)对术后早期腹腔化疗条件下结肠吻合口愈合的影响。方法Wister大鼠48只,随机分为4组:对照组,单纯行肠切除吻合;腹腔化疗组(5-FU组),术后连续3 d腹腔内注射氟尿嘧啶(5-FU)(20 mg·kg~(-1)·d~(-1));FS组,术中应用FS、术后给予早期腹腔化疗;GH联合FS组,术中应用FS、术后给予GH(1 mg·kg~(-1)·d~(-1))及早期腹腔化疗。术后8 d处死各组大鼠,剖腹切取吻合口进行评分(评分越高提示吻合口与周围组织的粘连越重)和测量吻合口破裂压力及进行吻合口组织学评分。结果GH联合FS组的吻合口评分2.00,低于FS组(2.13)和对照组(2.38),但P>0.05,差异无统计学意义;GH联合FS组显著低于5-FU组的2.75,P<0.05;差异有统计学意义。GH联合FS组的吻合口破裂压力229.9475,高于对照组(199.7525)和FS组(199.4338),P<0.05;显著高于5-FU组(171.7938),P<0.01。GH联合FS组的吻合口组织学评分2.9,高于FS组(2.3)和对照组(2.1)及5-FU组(1.7),P<0.05。结论联合应用GH和FS能提高吻合口愈合强度,促进术后早期腹腔化疗条件下结肠吻合口的愈合。  相似文献
