BackgroundTechnological resources, such as smartphones can contribute to the quantitative assessment of posture.PurposesTest the validity and reliability of using a postural assessment application to quantify the frontal plane knee posture in orthostatism and to test the influence of the use of external markers on the precision of this measure.DesignMethodological study.MethodsThe knee frontal plane posture of 30 volunteers were analyzed by two independent examiners. The photographs were taken with different external marker arrangements. The photographs were analyzed at two moments using the Kinovea software and PhysioCode Posture (PCP) application. Reliability was analyzed using the intraclass correlation coefficient (ICC) between measures with each instrument conducted at two moments with a 7-day interval. Concurrent validity of PCP with Kinovea measure was analyzed using Pearson's correlation coefficient. Standard error measurement (SEM), minimum detectable change (MDC) and Bland Altman plots were analyzed.ResultsPCP demonstrated excellent intra-rater [ICC = 0.92 (95% confidence interval [CI] 0.90–0.93)] and inter-rater [ICC = 0.88 (95%CI 0.85–0.90)] reliability. Concurrent validity analysis showed excellent agreement between PCP and Kinovea software (r = 0.88). The use of markers, independent of positioning, did not influence the measurement properties of measures with both softwares. The SEM was inferior to 1.2°, and the MDC was below 2.85°. No systematic errors were observed in the Bland Altman graphs.ConclusionsThe use of PCP application to measure knee posture was valid and demonstrated excellent intra- and inter-rater reliability levels. The use of external markers did not influence the measurement.  相似文献

[Purpose] Postural problems of adolescents needs to be evaluated accurately because they may lead to greater problems in the musculoskeletal system as they develop. Although photographic posture analysis has been frequently used, more simple and accessible methods are still needed. The purpose of this study was to investigate the inter- and intra-rater reliability of photographic posture analysis using MB-ruler software. [Subjects and Methods] Subjects were 30 adolescents (15 girls and 15 boys, mean age: 16.4±0.4 years, mean height 166.3±6.7 cm, mean weight 63.8±15.1 kg) and photographs of their habitual standing posture photographs were taken in the sagittal plane. For the evaluation of postural angles, reflective markers were placed on anatomical landmarks. For angular measurements, MB-ruler (Markus Bader- MB Software Solutions, triangular screen ruler) was used. Photographic evaluations were performed by two observers with a repetition after a week. Test-retest and inter-rater reliability evaluations were calculated using intra-class correlation coefficients (ICC). [Results] Inter-rater (ICC>0.972) and test-retest (ICC>0.774) reliability were found to be in the range of acceptable to excellent. [Conclusion] Reference angles for postural evaluation were found to be reliable and repeatable. The present method was found to be an easy and non-invasive method and it may be utilized by researchers who are in search of an alternative method for photographic postural assessments.Key words: Adolescents, Posture assessment, Reliability  相似文献

ObjectiveThe purpose of this study was to propose a quantitative evaluation for mandibular opening–closing movement asymmetries and to verify the intraexaminer and interexaminer reliability using photogrammetry in individuals with and without myogenic temporomandibular disorders.MethodsForty-nine female participants between ages 18 and 40 were enrolled in this study. They were assigned to 2 different groups: a temporomandibular disorder group, (n = 25; 28.1 ± 3.6 years) and an asymptomatic group (n = 24; 25.6 ± 5.1 years). Data were collected through photogrammetry using Corel Draw X3 software (Corel Corp, Ottawa, Ontario, Canada) for angle measurements. Reliability analysis was done on the total sample, and the photographs were obtained by a singular examiner on 2 occasions (intraexaminer) 1 month apart and from measurement made by another examiner (interexaminer) on different days. The intraclass correlation coefficient (ICC) was applied with a significance level of 5%.ResultsThe photogrammetry had excellent intrarater and inter-rater reliability for the evaluation of opening and closing movements of the jaw (intrarater: opening ICC = 0.99; closing ICC = 0.98; inter-rater: opening ICC = 0.89 and closing ICC = 0.82). Photogrammetry also demonstrated excellent intra- and inter-rater reliability in the evaluation of head posture (intra-rater: head deviation ICC = 0.96; head position ICC = 0.75; inter-rater: head deviation ICC = 0.98; head position ICC = 0.98).ConclusionUnder these experimental conditions, most angular values presented excellent intra- and interexaminer reliability.  相似文献

[Purpose] Assessment of posture is an important goal of physical therapy interventions for preventing the progression of forward head posture (FHP). The purpose of this study was to determine the inter- and intra-rater reliabilities of the assessment of FHP. [Subjects and Methods] We recruited 45 participants (20 male subjects, 25 female subjects) from a university student population. Two physical therapists assessed FHP using images of head extension. FHP is characterized by the measurement of angles and distances between anatomical landmarks. Forward shoulder angle of 54° or less was defined as FHP. Intra- and inter-rater reliabilities were estimated using Kendall’s Taub correlation coefficients. [Results] Intra-class correlation of intra-rater measurements indicated an excellent level of reliability (0.91), and intra-class correlation of inter-rater measurements showed a good level of reliability in the assessment of FHP (0.75). [Conclusion] Assessment of FHP is an important component of evaluation and affects the design of the treatment regimen. The assessment of FHP was reliably measured by two physical therapists. It could therefore become a useful method for assessing FHP in the clinical setting. Future studies will be needed to provide more detailed quantitative data for accurate assessment of posture.Key words: Forward head posture, Reliability, Posture assessment  相似文献

The WorkHab Functional Capacity Evaluation (FCE) is widely used in Australian workplace injury management and occupational rehabilitation arenas; however, there is a lack of published literature regarding its reliability and validity.

Purpose.?This study investigated the intra- and inter-rater reliability of the manual handling component of this FCE.

Method.?A DVD was produced containing footage of the manual handling components of the WorkHab conducted with four injured workers. Therapist raters (n == 17) who were trained and accredited in use of the WorkHab FCE scored these components and 14 raters re-evaluated them after approximately 2 weeks. Ratings were compared using intraclass correlation coefficients (ICCs), paired sample t-tests (intra-rater), chi-squared (inter-rater) and percentage agreement.

Results.?Intra-rater agreement was high with ICCs for the manual handling components and manual handling score showing excellent reliability (0.94–0.98) and good reliability for identification of the safe maximal lift (ICC: 0.81). Overall inter-rater agreement ranged from good to excellent for the manual handling components and safe maximal lift determination (ICC > 0.9). Agreement for safe maximal lift identification was good.

Conclusions.?Ratings demonstrated substantial levels of intra-rater and inter-rater reliability for the lifting components of the WorkHab FCEs.  相似文献

ObjectiveA high prevalence of poor standing posture among elementary schoolchildren indicates the need for the introduction of school-based interventions for improvement of postural alignments. For assessing the effect of intervention programs, reliable quantitative measures of standing posture should be used. Therefore, the purpose of this study was to examine the reliability of a photographic method for assessment of standing posture among elementary schoolchildren.MethodsThe study was conducted on a convenience sample of 273 male students between 10 and 13 years old. Each subject was photographed in habitual standing posture, 3 times in a front view and 3 times in a side view. Deviations from ideal postural alignment were calculated with Posture Image Analyzer software and UTHSCSA ImageTool software. Interitem reliability was estimated using intraclass correlation coefficient.ResultsIntraclass correlation coefficients for posture deviations assessed with Posture Image Analyzer ranged from 0.81 for knee joints deviation and ankle joints deviation in the coronal plane to 0.92 for trunk deviation and knee joints deviation in the sagittal plane. Intraclass correlation coefficients for posture deviations assessed with UTHSCSA ImageTool ranged from 0.80 for knee joints deviation in the coronal plane to 0.91 for trunk deviation and knee joints deviation in the sagittal plane.ConclusionsThis study showed a satisfactory interitem reliability of a photographic method for the assessment of standing posture among elementary schoolchildren.  相似文献

The Modified Head Posture Spinal Curvature Instrument (MHPSCI) is an extension of the Head Posture Spinal Curvature Instrument. Two specific modifications were made in the original design by adding a third arm projecting horizontally from the protractor to objectively fix the pivot exactly over the C7 vertebra and the addition of a spirit-level to properly align the instrument. In order to demonstrate reliability and validity, this study was conducted using patients with postural neck pain (N = 65) and healthy subjects (N = 20). All the subjects were working at a selected Information Technology Industry in India and had been recruited using a criterion-based sampling approach. The craniovertebral (CV) angle of each subject was evaluated by two raters consecutively. The measurements were taken by using both MHPSCI and the standard photographic method in a standardized sitting posture for the purpose of establishing criterion-validity of the instrument. The results of this study indicate a good inter-rater reliability (ICC = 0.76; CI = 0.65–0.84) as well as intra-rater reliability (ICC = 0.87; CI = 0.82–0.91) between three successive CV angle measurements (with 2 minutes interval between each measurement) through MHPSCI. While keeping the digital photographic measurement as a standard, this study established that the MHPSCI is a valid tool for measuring the CV angle as shown by non-significant difference (p > 0.01) and high correlation between the two methods (r = 0.79–0.84). This study demonstrates that the MHPSCI is a reliable and valid instrument for measuring CV angle in subjects with or without postural neck pain.  相似文献

The use of ultrasound (US) to perform quantitative measurements of musculoskeletal tissues requires accurate and reliable measurements between investigators and ultrasound machines. The objective of this study was to evaluate inter-rater and intra-rater reliability of patellar tendon measurements between providers with different levels of US experience and inter-machine reliability of US machines. Sixteen subjects without a history of knee pain were evaluated with US examinations of the patellar tendon. Each tendon was scanned independently by two investigators using two different ultrasound machines. Tendon length and cross-sectional area (CSA) were obtained, and examiners were blinded to each other's results. Tendon length was measured using a validated system involving surface markers and calipers, and CSA was measured using each machine's measuring software. Intra-class correlation coefficients (ICCs) were used to determine reliability of measurements between observers, where ICC > 0.75 was considered good and ICC > 0.9 was considered excellent. Inter-rater reliability between sonographers was excellent and revealed an ICC of 0.90 to 0.92 for patellar tendon CSA and an ICC of 0.96 for tendon length. ICC for intra-rater reliability of tendon CSA was also generally excellent, with ICC between 0.87 and 0.96. Inter-machine reliability was excellent, with ICC of 0.91–0.98 for tendon CSA and 0.96–0.98 for tendon length. Bland–Altman plots were constructed to measure validity and demonstrated a mean difference between sonographers of 0.03 mm2 for CSA measurements and 0.2 mm for tendon length. Using well-defined scanning protocols, a novice and an experienced musculoskeletal sonographer attained high levels of inter-rater agreement, with similarly excellent results for intra-rater and inter-machine reliability. To our knowledge, this study is the first to report inter-machine reliability in the setting of quantitative musculoskeletal ultrasound.  相似文献

The aim of this reliability study was to identify the clinimetric properties, specifically intra- and inter-rater reliability, for measuring the functionally and clinically important hand behind back (combined shoulder internal rotation/adduction and elbow flexion) range of motion using a modified technique. Sixty asymptomatic participants (20 male, 40 female) aged 45.4 ± 11.7 years (mean ± SD). Hand behind back was measured as the distance from the mid-line between the posterior superior iliac spines (PSIS) to the tip of the thumb and recorded in centimetres above the PSIS (a positive measure) or below the PSIS (a negative measure). The intra- and inter-rater reliability for the measurement were excellent, with ICC (2,1) of 0.95 for intra-rater and ICC (2,2) of 0.96 for inter-rater reliability. The standard error of the measurement (SEM) and smallest real difference (SRD) values showed acceptable levels of measurement error, SEM 4.3 mm and SRD 12.8 mm for intra-rater reliability and SEM 2.6 mm and SRD 7.7 mm for inter-rater reliability. The assessment of hand-behind-back motion from a point equidistant between the PSISs to the tip of the thumb is more reliable than traditional methods and a useful clinical tool.  相似文献

Reliable methods of measuring turnout in dancers and comparing active turnout (used in class) with functional (uncompensated) turnout are needed. Authors have suggested measurement techniques but there is no clinically useful, easily reproducible technique with established inter-rater and intra-rater reliability. We adapted a technique based on previous research, which is easily reproducible. We hypothesized excellent inter-rater and intra-rater reliability between experienced physical therapists (PTs) and a briefly trained faculty member from a university’s department of dance. Thirty-two participants were recruited from the same dance department. Dancers’ active and functional turnout was measured by each rater. We found that our technique for measuring active and functional turnout has excellent inter-rater and intra-rater reliability when performed by two experienced PTs and by one briefly trained university-level dance faculty member. For active turnout, inter-rater reliability was 0.78 among all raters and 0.82 among only the PT raters; intra-rater reliability was 0.82 among all raters and 0.85 among only the PT raters. For functional turnout, inter-rater reliability was 0.86 among all raters and 0.88 among only the PT raters; intra-rater reliability was 0.87 among all raters and 0.88 among only the PT raters. The measurement technique described provides a standardized protocol with excellent inter-rater and intra-rater reliability when performed by experienced PTs or by a briefly trained university-level dance faculty member.  相似文献
