孕妇血清锌水平与新生儿神经管缺陷发生关联的Meta分析

摘    要:目的 综合评价孕妇血清锌水平与新生儿神经管缺陷发生的关联,为出生缺陷防治提供科学依据。方法 计算机检索Pubmed、the Cochrane Library、Web of Science、中国知网和万方数据等数据库,收集了从1950年至2020年3月19日的关于关孕妇血清锌水平与神经管缺陷发生关联的人群流行病学研究,由两名人员按既定的纳入与排除标准筛选文献并作出质量评价,采用Stata 14.0软件对研究资料进行Meta分析。结果 总共纳入10篇文献,采用随机效应模型进行Meta分析,结果显示,孕妇血清锌水平较低者,其新生儿更易患有神经管缺陷(SMD=-2.07,95%CI:-3.07~-1.07)。亚组分析中发现,锌矿储量情况为异质性来源。敏感性分析表明研究结果相对稳健。Begg秩相关分析(z=2.68,P=0.007)和Egger回归分析(t=-4.72,P=0.001)提升存在一定发表偏倚。结论 孕期低血清锌水平可增加新生儿神经管缺陷发生的危险性。

Abstract:To evaluate the relationship between serum zinc level of pregnant women and the occurrence of neuraltube defects of newborns,so as to provide scientific basis for the prevention and treatment of birth defects.Methods Thedatabase of Pub Med,the Cochrane Library,web of science,CNKI and Wan Fang were searched by computer.The populationepidemiological study on the relationship between serum zinc level of pregnant women and neural tube defects from 1950 toMarch 19,2020 was collected.According to the established inclusion and exclusion criteria,two persons screened theliterature and made a quality evaluation.The research data were Meta-analyzed by using Stata 14.0.Results A total of 10articles were included and Meta-analysis was carried out by using the random effect model.The results showed that thenewborns with lower serum zinc level in pregnant women were more likely to suffer from neural tube defects(SMD=-2.07,95%CI:-3.07~-1.07).In the subgroup analysis,it was found that the zinc ore reserves were heterogeneous sources.Sensitivity analysis showed that the results were relatively stable.There was publication bias in the promotion of Begg’s rankcorrelation analysis(z=2.68,P=0.007)and Egger’s regression analysis(t=-4.72,P=0.001).Conclusion Lowserum zinc level during pregnancy can increase the risk of neural tube defects in neonates.
