泰州市急性胃肠炎学校聚集性疫情病原学鉴定及分子溯源

摘    要:目的 了解2018年10月至2019年1月间泰州地区急性胃肠炎疫情主要致病原,掌握其传播流行规律。方法 采集学校聚集性疫情中患者的咽拭子和肛拭子标本,进行病原学核酸检测,测定阳性疫情株VP1基因序列,构建系统发生树,分析疫情株所属流行亚群,推测该流行亚群做传染病人口学模型和传播路径,以及VP1基因变异性分析。结果 共采集疫情样100人份,检出诺如病毒GII型阳性45份,测定其中20份标本VP1基因序列,除1份为GII.3型外,其余均为GII.2亚型,该GII.2型疫情株间同源性平均达99.4%,其与2016-2017年中国、日本流行株属于同一流行亚群,该亚群起源于上海地区,且仍处于流行上升阶段。结论 本时间段泰州市急性胃肠炎学校聚集性疫情的主要致病原是GII.2型诺如病毒,其属于一个活跃的流行亚群。

Abstract:To investigate the major pathogen prevalent in the epidemic cases of acute gastroenteritis in Taizhou from October 2018 to January 2019, and to understand epidemic patterns. Methods The throat and anal swabs were collected from patients in each epidemic case, the specimens were subject to nucleic acid testing and sequenced VP1 gene sequences for the positive detected strains. Phylogeny was reconstructed basing on the sequences, for analyzing which epidemic subgroup the strains belonging to. The epidemic demographic model and transmission pathway of the subgroup were further inferred, with mutation pattern of VP1 genes analyzed. Results A total of 100 specimens were collected and 45 of them were positive detected for Norovirus, with 20 of strains subjected to VP1 sequencing. Except one specimen being Norovirus GII.3, the rest were typed as Norovirus GII.2 with the average similarity being 99.4%, which belonged to the same epidemic subgroup together with the strains prevalent in China and Japan during 2016-2017. The subgroup was inferred to origin around Shanghai and still undergoing an epidemic rising. Conclusion The major pathogen responsible for the epidemic cases of acute gastroenteritis in Taizhou is Norovirus GII.2, which belongs to an active epidemic subgroup.
