DCE-MRI定量参数分析在早期乳腺癌诊断中的应用价值

摘    要:目的 研究动态增强磁共振成像(dynamic contrast-enhanced magnetic resonance imaging,DCE-MRI)定量参数分析在早期乳腺癌诊断中的应用价值。方法 选取浙江省台州医院2020年6月至2021年6月收治的乳腺癌患者70例为恶性组,另选取同期浙江省台州医院收治的乳腺纤维腺瘤患者30例为良性组,采用DCE-MRI定量参数对乳腺癌和乳腺纤维腺瘤进行鉴别诊断分析,比较两组的DCE-MRI定量参数容量转移常数(volume transfer constant,Ktrans)、反流速率常数(reflux rate constant,Kep)、血管外细胞外间隙容积比(volume of the external vascular extracellular space,Ve),比较不同病理类型DCE-MRI定量参数Ktrans、Kep、Ve及DCE-MRI定量参数分析与乳腺癌患者预后的相关性及诊断敏感度、准确性。结果 良性组的DCE-MRI定量参数Ktrans、Kep、Ve均低于恶性组(P<0.0001);导管内癌组的Ktrans、Kep、Ve及非特殊类型浸润性癌的Ktrans、Kep、Ve均低于特殊类型癌(P<0.05);雌激素受体(estrogen receptor,ER)表达阴性组的Kep高于ER阳性组(P<0.05);孕激素受体(progesterone receptor,PR)阴性组的Kep高于PR阳性组(P<0.05);ER、PR阴性组的Ktrans、Ve高于ER、PR阳性组;人类表皮生长因子受体-2低表达和过表达组的Ktrans、Kep、Ve比较,差异无统计学意义(P>0.05);与Ktrans、Kep、Ve单一诊断比较,联合诊断对乳腺癌诊断价值较高(P<0.05)。结论 DCE-MRI定量参数分析用于早期乳腺癌的诊断,能有效鉴别不同疾病的类型及预后,诊断价值较高。

Application of DCE-MRI quantitative analysis in early breast cancer
Abstract:Objective To investigate the application value of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) quantitative parameter analysis technology in early breast cancer. Methods Seventy patients with breast cancer admitted to Taizhou Hospital of Zhejiang Province from June 2020 to June 2021 were selected as malignant group, and 30 patients with breast fibladenoma admitted to Taizhou Hospital of Zhejiang Province during the same period were selected as benign group, all of which were diagnosed by DCE-MRI quantitative parameter analysis technique. DCE-MRI differential diagnosis, DCE-MRI quantitative parameters volume transfer constant (Ktrans) and reflux rate of breast cancer and breast fibriadenoma constant (Kep), volume of the external vascular extracellular space space (Ve) were compared, the DCE-MRI quantitative parameters Ktrans, Kep, Ve of different pathological types, correlation between DCE-MRI quantitative parameter analysis techniques and prognosis of breast cancer patients, diagnostic sensitivity and accuracy statistics were compared. Results The quantitative parameters of DCE-MRI Ktrans, Kep and Ve in benign group were lower than those in malignant group (P<0.0001). The parameters Ktrans, Kep, Ve of intraductal carcinoma group and the parameters Ktrans, Kep, Ve of non-special invasive carcinoma group were lower than those of special type carcinoma (P<0.05). Kep in estrogen receptor (ER) negative group was higher than that in ER positive group (P<0.05). Kep in progesterone receptor (PR) negative group was higher than that in PR positive group (P<0.05). Ktrans and Ve in ER and PR negative group were higher than those in ER and PR like group (P<0.05). There was no significant difference between the low expression group of human epidermal growth factor receptor (HER)-2 and the over-expression group of Ktrans, Kep and Ve (P>0.05). Compared with the single diagnosis of Ktrans, Kep and Ve, the combined diagnosis has a higher diagnostic value for breast cancer (P<0.05). Conclusion DCE-MRI quantitative parameter analysis technology can be used in the diagnosis of early breast cancer, can effectively identify different types of diseases, predict the prognosis of patients, and has high diagnostic value.
