The increased use of interventional procedures and laparoscopic cholecystectomy in the management of hepatobiliary disorders is associated with an increased incidence of hemobilia and hepatic artery aneurysm. Here we report a case of hepatic artery pseudoaneurysm associated with a plastic biliary stent. Multiple factors were involved in the formation of the hepatic artery aneurysm (HAA) and it was successfully treated by embolization.  相似文献

We report a case of successful endovascular treatment of bilateral carotid artery occlusion with concurrent basilar apex aneurysm. An elderly female patient with subarachnoid hemorrhage (SAH) onset was admitted to the hospital. Computed tomography (CT) and digital subtraction angiography (DSA) confirmed the presence of bilateral carotid artery occlusion with concurrent basilar apex aneurysm. Brain blood supply was provided by the bilateral vertebral artery through the basilar artery. We treated the aneurysm with the endovascular approach by embolizing the aneurysm with three coils. The patient recovered well after surgery and showed no recanalization of the aneurysm on a one-year follow-up DSA. We also reviewed six similar cases found with a PUBMED database search (1980-2010), including those with bilateral common carotid artery occlusion. In conclusion, by using the endovascular approach, bilateral carotid artery occlusion with concurrent basilar apex aneurysm was efficiently treated.  相似文献

PURPOSE: The goal of this study was to directly measure the association between the internal carotid artery (ICA) morphometry and the presence of ICA-posterior communicating artery (PCOM) aneurysm. MATERIALS AND METHODS: The authors intraoperatively measured the length of the supraclinoid ICA because it is impossible to radiologically determine the exact location of the anterior clinoid process. We used an image analyzer with a CT angiogram to measure the angle between the skull midline and the terminal segment of the ICA (ICA angle), as well as the diameter of the ICA. The lengths and diameters of the supraclinoid ICA and the ICA angle were compared among PCOM aneurysms, anterior communicating artery (ACOM) aneurysms, and middle cerebral artery (MCA) bifurcation aneurysms (n=27 each). Additionally, the lengths and the diameters of M1 and A1 were compared for each aneurysm. RESULTS: The lengths of the supraclinoid ICA were 11.9+/-2.3 mm. The lengths of the supraclinoid ICA in patients with ICA-PCOM aneurysms (9.7+/-2.8 mm) were shorter than those of patients with ACOM aneurysms (13.8+/-2.2 mm, Student's t-test, p<0.001) and with MCA bifurcation aneurysms (12.2+/-1.9 mm, Student's t-test, p<0.001). The diameters of the supraclinoid ICA and A1 in patients with ACOM aneurysms were larger than those in patients with MCA bifurcation aneurysms (Student's t-test, p<0.05). There were no significant differences in the lengths of M1 and A1, ICA angle, or diameter of M1 for each aneurysm. CONCLUSION: These results suggest that the relatively shorter length of the supraclinoid ICA may be a novel risk factor for the development of ICA-PCOM aneurysm with higher hemodynamic stress.  相似文献

目的探讨手术夹闭存在胚胎型大脑后动脉的后交通动脉瘤(PCoAA)预后的影响因素。方法选择徐州医科大学附属医院神经外科2015年1月至2019年1月经显微手术夹闭合并胚胎型大脑后动脉的PCoAA患者80例,术后6个月采用改良Rankin量表评价患者预后,采用单因素分析和多因素Logistic回归分析影响患者预后的因素。结果性别、动脉瘤侧别、宽颈、动脉瘤大小、脑梗死史、手术时机、高血压史对手术预后影响不大;而年龄、Hunt-Hess分级、改良Fisher分级是影响手术预后的危险因素。结论年龄、术前Hunt-Hess分级以及改良Fisher分级是患者预后不良的独立影响因素;对于年龄不大、Hunt-Hess低分级、改良Fisher低分级这类患者,更早进行显微外科手术治疗,安全性更高,预后更好。  相似文献

We report a rare case of lower limb swelling due to compression of the superficial femoral vein by a solitary deep femoral artery aneurysm. The patient was a 58-year-old man presenting with acute swelling of the right lower limb caused by deep venous thrombosis. A multi-detector computed tomographic scan (CT) confirmed the diagnosis of a deep femoral artery aneurysm and revealed no evidence of aneurysms or occlusive lesions in the other arteries. Aneurysmectomy and ligation of the deep femoral artery were successfully performed. Preoperative multi-detector CT scanning is a valuable, non-invasive diagnostic tool to delineate not only the state of the aneurysm, but also that of the distal arterial tree.  相似文献

目的探讨锁孔神经外科手术治疗后交通动脉瘤的手术技巧和适应证。方法回顾性分析32例破裂的大脑中动脉瘤病例,全部经DSA或CTA确诊,出血2周后行眉弓上锁孔开颅,显微神经外科手术动脉瘤夹闭。结果 32例全部夹闭,无1例死亡,治愈率100%。术后1周内头部CTA,动脉瘤消失,载瘤动脉通畅,动脉瘤夹固定良好。10例于术后3~6个月进行了随访(CTA或DSA),未见复发。结论锁孔手术是治疗大脑中动脉瘤的微创治疗技术,减少了手术创伤及并发症,恰当地选择适应证和手术时机及应用显微外科技术是治疗的关键。  相似文献

目的 :显示对比正常侧和病变侧颈内动静脉与周围骨结构的解剖关系 ,以完善术前准备。方法 :10例颌颈部肿瘤患者 ,术前行颈动脉三维CT血管成像 (CTA) ,同步注射造影剂 ,螺旋CT对病变区血管段进行重点薄层螺旋扫描 ,由EasyVision工作站三维重建。结果 :5例颈外动脉和颈内静脉受压狭窄 ,其中 3例出现颈总动脉、颈内动脉向内侧移位 ,另 1例左侧颈内动脉动脉瘤清晰定位。CTA能够显示病变区血管段 (包括颈总动脉、颈内动脉、颈外动脉、颈内静脉 )与下颌骨、舌骨、颈椎、颅底骨结构的三维解剖关系 ,进行旋转、放大 ,三维测量 ,多视角观察 ,无重叠干扰。结论 :对颌颈、口咽部肿瘤患者术前CTA检查 ,能够判断颈部大血管的情况及颈部巨大肿瘤造成颈内动脉移位 ,CTA可显示血管性肿瘤的三维形状和与颅底骨空间关系 ,为手术入路及术式提供参考 ,有助于降低手术风险  相似文献

Coronary artery aneurysm or ectasia was reported in approximately 15% to 25% of the affected children, particularly in the proximal end of the main blood vessel and the left anterior descending part. Rare patients have been reported with aneurysm in the distal end of the right coronary artery. In this case report, we present a rare case with aneurysm in the distal end of the right coronary artery. Multi-slice computed tomography was performed for the coronary angiography. Aspirin (10 mg/kg body weight per day) and gamma globulin (2 kg/kg body weight) was administrated via intravenous injection. The patient is currently in a healthy status with a 12-month follow up.  相似文献
