肿瘤标志物联合中性粒细胞/淋巴细胞比值在肺癌诊断中的应用价值

摘    要:目的:探讨中性粒细胞/淋巴细胞比值(NLR)与肿瘤标志物胃泌素释放肽前体(ProGRP)、鳞状细胞癌抗原(SCCA)、人细胞角蛋白19片段(CYFRA21-1)、神经元特异性烯醇化酶(NSE)联合检测在肺癌诊断中的应用价值。方法:采用回顾性研究方法收集2019-01-02至2019-12-23期间在南部战区总医院就诊并明确诊断为肺癌的134例患者作为肺癌组,另选取80例同时段的肺部良性病例患者作为对照组,收集以上患者的临床资料,采用t检验和ROC等数据分析方法,回顾性分析各个指标单独或联合在肺癌诊断中的诊断价值。结果:肺癌患者血清ProGRP与患者年龄有关(P<0.05);SCCA与年龄、淋巴结转移(P<0.05)和性别(P<0.01)有关;CYFRA21-1与肿瘤分期有关(P<0.05);NSE与肿瘤大小(P<0.01)及淋巴结转移有关(P<0.05);NLR与患者年龄和肿瘤分化程度有关(P<0.05),且肺癌组患者肿瘤指标与NLR均高于对照组。根据ROC曲线按照Youden指数确定各指标的临界值,并依据临界值进行进一步分组比较ROC曲线下面积。四种肿瘤标志物诊断肺癌的AUC值分别为0.647、0.615、0.606与0.58,均小于与NLR联合后的AUC。结论:肺癌患者术前外周血肿瘤标志物与NLR均高于对照组,外周血NLR与肿瘤分化程度有关,将肿瘤标志物与NLR联合可提高对肺癌的诊断效能。

Abstract:Objective:To investigate the effect of NLR and tumor markers ProGRP,SCCA,CYFRA21-1 and NSE combined detection in the diagnosis of lung cancer.Methods:A retrospective study was performed to collect 134 patients who had been diagnosed with lung cancer in cardiothoracic surgery admitted to our hospital between January 02,2019 to December 23,2019 as the lung cancer group,and another 80 benign cases diagnosed during the same period were selected as control group.Clinical data of the above patients were collected,and the diagnostic value of each index alone or in combination in the diagnosis of lung cancer was retrospectively analyzed by t test and ROC data analysis methods.Results:Serum ProGRP in patients with lung cancer was correlated with age(P<0.05),SCCA was corrlated with age,lymph node metastasis(P<0.05) and gender(P<0.01),Cyfra21-1 was associated with tumor stage(P<0.05) and NSE was associated with tumor size(P<0.01) and lymph node metastasis(P<0.05).NLR was associated with age and degree of tumor differentiation(P<0.05),and the tumor markers and NLR in lung cancer group were higher than those in control group.The cut-off of each index were determined according to the ROC curve and Youden index,and the areas under the ROC curve were further grouped according to the critical values.The AUC values of the four tumor markers were 0.647,0.615,0.606 and 0.58,respectively,which were lower than that of the combination of those tumor markers with NLR.Conclusion:Preoperative tumor markers and NLR in peripheral blood of patients with lung cancer are higher than those in the control group.NLR in peripheral blood is related to the degree of tumor differentiation,and combined detection of tumor markers with NLR can improve the diagnostic value in lung cancer.
