| 摘 要： | 目的 探讨适合河南地区新生儿呼吸窘迫综合征（NRDS）患者住院费用的 DRGs病例组合方案，制定该病住院费用的参考标准。方法 收集河南某三级妇幼专科医院2017-2019年4002例NRDS患者病例，采用单因素方差分析和多元线性逐步回归分析，确定住院费用的主要影响因素，利用决策树对NRDS患者进行病例组合分析。结果 住院费用的主要影响因素有:呼吸机使用时长、有无操作、合并症或并发症的个数、新生儿出生体重、早产与否。通过对4002例NRDS患者分组共形成7个DRGs组合和相对应住院费用标准。结论 基于NRDS患者住院费用DRGs病例组合方案，可为制定适合河南省医保支付费用标准，切实降低医疗费用提供参考依据。

| Abstract: | To explore the combination of case－mix groups based on DＲGs for the hospitalization expenses ofneonatal respiratory distress syndrome in Henan Province，and to establish the reference standard for the medical expenses ofNＲDS.Methods 4 002 cases with NＲDS were collected from 2017 to 2019 in a tertiary hospital of maternal and child inHenan Province.One－way ANOVA and multiple linear regression analysis were used to analyze the main influencing factors ofhospitalization expenses，and case－mix groups of neonatal respiratory distress syndrome were conducted by using decisiontrees.Ｒesults The main influencing factors of hospitalization cost were:duration of using ventilator，operation or not，thenumber of comorbidities or complications，birth weight of the newborn，premature delivery or not.7 DＲGs and correspondinghospitalization cost standards were formed from 4002 cases.Conclusion Case－mix groups of medical expenses of NＲDSbased on DＲGs can provide references for the development of medical insurance payment standards in Henan province and can |
