Twenty patients with periappendiceal abscesses diagnosed by CT underwent percutaneous catheter drainage. Eighteen (90%) of the 20 patients were successfully treated by percutaneous drainage and antibiotic therapy alone. In two of these patients, however, small recurrent abscesses were diagnosed by CT 4 and 6 weeks after drainage, but these abscesses resolved with additional antibiotic therapy. Surgery was performed in two patients in whom percutaneous drainage failed. One was a diabetic patient who had a gas-forming abscess that extended into the posterior pararenal space. Surgery was necessary in a second patient because of a technical failure of catheter insertion, which resulted in perforation of the abscess cavity and peritonitis. Abscess sinograms in eight (44%) of 18 patients showed fistulas between the abscess cavity and the base of the cecum or appendix. However, the fistulas did not pose a problem in clinical management, and all closed within 14 days. Percutaneous drainage is an effective alternative to surgery in selected patients with periappendiceal abscesses.  相似文献

Internal drainage was performed with expandable metallic stents in a 79-year-old male with lung abscess secondary to bronchogenic carcinoma. Initial external drainage was prolonged, and internalization of drainage allowed removal of the external drainage catheter. Connection of abscess cavity to obstructed bronchus was created after passing a steerable guidewire percutaneously through the abscess into the bronchus. This new type of internal drainage technique may be applicable to patients with prolonged standard percutaneous external drainage of lung abscesses.  相似文献

Percutaneous drainage of multiseptated hepatic abscess   总被引:3,自引:0,他引:3  
Thirteen patients with multiseptated, hepatic abscesses were initially treated by percutaneous drainage using computed tomographic guidance. One of these 13 patients required surgery because of failure of percutaneous drainage due to the viscosity of the debris within the multiseptated cavities. Another patient required a second percutaneous drainage procedure due to recurrent hepatic fluid collection. Eleven of the 12 successful drainages were accomplished with one catheter because there was communication between the multiseptated "pockets." Our approach to hepatic abscess drainage is discussed with specific attention to the multiseptated abscess. The multiseptated abscess should not discourage an attempt at percutaneous catheter drainage.  相似文献

 目的 对比研究超声引导下经皮肝穿刺细针抽吸术与置管引流术治疗细菌性肝脓肿的疗效。方法 细菌性肝脓肿98例,其中直径≥5 cm 46例,直径<5 cm 52例。对以上两种大小的脓肿,分别采取超声引导下经皮肝穿刺细针抽吸术和置管引流术两种方法治疗,比较两种方法治疗后脓肿消失时间、患者住院时间、血象、体温恢复正常时间等。结果 直径≥5 cm肝脓肿,置管引流术组在住院天数、脓腔消失时间等方面优于细针抽吸术组(P<0.05),在术后血象、体温恢复正常时间方面两种方法无统计学差异(P>0.05)。直径<5 cm肝脓肿,两种方法在住院天数、脓腔消失时间、术后血象、体温恢复正常时间方面无统计学差异(P>0.05)。结论 对于直径≥5 cm、直径<5 cm的细菌性肝脓肿,应分别首选超声引导下经皮肝穿刺置管引流术、细针抽吸术。  相似文献

Ultrasonography and computed tomography were used to diagnose, localize, and guide the percutaneous drainage of 24 intraabdominal and retroperitoneal abscesses in 23 surgical and medical inpatients 16 of whom had abscess formation as a postoperative complication. On the basis of anatomic detail from sectional imaging, safe percutaneous drainage routes similar to surgical approaches were defined. After confirmation by needle aspiration, one of two methods of percutaneous catheter introduction were used: (1) a modified Seldinger technique for placement of an 8 French pigtail catheter and (2) a trocar catheter technique for placement of a 12 or 16 French catheter. Once in position, the catheters were managed as a surgically placed drain. In 22 of 24 cases, percutaneous catheter drainage was curative with no surgery required. There were no deaths or recurrences with an average follow-up period of 12 months ranging from 1 week to 3 years. Percutaneous aspiration and drainage should be considered as an alternative to surgery in the treatment of intraabdominal or retroperitoneal abscess.  相似文献

Experience with percutaneous drainage in four cases of splenic abscess is presented. Percutaneous drainage and antibiotics were curative in three of four cases. The one case not adequately treated by antibiotics and percutaneous drainage was also complicated by ascites and multilocular collections. Percutaneous drainage in this case did not adversely affect subsequent curative surgery. Percutaneous catheter drainage of splenic abscess under radiologic guidance can be a safe, effective treatment.  相似文献

目的通过肝脓肿介入性超声不同治疗方法疗效对比分析,探讨肝脓肿的最佳治疗方法。方法将68例肝脓肿患者随机分为三组,在全身有效抗生素应用的同时,分别行超声引导下经皮穿刺抽脓后无水酒精冲洗治疗组(26例)、穿刺抽脓后抗生素冲洗治疗组(30例)、经皮穿刺置管引流组(12例)。观察并记录了其平均治愈时间、单病灶治疗次数、术后并发症及发烧患者术后体温恢复时间等,结果68例病人中,除一例合并肺脓疡的患者介入治疗效果不佳转入外科手术,并且术后死于并发症外,其余均治愈,无并发症出现,酒精治疗组平均治愈时间及术后体温恢复正常时间最短,置管引流组最长。三组之间差异有显著性(P<0.01)。结论超声引导下经皮穿刺无水酒精冲洗肝脓肿安全、经济、效果良好,值得进一步推广。  相似文献

The failure of medical therapy for amebic liver abscess may be followed by its perforation, a complication associated with high mortality. We assessed the role of percutaneous catheter drainage in management of the sequelae of ruptured amebic abscesses in 13 critically ill patients; 22 intrahepatic lesions, three of which were multiloculated, were drained. Catheters were also placed in 17 extrahepatic collections: pleural space (n = 5), subphrenic (n = 7), perihepatic/subhepatic (n = 3), greater sac of peritoneum (n = 2). No attempt at percutaneous drainage failed. Prompt resolution of clinical features following drainage was a uniform feature. Successful resolution of the abscesses occurred within 20 days in 11 patients. In the remaining two, catheters needed to be retained in situ for 35 and 50 days. The mean hospital stay was 15 days (range 10–20 days). 100% patient survival was achieved, without a single morbid episode. Our results suggest that patients with ruptured amebic abscesses can be effectively and safely managed by percutaneous catheter drainage irrespective of the extent of extrahepatic contamination.  相似文献

OBJECTIVES: To determine the utility of percutaneous computed tomography (CT) guided drainage of abscesses formed secondary to retained appendicoliths. MATERIALS AND METHODS: A retrospective review was conducted over a 5-year period to identify patients who underwent CT guided drainage of abscesses related to retained appendicoliths. Inclusion criteria were a history of prior proven appendicitis treated by laparoscopic appendicectomy, identification of a post-operative abscess related to a visualised retained appendicolith and initial treatment by CT guided drainage. Data regarding initial technical success as well as long-term outcome was recorded in each case. RESULTS: In total, five patients were identified who underwent CT guided abscess drainage related to retained appendicoliths. There were three males and two females (age range 12-54 years). Initial drainage under CT guidance was technically successful in all cases with successful catheter placement and resolution of the abscess cavity. In all five cases however, there was recurrence of abscess formation following catheter removal. In one case, a second attempt with CT guided drainage was performed. Again this was initially successful with abscess recurrence following catheter removal. In all five cases, formal surgical drainage with removal of retained appendicolith resulted in a successful outcome. CONCLUSION: CT guided percutaneous drainage of intra abdominal abscess secondary to retained appendicoliths is only successful in the short term. Formal surgical drainage and removal of the appendicolith is required for long-term success.  相似文献

Percutaneous abscess drainage was performed in 21 patients who had periappendiceal abscesses. Fifteen patients had de novo abscesses, while six patients had persistent postsurgical abscesses. Nineteen of the 21 percutaneous drainages were successful. After percutaneous abscess drainage, interval appendectomy was simple and uneventful in all 14 patients in whom it was performed; four patients had appendices removed prior to percutaneous abscess drainage, and three elderly patients have not required appendectomy (follow-up 1 1/2-3 1/2 years). Percutaneous catheter drainage of periappendiceal abscess performed with computed tomographic guidance is effective and safe. Its benefits include imaging demonstration of the abscess; avoidance of an operation for abscess drainage; temporization of extremely ill patients; simplification of appendectomy, which is made elective; obviation of all operations in selected patients (e.g., elderly or with cardiopulmonary disease); and reduction of hospital stay and cost.  相似文献

Eleven cases of inadvertent catheter insertion into the small bowel or stomach during percutaneous abdominal abscess drainage are reviewed. Recognition of the intraluminal catheter position was made by contrast-enhanced fluoroscopy in all patients 1-6 days after catheter insertion. No evidence of leakage of intestinal contents into the peritoneal cavity was noted either clinically or radiologically. All cases occurred after drainage of a centrally located midabdominal collection, but the ultimate outcome was not compromised. In nine of 11 cases, catheter drainage alone was sufficient; two cases required additional surgical drainage. Considerations for management include a prolonged period of catheter drainage to allow evolution of a fibrous tract and gradual catheter withdrawal. These data confirm the growing experience with purposeful percutaneous gastrostomy that percutaneous catheterization of the gastrointestinal tract can occur without major sequelae.  相似文献

Complicated pancreatic abscesses: problems in interventional management   总被引:7,自引:0,他引:7  
Twenty-five patients with grade D or E pancreatitis underwent percutaneous drainage. These patients required multiple computed tomography (CT) examinations, multiple catheter insertions, multiple catheter manipulations, and long-term catheter drainage. Eight of the 25 patients were successfully treated with catheter drainage alone. Sixteen underwent surgical drainage, ten after attempts at percutaneous drainage and six prior to radiologic drainage. Of the ten patients who had initial percutaneous drainage, only four were clinically improved from the drainage procedure alone. Although the fluid component of the abscess was often adequately drained in all ten patients, surgery was required to remove pieces of necrotic debris. Six patients who underwent surgical debridement had residual abscesses in the post-operative period and were all successfully treated with percutaneous drainage. One patient died from unrelated causes. Successful interventional management of patients with pancreatic abscesses requires intensive radiologic intervention and monitoring and may be better served by a combination of radiologic and surgical means.  相似文献

Twenty-four patients with acute sigmoid diverticulitis and associated pelvic fluid collections seen on computed tomographic scans underwent percutaneous catheter drainage as an adjunct to surgical therapy. Fourteen of the 24 underwent a single-stage surgical procedure within 10 days of drainage. Five patients required two-stage surgical procedures because localized inflammatory changes precluded a primary resection despite the absence of a residual abscess at surgery. Two of the three remaining patients initially had no surgery, but they had recrudescences of their symptoms that required surgical drainage within 8 months. One patient in whom surgical resection was deferred remained asymptomatic 10 months after percutaneous drainage. A retrospective review of 87 patients undergoing surgery for diverticulitis suggested that the percentage of two-stage surgical procedures has decreased in the last 5-10 years, but there remains a substantial number of patients who might benefit from percutaneous catheter drainage of diverticular abscess of the sigmoid colon.  相似文献

Percutaneous catheter drainage was performed in 16 patients with diverticulitis complicated by abscesses. Each patient had resolution of fever within 72 hours. Eleven patients subsequently underwent simultaneous sigmoid resection and operative anastomosis 10-40 days after percutaneous drainage. One patient required a three-stage procedure after percutaneous drainage, and one patient was too unstable for operation at any time during her course and eventually died of respiratory failure. Three patients did not undergo resection after catheter drainage and have remained asymptomatic for 1-2 1/2 years. Ten of 16 patients had fistulas, eight of which closed spontaneously. Experience with percutaneous drainage of diverticular abscesses suggests that it obviates surgical abscess drainage and permits a single operation (sigmoid resection and closure) to be performed safely. Percutaneous abscess drainage has cost-saving implications, since one or two operations may be avoided in most patients, and in some high-risk elderly patients all operations may be obviated.  相似文献

OBJECTIVE: The transgluteal approach to abscess drainage through the greater sciatic foramen has been described in adults, but this route has not been as extensively studied in children. We performed CT-guided transgluteal percutaneous abscess drainage in seven children and assessed the results of drainage and catheter tolerance. CONCLUSION: Transgluteal catheters are well tolerated by children, and the transgluteal route is an effective approach to selected pelvic abscesses in children.  相似文献

目的观察CT引导下经皮肝脓肿穿刺置管引流治疗肝脓肿的疗效和临床应用价值。方法对临床诊断明确并经实验室及影像学检查证实的36例患者在CT引导下经皮肝脓肿穿刺引流,术中置管,术后定时冲洗并积极进行有效的抗生素治疗。结果 36例肝脓肿患者经皮穿刺置管引流术治疗后,临床症状消失,无并发症发生,经B型超声或CT复查脓肿消失,全部治愈出院。结论 CT引导下经皮肝脓肿穿刺置管引流术具有操作简便、手术创伤小、住院费用低、疗效肯定等优点,是临床治疗肝脓肿的有效方法之一。  相似文献

A large cervical abscess originating from the thyroid gland was treated by means of irrigational catheter drainage performed percutaneously with CT guidance. Culturing resulted in the growth of Streptococcus. Catheter drainage associated with appropriate antibiotic therapy resulted in healing of the abscess after only 3 days with no complications occurred. This case suggests that CT-guided percutaneous catheter drainage may be an effective and safe alternative to surgical therapy in the neck area as well as in other regions of the body.  相似文献

Definitive computed tomography-guided percutaneous drainage of right upper abdominal abscesses was performed in eight patients (11 procedures). A lateral perpendicular approach was used in all cases. An Argyl trocar catheter was used in nine procedures and a pigtail catheter in two. The drainage was successful in seven patients. In one patient no drainable abscess was found. Percutaneous abdominal abscess drainage should be considered a satisfactory alternative to surgical drainage in poor-risk patients as well as in patients who have had repeated operations in whom any further surgery may be unduly difficult.  相似文献

 目的评价经皮穿刺引流在阑尾周围脓肿治疗中的临床应用价值。方法将60例阑尾周围脓肿患者随机分成常规治疗组和经皮引流组,每组30例。常规治疗组采用抗生素全身抗感染及对症支持治疗;经皮引流组在全身抗感染及对症支持治疗基础上,经腹壁脓腔置管引流。结果经皮引流组第2周治愈率40.00%(12/30)、有效率90.00%(27/30),第3周治愈率80.00%(24/30)、有效率96.67%(29/30),与常规治疗组比较差异有统计学意义(P<0.05或P<0.01),而且经皮引流组在体温恢复正常时间、血象恢复正常时间、脓腔消失时间、腹痛消失时间和抗生素使用时间方面明显优于常规治疗组(P<0.01)。治疗期间经皮引流组无中转手术治疗病例。结论对液化相对完全且局限的阑尾周围脓肿,经皮穿刺引流可缩短疗程,提高疗效,是一种安全可靠的治疗方法。  相似文献
