Fractured implant abutment screws can be retrieved with repair or rescue devices. However, whether the screw was completely retrieved or the inner implant body was damaged may be uncertain. A silicone replica technique was recently implemented in Bern and may be the most predictable method available at the moment for clinically assessing the internal implant body. This technique is straightforward and precise and may help dentists determine the internal implant condition when managing a fractured screw.  相似文献

This article describes a procedure in which articulating paper is modified for the intraoral assessment of static occlusal contacts. The rectangular-shaped articulating paper is modified by creating parallel cuts at 2- to 3-mm intervals perpendicular to its long side without completely separating the paper. This modification may improve the accuracy in determining occlusal contacts and therefore facilitate an occlusal adjustment procedure.  相似文献

Statement of problem

Visual shade matching is subjective and a cause of concern for clinicians. Different measurement devices have been developed to assist in tooth color selection and to achieve better esthetic results. However, consensus is lacking as to which method of tooth shade selection provides more predictable results.

Purpose

The purpose of this clinical study was to compare the reliability of different visual and instrumental methods for dental shade matching.

Material and methods

Visual shade matching was performed by 3 experienced clinicians using 2 different shade guides (VITA Classical A1-D4 and VITA Toothguide 3D-MASTER with 29 tabs; VITA Zahnfabrik) with and without the aid of a light-correcting device (Smile Lite; Smile Line). An intraoral scanner (TRIOS; 3Shape A/S) and a spectrophotometer (VITA Easyshade Advance 4.0; VITA Zahnfabrik) were also used for color shade matching. The instrumental methods were repeated 3 times to determine repeatability. Shade-matching sessions for each method were performed under controlled lighting on the middle third of the maxillary right central incisor of 28 participants. The Fleiss' kappa statistical test was used to assess the reliability of each method. The weighted kappa statistical test was used to assess the agreement between the shades matched by different methods (α=.05).

Results

Instrumental methods were more accurate than visual methods. The best performance was found for the intraoral scanner configured for the 3D-MASTER scale (Fleiss' kappa value of .874) and for the spectrophotometer configured for the VITA Classical scale (Fleiss' kappa value of .805). The best visual shade-matching method was the VITA Classical scale associated with the light-correcting device (Fleiss' kappa value of .322). The Classical scale without the light-correcting device showed the poorest reliability (Fleiss' kappa value of .177) (P<.05).

Conclusions

Instrumental methods for color shade matching were more reliable than the visual methods tested.  相似文献

Statement of problem

Currently, dental articulators can recreate mandibular movements and occlusal contacts. However, whether virtual articulators can also provide information about occluding dental surfaces, functional movements, and the mandibular condyles is unclear.

Purpose

The purpose of this in vitro study was to evaluate the occluding surfaces on dental casts obtained from a patient and approximate them to a hemispherical contact model. Both models were tested by digitizing the Dentatus ARL dental articulator.

Material and methods

A combination of photogrammetry and structure from motion methods were used to scan a Dentatus ARL articulator and representative dental casts. Using computer-aided engineering and finite element analysis, contact points and action vectors to the forces on occluding surfaces and condyles were obtained for cast and hemispherical models. This experiment was performed using centric occlusion and 3 different condylar inclinations. The Kruskal-Wallis 1-way analysis of variance on ranks test was used to allow all pairwise comparisons between condylar inclination and mechanical action vector values in each location (α=.05).

Results

Action vectors from the cast model and each location of the hemispherical model were calculated to show the mechanical consequences and the similarity among models. Overall, no significant differences were observed for action vectors (A20 versus A40 versus A60) at each location (dental cast/hemisphere, right condylar, and left condylar) in the analysis of dental casts and the hemisphere model (.382≤P≤.999).

Conclusions

This study provided graphical information that may assist the dental professional in determining which occlusal contacts should be modified to attain condylar and balanced centric occlusion.  相似文献

Statement of problem

Color matching in restorative and prosthetic dentistry is important for the success of dental treatments, although communication with the dental laboratory remains subjective, and studies of the performance of objective communication methods are lacking.

Purpose

The purpose of this study was to evaluate color differences (ΔE) and compare color luminosity (L*) values among different types of digital photography equipment used to document tooth color, with and without a gray reference card, and to determine whether the gray card could be used to standardize color assessment in dental photography.

Material and methods

Sixty photographs were made (n=10) using different equipment: a D7000 digital camera (Nikon Corp) with an 85-mm lens and wireless close-up flash (DC+WCF); close-up flash surrounded by 80 grams per square meter (gsm; specification of paper thickness) white printing paper (DC+WPP); ring flash (DC+RF); close-up flash attached to a dual-point rigid flash bracket (DC+DPRF); cross-polarizing filter attached to a close-up flash (DC+CPF); and iPhone 7 (I7). For all photographs, a gray reference card with known color values was positioned at the patients’ mandibular teeth, acting as a parameter for the analysis of white-balanced digital photographs. Each photograph underwent white balance with the reference card and software. ΔE were obtained from each piece of equipment by comparing images with and without white balance (original photo) with software and the smallest ΔE achieved was used as the gold standard for comparisons of luminosity. Values of luminosity were subsequently obtained for the different equipment with and without white balancing the photographs; these values were compared using a general estimating equation with Huber-White standard error (α=.05).

Results

The use of a cross-polarizing filter was used as the gold standard for luminosity evaluation, as the smallest ΔE (3.4) among photographs were observed when those with and without white balance were compared. Luminosity results from the cross-polarizing filter method (DC+CPF) were not significantly different from those of the DC+DPRF (P=.73), DC+WPP (P=.106), and DC+WCF (P=.551) groups but were statistically different from DC+RF (P=.028) and I7 groups (P<.001). Use of a gray card was significant when a ring flash (P=.008) or the iPhone (P=.023) were used but not statistically significant for the other groups (P>.05).

Conclusions

The use of a cross-polarizing filter results in more color-standardized photographs, while the ring flash system and the iPhone 7 result in less standardized photographs. The gray reference card had a significant effect when a ring flash system or iPhone 7 was used.  相似文献

Statement of problem

Indirect composite resins (ICR) are promising alternatives as veneering materials for zirconia frameworks. The effects of core-to-dentin thickness ratio (C/Dtr) on the mechanical property of bilayered veneer ICR/yttria-tetragonal zirconia polycrystalline (Y-TZP) core disks have not been previously studied.

Purpose

The purpose of this in vitro study was to assess the effects of C/Dtr on the biaxial flexural strength, reliability, and fracture mode of bilayered veneer ICR/ Y-TZP core disks.

Material and methods

A total of 180 bilayered 0.6-mm-thick composite resin disks in core material and C/Dtr of 2:1, 1:1, and 1:2 were tested with either core material placed up or placed down for piston-on-3-ball biaxial flexural strength. The mean biaxial flexural strength, Weibull modulus, and fracture mode were measured to evaluate the variation trend of the biaxial flexural strength, reliability, and fracture mode of the bilayered disks with various C/Dtr. One-way analysis of variance (ANOVA) and chi-square tests were used to evaluate the variation tendency of fracture mode with the C/Dtr or material placed down during testing (α=.05). Light microscopy was used to identify the fracture mode.

Results

The mean biaxial flexural strength and reliability improved with the increase in C/Dtr when specimens were tested with the core material either up and down, and depended on the materials that were placed down during testing. The rates of delamination, Hertzian cone cracks, subcritical radial cracks, and number of fracture fragments partially depended on the C/Dtr and the materials that were placed down during testing.

Conclusion

The biaxial flexural strength, reliability, and fracture mode in bilayered structures of Y-TZP core and veneer ICR depend on both the C/Dtr and the material that was placed down during testing.  相似文献

Statement of problem

Variation in the baseline mechanical properties of polyamide thermoplastic polymers used in the fabrication of prosthetic dental appliances and the effects of nonaldehyde disinfectants on the mechanical properties of these polymers are unclear.

Purpose

The purpose of this in vitro study was to compare the flexural and impact strengths of 2 flexible denture materials (Valplast and Sunflex) and evaluate the effect of 24-hour immersion in nonaldehyde disinfectant (Perform) on their flexural and impact strengths.

Material and methods

Of 48 specimens of Valplast and Sunflex, half were immersed in nonaldehyde disinfectant solution containing the active ingredient 2% peroxymonosulfate for 24 hours. Flexural and impact strengths were measured using a universal testing machine. The Student t test with Bonferroni correction was used (α=.008).

Results

For the Valplast group, the mean ±standard deviation flexural strength was 27.8 ±0.57 MPa, and the impact strength was 3.5 ±0.98 kJ/m2. For the Sunflex group, the mean flexural strength was 57.4 ±4.09 MPa, and the impact strength was 6.0 ±3.11 kJ/m2. Sunflex showed greater flexural strength (P≤.001) and impact strength (P=.001) than Valplast. A significant increase in the impact strength (P≤.001) but not in the flexural strength of Valplast was observed after exposure to the disinfectant solution. Immersion disinfection had no significant effect on the strength of Sunflex.

Conclusions

The flexural and impact strengths of Sunflex were significantly greater than those of Valplast. Immersion disinfection with peroxymonosulfate had no significant effect on Sunflex but increased the impact strength of Valplast.  相似文献

Statement of problem

Whether masking or translucency should be the goal for achieving an esthetic restoration with ceramic materials is unknown.

Purpose

The purpose of this in vitro study was to examine the effect of the number of firings on the color and translucency of ceramic core material with different thicknesses of veneer ceramic.

Material and methods

Three different ceramic systems were used: IPS Empress (IE), IPS e.max Press (IEP), and Turkom Cera (TC). Ninety disk-shaped cores, 10 mm in diameter and 0.8 mm in thickness were prepared using each system (n=30). The specimens were divided into 3 subgroups (n=10) according to the thickness of the veneering ceramic: 0.5, 1.0, and 1.5 mm. The specimens were fired 1, 3, and 5 times, and the color parameters of the specimens were measured. The color measurements were repeated, and the data were statistically evaluated using 2-way and multifactorial analysis of variance, the Duncan test, and the Bonferroni post hoc test.

Results

The number of firings affected the translucency parameter and CIELab values. Increased porcelain thickness led to decreased translucency parameter and L* values and increased a* and b* values (P<.05). The IE system was the most translucent (P<.001). The changes with increased firing periods were not statistically significant (P>.05). The number of firings affected the color change of the IE and IEP systems but not of the translucency parameter system.

Conclusions

Repeated firings and porcelain thickness affected the final color and translucency of the ceramic systems tested. These 2 factors adversely affected the esthetic result and should be considered in both the preparation and the laboratory phase.  相似文献

目的: 观察SuperBond C&B粘结系统对牙周炎松动前牙及后牙作松牙固定后的临床效果,并评价其与结扎丝复合树脂夹板、百强纤维夹板的各自临床特点。方法: 经牙周基础治疗后,松动Ⅱ°~Ⅲ°的前牙(41人,228牙),随机分组,制作 SuperBond(A组)、结扎丝复合树脂(B组)、百强纤维(C组)牙周夹板,分别对3组固定后1月、3月的夹板状况、牙周指数、患者主观感受进行比较。松动I°~Ⅲ°的后牙(6人,10牙)经SuperBond C&B固定后3月的夹板状况。结果: 对松动前牙,固定后1月、3月时A组的夹板粘结断裂率高于B组和C组(P<0.005),3月时A组的牙石指数、牙龈指数低于B与C组(P<0.05),3月时PD、AL、切割功能无显著差异。对于松动后牙,SuperBond固定后1月、3月夹板断裂率较高。结论: SuperBond C&B粘结系统用于固定松动前牙有更为美观、舒适的优势,可用于暂时固定松动后牙。  相似文献

To determine the relationship between the plane of occlusion and the Camper's line (ala-tragus line). Lateral cephalograms of 105 dentulous subjects were obtained after outlining the tragus and the base of the ala of the nose with radiopaque markers. Tracings of the cephalograms were done and the relationship between the plane of occlusion and the Camper's line (ala-tragus line) was noted. The most common tragal reference as a posterior landmark for determination of plane of occlusion was found to be below inferior (in 30.48% of subjects), and inferior (in 24.76% of subjects). The least common tragal reference was found to be above superior (in 3.82% of subjects) followed by superior of tragus and the point between superior and middle of the tragus (in 6.66% of subjects). The tragal reference in this study population was more towards the inferior of the tragus, with most of the times being below the inferior border. Therefore, the orientation of the plane of occlusion using the superior of tragus as a posterior landmark (according to the widely accepted definition of Camper's line) may be considered to be questionable. Further, the use of the tragus as a posterior landmark for the orientation of the plane of occlusion may be questioned on the basis of the findings of this study.  相似文献
