ABSTRACT

Objectives: Current industry classification systems in the United States do not differentiate mechanized and nonmechanized logging operations. The objectives of this article are to quantify injury risk differences between mechanized and nonmechanized logging operations in Washington State and to evaluate for potential injury risk tradeoffs, such as decreasing traumatic injuries while increasing nontraumatic injuries that might occur when mechanized logging operations are substituted for nonmechanized logging operations.

Methods: Using Washington State workers’ compensation insurance risk classes to differentiate mechanized and nonmechanized logging operations, injury and illness claims data and employer reported hours were used to compare claim rates and to characterize injuries by type of logging operation.

Results: From 2005 to 2014, the accepted Washington State worker’s compensation claim rate for nonmechanized logging was 46.4 per 100 full-time equivalent employees compared to 6.7 per 100 full-time equivalent (FTE) for mechanized logging activities. The rate ratio for comparing nonmechanized to mechanized logging claims rates for all accepted claims was 6.9 (95% Confidence Interval 6.4–7.5). Claim rates for traumatic injury and nontraumatic injuries in nonmechanized logging exceeded comparable rates in mechanized logging activities, although the distribution of types of injury differed by type of logging operation. A greater percentage of accepted claims in nonmechanized logging were traumatic injuries than in mechanized logging (92.2% vs. 85.0%, respectively). In addition, nonmechanized logging had higher total claim and medical costs per FTE and had a higher proportion of claims with lost work time than mechanized logging.

Conclusion: Mechanized logging offers a considerable safety advantage over nonmechanized logging operations. Continued efforts to increase the mechanization of logging operations will result in decreased injury rates.  相似文献

Introduction. Immigrants often come to Canada for the purpose of employment and make up a large proportion of our labour force. Yet, these workers' labour market experience may not always be positive – new immigrant workers can have difficulties finding a job in their field and may end up working in ‘survival jobs’ that expose them to workplace hazards. Workers who are new to Canada may not be familiar with legislation designed to protect them at work or with social programs that can help after a work-related injury.

Methods. Through a series of in-depth interviews this study examined the experiences of new immigrants after they were injured on the job.

Results. The analysis revealed that many workers were in manual, ‘survival jobs’ and had not received job or occupational health and safety training. Many did not speak the English language well and knew little about their rights. While workers often felt trepidation about reporting their injury, most told a health care provider or employer that they were injured or in pain. This, however, rarely led to timely or appropriate claim filing. Workers were often discouraged from filing a claim, misinformed about their rights or offered ‘time off work’ in lieu of reporting the injury to worker's compensation. In instances where a claim was filed, communication problems were common and led to mistakes being made on forms and misunderstandings with the adjudicator and employer. Interpretation services were not always offered consistently or at the correct time.

Conclusion. Efforts must be made to systematically inform new immigrants of their health and safety rights, responsibilities and entitlements as they are entering the labour market. Systems must be put in place to ensure that immigrants can access the compensation system in the event of a work-related injury and that employers and healthcare providers fulfil their reporting responsibilities.  相似文献

Objective: To characterize occupational injuries of aquaculture workers in Washington State.

Methods: We reviewed accepted Washington State workers’ compensation claims from January 1, 2006 to December 31, 2014. Trends in employment, worker demographics (including age, sex, and body mass index), claim rate, and injury characteristics were summarized for accepted claims in the Washington State risk classes that include aquaculture workers.

Results: During the nine-year study period, there were 1,180 accepted claims, 836 (65.1%) were medical-aid only and 344 (26.8%) were compensable. Most commonly reported injury types included being struck by/against an object (n = 420) and work-related musculoskeletal disorders (n = 310). Over the study period, there was no statistically significant change in claim rates in aquaculture (p = 0.77), though they were elevated compared to claim rates for all WA industries combined. A significant upward trend (p = 0.003) in aquaculture employment was observed during our study period.

Conclusions: Given the growth in aquaculture production, and that aquaculture workers in Washington are experiencing higher occupational injury rates compared to other workers in the state, our study suggests that the industry should devote greater attention to prevention of work-related injuries, especially those resulting in being struck by/against objects or work-related musculoskeletal disorders. Enhanced occupational safety and health programs for this industry could help reduce injuries, as well as the cost of workers’ compensation claims.  相似文献

Background: Outsourcing labor is linked to negative impacts on occupational health and safety (OHS). In British Columbia, Canada, provincial health care service providers outsource support services such as cleaners and food service workers (CFSWs) to external contractors.

Objectives: This study investigates the impact of outsourcing on the occupational health safety of hospital CFSWs through a mixed methods approach.

Methods: Worker’s compensation data for hospital CFSWs were analyzed by negative binomial and multiple linear regressions supplemented by iterative thematic analysis of telephone interviews of the same job groups.

Results: Non-significant decreases in injury rates and days lost per injury were observed in outsourced CFSWs post outsourcing. Significant decreases (P < 0.05) were observed in average costs per injury for cleaners post outsourcing. Outsourced workers interviewed implied instances of underreporting workplace injuries.

Conclusions: This mixed methods study describes the impact of outsourcing on OHS of healthcare workers in British Columbia. Results will be helpful for policy-makers and workplace regulators to assess program effectiveness for outsourced workers.  相似文献

This paper examines the compensation process for work-related injuries and illnesses by assessing the trajectories of a sample of immigrant and non-immigrant workers (n = 104) in Montreal. Workers were interviewed to analyze the complexity associated with the compensation process. Experts specialized in compensation issues assessed the difficulty of the interviewees’ compensation process. Immigrant workers faced greater difficulties with medical, legal, and administrative issues than non-immigrants did. While immigrant workers’ claim forms tended to be written more often by employers or friends (58% vs. 8%), the claims were still more often contested by employers (64% vs. 24%). Immigrant workers were less likely to obtain a precise diagnosis (64% vs. 42%) and upon returning to work were more likely to face sub-optimal conditions. Such results throw into relief issues of ethics and equity in host societies that are building their economy with migrant workers.  相似文献

Background: Modern U.S. dairy farm operations present multiple safety hazards which may result in worker injuries, illnesses and fatalities. The U.S. industry trend towards large-herd milking operations, especially in the Western U.S., necessitates an increased number of workers on each farm. The U.S. dairy workforce is comprised of an increasing percentage of immigrant workers with limited English proficiency which presents a safety training challenge. Our objective was to develop, deliver and evaluate safety awareness training to U.S. dairy farm workers using mobile tablets as a personal learning environment. Methods: A non-random sample of 1,436 dairy workers received safety awareness training on a mobile device platform. Kirkpatrick Levels 1, 2 and 3 training effectiveness evaluations were performed. Results: Approximately 58.7% of training participants received a middle school education, and 22.7% were from Guatemala who reported K’iche’ as their native language. A statistically significant large effect size suggests safety knowledge was gained among training participants when comparing post-test scores to pre-test scores (Level-2). Training participants rated their training experience favorably (Level-1), and applied knowledge gained in their daily work activities three months after training (Level-3). Conclusion: Our findings suggest the utilization of mobile learning techniques is an effective means to deliver safety awareness training content to dairy workers. Dairy worker safety training should be culturally, linguistically, and literacy appropriate and comprehensive in nature to include all applicable dairy safety hazards. Our observations support the use of mobile devices as just one component of a more comprehensive health and safety management program on farms.  相似文献

BACKGROUND: To investigate occupational health in urban immigrant Latino workers, using a community-based method. METHODS: A survey was administered through consecutively selected door-to-door interviews. RESULTS: Response rate was 80% (n = 427). Average time in the US was 7.6 years, and average job tenure was 2.8 years. Twenty-five reported exposures to over 10 different hazards, and 18% thought these hazards had harmed their health. Only 31% received any job safety training; 55% had no workers' compensation coverage. Of the 47 (11%) with a work injury in the past 3 years, 27% reported difficulty obtaining treatment, 91% lost time from work (median = 13 days) and 29% had to change jobs because of the injury. The annual occupational injury rate was 12.2/100 full-time workers, compared to an expected rate of 7.1. CONCLUSIONS: Urban immigrant workers have increased risk of occupational injuries, with adverse outcomes.  相似文献

BACKGROUND: Poultry processing is the largest sector of the meat products industry. Many workers are immigrants; few data exist on their illness and injury rates and the impact of workplace safety environment. METHODS: Survey interviews were conducted with a representative sample of 200 Latino poultry workers employed by three different companies in western North Carolina; topics included symptoms, work-related illnesses and injuries, and plant safety climate. RESULTS: Most respondents were <35 years of age and had been in the US <10 years. Frequency of self-reported symptoms was high, particularly for musculoskeletal symptoms. Despite symptoms, workers reported not missing work or seeking medical care. Occupational injuries and illnesses and symptoms varied by company. Between-company differences in injury and illness rates were consistent with perceived safety and company provision of personal protective equipment. CONCLUSIONS: Symptoms reported exceeded rates reported by other community, clinical, and occupational samples. Findings suggest policy changes and research are needed to reduce the high rates of occupational illnesses and injuries in this vulnerable population.  相似文献

OBJECTIVES: This study aimed to assess the adequacy of safety training provided to young Latino immigrant construction workers. The study posited that, because of their youth and immigrant status, these workers would be less likely to receive adequate training. METHODS: We interviewed 50 youths aged < or =21 who had worked at least 10 days in construction in the previous year. The in-person interview included 140 questions covering a range of construction work and health and safety experiences. RESULTS: Participants reported performing a range of hazardous tasks, some while under the age of 18. Of these, 68% to 72% reported receiving some training, but median training time was only 1 hour. Only 24% reported receiving written training material. Those with less English ability received less training. CONCLUSIONS: Young Latino immigrants in this study received inadequate training given the hazardous work they performed. CLINICAL SIGNIFICANCE: Results of this research, especially the relatively low level of English communication skills among young Latino workers, point to the need for increased bilingual services not just in worker safety training programs, but also in medical clinics and emergency rooms that treat Latino workers.  相似文献

The industrialization of pork production is a gradual process. While operators are focused increasingly on production which is efficient and of high quality they must also be aware of the potential costs to their employees. The use of confinement facilities exposes workers to health and safety risks, which in turn may increase operating cost through increased health care and insurance costs. In order to protect employees, the employer needs knowledge about these risks, their impact on the operation, and methods of preventing injuries and illnesses. ABSTRACT

Vermont is a new Latino destination where many Spanish-speaking migrants have found work on dairy farms. One hundred twenty Latino workers were surveyed on 59 Vermont dairy farms to develop a demographic profile and evaluate their self-assessed health status and barriers to care. The study found, similar to other studies, the majority of workers were young, male Mexicans. However, the workers in this study, as compared to others, originated farther south in Mexico and there were significant regional differences in educational attainment. Workers defined health in terms of their ability to work and the majority believed themselves to be in good health. The majority felt that moving to the United States has not changed their health status. The most common health issue reported was back/neck pain, followed by dental and mental health issues. Workers are both physically and linguistically isolated and reported isolation as the most challenging aspect of dairy farm work. Fear of immigration law enforcement was the primary barrier to care. Community-based initiatives, including partnerships with colleges and universities, outreach to farm employers and the adoption of “bias-free policing” are strategies that can increase access to health care for Latino dairy farmworkers in the United States.  相似文献

ABSTRACT: BACKGROUND: No national study has investigated whether immigrant workers are less likely than U.S.-workers to seek medical treatment after occupational injuries and whether the payment source differs between two groups. METHODS: Using the 2004-2009 Medical Expenditure Panel Survey (MEPS) data, we estimated the annual incidence rate of nonfatal occupational injuries per 100 workers. Logistic regression models were fitted to test whether injured immigrant workers were less likely than U.S.-born workers to seek professional medical treatment after occupational injuries. We also estimated the average mean medical expenditures per injured worker during the 2 year MEPS reference period using linear regression analysis, adjusting for gender, age, race, marital status, education, poverty level, and insurance. Types of service and sources of payment were compared between U.S.-born and immigrant workers. RESULTS: A total of 1,909 injured U.S.-born workers reported 2,176 occupational injury events and 508 injured immigrant workers reported 560 occupational injury events. The annual nonfatal incidence rate per 100 workers was 4.0% (95% CI: 3.8%-4.3%) for U.S.-born workers and 3.0% (95% CI: 2.6%-3.3%) for immigrant workers. Medical treatment was sought after 77.3% (95% CI: 75.1%-79.4%) of the occupational injuries suffered by U.S.-born workers and 75.6% (95% CI: 69.8%-80.7%) of the occupational injuries suffered by immigrant workers. The average medical expenditure per injured worker in the 2 year MEPS reference period was $2357 for the U.S.-born workers and $2,351 for immigrant workers (in 2009 U.S. dollars, P=0.99). Workers' compensation paid 57.0% (95% CI: 49.4%-63.6%) of the total expenditures for U.S.-born workers and 43.2% (95% CI: 33.0%-53.7%) for immigrant workers. U.S.-born workers paid 6.7% (95% CI: 5.5%-8.3%) and immigrant workers paid 7.1% (95% CI: 5.2%-9.6%) out-of-pocket. CONCLUSIONS: Immigrant workers had a statistically significant lower incidence rate of nonfatal occupational injuries than U.S.-born workers. There was no significant difference in seeking medical treatment and in the mean expenditures per injured worker between the two groups. The proportion of total expenditures paid by workers' compensation was smaller (marginally significant) for immigrant workers than for U.S.-born workers.  相似文献

ABSTRACT

Objective: This article describes how perceived discrimination shapes the way Latino farmworkers encounter injuries and seek out treatment. Methods: After 5 months of ethnographic fieldwork, 89 open-ended, semistructured interviews were analyzed. NVivo was used to code and qualitatively organize the interviews and field notes. Finally, codes, notes, and co-occurring dynamics were used to iteratively assess the data for major themes. Results: The primary source of perceived discrimination was the “boss” or farm owner. Immigrant status was also a significant influence on how farmworkers perceived the discrimination. Specifically, the ability to speak English and length of stay in the United States were related to stronger perceptions of discrimination. Finally, farm owners compelled their Latino employees to work through their injuries without treatment. Conclusions: This ethnographic account brings attention to how discrimination and lack of worksite protections are implicated in farmworkers’ injury experiences and suggests the need for policies that better safeguard vulnerable workers.  相似文献

ABSTRACT

Migrant farmworkers are a vulnerable population. Migrant farmworkers with H-2A visas are the only agricultural workers with temporary work permits. Little research has directly focused on the job characteristics and work safety of workers with H-2A visas. This analysis (1) describes their personal and job characteristics, job hazards, and stressors; (2) describes their perceived work safety climate; and (3) examines associations of perceived work safety climate with job characteristics, job hazards, and stressors. Data are from a cross-sectional component of a larger study of farmworker pesticide exposure; in 2012 interviews were conducted with 163 migrant farmworkers with H-2A visas in North Carolina. The sample was limited to men aged 30 to 70 years. Migrant farmworkers with H-2A visas experience the same hazards as do other farmworkers. Their mean score on the Perceived Work Safety Climate Scale 25.5 (SD = 3.7) is similar to that of other farmworkers and other immigrant workers. Perceived work safety climate is associated with hours worked per week (P = .02), precarious employment (P < .001), planting and cultivating (P = .002), topping tobacco (P = .0012), and stress (P = .02). Perceived work safety climate is particularly important for migrant farmworkers with H-2A visas because their labor contracts limit their options to change employers. Additional research on the status of work safety climate among agricultural workers is needed, as well as on the factors that affect work safety climate and on the safety characteristics that are affected by work safety climate. Policy changes that lead to improved work safety climate should be considered.  相似文献

The authors report here results from the first follow-up survey of the Mexican Immigration to California: Agricultural Safety and Acculturation (MICASA) cohort of community-dwelling immigrant Hispanic farm workers in California's Central Valley. Among 560 participants the authors observed cumulative 1-year injury incidence of 6.6% (all injuries) and 4.3% (agricultural injuries). Increased prospective injury risk was associated with males, US birth, years lived in the United States, family income, and poor self-rated health. Agricultural injuries were associated most frequently with being struck by an object, falls, and cutting instruments, whereas over two thirds of nonagricultural injuries involved motor vehicles. Prevention should focus on safe handling of tools and materials, falls, and motor vehicle safety.  相似文献

Abstract

Agricultural work is one of the riskiest occupations for the eyes. Protective eyewear can prevent eye injuries in upward of 90% of cases. Latino migrant and seasonal farm workers (LFWs) are at particular risk of eye injury because of economic, social, and cultural barriers to safe employment. The goal of this project was to determine the perceived benefits and barriers to use of protective eyewear during agricultural work among LFWs. In the year 2000, 55 workers who spent at least part of their time working in Illinois and Michigan as seasonal, hired farm workers were interviewed either individually (9) or in six focus groups (46) regarding their perceptions of eye hazards and the factors that encourage or discourage the use of protective eyewear. Subsequently, safety glasses and training on eye safety were delivered by promotores de salud to LFWs from the same cohort over two summer seasons. During the second season, the promotores observed the use of safety eyewear by farm workers at least once per week on 17 farms over a three-month period. At each point of observation, the promotores asked those wearing the glasses why they chose to wear them and those who were not wearing them, why not. At both points of the study (pre-intervention interviews and post-intervention observations), the reasons for use fell into the following categories: protection from hazards, appearance, whether it was mandated/provided by the employer, and whether others used it. The reasons for non-use include perceived lack of protection, discomfort, undesirable appearance, interference with visual acuity, slowing down the work pace, and no mandate from employers. Since eliminating the eye injury hazard is not possible in most agricultural settings, administrative strategies and use of personal protective equipment are critical. Perceptions of the target audience, LFWs, should be used to develop a comprehensive eye safety program; this should include allowing LFWs to select a style from effective prototypes, providing eyewear and promoting/mandating its use in hazardous job tasks, and implementing a comprehensive PPE program for eye safety. In addition, addressing functional problemsfalling off, fogging, loss and forgetting glasses, the pace slowdown that reduces production and leads to lower wages for workersshould be addressed.  相似文献

Objective: Migrant seasonal agricultural workers who are employed in one of the most hazardous occupations in Turkey experience difficulties in accessing health-care services. The aim of this study is to investigate the living conditions, access to health-care services, and occupational health and safety conditions of migrant seasonal agricultural workers in the Çukurova region.

Methods: Four hundred migrant seasonal agricultural workers were contacted in the county of Karata? located in the province of Adana, Turkey. A four-section questionnaire was administered using face-to-face interview techniques.

Results: The mean age of the participants was 32.8 ± 12.4 years. One-fourth of the participants did not have any social security coverage. Almost all had a monthly income below minimum wage, and 98% were living in tents. One-fifth experienced health problems in the last year, and 63% of them visited a health-care facility for diagnosis and treatment. About 3.3% of the participants sustained injuries at work, and only 23% of them visited a health-care facility.

Conclusion: The findings of the study show that migrant seasonal agricultural workers in Turkey have insufficient working conditions, wages, accommodation facilities, nutrition opportunities, health conditions, and occupational health and safety conditions. This insufficiency negatively affects life qualities of the workers, utilization of health-care services, and their occupational health status and safety.  相似文献

This article examines the choices made by a sample of Latino immigrant restaurant workers in regard to their health management, particularly in response to illness and injury. I draw on 33 interviews with kitchen staff employed in the mainstream restaurant industry in San Jose, California, and Houston, Texas, in 2006 and 2007. I argue that workers must consider complex power relationships at work in weighing the advantages of calling in sick, using protective equipment, seeking medical care, or filing a workers’ compensation claim. These decisions implicate direct and opportunity costs, such as risk of job loss and missed opportunities for advancement. Workers consequently leverage their health capital to meet their economic needs, to assert their autonomy at the workplace, and to ultimately reject the stigma of illness and injury.  相似文献

Objective: This study aimed to quantify the burden of fatal and workers’ compensation–related injuries on the dairy sector in Australia and to assess current financial inputs for work health and safety actions. Methods: The first and second phases entailed a review of the financial costs associated with fatal (2001–2013) and workers’ compensation cases (2008–2009 to 2012–2013). The third element was a case-study approach focusing on actual production and cost data from a small sample of dairy farms (n = 6) to ascertain an estimate for current financial inputs to work health and safety. Results: The estimated financial impact was approximately AUD$12.4 million each year, comprising AUD$6.8 million for fatalities and AUD$5.6 million for compensable injuries. All sample businesses were investing in health and safety initiatives; however, this was modest and on average represented 0.4% of total expenditure (AUD$4,472) or <AUD$0.02 of the costs per kilogram of milk solids produced. When projected nationally, the inputs were around AUD$13.2 million annually. Conclusions: Despite data limitations, there is a compelling financial case to increase strategic investments into work health and safety by dairy farmers in Australia. Increased investment has the potential to not only minimize the burden of injury and costs to dairy farmers, but will also significantly reduce the devastating impacts that these incidents have on individuals, their families and communities.  相似文献

Introduction: Hospital accreditation can be an incentive to improve occupational health and safety (OHS) performance.

Objective: This study assesses the relationship between status of accreditation among private Lebanese hospitals and compliance with OHS accreditation standards.

Methods: A survey was administered to 68 private Lebanese hospitals to assess accreditation status and specific indicators related to each of the 9 OHS codes in the Lebanese accreditation manual. Chi-square, Fisher’s exact test, and independent sample t-tests compared the OHS standards between accredited and non-accredited hospitals.

Results: Fifty-six percent of participating private hospitals were accredited. Accredited hospitals reported statistically better OHS performance than non-accredited hospitals based on the standards outlined in the accreditation manual. However, there was inconsistent performance on numerous OHS indicators among participating hospitals.

Conclusion: The gaps in OHS performance suggest the need for strengthened OHS guidelines in the national accreditation process to safeguard workers’ health. Strategies to fortify OHS performance include tying service reimbursement to OHS compliance and linking OHS standards with national labor legislation.  相似文献
