急性心肌梗死患者血清miR-423-5P,Bcl-2 和LC3-II 水平表达与预后相关性研究

摘    要:目的 探讨血清miR-423-5P,Bcl-2 和LC3-II 对急性心肌梗死(acute myocardial infarction,AMI)患者预后的影响。方法 筛选2017 年3 月~ 2018 年10 月收治于邯郸市第一医院心内科的122 例AMI患者为研究对象,根据随访期间(一年)是否发生主要不良心血管事件(major adverse cardiovascular events,MACE)分为MACE组(n=43)和非MACE组(n=79),同期体检健康志愿者(n=50)作为对照组,分别采用聚合酶链式反应(polymerase chain reaction,PCR)和酶联免疫吸附测定(enzyme linked immunosorbent assay,ELISA)检测患者血清miR-423-5P,Bcl-2 和LC3-II 表达水平。比较两组患者血清miR-423-5P,Bcl-2 和LC3-II 的表达差异,并用受试者工作特征(receiver operator characteristic, ROC) 曲线图分析对急性心肌梗死患者预后的价值。结果 对照组miR-423-5P(0.91±0.12),Bcl-2(1.13±0.21ng/ml)和LC3-II(0.73±0.16 ng/ml)均低于MACE组(1.42±0.22,4.25±0.34 ng/ml,1.25±0.31g/ml)和非MACE组(1.13±0.11,2.01±0.28ng/ml,1.21±0.06 ng/ml),差异均具有统计学意义(F=151.155,1 880.468,115.625,均P < 0.05);MACE 组血清mIR-423-5P 和Bcl-2 表达均高于非MACE 组,差异具有统计学意义(t=9.716, 39.093,均P < 0.05),两组间血清LC3-II 的表达差异无统计学意义(t=1.113, P>0.05)。ROC 曲线显示,miR-423-5P,Bcl-2 预测急性心肌梗死患者预后的曲线下面积分别为0.813 和0.776,截断值分别为1.213 和4.012 ng/ml,灵敏度分别为0.721 和0.702,特异度分别为0.734和0.737;二者联合检测时,曲线下面积、灵敏度和特异度分别为0.852, 0.832,0.827。结论 血清mIR-423-5P,Bcl-2水平与急性心肌梗死患者预后密切相关,有可能成为判断预后的标志物,且两者联合应用的效果优于单独应用。

Abstract:Objective Aim to investigate the effects of serum miR-423-5P,Bcl-2 and LC3-II on the prognosis of patients with acute myocardial infarction(AMI). Methods 122 patients with AMI treated in the Department of Cardiology of the First Hospital of Handan City from March 2017 to October 2018 were selected as the research objects. According to whether major adverse cardiovascular events (MACE) occurred during the follow-up period (1 year), they were divided into mace group (n=43 ) and non mace group (n=79 ). Healthy volunteers (n=50 ) in the same period were used as the control group. The expression levels of serum miR-423-5p, Bcl-2 and LC3-II were detected by PCR and ELISA. The expression differences of serum miR-423-5p, Bcl-2 and LC3-II between the two groups were compared, and the value of ROC curve in the prognosis of patients with acute myocardial infarction was analyzed. Results MiR-423-5p (0.91 ± 0.12), Bcl-2 (1.13 ± 0.21ng/ml) and LC3-II (0.73 ± 0.16 ng/ml) in the control group were lower than those in MACE group (1.42 ± 0.22, 4.25 ± 0.34 ng/ml, 1.25 ± 0.31 ng/ml) and non MACE group (1.13 ± 0.11, 2.01 ± 0.28 ng/ml, 1.21 ± 0.06 ng/ml), and the differences were statistically significant (F=151.155,1 880.468,115.625,all P < 0.05). The expressions of serum miR-423-5p and Bcl-2 in the MACE group were higher than those in the non-MACE group, with statistically significant differences (t=9.716, 39.093, all P< 0.05), while the differences in LC3-II expression between the two groups were not statistically significant (t=1.113, P> 0.05). The area under the curve of miR-423-5p and Bcl-2 in predicting the prognosis of patients with AMI were 0.813 and 0.776, cut-off value were 1.213 and 4.012 ng/ml, sensitivity were 0.721 and 0.702, specificity were 0.734 and 0.737, respectively. When the two were combined, the area under the curve, the sensitivity and the specificity were 0.852, 0.832, 0.827. Conclusion Serum miR-423-5p and Bcl-2 levels are closely related to the prognosis of patients with acute myocardial infarction, and may be used as prognostic markers, and the combined application of the two is better than the single application.
