In a review of the organisation of higher medical training in Britain leading to accreditation for those intending to specialize in venereology within the National Health Service suggestions are made for improving the present system. These include the enrollment of trainees in general medicine as well as in venereology to ensure a broad general experience of medicine. The establishment of an agreed curriculum in venereology for trainees is thought desirable. Improvement in undergraduate education in venereology is necessary if recruitment of trainees to the specialty is to be increased. The diploma courses in venereology are mainly attended by foreign graduates; the numbers are likely to be reduced by the recent heavy increases in fees. Although adequate practical experience of the tropical venereal diseases cannot be provided, these courses give a good general experience of venereology as ideally practised, much of which is relevant to the conditions under which the overseas trainees will have to work on return to their own countries. Secondment of our own trainees work in centres in developing countries as an approved part of their training would benefit both them and their hosts.

Although, at present dermatology and venereology in the United Kingdom and Eire are practiced separately, the influence of the European Economic Community may cause certain changes. This article refers to events over the course of centuries which have caused the disciplines to develop separate paths.

Andrology evolved more than 100 years ago in order to create an equivalent to gynecology for specific male problems in reproductive medicine. The first stimuli for the clinical and scientific development of this specialty in Germany arose from dermatologists. Andrology thus appears as a moiety of dermatology. In 2003 the visibility of andrology among the medical disciplines was accentuated by the introduction of an additional specialty specification for dermatologists, endocrinologists and urologists, since the other two disciplines also worked on andrological problems. Today, the majority of the members of the Society of Andrology are urologists and also urologists form the majority of medical practioners with this additional specialty. Besides reproductive medicine, there are several other topics which underline the conjunction of dermatology and andrology. Hence andrology should be observed as a part of dermatology not only for historical reasons. Andrology is a living subspecialty of dermatology and young dermatologists should be motivated to become engaged in it.

Change within dermatology as a clinical discipline is expected and inevitable. However dermatology may change as a medical specialty in the new millennium, there will still be patients with medical dermatologic disease whose optimal care will depend on skin disease specialists' having the highest level of training and experience in medical dermatology. Dermatologists who have subspecialized in medical dermatology will provide the role models for new generations of dermatologists, perform the patient-oriented research, and care for the more complicated patients. Thus, if during its evolution, dermatology loses the ability to train and support medical dermatologists, it will be weakened as the discipline that can best care for skin disease. Clearly, the loss of talented academicians such as the person whose career was outlined in the case report presented at the beginning of this article should be a huge warning sign that the future of medical dermatology as a specialty is uncertain. The Medical Dermatology Society hopes to develop a coalition with all other leadership organizations within dermatology to deal with this problem effectively. There is a need for a broader discussion within organized dermatology of the growing crisis in this area and how all dermatology leadership organizations working together can develop an action plan for effectively dealing with this important but challenging problem. Dermatology must ask itself what it wants to look like as a medical specialty in the future. Without an steady stream of young clinician-investigators focused on the many challenging problems in medical dermatology, dermatology will not exist as the specialty it is today.

This is a tribute to one of the fathers of modern European dermatology and venereology, Dr Joseph Plenck, the Viennese protodermatologist of two hundred years ago, 1735-1807, who was professor and secretary of the Imperial Medical Josephine Academy, where today is the Institute for the history of medicine. Plenck introduced the systematization of dermatovenereological diseases based on their paradigmatic differences, which is deemed as "the only (system) with pretensions to accuracy". A man of diversified interests and a fruitful author, he wrote many treatises covering various fields from dermatology, venereology, and dentistry to surgery, obstetrics, anatomy, pediatrics, pharmacology, and botany. This remarkable physician definitely left a deep trace in the history of dermatology and venereology, and marked the transition from text-based to visually dependent culture in the field of dermatovenereology.

The Belgrade dermatovenereologic moulge collection is formed during the period between 1925 and 1958, with one break between 1940 and 1950. The first moulages have been created after the foundation of the Belgrade University's School of Medicine and the Institute of Dermatology and Venereology (IDV), when this form of education became very significant. The collection is now comprised of 350 moulages, of which 280 pieces are completely preserved, whereas 70 are previously damaged. This article describes the work of two authors of moulages: one was Dr. Sergej Pavlovic Alisov, physician, amateur sculptor and painter, and the other was academic sculptor Vojislav Sikoparija. Wide variety of presented cases of skin and venereal diseases have been analysed; before World War I, there was a high proportion and florid pathology of venereal diseases and leprosy, whereas after World War II, these diseases became infrequent. Moulages at the IDV display high authenticity, which in some cases, made possible the correction of original diagnosis according to current criteria. Moulages represent a very significant subject of medical profession, and the need of an expert opinion on the conservation and reparation of moulages, which could be implemented by those in possession of such collections, is emphasized.

Decrease or loss of sexual function in many chronic diseases has recently attracted significant attention owing to its impact on quality of life. Generic and disease-specific quality-of-life questionnaires measure changes in work, school, social life and emotional status regarding the disease and its treatment. Specific questionnaires have been designed to evaluate changes in sexuality and sexual function. Sexual dysfunction, especially female sexual dysfunction, in different diseases became a popular and important health concern in recent years. There are a lot of studies about sexual dysfunction in the areas of other specialities of medicine, but there are only a few studies in dermatological diseases. In this paper, sexual dysfunction and the studies performed about this subject in dermatology will be reviewed.

Chronic skin diseases may result in a variety of psychological problems, including distress, demoralization, poor self esteem, sleep disturbances, social phobia, anxiety, depression and obsessive‐compulsive disorder. It is known that skin diseases may also cause sexual problems. Sexual activity remains important for most men throughout their adult lives and into old age. Impairment in their sexual experience can cause significant personal and interpersonal distress at any age. Sexual dysfunction in chronic systemic diseases has become a popular and important health concern in recent years. This subject is quite new in the specialty of dermatology. We explore the relationships between dermatologic diseases and male sexual dysfunction.
