Summary Although the prognosis of high grade malignant glioma patients is generally poor, it is possible to identify groups of patients with varying prognoses. Basing our results on the first MRC glioma study, multivariate methods were used to identify prognostic factors independently associated with the length of survival. Young age, the presence of fits, especially of long duration, extensive surgical removal of tumour and good clinical performance status were found to be the most important predictors of longer survival. The effect of tumour grade (3 or 4) was not significant, being considerably diluted by an association with extent of neurosurgery. A prognostic index was derived which split the patients into 6 groups of varying prognoses, with 2-year survival rates of between 1 and 32%. The results were verified in patients entered into a subsequent MRC trial. The successful identification of different prognostic groups suggests the use of this index as an aid in making treatment decisions for individual patients, and in interpreting the results of uncontrolled phase II studies.This report was prepared on behalf of the participating members by S.P. In October 1979 we started an accelerated hyperfractionation program consisting of 1.6 Gy/fraction, 2 fractions/day for 12 days or 38.4 Gy. Due to acute toxicity, the patients were then given a 2-week break and resumed once daily radiation therapy with 1.8 Gy/fraction up to 65 Gy, designated as the b.i.d.-q.d. program. In August 1982, the program was changed and the latter part of the treatment was continued on the twice daily program with 1.6 Gy/fraction for a total of 64 Gy, designated as the b.i.d.-b.i.d. program. We evaluated the local control rates of 140 patients with squamous cell carcinomas arising from the oropharynx, that is faucial tonsil and base of tongue. After these treatment regimens, the 36 month actuarial local rates for the T1-4 lesions were 56% for b.i.d.-q.d. (52 patients) and 85% for b.i.d.-b.i.d. (88 patients) with a p value of 0.0013. For the T1-2 lesions the corresponding rates were 81% (13 patients) and 97% (44 patients) with a p value of 0.53. The difference was marked for the T3-4 lesions, that is 47% vs 77% respectively with a p value of 0.017, and those patients without nodal metastases, that is 46% vs 93% with a p value of 0.00043. The improvement was probably due to marked shortening of the overall treatment course following b.i.d.-b.i.d. accelerated program--in this case 1 1/2 weeks. Our preliminary data suggests that the b.i.d.-b.i.d. program is superior to the b.i.d.-q.d. program. The seemingly improved local control rates for the treatment of oropharyngeal carcinomas requires a clinical prospectively randomized trial for confirmation, that is b.i.d.-b.i.d. versus b.i.d.-q.d. versus standard fractionation (q.d.) program.  相似文献

Xenografts (s.c. and i.p.) of human ovarian cancer, shown to express the tumor associated antigen defined by the monoclonal antibody HMFG2, were used to investigate in vivo localization of the radioiodinated antibody after i.p. and i.v. injection. Following i.v. injection, maximum uptake (31.4 +/- 3.5%/g s.c. tumor) was seen in s.c. tumors at 48 h after injection. Tumor:normal tissue ratios increased with time to 240 h. Uptake by ascites and i.p. tumors was less, with a maximum of 10.0 +/- 8%/g ascites reached at 16 h and 11.4 +/- 3.2%/g i.p. tumor at 96 h. After i.p. injection uptake in s.c. xenografts was maximal (14.8 +/- 2.0%/g) at 20 h. For ascites, the i.p. route of administration resulted in high uptake (27.7 +/- 5.8%/g) early (2 h) with an ascites:normal tissue ratio of 69.3 and a specific antibody:nonspecific antibody ratio of 30.8. Except for data at one time point, uptake by i.p. solid tumor was similar to that seen for s.c. tumor. These data strongly suggest that large concentration advantages can be achieved in ascites cells by regional i.p. injection, but that i.p. solid tumor may rely on i.v. delivery of antibody before uptake.  相似文献

INTRODUCTION: The i.v. administration of the cytoprotective agent amifostine is associated with reversible clinical hypotension, protracted emesis, and malaise in a various percentage of patients. We evaluated, prospectively, whether the s.c. route is a better tolerated alternative to the i.v. route in patients receiving chemotherapy. PATIENTS AND METHODS: Fifty-nine patients treated with "once every 2 weeks" regimens received 1000 mg of amifostine i.v. before chemotherapy. Patients who developed protracted vomiting and malaise and/or clinical hypotension for two consecutive i.v. administrations received the same dose of amifostine s.c. for the subsequent cycles (i.v./s.c. study). In an additional cohort of 12 patients (s.c. study), 1000 mg of amifostine were given s.c. since the first chemotherapy cycle. RESULTS: In the i.v./s.c. study, 8 (13.5%) patients showed protracted emesis/malaise and/or clinical hypotension during the first two cycles. An additional 4 (6.6%) patients developed similar side effects during the subsequent cycles. Switching to the s.c. route, an improved tolerance was noted. In the s.c. study, a total of 76 injections was administered. Protracted vomiting or clinical hypotension was absent, and this tolerance profile was significantly better than the i.v. one (P = 0.001). There were no other systemic side effects related to the s.c. administration. CONCLUSIONS: Amifostine, at a dose of 1000 mg, is better tolerated when administered s.c. Switching to the s.c. route in patients with poor tolerance using the i.v. administration allows the continuation of cytoprotection with minor side effects. Although preliminary, 1000 mg of amifostine effectively protected against the lower, still more frequently administered doses of chemotherapy given once every 2 weeks.  相似文献

Irinotecan (CPT-11) is a new drug active in colorectal cancer. A comparison was made of the efficacy and pharmacokinetics of CPT-11 after i.p. versus i.v. administration to mice. We found that i.p. administration of CPT-11 to mice bearing C26 colon cancer was more efficient and less toxic than i.v. administration; a 100-mg i.p. dose induced an increase in life span equivalent to that produced by a 300-mg i.v. dose, and toxic deaths appeared after doses of 400 mg/kg given i.v. and 800 mg/kg given i.p. Pharmacokinetic parameters of CPT-11 and SN-38 were compared after i.v. or i.p. administration in mice bearing P388 leukemia ascites. Peritoneal CPT-11 and SN-38 AUC values were higher after i.p. administration than after i.v. injection. Plasmatic AUC values remained equivalent. Moreover, peritoneal CPT-11 clearance was 10-fold lower after i.p. versus i.v. administration. If the survival and pharmacologic advantage of i.p. CPT-11 in the murine model considered can be translated to a safe and practical mode of therapy in patients and if local toxicity does not prove to be a major adverse effect, then a potentially useful agent could be added to the drugs known to be active when given by the i.p. route for adjuvant therapy in colon cancer.
