螺旋CT三维重建在髋臼骨折的临床应用

摘    要:目的 探讨螺旋CT三维重建 (3DCT)对髋臼骨折的临床应用价值。方法 对 3 3例髋臼骨折行单层螺旋CT(2 3例 )及多层螺旋CT(10例 )薄层容积扫描 ,并进行表面遮蔽显示法 (SSD)及容积再现法 (VR)三维重建 ,并对 2DCT和3DCT的结果进行分析、比较。结果  3DCT能立体、直观、准确地评估髋臼骨折的形态、类型、移位及是否合并脱位等空间信息 ,但 3DCT对微小骨折的显示率低于 2DCT ,且存在着“假骨折”和“假孔”现象。结论  3DCT在显示髋臼骨折的空间关系上具有优势 ,具有较好的临床应用价值

Abstract:Objective To evaluate clinical value of spiral CT three dimension reconstruction (3DCT) in diagnosis of acetabular fracture. Methods Thirty three cases of acetabular fracture were underwent thin slice volume scan and then 3D reconstruction in ways of surface shaded display (SSD) and volume rendering (VR) by single slice (23 cases) or multi slice (10 cases)spiral CT;results of 2DCT and 3DCT were analyzed and compared. Results 3DCT could evaluate stereoscopically and accurately conformation,type,fracture displacement,hip dislocation and other space information,but was worse than 2DCT in detecting small fracture and "false fracture" and "false hole" existed in 3DCT. Conclusion 3DCT takes advantages of displaying space relation of acetabular fracture and has obvious clinical value.
