双源CT双能量扫描在肺病灶良恶性诊断中的应用研究

摘    要:目的:本实验探讨双源CT双能量技术在肺病灶良恶性诊断中的应用。方法:收集病例140例,其中腺癌82例、鳞癌41例、炎症17例,均行双源CT双能量扫描。测量所有病变的碘浓度、标准化碘浓度及能谱曲线斜率并记录。应用统计学方法分析,判断三种病变之间的差异。结果:腺癌、鳞癌及炎症的碘浓度分别为(2.102±0.794) mg/mL、(1.871±0.654) mg/mL、(2.400±0.895) mg/mL,鳞癌与炎症之间差异有统计学意义,腺癌与鳞癌、腺癌与炎症之间差异无统计学意义。腺癌、鳞癌及炎症的标准化碘浓度分别为(48.728±17.740)%、(49.834±19.418)%、(59.176±16.032)%,腺癌与炎症之间差异有统计学意义,腺癌与鳞癌、鳞癌与炎症之间差异无统计学意义。腺癌、鳞癌及炎症的能谱曲线斜率分别为(-2.121±0.593)、(-1.711±0.548)、(-2.499±0.937),两两之间差异均有统计学意义。结论:双源CT双能量扫描技术对肺病灶性质的鉴别诊断有一定的临床价值。

Abstract:Objective:To investigate the application of dual-source CT dual-energy technique in the benign and malignant pulmonary lesions.Methods:140 cases were collected,including 82 cases of adenocarcinoma,41 cases of squamous cell carcinoma and 17 cases of inflammatory lesions.All patients underwent enhanced dual-source CT scanning in dual-energy pattern.The iodine concentration,normalized iodine concentration and the slope of energy spectrum curve of all lesions were measured and recorded.Statistical methods were used to analyze the differences among the three lesions.Results:The iodine concentrations of adenocarcinoma,squamous cell carcinoma and inflammatory lesions were(2.102±0.794)mg/mL,(1.871±0.654)mg/mL,(2.400±0.895)mg/mL,respectively.The difference between squamous cell carcinoma and inflammatory lesions was statistically significant.There was no statistical difference between adenocarcinoma and squamous cell carcinoma,adenocarcinoma and inflammatory lesions.The normalized iodine concentrations of adenocarcinoma,squamous cell carcinoma and inflammatory lesions were(48.728±17.740)%,(49.834±19.418)%,(59.176±16.032)%,respectively.The difference between adenocarcinoma and inflammatory lesions was statistically significant.There was no statistical difference between adenocarcinoma and squamous cell carcinoma,squamous cell carcinoma and inflammatory lesions.The slopes of energy spectrum curves of adenocarcinoma,squamous cell carcinoma and inflammatory lesions were(-2.121±0.593),(-1.711±0.548),(-2.499±0.937).And the differences were statistically significant.Conclusion:Dual-source CT dual-energy scanning technology has certain clinical value in the differential diagnosis of lung lesions.
