Humoral immunity was evaluated in seven children undergoing continuous ambulatory peritoneal dialysis (CAPD). Five children (71%) had serum immunoglobulin levels which were more than 2 SD below the mean for age. One mechanism contributing to hypogammaglobulinemia was loss via the peritoneal membrane. Daily losses of IgG into peritoneal dialysis effluents represented 7%–29% of normal daily IgG synthesis, and losses increased with duration of CAPD therapy. Despite the observed hypogammaglobulinemia, all children had protective antibody responses to immunization.  相似文献

We monitored serum zinc and copper levels for 4 months in six patients treated with continuous ambulatory peritoneal dialysis (CAPD). Zinc and copper fluxes were studied during a single dialysis exchange and over a 3-day period. Routine oral trace element supplements were then discontinued for 2 months. Serum zinc levels declined but serum copper levels remained unchanged. One month after oral supplements had been restarted, serum zinc levels returned to normal and serum copper levels rose above initial values. Zinc and copper concentrations in dialysis exchange indicated that the patients absorbed zine and lost copper in significant amounts. The patients had poor dietary intakes of both minerals. These data suggest that patients treated with CAPD benefit from oral zinc supplementation.  相似文献

This study was undertaken to ascertain whether 19 patients maintained on continuous ambulatory peritoneal dialysis (CAPD) for at least 1 year experienced any deterioration in peritoneal membrane function. Selected serum chemistries and skinfold measurements were also evaluated to determine whether patients dialyzed by CAPD could maintain a normal nutritional status. This study demonstrates that patients maintained on CAPD had stable dialysate protein losses, glucose absorption from the dialysate, and constant urea, creatinine, and sodium removal. When these patients were subdivided by incidence of peritonitis, the group with a lower incidence of peritonitis (one episode every 349 +/- 155 SEM days) showed stable serum protein concentration and improvement in upper arm area whereas the group with a high incidence of peritonitis (one episode every 95 +/- 7 SEM days) showed a reduction in upper arm muscle area. Thus, our data suggest that over a 1-year period, there is no deterioration in peritoneal membrane characteristics and CAPD is effective in maintaining the nutritional status of the patient. However, both membrane function and nutritional status may be impaired by frequent episodes of infection.  相似文献

目的 探讨狼疮肾炎(LN)腹膜透析(腹透)患者的长期预后.方法 入选1995年5月1日至2013年4月30日期间在本院开始腹透且资料完整的LN患者(n=33),同时入选与其年龄、性别、并发症匹配的非LN腹透患者(n=33)作为对照组.所有入选患者均随访至死亡、退出腹透、转其他中心或至研究终止.采用Kaplan-Meier生存分析和Log-Rank检验比较两组患者的生存率、技术生存率和无腹膜炎生存率.结果 腹透开始时,LN组患者的估算肾小球滤过率(eGFR)、抗双链DNA (anti-dsDNA)和高敏C反应蛋白(hs-CRP)水平均明显高于对照组(均P< 0.05).截至研究终止,LN组患者有13例(39.4%)死亡,8例(24.2%)转血液透析(血透),5例(15.2%)肾移植,2例(6.1%)转其他中心.LN组患者最常见的死亡原因是感染(9例,69.2%),其中又以腹膜炎最常见(6例,46.2%),而对照组患者最常见的死亡原因是心血管疾病(5例,83.3%).Kaplan-Meier分析显示LN组患者的1、3、5年患者生存率为82%、49%、49%,明显低于对照组(x2=8.455,P=0.004).LN组患者的技术生存率也明显低于对照组(x2=6.753,P=0.009).LN组腹膜炎发生率为1次/20.5病人月,而对照组腹膜炎发生率为1次/67.6病人月.LN组患者的无腹膜炎生存率显著低于对照组(x2=8.256,P=0.004).结论 LN腹膜透析患者的长期预后较差.腹膜炎是LN腹透患者死亡和技术失败的主要原因.  相似文献

Peritoneal clearances and dialysate protein losses occurring in paediatric patients undergoing different continuous cycling peritoneal dialysis (CCPD) regimens have not been well defined. We, therefore, evaluated 10 children aged 15.8 +/- 2.5 (SD) years who were maintained on home peritoneal dialysis for 20.5 +/- 10 months. All patients had at least 3 months of CCPD. The patients were admitted to the Clinical Research Center for 48 hours and allocated to five different dialysis protocols. In protocol I, the frequency of exchanges was 10 per 10 hours; in Protocol II it was 5 per 10 hours; and in Protocol III it was 3 per 10 hours. Protocol II D and III D had, in addition, a daytime dwell of one-half the night-time volume. A 1.5% glucose dialysate solution was used for night-time dialysis, and 4.25% glucose dialysate solution for the daytime dwell. The mean inflow dialysate volume per exchange was 36.7 +/- 5.6 ml/kg body weight and was constant in each patient for each study protocol. BUN and creatinine clearances for each protocol were calculated and dialysate protein losses were measured. The data indicate that hourly night-time dialysis (Protocol I) provides best solute clearance. A daytime dwell further enhances the total solute clearance and should be used preferably in anuric patients. Residual urine output contributes significantly to the total solute clearance. Protein losses are maximum with low-frequency exchanges and a daytime dwell. No significant differences in the serum albumin concentrations were found during the different protocols; however, the long-term effect of the protein loss on the nutritional status of the patients requires further evaluation.  相似文献

目的:探讨持续非卧床性腹膜透析(CAPD)患者并发胸腹瘘(AF)的临床表现、诊断方法及治疗与转归。方法观察广东省佛山市南海区人民医院肾内科2009年1月~2012年2月收治的3例CAPD并发胸腹瘘患者,对其临床表现、诊断方法及治疗与转归进行分析与总结。结果3例患者均出现气促、胸闷、不能平卧,伴超滤量明显减少,影像学提示中-大量右侧胸腔积液;应用胸水生化成分分析鉴定胸水性质,同时以标记放射性核素99Tcm-乙三胺五乙酸(DPTA)注入透析液后,在胸膜腔中探测到99Tcm确诊;经抽取胸水、暂停CAPD、改行间隙性腹膜透析等治疗方法效果欠佳,3例患者最终都转为血液透析治疗。结论从临床表现可早期发现胸腹瘘,用核素扫描结合胸水生化成分分析诊断胸腹瘘敏感性高,不良反应少。出现胸腹瘘后患者较难再维持腹膜透析治疗。  相似文献

Since 15 December 1991 four swan neck presternal catheters (SNPC) have been implanted in four children aged 2–11 years. The observation period ranged from 4 to 10 months. The aim of this study was to evaluate the usefulness of a new peritoneal dialysis catheter implantation method in paediatric patients. The indications for insertion of the SNPC were: young age, use of nappies, obesity and recurrent exit site infection (ESI). The surgical technique of the SNPC implantation was similar to that used for adults. The chest location of the catheter exit site is advantageous for the following reasons: (1) easier care of a small child because of greater distance from nappies, (2) better healing and decreased risk of ESI in the area with less fat thickness and (3) less trauma. A larger number of children with a longer follow-up is necessary for better evaluation of the SNPC, as well as for estimation of frequency of ESI and peritonitis.  相似文献

Background: During the past few decades, the demographics of end stage renal disease have been changed significantly with the emerging predominance of elderly patients. Elderly dialysis patients are usually more dependent and may need long-term placement in a long-term care facility. Failure to meet the needs of these patients may have a significant impact on the peritoneal dialysis program. We report our experience of starting peritoneal dialysis program in a community-based Long Term Care Facility (LTCF).Methods: During the period of 2000–2001, after appropriate training of nursing home personnel, we admitted 8 peritoneal dialysis patients to one community-based nursing home. All information presented here has been collected through chart review.Results: At the time of admission the average age of the 8 patients was 77.3 ± 7.2 years (range 69 to 91 years). All patients had several comorbid diseases and six of the eight were bed-ridden. The patients stayed in the facility for a total of 29.57 patient months. One patient had three episodes of peritonitis within three months (all culture negative) and has been excluded from the analysis of the overall peritonitis rate. The peritonitis rate for the other seven patients was 1 per 7.54 patient month. Six patients were readmitted to hospital because of peritonitis [4], severe malnutrition [1] and hip fracture [1]. Four of them died in the hospital. One died in the nursing home. One patient remains in the nursing home at the present time.Conclusions: Our experience suggests that peritoneal dialysis can be achieved in a community-based nursing home. This requires a systematic training program for the LTCF personnel and the availability of a ``dedicated' nephrology dialysis staff. This is crucial to the success of the program. It is important that patients, their families and ESRD care professionals are informed of the limited survival expectation particularly for very old and severely impaired patients.  相似文献

Objectives To investigate the effects of seasonal changes on peritoneal dialysis associated peritonitis (PDAP) in patients on peritoneal dialysis (PD), and to provide evidence for clinical prevention and treatment of PDAP. Methods All episodes of PD-related peritonitis during clinic follow-up in maintenance PD patients from Jan 1st, 2007 to Dec 31st, 2015 in Peking University People's Hospital were reviewed. The incidence of peritonitis, laboratory indexes, pathogens and clinical outcomes in different seasons were recorded and analyzed. One-way ANOVA and chi square test were employed to compare the incidence of PDAP and related data in different seasons, and Pearson correlation was used to analyze correlations between PDAP rate and monthly mean temperature and mean humidity. Results During nine years, a total of 119 PD patients occurred 190 times of peritonitis during home PD. The PDAP rate in summer was the highest, 0.21 episodes/year, followed by spring (0.16 episodes/year) and autumn (0.16 episodes/risk year), but there was no significant difference among peritonitis rates in four seasons. There were significant positive correlation between monthly mean temperature, monthly mean humidity and the peritonitis rate (mean temperature: r=0.828, P<0.01; mean humidity r=0.657, P<0.05). (2) As for bacteria, in Summer the PDAP rate caused by Staphylococcus aureus and Coagulase negative staphylococcus (CoNS), and Gram-negative bacteria was higher than that in other seasons, but there was no statistical difference. There were significant positive correlation between monthly mean temperature, mean humidity and the rate of CoNS peritonitis (mean temperature: r=0.704, P<0.05; mean humidity: r=0.607, P<0.05). (3) There were no statistical difference among results of PD related peritonitis in different seasons about general situation, clinical manifestation, causes of peritonitis and laboratory index before peritonitis episodes. PD procedure-related problems were the main cause of peritonitis in summer and autumn. (4) The cure rate of all peritonitis was 90%. The highest cure rate was in autumn and winter, while the lowest cure rate was in summer, but no statistical difference. Among the peritonitis episodes with treatment failure, 52.6% occurred in summer. Conclusions There is some correlation between the rate of PDAP and seasons. Higher temperature and higher humidity were significantly correlated with higher peritonitis rate, especially the rate of CoNS peritonitis. The prognosis of PDAP in summer was relatively poor, with higher proportion of hospitalization and lower cure rate.  相似文献

目的 研究持续不卧床腹膜透析(CAPD)患者24 h腹透液总蛋白量与临床心血管疾病(CVD)的关系。 方法 选择我中心随访的CAPD患者178例,完成24 h腹透液总蛋白量检测及有关病史采集,完成颈动脉彩超和心脏彩超检测,并进行前瞻性随访≥12个月,观察新发临床CVD的发生。 结果 CAPD患者24 h腹透液总蛋白量平均为(5.0±1.8) g。CAPD患者有既往心血管疾病史或糖尿病史者或有颈动脉硬化者24 h腹透液总蛋白量均较无相应病史者多(分别是t = 2.082,P = 0.039;t = 2.601,P = 0.010;t = 2.217,P = 0.029)。CAPD患者24 h腹透液总蛋白量与舒张末期左室内径(LVDd,单位:mm)、舒张末期室间隔厚度(IVSd,单位:mm)、舒张末期左室后壁厚度(LVPWd,单位:mm)、左室心肌重量指数(LVMI,单位:g/m2)均呈正的直线相关关系(分别是r = 0.222,P=0.040;r = 0.217,P=0.043; r = 0.339,P = 0.002;r = 0.305,P = 0.007),与左室射血分数(LVEF)呈负的直线相关关系(r = -0.221,P = 0.040)。其中114例CAPD患者完成了前瞻性随访,平均随访(15.3±1.5)个月。以24 h腹透液总蛋白量的第50位数(P50)为界分为高腹透液蛋白组和低腹透液蛋白组,卡方检验显示,高腹透液蛋白组新发CVD发生率显著高于低腹透液蛋白组(40.4%比19.3%,χ2 = 6.035,P = 0.014)。多因素logistic回归分析显示,24 h腹透液总蛋白量与年龄、血清白蛋白、LVDd均为CAPD患者新发CVD的独立危险因素。 结论 CAPD患者24 h腹透液总蛋白量与既往心血管疾病史、糖尿病史、颈动脉硬化有关,且与左心室结构和功能障碍相关,是CAPD患者新发CVD的独立危险因素。  相似文献

Abstract:  Volume control is critical for peritoneal dialysis. Although peritoneal equilibration test (PET) has been used to clarify the peritoneal membrane characteristics, it is not able to adequately predict peritoneal fluid removal and optimize appropriately the dwell time. In the present study, we applied computer simulation and performed a more detailed evaluation of the fluid kinetics in patients with different ultrafiltration (UF) capacity. Patients who used three to four exchanges of 2.27% glucose dialysate per day (poor UF capacity group), and patients who used three to four exchanges of 1.36% glucose dialysate per day (good UF capacity group) to achieve adequate amount of peritoneal fluid removal were included in the present analysis. All included patients were asked to record appropriately their dialysis exchanges for the assessment of their peritoneal fluid transport characteristics. Seventeen continuous ambulatory peritoneal dialysis patients were selected in the present study, nine in poor UF capacity group and eight in good UF capacity group. Patients in poor UF capacity group had significantly higher daily glucose exposure, higher dialysate-to-plasma ratio of creatinine (D/P creatinine) values, and higher peritoneal fluid absorption rate, K e, as compared to patients with good UF capacity. Our results suggest that patients with poor UF capacity have significant higher peritoneal small solute transport rate, and more importantly, higher peritoneal fluid absorption rate as compared to patients with good UF capacity.  相似文献

Peritonitis and catheter-related (exit-site/tunnel) infections are major causes of morbidity in children receiving peritoneal dialysis (PD). Our objective was to evaluate the impact of a combination of prophylactic measures on the rate of peritonitis and catheter-related infections subsequent to their implementation in 2001. This is a single center review of incident patients who received automated peritoneal dialysis (APD) from 1997 to 2004. The causal microorganisms, annualized peritonitis and catheter-related infections rates and the time to infection were reviewed using pooled data from 1997 to 2000 and from 2001 to 2004. Fifty-four patients received PD over 1099 patient months (pm). Twenty-eight peritonitis episodes occurred in 15 patients over 599 pm from 1997 to 2000 (annualized rate (AR): 0.56 infections/patient year). Eight episodes of peritonitis occurred in five patients over 500 pm from 2001 to 2004 (AR: 0.19 infections/patient year) (P = 0.01). Prior to 2001, the median time from dialysis initiation to the first peritonitis episode was 500 days (95% CI, 400–660 days), compared to 1137 days (95% CI, 1050 to +Infinity) from 2001 to 2004 (P = 0.008). The rate of catheter-related infections and time to initial infection during the two periods was not different. We conclude that measures to decrease the frequency of peritonitis can be successfully applied to children and should be incorporated as part of standard care.  相似文献

We report the development of chylous ascites in a neonate as an uncommon complication during continuous peritoneal dialysis. Cloudy dialysis fluid containing many white blood cells might confuse the diagnosis of chylous ascites with infective peritonitis and result in inappropriate use of antibiotics. Resolution may be critical, since chyle removal during dialysis may result in profound immunosuppression and malnutrition due to lymphocyte and fat losses. After 4 weeks on a modified diet, the chyle leak resolved. The patient returned to breast milk and continues nighttime continuous-cycle peritoneal dialysis without further chyle leak.  相似文献

Continuous ambulatory peritoneal dialysis (CAPD) has come to be extensively used for the treatment of end-stage renal failure in children, and especially infants, such that now more than half of children on dialysis worldwide receive treatment by this means. Peritonitis, however, is commoner in children than in adults receiving treatment, and is a major source of morbidity and treatment failure in children started on CAPD. Only recently has the immunology of the normal peritoneum been studied extensively, with the need to assess the impact of the installation of large volumes of fluid into the peritoneal sac during dialysis. The main phagocytic defences of the peritoneum depend upon a unique set of macrophages which are present free in the peritoneal fluid but also in the submesothelium and in perivascular collections together with B lymphocytes in the submesothelial area. Both the number of macrophages per unit volume and the concentration of opsonic proteins, such as IgG, complement and fibronectin, are reduced to between only 1% and 5% when dialysis fluid is continuously present in the peritoneal sac. In addition, the fluids used for CAPD are toxic to both macrophages and to mesothelial cells. Thus minor degrees of contamination frequently lead to peritonitis and in addition the majority of patients have catheters inserted in their peritoneum which become colonised with organisms capable of producing exopolysaccharide (slime), which promotes adhesion of the organism to the plastic and protects them against phagocytic attack and the penetration of antibiotics. Thus the peritoneum is in a state of continual inflammation, as well as being a markedly more vulnerable site than the normal peritoneum to the entry of organisms. Whether clinical peritonitis appears in this state of chronic contamination probably depends on perturbation in the balance between host defences and the organism. WhilstStaphylococcus epidermidis is the commonest, cause of peritonitis,Staphylococcus aureus and Gram-negative organisms are much more serious and more frequently lead either to temporary catheter removal or discontinuation of dialysis altogether. Expeditious diagnosis of peritonitis remains a significant goal in the management of patients maintained on peritoneal dialysis. Several attempts to use leukocyte esterase reagent strips to diagnose peritonitis have been described. In this study we examined the usefulness of a new reagent strip, the PeriScreen Test Strip, in the diagnosis of peritonitis. A series of 72 peritoneal effluent samples obtained from 22 maintenance peritoneal dialysis patients is reported. In this study, the test strips had a sensitivity of 100% and a specificity of 98.3% as compared to an abnormal leukocyte count. Thus, in the diagnosis of peritonitis we believe that the PeriScreen Test Strip can be used as a simple bedside screening test to exclude peritonitis in peritoneal dialysis patients.  相似文献

BackgroundHypoalbuminemia at baseline is a powerful predictor of long-term outcomes in peritoneal dialysis patients. However, the levels of serum albumin are dynamically changed during PD. The present study investigated whether the improvement of hypoalbuminemia during PD can affect the patients’ outcomes.Methods436 consecutive incidents continuous ambulatory peritoneal dialysis patients were involved in this study. Demographic, hematologic, biochemical, and dialysis-related data at baseline as well as 1 year after PD were collected. All patients were followed for at least 1 year for mortality.ResultsAmong the 436 patients, the mean age was 48.44 ± 14.98 years, with 58.26% males and 18.12% prevalence of diabetes. The mean follow-up time was 48.25 ± 24.05 months. During the follow-up period, a total of 68 patients died. Serum albumin was 34.35 ± 5.65 g/L at baseline, which increased to 37.39 ± 5.05 g/L at 1 year after PD. Multivariate linear regression analysis showed that sex, age, BMI, diabetic nephropathy, as well as albumin at baseline were independently associated with albumin at 1 year. Every 1 year of age rise would result in a 3.9% increase in the risk of mortality (HR = 1.039, 95%CI 1.016–1.061, p = 0.001). Every 1 g/L increase in albumin at 1 year after PD confers an 8.7% decrease in the risk of mortality (HR = 0.913, 95%CI 0.856–0.973, p = 0.005).ConclusionThe level of serum albumin was increased in the first year of PD. Serum albumin after 1 year of PD predicted mortality in peritoneal dialysis.  相似文献

目的探讨腹膜透析相关性腹膜炎的致病菌和耐药性,及其与转归之间的关系。方法回顾性分析169例次腹膜透析相关性腹膜炎的临床资料、致病菌与耐药性,探讨其与转归的联系。结果116例次培养阳性,培养总阳性率达68.6%,近5年高达87.7%。致病菌中G^+球菌占58例次(50%),G^-杆菌45例次(38.8%),真菌6例次(5.2%),G^+杆菌、G^-球菌及混合感染7例(6%)。G^+球菌中最常见为葡萄球菌(56.8%);G^-杆菌中大肠杆菌阳性率最高(62.2%)。从耐药性看,G^-杆菌对氨苄西林的耐药率最高(76.7%),对阿米卡星和哌拉西林/他唑巴坦的耐药率较低(2.9%、3.4%)。G^+球菌对万古霉素均敏感,对利福平和哌拉西林/他唑巴坦的耐药率较低(2.1%、7.9%)。腹膜炎致持续非卧床腹膜透析(CAPD)退出率为10.1%(17/169),以真菌腹膜炎为主。结论近5年本中心培养阳性率较高,及时采用血培养瓶留取标本很关键。G^-杆菌对阿米卡星和哌拉西林/他唑巴坦较敏感,而万古霉素、利福平和哌拉西林/他唑巴坦宜作为抗G^+球菌的经验用药。真菌性腹膜炎是导致腹膜透析退出的主要原因。  相似文献

目的 分析持续性非卧床腹膜透析(CAPD)患者的死亡原因,为延长患者生存时间提供依据.方法 回顾性分析119例CAPD患者的死亡原因,并与同期126例CAPD存活患者相比较,以寻找其死亡的高危因素.结果 心血管并发症、脑血管意外和营养不良是最主要的死亡原因,分别占30.3%、24.4%和16.8%.与存活组相比,死亡组的年龄、动脉血压(收缩压和舒张压)、胆固醇和C反应蛋白升高(P〈0.05或P〈0.01),死亡组左心室肥大的比例升高(P〈0.01),而心脏射血分数、血红蛋白、血浆白蛋白、主观综合性营养评估、尿素氮清除率、透析前残余肾功能和透析前尿量降低(P〈0.01).结论 心脑血管并发症和营养不良是CAPD患者最主要的死亡原因,与残余肾功能减退、左心室肥大、高血压、蛋白质丢失和微炎症等因素有关.保护残余肾功能、积极有效地控制高血压、纠正营养不良和减少腹膜炎的发生率将有助于延长患者生存时间,降低死亡率.  相似文献
